[Skin and skin tissue infections: main clinical patterns/pictures].

Skin and soft tissue infections represent a heterogeneous group of clinical entities that require to be accurately identified for an appropriate and immediate management. Clinicians are challenged by the need to rapidly select those patients requiring hospitalization and medical therapy only and those to be immediately submitted to surgery. Erysipelas and several forms of cellulitis, involving the superficial structures of epidermis and dermis, are medical conditions; some cutaneous abscess may require surgical drainage, and all the necrotizing infections, involving the subcutaneous tissue (ne

Intracardiac thrombus in Behçet's disease: two case reports.

UNLABELLED: Intracardiac thrombus in Behçet's disease is an extremely rare manifestation. We report two such cases. A 20-year-old man presented with dyspnoea, cough and haemoptysis. Right heart thrombus associated with pulmonary artery aneurysm and thromboembolism was identified by helical CT and transoesophageal echocardiography. The second case was a 29-year-old male admitted for fever and chest pain. A diagnosis of right atrial thrombosis associated with pulmonary embolism and hyperhomocysteinemia was made. Due to the absence of haemodynamic compromise, medical management consisting of immu

Clinical differentiation of parkinsonian syndromes: prognostic and therapeutic relevance.

Parkinson disease is the most common cause of parkinsonism, but other causes should always be excluded because they have a different prognosis, respond differently to medical treatment, and should not be managed by surgical means. However, diagnosis, even by experts, is challenging; one autopsy series showed an error rate of 24%. Distinction between various diagnostic possibilities depends on the history and examination findings. The use of certain medications, the rapid rate of disease progression, early onset of falling, the presence of certain dysautonomic symptoms, cognitive or behavioral

Diving and hyperbaric ophthalmology.

Exposure of the human body to ambient pressures greater than that at sea level may result in various disorders, some of which have ocular manifestations. Additionally, some eye disorders and postoperative states may be adversely affected by the underwater environment or other hyperbaric exposures. The prevalence of recreational, military, and commercial diving, as well as the medical use of hyperbaric oxygen therapy, requires that ophthalmologists be familiar with the effects of the hyperbaric environment on the normal and diseased eye. The ophthalmology and diving medical literatures were sur

Agoraphobia. Is fear the basis of symptoms?

All human beings are biologic, psychological, and sociologic creatures. The treatment program for agoraphobia at the University of Kansas Medical Center School of Medicine, Kansas City, is designed to deal effectively with problems in these three areas. The biologic area is dealt with by medication; tricyclic antidepressants are used to block panic attacks, and benzodiazepines are given, as needed, for anticipatory anxiety. Group therapy is particularly effective in the areas of individual psychopathology and social anxiety. We have not found agoraphobia difficult to treat and have demonstrate

Management of the mildly abnormal Pap smear: a conservative approach.

Follow-up and management of the mildly abnormal Pap smear has been the subject of controversy in the medical literature. At Fitzsimons Army Medical Center, patients with initial Pap smears reported as "inflammatory atypia" or mild dysplasia were treated with specific therapy for vaginitis and/or cervicitis. Follow-up smears were performed 6 to 8 weeks later. Of the above groups, 80 and 58%, respectively, were negative on repeat smear. A third group of patients with Pap smears reported as consistent with human papillomavirus (HPV) were not treated but had repeat smears performed at 6 to 8 weeks

Evidence-informed guidance for the clinical use of oral semaglutide in obesity management.

Oral semaglutide, the first oral glucagon-like peptide-1 (GLP-1) receptor agonist therapy approved for the treatment of type 2 diabetes, is now approved for obesity management and cardiovascular risk reduction in adults, demonstrating weight loss comparable to that of subcutaneous GLP-1 therapies, alongside improvements in cardiometabolic risk factors. The availability of oral semaglutide for the treatment of obesity provides healthcare professionals with additional opportunities to individualize therapy based on patient preferences, lifestyle, and clinical circumstances. However, the oral sem

A Korean Clinical Practice Framework for Thyroid Eye Disease: Integrating International Recommendations with Survey-Informed Consensus.

Thyroid eye disease (TED), the most common extrathyroidal manifestation of Graves' disease, requires individualized management that integrates medical, radiotherapeutic, and surgical interventions. In this article, we present a society-endorsed, survey-informed Korean clinical practice framework, including consensus elements, for TED management. This framework integrates the 2022 American Thyroid Association/European Thyroid Association consensus, the 2021 European Group on Graves' Orbitopathy (EUGOGO) guideline, and contemporary Korean specialist practice patterns. A 2025 nationwide survey of

Clinical Features and Outcomes of Spontaneous Tumor Lysis in Testicular Germ Cell Tumors: A Case Series From a Cancer Center in Lahore, Pakistan.

Background Testicular germ cell tumors (TGCTs) are the most common solid malignancies in young adult men and typically have excellent outcomes with modern therapy. Tumor lysis syndrome (TLS) is a rare but potentially fatal complication in TGCTs, particularly when it occurs spontaneously in the absence of cytotoxic treatment. Data describing spontaneous TLS in this setting remain limited. Methods We report a case series of six patients who presented with spontaneous TLS secondary to TGCTs. Clinical features, imaging findings, laboratory results, management strategies, and outcomes were reviewed

Therapeutic Approaches for Toxic Optic Neuropathies: Insights from Methanol-Induced Optic Neuropathy and NAION Treatments.

Background/Objectives: Toxic optic neuropathy (TON) represents a spectrum of optic nerve damage caused by exposure to toxins, including drugs, alcohol, and industrial chemicals. It is characterized by progressive vision loss, dyschromatopsia, and optic nerve pallor and poses a clinical challenge in diagnosis and management due to overlapping features with other optic neuropathies. Non-arteritic anterior ischemic optic neuropathy (NAION), although distinct, shares common pathophysiological mechanisms such as oxidative stress and mitochondrial dysfunction. This review aims to evaluate therapeuti

Palliative Management of Inoperable Malignant Bowel Obstruction: Prospective, Open Label, Phase 2 Study at an NCI Comprehensive Cancer Center.

CONTEXT: Malignant bowel obstruction (MBO) is a common complication of intra-abdominal cancer, frequently seen in advanced gastrointestinal and gynecologic cancer. Management of MBO can be challenging, particularly if the patient is not a surgical candidate. No consensus exists on how best to manage these patients medically. Retrospective studies suggest that the combination of dexamethasone, octreotide and metoclopramide may lead to relief of obstruction and improvement in symptoms associated with the obstruction. OBJECTIVES: This study seeks to prospectively evaluate the combination of drug

Management of invasive ductal carcinoma in hypermobile Ehlers-Danlos syndrome: a case report.

Literature is sparse regarding the management and long-term outcomes of breast cancer in patients with Ehlers-Danlos syndrome (EDS). Of the EDS subtypes, hypermobile Ehlers-Danlos Syndrome (hEDS) is associated with cardiovascular dysautonomia which manifests as spontaneous episodes of tachycardia and hypotension. Given this clinical autonomic system impact, hEDS is known to have significant intraoperative risk and postoperative complications. However, outcomes of hEDS patients have not been specifically studied in the field of breast cancer surgery. Here we present a case of a 62-year-old fema

Serum trough levels of infliximab are not associated with peripheral arthralgia activity in patients with inflammatory bowel disease.

BACKGROUND: Infliximab is an efficacious therapy for inflammatory bowel disease and may play a role in management of some extraintestinal manifestations. While higher trough levels of infliximab are associated with higher rates of disease remission, the association between trough levels of infliximab and arthralgia activity characterised as an extraintestinal manifestation has yet to be defined. OBJECTIVE: We aimed to assess the association between serum trough levels of infliximab and peripheral arthralgia activity in patients with inflammatory bowel disease. DESIGN: In this cross-sectional

Bell's palsy: a summary of current evidence and referral algorithm.

Spontaneous idiopathic facial nerve (Bell's) palsy leaves residual hemifacial weakness in 29% which is severe and disfiguring in over half of these cases. Acute medical management remains the best way to improve outcomes. Reconstructive surgery can improve long term disfigurement. However, acute and surgical options are time-dependent. As family practitioners see, on average, one case every 2 years, a summary of this condition based on common clinical questions may improve acute management and guide referral for those who need specialist input. We formulated a series of clinical questions like

The management of intractable pain with adjuvant pulsed electromagnetic field therapy.

This case describes a 51-year-old woman who reported experiencing severe, constant pain, diffusely located in the region of her right mandible neck (primarily involving the mandible, lower right molars, the neck, the upper back, and the shoulder) during the course of several years. Surgical interventions (root canal, spinal fusion) were performed to address potential sources of pain. Despite these interventions, the patient reported severe pain after both surgeries, which persisted beyond the acute postoperative period. Additional pharmacological interventions and physical therapy were also at

Cetuximab: adverse event profile and recommendations for toxicity management.

Cetuximab (Erbitux, IMC-C225, ImClone Systems Incorporated, New York, NY) is a monoclonal antibody targeted to the epidermal growth factor receptor. Expression of the epidermal growth factor receptor is associated with disease progression, poor survival, poor response to therapy, and the development of resistance to therapy in many solid tumors. Cetuximab blocks the binding of natural ligands to the epidermal growth factor receptor, thus inhibiting oncogenic processes associated with its activation. Infusion reactions, acneform skin rash, and nail disorder are the most clinically relevant adve

Therapeutic outcomes of CAL/BDP PAD-based cream in patients with mild-to-moderate psoriasis: insights from four case reports.

Topical medications are commonly used to manage mild-to-moderate psoriasis as adjunctive treatments in patients receiving phototherapy, systemic agents or biologic therapies. Amongst available topical options, the fixed-dose combination of calcipotriol (CAL) and betamethasone dipropionate (BDP) has demonstrated efficacy and tolerability in clinical trials, with most adverse events being mild and transient local reactions. CAL/BDP has recently been formulated as a cream using polyaphron dispersion (PAD) technology to improve its rheological behaviour and sensory characteristics. We describe the
