Perinatal Mortality among All Deliveries in a Tertiary Care Center: A Descriptive Cross-sectional Study.

INTRODUCTION: Perinatal mortality comprises the number of stillbirths and death of newborns within seven days of life which is the main contributor to infant and maternal mortality. The aim of this study was to find out the prevalence of perinatal mortality among all the deliveries in a tertiary care center of a remote part of Nepal. METHODS: This was a descriptive cross-sectional study conducted in a tertiary care center located in Jumla among 3798 deliveries (childbirth) from August 2014 to April 2020. Ethical approval was taken from the institutional review committee (2076/2077/05) of the

Bacteriological research and 'puerperal' fever: female health and childbirth in late colonial India.

This article explores female healthcare at the crossroads of bacteriology and obstetric research. Puerperal fever or childbed fever manifested as an epidemic since the nineteenth century, and in both Europe and America, it charted a distinct course for bacteriological research. With the identification of bacteriological causes, new sets of public health regimes were instituted in both regions. The experience of the colonies, however, differed. This paper focusses on how colonial discourse on obstetric nursing, midwifery, clinical hygiene, and maternal healthcare can be positioned in this globa

Acute kidney injury in pregnancy-current status.

Pregnancy-related acute kidney injury (PR-AKI) causes significant maternal and fetal morbidity and mortality. Management of PR-AKI warrants a thorough understanding of the physiologic adaptations in the kidney and the urinary tract. Categorization of etiologies of PR-AKI is similar to that of acute kidney injury (AKI) in the nonpregnant population. The causes differ between developed and developing countries, with thrombotic microangiopathies (TMAs) being common in the former and septic abortion and puerperal sepsis in the latter. The incidence of PR-AKI is reported to be on a decline, but the

[The perinatal mortality in a general hospital].

BACKGROUND: the perinatal mortality (PM) represents a partial expression of a developing country and its health programs. The aim was to determine the rate of PM in a second level hospital that lacks the support of a perinatal care unit, and to help with some epidemiological elements to plan strategies to give a better care in the perinatal stage. METHODS: we collected all records of deaths within the parameters of perinatal death. DATA INCLUDED: maternal age, prenatal history, cause of the perinatal death and birth control information. RESULTS: out of 117 deaths, 59.8 % were in the late fet

Getting to the heart of the matter: epidemiology of cyanotic heart defects.

Congenital heart defects (CHDs) are the most common type of birth defect, making significant contributions to infant morbidity and mortality, but not all CHDs contribute equally to such outcomes. Although cyanotic CHDs constitute some of the most serious CHDs, its epidemiology is poorly understood. We present a comprehensive systematic review of the literature on the epidemiology of cyanotic CHD, with emphasis on the most current knowledge on identified risk/etiologic factors. Literature for this review was identified by searching the PubMed database from the National Center for Biotechnology

Genes regulating embryonic and fetal survival.

Embryonic mortality in both farm animals and humans occurs most frequently during the first few weeks after conception. It can be attributed to abnormalities in the earliest developmental processes during embryogenesis that include implantation, maternal recognition of pregnancy, and formation of the placenta and cardiovascular system. The molecular mechanisms that are essential for all of these early processes are being elucidated at a rapid pace using transgenic and gene knockout approaches in mice. Two important general conclusions have emerged from this work. First, placental defects can o

Fetoprotectivity of the flavanolignan compound siliphos against ethanol-induced toxicity.

Of the three flavanolignans that are found in silymarin (Silybum marianum [L.] Gaertn.), silybin is thought to be the primary therapeutic constituent. To test the capacity of silybin to protect the rat fetus from toxic effects of maternally ingested EtOH we did the following: Adult female rats were assigned to one of four groups; EtOH, EtOH/silybin, pair-fed control, and chow fed control. Silybin was orally administered as Siliphos(R), which is one part silybin to two parts phosphatidylcholine. All groups except the chow-fed control were maintained on a liquid diet throughout pregnancy. On day

Embryotoxicity and genotoxicity of nickel.

Prenatal effects of nickel result from direct insults to the mammalian embryo as well as from indirect ones through maternal damage. Nickel may upset the hormonal balance of the mother and can impair the development of the preimplantation embryo. The metal can cross the feto-maternal barrier and enter the fetus. In addition to an increase in prenatal and neonatal mortality, nickel can produce different types of malformations in the surviving embryos but its teratogenic action seems to be delayed, probably as a result of retarded transfer via the placenta. No definite conclusions can be reached

Catching Up on Mortality, Falling Behind on Disability: A Dynamic Decomposition of the Health-Adjusted Life Expectancy Gap between Higher-Income and Lower-Income Countries.

Background: Global health inequalities are conventionally assessed through the mortality gap, yet the joint contribution of mortality and disability to health-adjusted life expectancy (HALE) gap and their dynamic evolution remain poorly understood. We aim to decompose the HALE gap between higher-income and lower-income countries into mortality and disability drivers, separate long-term trends from transient fluctuations, and reveal heterogeneity within the low-income group. Methods: Using data from the Global Burden of Disease Study for 199 countries (122 higher-income and 77 lower-income) fro

A copula-based geo-additive approach to childhood health outcomes in five West African nations.

BACKGROUND: Anemia and malnutrition are major contributors to child morbidity and mortality in Sub-Saharan Africa, particularly in West Africa where their prevalence remains high due to multifaceted socio-economic and environmental factors. Malnutrition, as defined by WHO, includes both undernutrition and overnutrition and is linked to increased risk of non-communicable diseases. Anemia, characterized by low hemoglobin levels, impairs oxygen transport and is associated with developmental delays and increased mortality in children. Given their interrelated nature, a joint modeling approach is e

PRESHA - Preventing severe hypertensive adverse events in pregnancy and childbirth: a study protocol for establishing a prospective cohort study and biobank in urban Tanzania.

OBJECTIVES: Hypertensive disorders of pregnancy are a leading cause of maternal and perinatal mortality in low- and middle income settings. Yet, to date few large-scale prospective clinical studies in this field are conducted in sub-Saharan Africa. This paper describes the study protocol of a prospective cohort study, the PRESHA (PREventing Severe Hypertensive Adverse Events) cohort in Mwanza, Tanzania. The PRESHA study aims to reduce maternal and neonatal mortality and morbidity related to preeclampsia (PE) through improved prediction, prevention and clinical management. More specifically, wi

Using RE-AIM to inform the adaptation of a clinical decision support application for hypertension disorders of pregnancy: a qualitative study of provider perspectives.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are significant contributors to maternal morbidity and mortality in the United States, which has one of the highest maternal mortality rates among high-income nations. Home blood pressure monitoring (HBPM) offers promise for improving care, but its integration into workflows is poorly understood. This study explores clinicians' perspectives and identifies implementation challenges for an HDP-specific app. METHODS: We used a snowball approach to invite perinatal clinicians at a large academic health center to participate in qualitative inte

The magnitude and associated factors of immediate postpartum anemia among women who gave birth in Ethiopia: systematic review and meta-analysis, 2023.

BACKGROUND: The immediate postpartum period is a very crucial phase for both the life of the mother and her newborn baby. Anemia is the most indirect leading cause of maternal mortality. However, anemia in the immediate postpartum period is a neglected public health problem in Ethiopia. Therefore, this systematic review and meta-analysis aimed to estimate the pooled magnitude of immediate postpartum anemia and the pooled effect size of associated factors in Ethiopia. METHODS: Searching of published studies done through PubMed, Medline, Cochrane, African index Medicus, List of Reference Index,

Essential components of postnatal care - a systematic literature review and development of signal functions to guide monitoring and evaluation.

BACKGROUND: Postnatal Care (PNC) is one of the healthcare-packages in the continuum of care for mothers and children that needs to be in place to reduce global maternal and perinatal mortality and morbidity. We sought to identify the essential components of PNC and develop signal functions to reflect these which can be used for the monitoring and evaluation of availability and quality of PNC. METHODS: Systematic review of the literature using MESH headings for databases (Cinahl, Cochrane, Global Health, Medline, PubMed, and Web of Science). Duration of birth interval and its predictors among reproductive-age women in Ethiopia: Gompertz gamma shared frailty modeling.

BACKGROUND: The World Health Organization recommended a minimum of 33 months between consecutive live births to reduce the incidence of adverse pregnancy outcomes. Poorly spaced pregnancies are associated with poor maternal and child health outcomes such as low birth weight, stillbirth, uterine rupture, neonatal mortality, maternal mortality, child malnutrition, and maternal hemorrhage. However, there was limited evidence on the duration of birth interval and its predictors among reproductive-age women in Ethiopia. Therefore, this study aimed to investigate the duration of birth interval and i

Setting priorities in child health research in India for 2016-2025: a CHNRI exercise undertaken by the Indian Council for Medical Research and INCLEN Trust.

BACKGROUND: Millennium Development Goal 4 (MDGs) mobilised countries to reduce child mortality by two thirds the 1990 rate in 2015. While India did not reach MDG 4, it considerably reduced child mortality in the MDG-era. Efficient and targeted interventions and adequate monitoring are necessary to further progress in improvements to child health. Looking forward to the Sustainable Development Goal (SDG)-era, the Indian Council of Medical Research and The INCLEN Trust International conducted a national research priority setting exercise for maternal, child, newborn health, and maternal and chil

Factors affecting the outcome of pregnancy with rheumatic heart disease: an experience from low-middle income country.

Studies on pregnancy with rheumatic heart disease (RHD), still common in the developing world, are relatively old and small. This retrospective study was conducted to study the outcome of pregnancy in women with RHD and factors associated with poor outcome. We studied 353 pregnancies in 273 women. In 35% of the patients, the diagnosis was first made during index pregnancy. Women with severe MS had lesser gestational age at delivery and birth weight than those with mild-to-moderate MS. Women with NYHA III-IV status delivered at lesser gestational age had lesser birth weight and had higher perin
