We compared a universal fungal PCR assay with fluorescence microscopy for the diagnosis of Pneumocystis carinii pneumonia. 82 bronchoalveolar lavages (BALs) of 64 immunocompromised patients with atypical pneumonia and 50 BALs of 50 immunocompetent adults without lung disease were examined. 10 immunocompromised patients were clinically and/or histologically proven to suffer from PCP. For fluorescence microscopy, sensitivity and specificity in detecting P. carinii were 80.0% and 98.1%, for the PCR assay 100.0% and 96.2%, respectively. Cutaneous sporotrichosis is clinically divided into two main types: lymphocutaneous and fixed plaque type. Our report represents an unusual case with both types simultaneously. Fluconazole 150 mg once weekly was used as the treatment. The fixed type lesions responded very well and were healed after 4 months, but the lymphocutaneous lesions were not controlled even after 6 months of the treatment.

A total of 12 Paracoccidioides brasiliensis yeast form strains, including clinical and environmental isolates, were tested for their susceptibilities to amphotericin B, ketoconazole, fluconazole, and itraconazole. The tests were determined using a broth macrodilution procedure. In general, there was a remarkable homogeneity of results for all strains, and comparable MICs were found for environmental and clinical isolates.

Serum samples from 60 subjects with confirmed HIV-1 infection including 28 AIDS patients and 32 carriers were examined by immunodiffusion for precipitating antibodies to antigens of Histoplasma, Blastomyces and Candida. UNLABELLED: Funguria, indirect anti-Candida haemagglutination test (C-IHT) and Candida-specific immunoglobulins C-IgM, C-IgG and C-IgA were investigated under suspicion of systemic candidosis in critically ill patients. A total of 143 urine cultures were studied for Candida from 74 adults and a median count of log 3.0 CFU ml-1 was found. Most isolated Candida species were Candida albicans and Candida glabrata. We report the case of a patient suffering from vitiligo and discoid lupus erythematosus. The nails of the left hand were involved with subungual hyperkeratosis. He was treated by local and systemic corticosteroid and chloroquine but hyperkeratotic lesions progressed on the affected sites and were found to be caused by Trichophyton violaceum.

In the last 50 years the spectrum of agents of dermatophytoses changed. The change is remarkable especially in the case of zoophilic dermatophytes. Microsporum canis has displaced Trichophyton verrucosum. In the period from 1966 to 1970 2822 dermatophytoses were registered as occupational dermatoses in the former German Democratic Republic. A 38-year-old male with history of trauma in the left gluteal region 20 years ago presented with a dark red skin eruption at the traumatized area. It gradually grew to form an erythematous plaque with a well-defined border. Clinical findings and mycological cultures resulted in the diagnosis of chromoblastomycosis due to Fonsecaea pedrosoi. After initial administration of 5-fluorocytosine and local heat an almost complete cure was achieved with terbinafine combined with local heat therapy. A review is given on the chromoblastomycosis cases observed in the Kitasato region in Japan.

We conducted upper intestinoscopies in 124 intensive care patients, six of whom had oesophageal candidosis. Of these, two also had Candida plaque in the stomach. The patients at the intensive care unit (ICU) had a mean Apache-II score of 26.7; whereas the score was 29.5 in patients with Candida oesophagitis. The objective of this study is to define the frequency of tinea pedis in patients with tinea cruris. Sixty patients (46 males and 14 females) entered our study. Epidermophyton floccosum was the most frequently isolated fungus in tinea cruris. Four patients (6.4%) had concurrent tinea pedis and the only isolate from the foot was Trichophyton mentagrophytes. In three of them, the corresponding dermatophyte in the groin was E. floccosum.

Diabetes mellitus is associated with a higher incidence of certain infections, including fungal infections like rhinocerebral zygomycosis (RCZ) and cutaneous candidosis. Pulmonary zygomycosis rarely occurs without pre-existing immunocompromised disease. A 72-year-old male was found to have a nodular shadow (3 cm x 4 cm) in the right S8 and S9 on a chest X-ray. Right lower lobectomy was performed and histological examination of the resected material demonstrated pulmonary zygomycosis. Hyphae stained positively not only with Grocott-Gomori methenamine silver staining, but also with an anti-Rhizopus oryzae polyclonal antibody.

Histoplasmosis has been little reported among HIV-infected children. We report a case of a 4-year old boy with AIDS who presented with disseminated histoplasmosis diagnosed by lung biopsy. The patient had a good clinical response to amphotericin B followed by itraconazole oral solution.

We compared the efficacy and tolerability of fluconazole (FCA) with amphotericin B/flucytosine (ABF) in neutropenic patients with haematological malignancies. Antifungal therapy started on day 4 when fever was unresponsive to antibiotics or on day 1 together with the antibiotics, if there was evidence of mycosis. We report two cases of invasive pulmonary aspergillosis due to Aspergillus flavus in one patient who with chronic nephritis and to A. fumigatus in another with malignant lymphoma. After receiving intravenous liposomal amphotericin B therapy for 31 and 35 days, respectively, the patients were cured and did not experience any severe adverse effects.

Tinea capitis is the most common type of dermatophytosis constituting 35.2% (173/491) of all cases of dermatophytosis seen at two out-patients dermatological clinics of the Basrah General Hospital and at two private dermatological clinics between October 1994 and December 1995. Males were more commonly affected than females represented by 60 and 40%, respectively. Out of 173 mycologically positive cases, 143 specimens were culture-positive, represented by five dermatophyte species; Trichophyton violaceum was the most frequent causative agent (38.5%), followed by Trichophyton verrucosum (28.7%)
