Interpretable prediction on treatment response of piperacillin-tazobactam for lower respiratory tract infections using machine learning.

Empiric piperacillin/tazobactam (PIPT) therapy is commonly used in high-risk patients with severe community-acquired pneumonia. However, its overuse may lead to antibiotic resistance and unwanted side effects. To this end, we developed and validated a machine learning (ML) model to assess the effectiveness of PIPT in the treatment of septic lower respiratory tract infections and to explore relevant influencing factors. The study was based on data from hospitalized patients treated with PIPT, and a dataset of bacterial lower respiratory tract infections was constructed by retrospective analysis

Development of Dual 'RT-LAMP-LFA' Rapid Detection Technology With Gold Magnetic Nanoparticles for Influenza Virus.

Seasonal and persistent outbreaks of influenza viruses represent a significant challenge to global public health. Rapid, convenient and accurate diagnosis methods of influenza viruses are crucial for timely treatment to mitigate morbidity and mortality during both seasonal epidemics and pandemics. However, current diagnostic tools often face limitations in speed, accuracy or complexity of result interpretation; there is a great need for more efficient detection technology for influenza virus, especially for use in resource-limited settings or during large-scale outbreaks. This study developed

Susceptibility and shedding in Mx1+ and Mx1- female mice experimentally infected with dairy cattle A(H5N1) influenza viruses.

BACKGROUND: Clade 2.3.4.4b highly pathogenic avian influenza A(H5N1) (HPAI H5N1) viruses have spread prolifically in dairy cattle in the US, resulting in dozens of human infections, some without well-established links to animal contacts. Many wild mammals have also been affected, including peridomestic house mice. METHODS: Here, we evaluated susceptibility, tissue tropism, and shedding in female PWK/PhJ and BALB/cJ mice, two laboratory strains derived from house mice that differ in expression of the antiviral restriction factor Mx1. PWK/PhJ mice, which were selected for their natural expressi

Combining 24-Hour Continuous Monitoring of Time-Locked Heart Rate, Physical Activity and Gait in Older Adults: Preliminary Findings.

Hemodynamic homeostasis is essential for adapting the heart rate (HR) to postural and physiological changes during daily activities. Traditional HR monitoring, such as 24 hour (h) Holter monitoring, provides important information on homeostasis during daily living. However, this approach lacks concurrent activity recording, limiting insights into hemodynamic adaptation and our ability to interpret changes in HR. To address this, we utilized a novel wearable sensor system (ANNE@Sibel) to capture time-locked HR and daily activity (i.e., lying, sitting, standing, walking) data in 105 community-dw

Serum microRNAs as peripheral markers of primary aldosteronism.

BACKGROUND: Primary aldosteronism (PA) is the principal cause of secondary hypertension; it leads to significantly elevated cardiovascular morbidity and mortality, but only a fraction of its cases ever get detected, partially due to diagnostic procedures that are difficult to perform and to interpret. More straightforward diagnostic methods are needed. Lateralized, or unilateral PA (uPA), is best treated by surgery. Bilateral PA (bPA) is treated medically. AIM: The aim of our study was to explore microRNA (miRNA) in peripheral blood as markers of PA, uPA and bPA. METHODS: In groups of subjec

Association of glycemic variability with short- and long-term mortality in critically ill patients with trauma.

The relationship between glycemic variability (GV) and mortality in critically ill trauma patients remains unclear. We evaluated whether GV, quantified by the coefficient of variation (CV) of glucose during the ICU stay, is associated with short- and long-term mortality in this population. Using the Medical Information Mart for Intensive Care IV (MIMIC-IV) database, we identified 4,009 adult trauma patients admitted to the intensive care unit and assessed the association between CV and 30-day and 1-year mortality with Cox proportional hazards models, Kaplan-Meier analysis, restricted cubic spl

Scapulothoracic Disorders: Anatomy, Kinematics, Clinical Assessment, and Multimodality Imaging.

Shoulder and neck concerns are prevalent musculoskeletal issues prompting medical attention, often stemming from scapulothoracic disorders that can serve as both the cause and consequence of other shoulder abnormalities. Scapular dyskinesis, fractures, benign and malignant masses, and neuropathies are frequently overlooked yet can substantially affect shoulder pain and function, particularly in athletes. Scapulothoracic disorders may lead to and/or be worsened by common shoulder abnormalities including those of the rotator cuff, neighboring bursae, and the glenohumeral articulation and labrum.
