Hormonal Contraception and Female Sexuality: Position Statements from the European Society of Sexual Medicine (ESSM).

INTRODUCTION: Hormonal contraception is available worldwide in many different forms. Fear of side effects and health concerns are among the main reasons for not using contraceptives or discontinuing their use. Although the safety and efficacy of contraceptives have been extensively examined, little is known about their impact on female sexual function, and the evidence on the topic is controversial. AIM: To review the available evidence about the effects of hormonal contraceptives on female sexuality in order to provide a position statement and clinical practice recommendations on behalf of t

Risk Factors of Sudden Cardiac Death in the Young: Multiple-Year Community-Wide Assessment.

BACKGROUND: Prevention of sudden cardiac arrest (SCA) in the young remains a largely unsolved public health problem, and sports activity is an established trigger. Although the presence of standard cardiovascular risk factors in the young can link to future morbidity and mortality in adulthood, the potential contribution of these risk factors to SCA in the young has not been evaluated. METHODS: We prospectively ascertained subjects who experienced SCA between the ages of 5 and 34 years in the Portland, Oregon, metropolitan area (2002-2015, catchment population ≈1 million). We assessed the cir

Prevalence and predictors of help-seeking for women exposed to spousal violence in India - a cross-sectional study.

BACKGROUND: Spousal violence against women is prevalent in India (29%). Studies from various countries have shown that few women exposed to intimate partner violence or spousal violence seek help, especially in low-income countries. The objective of this study was to estimate the prevalence and predictors of help-seeking among women in India who have experienced various types of spousal violence. METHODS: Cross-sectional data on 19,125 married, separated, divorced or widowed women in India who had experienced physical or sexual violence at the hands of their husbands were obtained from the In

Predictors of sinonasal quality of life and nasal morbidity after fully endoscopic transsphenoidal surgery.

OBJECT: Despite the increasing application of endoscopic transsphenoidal surgery for pituitary lesions, the prognostic factors that are associated with sinonasal quality of life (QOL) and nasal morbidity are not well understood. The authors examine the predictors of sinonasal QOL and nasal morbidity in patients undergoing fully endoscopic transsphenoidal surgery. METHODS: An exploratory post hoc analysis was conducted of patients who underwent endoscopic pituitary surgery and were enrolled in a prospective multicenter QOL study. End points of the study included patient-reported sinonasal QOL

A multi-center, randomized, double blind placebo-controlled trial of estrogens to prevent Alzheimer's disease and loss of memory in women: design and baseline characteristics.

BACKGROUND: Observational studies and small clinical trials suggested that hormone replacement therapy (HRT) decreases risk of cognitive loss and Alzheimer's disease (AD) in postmenopausal women and may have value in primary prevention. PURPOSE: A clinical trial was designed to determine if HRT delays AD or memory loss. This report describes the rationale and original design of the trial and details extensive modifications that were required to respond to unanticipated findings that emerged from other studies during the course of the trial. METHODS: The trial was designed as a multi-center,

Acute pesticide morbidity and mortality: California.

The California Pesticide Illness Surveillance Program collects, investigates, abstracts, and records reports received from physicians. A minority of the reports are received through the county health officers who are notified by physicians under a state requirement for reporting pesticide-caused conditions. Most of the cases are identified by review of workers' compensation records. All the cases identified are investigated by the agricultural commissioners of the counties where exposure occurred. The investigation reports are reviewed and abstracted by staff of the Worker Health and Safety Br

Minor head trauma.

This article reviews the persisting difficulty and the importance of the diagnosis of minor head trauma. The diagnosis has been complicated by pervasive disagreement regarding diagnostic criteria. This is primarily a result of the fact that evidence for actual injury is hard to obtain in minor cases because most symptoms tend to be subjective and have high base rates in the normal, uninjured population. At the same time, the diagnostic decision has important implications for patients in terms of treatment, expectancy for future function and lifestyle, and compensation for injuries. Decision th

The epidemiology of urban pediatric neurological trauma: evaluation of, and implications for, injury prevention programs.

OBJECTIVE: To describe the incidence and causes of pediatric head, spinal cord, and peripheral nerve injuries in an urban setting and to assess the implications of these data for injury prevention programs. METHODS: Pediatric deaths and hospital admissions secondary to neurological trauma included in the Northern Manhattan Injury Surveillance System from 1983 to 1992 were linked to census counts to compute incidence rates. Rates before the implementation of a nonspecific injury prevention program were compared with rates after the implementation, and rates for the target population were compa

COVID-19 Impact on Health Behaviors and Survey Engagement in Older African Americans: Longitudinal Cohort Study.

BACKGROUND: The COVID-19 pandemic disrupted older adults' daily lives, particularly concerning social interaction, physical activity, and sleep quality. Older African Americans were disproportionately affected yet remain underrepresented in research documenting the impact of the COVID-19 pandemic. OBJECTIVE: This study investigated changes in self-reported health, survey engagement, physical activity, and sleep duration among older African American adults in the Minority Aging Research Study (MARS) before and after Illinois' March 21, 2020, COVID-19 stay-at-home order, using online surveys an

Is degree attainment associated with serious psychological distress among Black men aged 24 and older?

OBJECTIVES: This study examines the relationship between degree attainment and serious psychological distress (SPD) among Black men in the United States. Using the Social Determinants of Mental Health (SDoMH) framework, we investigated whether degree attainment serves as a protective factor against SPD and explored how individual factors (age, sexual orientation, state of health), socioeconomic factors (income and marital status), and substance use factors (marijuana use and alcohol use) modify this relationship. DESIGN: We analyzed data from the 2022 National Survey of Drug Use and Health (n

A national snapshot of family caregivers in the United States: characterizing demographics and health among caregivers over age 50.

INTRODUCTION: As modern households diversify, many family caregivers face multi-generational demands, such as simultaneously caring for grandchildren and aging parents. Despite being the backbone of the care system, caregivers navigate highly varied needs and family dynamics. It is thus crucial to understand who family caregivers are and how they fare in terms of health and wellbeing. This descriptive study provides a profile of U.S. intergenerational caregivers over age 50. METHODS: Using data from 2022 RAND Health and Retirement Study (HRS) and HRS core files, we focus on four primary careg

Clustering Analysis of Partial Tooth Loss Patterns and Their Association With Cardiovascular Health Using a Japanese Claims Database.

OBJECTIVE: Cardiovascular health (CVH), defined by the American Heart Association, aims to reduce cardiovascular disease (CVD) deaths and improve overall health. Poor oral health, including tooth loss, is increasingly linked to higher CVD risk. However, the effects of specific partial tooth loss patterns on CVH remain unclear. This study aimed to identify clinically relevant tooth loss patterns and evaluate their association with CVH in a large Japanese population. DESIGN: Retrospective cohort study. SETTING AND PARTICIPANTS: Health insurance claims data were obtained from the JMDC database

Pig Xenotransplantation in Beta Cell Replacement: Addressing Challenges and Harnessing Potential for Type 1 Diabetes Therapy.

BACKGROUND: Parasites are among the main factors that negatively impact the health and reproductive success of organisms. However, if parasites diminish a host's health and attractiveness to such an extent that finding a mate becomes almost impossible, the parasite would decrease its odds of reproducing and passing to the next generation. There is evidence that Toxoplasma gondii (T. gondii) manipulates phenotypic characteristics of its intermediate hosts to increase its spread. However, whether T. gondii manipulates phenotypic characteristics in humans remains poorly studied. Therefore, the pr

Impact of the societal response to COVID-19 on access to healthcare for non-COVID-19 health issues in slum communities of Bangladesh, Kenya, Nigeria and Pakistan: results of pre-COVID and COVID-19 lockdown stakeholder engagements.

INTRODUCTION: With COVID-19, there is urgency for policymakers to understand and respond to the health needs of slum communities. Lockdowns for pandemic control have health, social and economic consequences. We consider access to healthcare before and during COVID-19 with those working and living in slum communities. METHODS: In seven slums in Bangladesh, Kenya, Nigeria and Pakistan, we explored stakeholder perspectives and experiences of healthcare access for non-COVID-19 conditions in two periods: pre-COVID-19 and during COVID-19 lockdowns. RESULTS: Between March 2018 and May 2020, we enga

HBV vaccination and PMTCT as elimination tools in the presence of HIV: insights from a clinical cohort and dynamic model.

BACKGROUND: Sustainable Development Goals set a challenge for the elimination of hepatitis B virus (HBV) infection as a public health concern by the year 2030. Deployment of a robust prophylactic vaccine and enhanced interventions for prevention of mother to child transmission (PMTCT) are cornerstones of elimination strategy. However, in light of the estimated global burden of 290 million cases, enhanced efforts are required to underpin optimisation of public health strategy. Robust analysis of population epidemiology is particularly crucial for populations in Africa made vulnerable by HIV co-

Chromium in the environment: an evaluation of exposure of the UK general population and possible adverse health effects.

Chromium in the hexavalent form, Cr(VI), has long been recognized as a carcinogen and there is concern as to the effects of continuous low-level exposure to chromium both occupationally and environmentally. This review summarizes the available exposure data and known health effects and evaluates the potential risk to human health in the United Kingdom. Chromium emissions to the environment in the United Kingdom are predominantly derived from fuel combustion, waste incineration, and industrial processes. The less toxic trivalent form of chromium [Cr(III)] is dominant in most environmental compa
