Diagnostic and therapeutic challenges in extragonadal yolk sac tumor with hepatoid differentiation: A case report.

Yolk sac tumors (YSTs) are rare aggressive tumors, arising most commonly in the gonads and occurring mainly in young adults. We herein report a case of an extragonadal YST with hepatoid differentiation localized in the brain and lung. A 41-year old man presented to our hospital with a generalized seizure. A head computed tomography and magnetic resonance imaging examination revealed a large mass in the left occipital lobe with associated edema. Following complete resection, the histopathological examination revealed that the mass was a highly malignant epithelial tumor with a hepatoid pattern.

Hypoxia and perfusion in breast cancer: simultaneous assessment using PET/MR imaging.

OBJECTIVES: Hypoxia is associated with poor prognosis and treatment resistance in breast cancer. However, the temporally variant nature of hypoxia can complicate interpretation of imaging findings. We explored the relationship between hypoxia and vascular function in breast tumours through combined 18F-fluoromisonidazole (18 F-FMISO) PET/MRI, with simultaneous assessment circumventing the effect of temporal variation in hypoxia and perfusion. METHODS: Women with histologically confirmed, primary breast cancer underwent a simultaneous 18F-FMISO-PET/MR examination. Tumour hypoxia was assessed u

Integrated versus separate reading of F-18 FDG-PET/CT and MRI for abdominal malignancies - effect on staging outcomes and diagnostic confidence.

OBJECTIVE: Abdominal cancer patients increasingly undergo multimodality imaging. This study evaluates effects of integrated reading of PET/CT and abdominal MRI on staging outcomes and diagnostic confidence compared to "routine" separate reading. METHODS: In total, N = 201 patients who underwent abdominal MRI and whole-body F-18 FDG-PET/CT within 14 days were retrospectively analyzed. Original MRI and PET/CT reports were retrieved and reported findings translated into a 5-point confidence score (1 = definitely benign to 5 = definitely malignant) for 7 standardized regions (primary tumor/region

[State of the art and projects of the Italian Fabry Disease Cardiovascular Registry].

Anderson-Fabry disease is a panethnic, rare disease caused by α-galactosidase A deficiency, with subsequent systemic intracellular accumulation of glycosphingolipids. Confined as a nephrological disease for many decades, the widespread use of multimodality imaging techniques over the last 20 years (like cardiac magnetic resonance) has allowed to highlight the frequent and heterogeneous cardiovascular involvement, with important impact on therapeutic strategies and prognosis. However, many grey zones and knowledge gaps remain, both in diagnostic and management approaches. Recently, the Italian

Unexpected metachronous multiple primary cancers: pilonidal sinus‑related squamous cell carcinoma and incidentally detected gastric gastrointestinal stromal tumour.

This case report presents a 62‑year‑old male with chronic pilonidal disease that underwent malignant transformation into squamous cell carcinoma (SCC), followed 18 months later by incidental detection of a gastric gastrointestinal stromal tumour (GIST) during routine oncologic surveillance. Initial contrast‑enhanced CT revealed the gluteal SCC, which was surgically excised and treated with adjuvant radiotherapy. Subsequent CT scan indicates a well‑circumscribed mass in the submucosa of the stomach. MRI and endoscopic biopsy confirmed a submucosal gastric GIST, which was completely resected. No

Pearls & Oy-sters: A Challenging Optic Neuropathy-What Not to Miss in Optic Nerve Sheath Enhancement.

Optic neuropathies include a wide range of disorders from ischemic, toxic, demyelinating, or inflammatory processes with acute/subacute onset to more gradual compressive or genetic etiologies. Accurate clinical history and multimodality optic nerve imaging including MRI and optical coherence tomography have greatly improved the diagnosis of patients with optic neuropathies. We report a case of a woman with severe monocular visual acuity deficit. Optic nerve sheath enhancement seen on MRI led to a broad differential diagnosis including demyelinating causes, optic nerve sheath meningioma (ONSM),

Radiomics in Lung Cancer Imaging: A Narrative Review of Current Evidence.

BACKGROUND: Lung cancer remains the leading cause of cancer-related mortality worldwide, and early diagnosis and accurate disease stratification are still major clinical challenges. Radiomics has emerged as a quantitative imaging approach that extracts high-dimensional features from radiological imaging, with applications in diagnosis, prognosis, radio genomics, and assessment of treatment response. However, its clinical translation is still limited by methodological heterogeneity and a lack of standardization. AIM: This narrative review synthesizes evidence from systematic reviews and meta-a

Discrepancy-based diffusion models for lesion detection in brain MRI.

Diffusion probabilistic models (DPMs) have exhibited significant effectiveness in computer vision tasks, particularly in image generation. However, their notable performance heavily relies on labelled datasets, which limits their application in medical images due to the associated high-cost annotations. Current DPM-related methods for lesion detection in medical imaging, which can be categorized into two distinct approaches, primarily rely on image-level annotations. The first approach, based on anomaly detection, involves learning reference healthy brain representations and identifying anomal

Mitral Annular Disease at Cardiac MRI: What to Know and Look For.

Accurate evaluation of the mitral valve (MV) apparatus is essential for understanding the mechanisms of MV disease across various clinical scenarios. The mitral annulus (MA) is a complex and crucial structure that supports MV function; however, conventional imaging techniques have limitations in fully capturing the entirety of the MA. Moreover, recognizing annular changes might aid in identifying patients who may benefit from advanced cardiac imaging and interventions. Multimodality cardiovascular imaging plays a major role in the diagnosis, prognosis, and management of MV disease. Transthorac

Image-guided enzyme/prodrug cancer therapy.

PURPOSE: The success of enzyme/prodrug cancer therapy is limited by the uncertainty in the delivery of the enzyme in vivo. This study shows the use of noninvasive magnetic resonance (MR) and optical imaging to image the delivery of a prodrug enzyme. With this capability, prodrug administration can be timed so that the enzyme concentration is high in the tumor and low in systemic circulation and normal tissue, thereby minimizing systemic toxicity without compromising therapeutic efficiency. EXPERIMENTAL DESIGN: The delivery of a multimodal imaging reporter functionalized prodrug enzyme, cytosi

Near-Infrared Theranostics: Cutting-Edge Innovations in Cancer Diagnosis and Therapy.

INTRODUCTION: Theranostics of near-infrared (NIR) imaging, which originated from the merging of diagnostic imaging and targeted therapy, is one of the most remarkable achievements in the early diagnosis and treatment of malignancies. The NIR is divided into two optical windows: the first NIR I (700-900 nm) and the second NIR II (1000-1700 nm), which enable improved tissue penetration, reduced autofluorescence, and higher resolution compared with visible light. METHODS: A literature search was conducted across databases, including PubMed, ScienceDirect, and the Cochrane Library. Eligibility cr

Depicting Characteristic Staghorn Vessels in Solitary Fibrous Tumor of the Liver With Contrast-Enhanced Ultrasound and Ultrasound Localized Microscopy: A Case Report.

BACKGROUND Solitary fibrous tumor of the liver (SFTL) is a rare mesenchymal neoplasm. Most patients are asymptomatic, and lesions are discovered accidentally. Lacking specific imaging manifestations, SFTL is prone to misdiagnosis, making accurate preoperative diagnosis vital for clinical treatment. Thus, multimodal imaging is indispensable for preoperative evaluation. CASE REPORT We report the case of a 55-year-old woman with an asymptomatic liver mass identified during a routine health examination. Initial contrast-enhanced ultrasound (CEUS) revealed a hypervascular lesion with abundant perip
