Efficacy and safety of a selective partial agonist for nociception/orphanin-FQ peptide (NOP) receptors in patients with insomnia disorder.

AIMS: Insomnia is a common sleep disorder, affecting up to 20% of the world population and adversely impacting productivity, health, and overall well-being. Although pharmacologic options exist to treat insomnia, the health-related quality of life for patients who are prescribed hypnotics is no higher than for those who are not, revealing a significant treatment gap. Sunobinop, an oral, selective, potent, partial agonist of the nociceptin receptor (NOP), was evaluated for efficacy and safety in the treatment of Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) insomn

Study protocol of a randomized controlled superiority trial of Huddinge Online Prolonged Exposure therapy (HOPE) for adults with posttraumatic stress disorder.

BACKGROUND: Cognitive behavioral therapy (CBT) is considered a first-line treatment for posttraumatic stress disorder (PTSD) but is rarely available in regular care. Action is clearly needed to increase treatment availability. A possible solution to improve access to treatment would be the use of remotely delivered digital intervention. Internet-delivered CBT may also carry several other advantages compared to traditional psychological treatments (e.g., less therapist time per patient, bridging geographical distances between therapist and patient, and provision of a standardized intervention).

Structured Personalized Oxygen and Supportive Therapies for Dyspnea in Oncology (SPOT-ON): A personalized randomized clinical trial protocol.

BACKGROUND: Low-flow supplemental oxygen (LFSO), high-flow nasal cannula (HFNC), and non-invasive ventilation (NIV) are often used for palliation of dyspnea in hospitalized patients. However, it is unclear how patients prefer to use these modalities and how they can be personalized for optimal dyspnea relief. The objective of this randomized clinical trial is to compare the effect of a personalized respiratory therapist (RT)-led intervention, termed SPOT-ON, and enhanced usual care on dyspnea intensity in both hypoxemic and non-hypoxemic hospitalized patients with cancer. METHODS: In this two

A Pilot Randomised Non-Inferiority Trial of Auto-Titrating Versus Fixed Continuous Positive Airway Pressure for Obesity Hypoventilation Syndrome With Severe Obstructive Sleep Apnoea.

BACKGROUND AND OBJECTIVE: Fixed CPAP (fCPAP) is the preferred first-line ventilatory therapy in patients with obesity hypoventilation syndrome with severe obstructive sleep apnoea (OHS + sOSA). Auto-titrating CPAP (APAP) is not recommended but has not been systematically evaluated in this population. Our pilot study aimed to provide feasibility data for a larger comparative effectiveness trial to determine whether APAP is non-inferior to fCPAP for improving hypercapnic respiratory failure. METHODS: Participants with OHS + sOSA were randomised to either APAP or fCPAP for 3 months. The primary

The role of neuromodulation in heart failure with preserved ejection fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a clinical syndrome with limited treatment options to improve long-term outcomes such as quality of life, exercise capacity, and mortality. Neuromodulation-based therapies have emerged as potential interventions to address autonomic dysregulation in HFpEF. This review discusses the long-term efficacy and safety of four key neuromodulation therapies: Renal Denervation (RDN), Baroreceptor Activation Therapy (BAT), Vagus Nerve Stimulation (VNS), and Greater Splanchnic Nerve (GSN) Ablation. Each therapy shows promise, but variab

Lymphovenous Anastomosis and Vascularized Lymph Node Transfer Reduce Long-term Cellulitis Events in Patients With Secondary Lymphedema: A Systematic Review and Meta-analysis.

BACKGROUND: Secondary lymphedema frequently occurs following surgery, malignancy, trauma, or radiation and is characterized by limb swelling, discomfort, and recurrent infections. Two advanced supermicrosurgical techniques-lymphovenous anastomosis (LVA) and vascularized lymph node transfer (VLNT)-have recently gained popularity as treatment options. However, long-term outcome data for these procedures remain limited. This systematic review and meta-analysis aim to evaluate the long-term efficacy of LVA and VLNT in patients with secondary lymphedema followed for more than 2 years postoperativel

Presence of Leptospira spp. in a Mosaic of Wetlands Used for Livestock Raising under Differing Hydroclimatic Conditions.

Knowledge about the life cycle and survival mechanisms of leptospires in the environment is scarce, particularly regarding the environmental factors associated with their presence in ecosystems subject to livestock farming, where precipitation, seasonal floods, and river overflows could act as facilitators of leptospire dispersion. This study aimed to identify and study the presence of Leptospira spp. in the Lower Delta of the Paraná River and describe the physical, chemical, and hydrometeorological conditions associated with their presence in wetland ecosystems impaired by livestock raising i

Salivary Biomarkers as Predictors of Obesity and Intermediate Hyperglycemia in Adolescents.

INTRODUCTION: Childhood obesity presents a major risk for metabolic diseases in adulthood. Noninvasive methods are needed for predicting the course of obesity in children and its complications. Using blood for longitudinal analyses of biomarkers to predict disease in children is not a convenient method. Saliva presents a noninvasive platform to detect inflammatory changes in biomarkers as possible predictive measures of future pathological events. OBJECTIVES: The aim of this study was to evaluate the relationship between specific salivary biomarkers, obesity, and intermediate hyperglycemia in
