Subjective assessment of temporomandibular joint sounds.

If it could be shown that the human ear was sufficiently sensitive to describe TMJ sounds, there would be no need to use sophisticated electronic equipment to analyse the sounds. To test this, the ability of normal listeners to distinguish the subtle changes in position, pitch, duration and latency present in TMJ sounds is measured using triangle tests to determine the just-noticeable differences. The results suggest that the human ear is a rather poor instrument for describing subtle differences in the position, duration and latency of TMJ sounds, but is capable of detecting small differences

Modified Hauser operation for patellar instability. Immediate mobilization of 35 knees, a 5-8 year follow-up study.

We performed a follow-up examination of 35 knees with chronic patellar instability treated by a modified Hauser operation: medialization and distalization of the tibial tubercle without dorsal transfer, and fixation of the tubercle rigidly with 2 cortical screws. Cast immobilization was not used and the patients were allowed to mobilize the knee immediately. The follow-up period was 6 (5-8) years. 27 of 35 knees were excellent or good at follow-up, while 8 patients were more or less dissatisfied with the result. 5 knees had arthrosis at follow-up. No severe complications occurred, nor did the

Diagnostic potential of pseudo-dynamic MRI (CINE mode) for evaluation of internal derangement of the TMJ.

In addition to the patient's history and a thorough clinical investigation, magnetic resonance imaging (MRI) of the temporomandibular joint (TMJ) has been introduced to complete the findings for the diagnosis of internal derangement of the TMJ. However, 'dynamic information' is desirable to help us to understand the mechanism of internal derangement. This information is given for example by electronic axiography recording systems. The lack of any ability to assess joint function dynamically in MRI is a point of criticism. Using a computer-driven pseudodynamic MRI system (CINE mode) 'dynamic in

A modified extraoral technique of mandibular manipulation in disk displacement without reduction.

A main reason for failure when manipulating a mandible in disk displacement without reduction is a lack of coordination, because of a sudden resistance on the patient's part when fingers are placed in his mouth. This work describes a new extraoral manipulation technique that avoids this problem. Basically, this technique involves three stages. In stage one, the patient performs rhythmic left and right sideways movements unassisted; in stage two, gentle finger pressure is applied by the clinician on the external aspect of the mandible in the same direction as the patient's movements; and in sta

Follow-up examination of the temporomandibular joint disk after splint therapy by magnetic resonance imaging--a case report.

A 38-year-old female patient presented with temporomandibular joint (TMJ) pain, bilateral clicking and intermittent locking. The patient was treated with a splint in order to decrease the symptoms and to obtain a proper disk-condyle relationship. The effect of the splint therapy was investigated after one month and after one year and five months using magnetic resonance imaging (MRI) with splints in place. After treatment for one year and six months, the clicking sound of the right TMJ disappeared, and she became free from locking. With the aid of MRI, it was found that the disk of the right T

Boutonniere deformity.

The boutonniere (French for button hole) deformity is a result of an injury to the finger extensor mechanism that causes a characteristic flexion attitude of the proximal interphalangeal joint and distal interphalangeal joint hyperextension. This article reviews the pertinent anatomy, the mechanisms of injury, the diagnosis, and a systematic approach to the prevention and treatment of the boutonniere deformity.

A new appraisal of abnormalities of the odontoid process associated with atlanto-axial subluxation and neurological disability.

It is well known that abnormalities of the odontoid process may be associated with subluxation and neuraxial compression. Modern computerized imaging is demonstrating that existing classifications are in need of revision since they no longer reflect current views on development, nor do they adequately explain the patterns of abnormality commonly encountered in clinical practice. This study is a description of 62 varied clinical cases examined by high definition computerized myelography or MRI in flexion and extension, some before and after stabilization procedures. From these descriptions, and

Evaluation of patellar shape in the sagittal plane. A clinical analysis.

A prospective study of 564 patients was performed to assess the correlation between patellar shape as seen on a lateral radiograph and 1) patellar pain and 2) the radiologic determination of patellar height. A classification of patellar shape was developed based on the ratio of the patellar length to the length of the articular surface. Three classification types were established, including the "long-nosed," "Cyrano" type. Patients were placed into one of two groups based on the presence or absence of patellar pain. Lateral radiographs were evaluated for patellar height using two published ind

[Shoulder arthroscopy in degenerative and inflammatory diseases].

A new concept of shoulder arthroscopy is presented based on experience with 800 shoulder arthroscopies. All open surgical procedure on the shoulder are preceded by arthroscopy. The procedure is performed with the patient lying on his or her side. The medium is CO2, and the advantages are less bleeding, a natural picture, no fluid edema. After diagnostic arthroscopy (with CO2), operative arthroscopy (with fluid) or open joint surgery follows. The surgery can easily be performed in the same position for good access with any approach. Preliminary diagnostic arthroscopy helps to plan the procedure

Atlanto-occipital subluxation in Down syndrome.

Atlanto-occipital subluxation (AOS) in individuals with Down syndrome is discussed using five new cases and nine patients previously presented in the literature. Although AOS is likely due to ligamentous laxity, it was associated with atlantoaxial instability in only two youngsters. Reducible C1-C2 rotary subluxation was present in a third. Posterior movement of the occiput with respect to C1 occurred on extension and reduced on flexion in all but one individual who demonstrated anterior subluxation. Neurological problems are described in only two individuals: one with severe atlantoaxial subl

Evaluation of patients with persistent symptoms after lateral retinacular release by kinematic magnetic resonance imaging of the patellofemoral joint.

The arthroscopic lateral retinacular release is typically performed to treat patellar pain and instability. This procedure was previously considered to be relatively benign with a low associated complication rate. However, a high incidence of medial subluxation of the patella was recently reported in patients with persistent symptoms after lateral retinacular release. Because the use of physical examination criteria may not always be sufficient to assess patellar alignment, 40 patients (43 knees) were evaluated by the newly developed technique of kinematic magnetic resonance imaging of the pat

Extensor carpi ulnaris and flexor carpi ulnaris tenodesis of the unstable distal ulna.

A procedure for stabilizing the distal ulna using the extensor carpi ulnaris and flexor carpi ulnaris is described. Seven patients who had sustained posttraumatic distal ulnar dorsal instability and articular degeneration and one patient with instability caused by rheumatoid arthritis were operated on. All eight obtained stable ulnae with mean motion of 62 degrees of supination and 86 degrees of pronation (mean increase of 32 degrees of supination and 43 degrees of pronation). Follow-up averaged 28 months (range, 18 to 63 months). The tenodesis, using a weave of a distally-based slip of flexor

Classification of patellofemoral disorders.

Patellofemoral disorders represent a large portion of the average orthopedist's practice. Despite the improvements in patellofemoral radiographs and arthroscopic diagnostic techniques, these disorders are too frequently misunderstood and frustrating to treat. This report proposes a clinical classification for patellofemoral disorders that, it is hoped, will aid our understanding and improve our results of treatment. A major feature of this classification is the recognition that a developmental and familial abnormality, patellofemoral dysplasia, is the etiology for most patellofemoral disorders

Clinical and roentgenological studies on malalignment disorders of the patello-femoral joint. Part I: Classification of patello-femoral alignments using dynamic sky-line view arthrography with special consideration of the mechanism of the malalignment disorders.

Dynamic sky-line view arthrography at 30, 60, and 90 flexion degrees was carried out on 1,242 knees of 621 cases who claimed trouble with at least one knee. The patient's age range was from 15 to 35 years. Alignment of the patello-femoral joint was classified into four groups; Grade I: The patello-femoral joint is congruent in all the three views. Grade II: The view taken at 30 degrees shows the joint space open in the medial facet but closed in the view at 60 degrees. Grade III: The medial interspace is open at 60 degrees but closed at 90 degrees. Grade IV: The medial joint space is open in a

[Acute ligamentous injuries of the Chopart and Lisfranc joint line].

The foot is a complex structure with numerous articular surfaces. As there are many potential complications (severe edema, compartment syndrome of the intrinsic foot musculature, bone and soft tissue necrosis, joint abnormalities, limitation of movement, deformities, etc.), traumatic surgeons must have a detailed knowledge of the relevant functional anatomy and treatment procedures. If there are multiple injuries, in the context of management, early definitive treatment for the injury is to be stressed. One requirement is an exact clinical and radiological diagnosis. As a rule, closed reductio

Computerized tomography as a guide in the diagnosis of temporomandibular joint disease.

As described, CT is a noninvasive method for providing detailed views of both the soft and hard tissues of the TMJ region. Because internal derangements of the joint involve both of these structures, CT can provide an invaluable tool in the study, diagnosis, and treatment of various disorders of this nature. It is extremely important, however, to create a close working relationship with the radiologist who will be performing these studies. Only with adequate clinical study and information communicated to the radiologist can true benefits be obtained. As CT scans are available nationwide, this

[Static and dynamic anatomo-radiology of the wrist].

Carpal instability is a difficult problem and a static as well as a dynamic radioanatomical study of the wrist is indispensable for correct diagnosis. This study is based on the description of the variations of the aspects of the bones of the proximal carpal row. An analysis of the projection of each individual bone was made, and thus, the author was able to describe the radioanatomical features of the wrist in three positions: neutral, radial and ulnar deviation. Secondly, the author points out the modifications produced on a normal wrist by active and passive loading. On the posteroanterior

Factors influencing elbow instability.

Stability or instability of the elbow is contingent upon three distinct but interrelated conditions: radiocapitellar articular continuity, coronoid process integrity and medial collateral ligament competency through an intact anterior oblique ligament. The latter, the anterior oblique ligament of the medial collateral ligament both in anatomic experimentation and clinical cases appears to be the most critical factor in elbow stability. The reconstruction of the unstable elbow must focus on this medial supporting ligament for a predictable result.

The arthroscope in the diagnosis of disorders of the patellofemoral joint.

Despite our current knowledge and accumulated information, orthopedic surgeons often experience difficulties in the diagnosis of internal derangement of the knee. Too often, the admission diagnosis I.D.K. means "I don't know." Disorders of the patellofemoral joint contribute to this difficulty and may simulate torn menisci and loose bodies, as well as the instability which results from rotary displacement. Patients often have difficulty describing and locating their symptoms, and 2 or more pathologic lesions may be present in the same joint. Early lesions involving the articular cartilage are
