Transmural coronary venous O2 saturations in normal and isolated hearts.

Blood O2 saturations were measured by microscopic oximetry in the small coronary veins of wither open-chest or isolated and blood-perfused dog hearts. Subendocardial saturations (average 34%) were significantly lower than subepicardial (average 52%) in isolated hearts contracting isovolumically at systolic and coronary perfusion pressures of 100 mmHg. Saturations of botb regions fell and were not significantly different from each other (both averaged 16%) with partial coronary occlusion. When MVo2 was increased by calcium infusion, subendocardial saturations fell sharply to about 2% and were s

Cellular Senescence in Heart Failure with Preserved Ejection Fraction: Mechanisms and Therapeutic Opportunities.

Heart failure with preserved ejection fraction (HFpEF) contributes to nearly 50% of the global heart failure (HF) cases, and remains a major clinical challenge because of its heterogeneous pathophysiology, substantial residual morbidity, and limited mechanism-based treatment options. HFpEF is increasingly recognized as a heterogeneous multisystem syndrome closely associated with aging, cardiometabolic comorbidities, and chronic low-grade inflammation. Cellular senescence and the senescence-associated secretory phenotype (SASP) have emerged as potential mechanisms linking several of these syste

Unusual Acute Myocarditis Resulting in Severe Active Flare of Ankylosing Spondylitis with Pulmonary and Ocular Involvement: A Case Report.

BACKGROUND: Acute myocarditis is an inflammatory disorder of the myocardium that can lead to severe cardiac dysfunction, arising from a broad spectrum of infectious and noninfectious etiologies. Ankylosing spondylitis, which primarily affects the axial skeleton, can involve the heart due to its systemic inflammatory nature, occasionally resulting in severe complications such as myocarditis. CASE PRESENTATION: We report an unusual case of acute myocarditis in a 49-year-old male with ankylosing spondylitis presenting with chest pain and systemic inflammatory symptoms. Diagnostic workup using th

Cardiac-homing peptide-targeted colchicine-based drugstores with inhibiting inflammatory response for myocardial infarction improvement.

One of the primary challenges in addressing cardiac injury is the limited cardiac specificity of therapeutic drugs. Lipid nanoparticles (LNPs) are a relatively novel class of delivery vehicles used for administering RNA and various drugs. A major challenge in the treatment of cardiac injury remains the poor cardiac specificity of most therapeutic agents. Despite their promise as a relatively novel delivery platform for RNA and other payloads, LNPs do not adequately overcome this limitation. Nevertheless, these LNPs exhibited preferential accumulation in the liver rather than the heart. To addr

The vagus nerve in myocardial infarction.

Myocardial infarction can result from and leads to sympathetic activation, whereas vagal engagement provides a counter-regulatory influence that can limit ischaemic injury. This article reviews the organization of vagal sensory and efferent pathways, their transmitters and receptors, and their targets within the myocardium, coronary and systemic vasculature, and other organs including the intestine and spleen. Atrial mechanosensitive afferents mediate the Bainbridge reflex, resulting in tachycardia, whereas activation of ventricular mechano- and chemosensitive afferents trigger the Bezold-Jari

Historical account of reperfusion therapy and the development of cardioprotection beyond reperfusion.

The only way to salvage myocardium from infarction is timely reperfusion of the occluded coronary artery. Timely reperfusion reduces infarct size and mortality. The history of reperfusion therapy from the first clinical use of intravenous streptokinase in patients with acute myocardial infarction by Sherry et al. in the late 1950s, over seminal experimental dog studies by Ross Jr. et al., demonstrating infarct size reduction by timely reperfusion in the early 1970s, to a more systematic use of initially streptokinase and a later tissue plasminogen activator in clinical trials (GISSI, TIMI), th

Cardiomyocyte-specific deletion of STING improves cardiac function, glucose homeostasis, and wound healing in diabetic mice.

AIMS: The present study aimed to investigate the effects and underling mechanisms of cardiomyocyte-specific STING knockout on cardiac function and wound healing in diabetes. MATERIALS AND METHODS: In this study, type 2 diabetes was induced in cardiomyocyte-specific STING knockout mice using a combination of a high-fat diet and streptozotocin. Cardiac function and remodeling were assessed by echocardiography and histopathological analysis. Glucose homeostasis was evaluated through insulin sensitivity tests and intraperitoneal glucose tolerance tests. Wound healing was quantified by measuring t

Transthyretin amyloid cardiomyopathy: Evolving therapies, expanding hope.

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive and life-threatening condition characterized by the deposition of misfolded transthyretin (TTR) protein in the myocardium, leading to restrictive cardiomyopathy and heart failure. Given its high mortality rate and historically limited therapeutic options, there is a growing need for effective treatments. Current therapeutic strategies focus on reducing the production of TTR using RNA-targeted therapies and stabilizing the native TTR tetramer to prevent misfolding and aggregation. In recent years, emerging therapies-including monoc

Induction of cardiac fibulin-4 protects against pressure overload-induced cardiac hypertrophy and heart failure.

The prevailing view of fibulin-4 deficient mice is that the cardiac phenotype is the result of aortic and/or valvular disease. In the present study, we have tested whether the cardiac phenotype is, at least in part, the consequence of primary cardiac effects of fibulin-4. We have found fibulin-4 expression to be activated throughout the myocardium in wildtype (fibulin-4+/+) C57Bl/6J;129 Sv mice subjected to transverse aortic constriction (TAC). In contrast, haploinsufficient fibulin-4+/R mice exposed to severe TAC do not show this increase in myocardial fibulin-4 expression, but display altere

A Case Report of a 36-year-old Male Diagnosed with a Spontaneous Coronary Artery Dissection.

UNLABELLED: This case report discusses a 36-year-old male who presented to the emergency department with an atypical story for acute coronary syndrome (ACS). Initially, the patient was felt to have a non-diagnostic electrocardiogram (ECG). Once the laboratory test results were obtained and the initial high-sensitivity troponin was noted to be elevated, the initial ECG was again reviewed. The patient was felt this time to have an abnormal ECG, demonstrating borderline ST elevation in leads I, aVL, and V2-V5. The interventional cardiologist on call was contacted, and the cardiac catheterization

An Efficient Staggered Scheme for Solving the Poromechanics Problem of Quasi-Static Cardiac Perfusion.

The ventricles can be considered a type of poroelastic material, where the mass and pressure of the interstitial fluid, along with the displacement of the skeleton, are the three primary physical quantities of interest. Based on the free energy function of the poroelastic material, we propose a simplified model that requires only two fields to be directly solved, with another quantity obtained through post-processing. To solve this model, we first discretize the equations with the backward Euler scheme and finite element method, leading to a nonlinear system of equations, which can be solved u

Differential Methylation of the Genes PAX8 and GATA4 Alters Their Expression in the Heart during Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is the most common hereditary heart disease with a prevalence ranging from 1 : 500 to 1 : 200 individuals. The development and clinical presentation of HCM do not always conform to the traditional view of its monogenic inheritance pattern. One key to addressing this issue may lie in identifying epigenetic mechanisms regulating gene expression, particularly DNA methylation, involved in the pathogenesis of the disease and modifying its course. Using previously obtained whole-genome data, we identified four extended genomic regions with reduced methylation levels

Incidental Cardiac Metastasis in Breast Carcinoma.

Background and Clinical Significance: Metastatic breast cancer is a major global health burden, with common metastatic sites including the bones, lungs, liver, and brain. Cardiac metastasis is rare and often clinically silent, leading to underdiagnosis. Recognizing cardiac involvement, even when asymptomatic, is important for understanding the full extent of disease and ensuring optimal patient care. Case Presentation: We report the case of a woman with advanced breast carcinoma who showed no clinical or imaging evidence of cardiac involvement throughout the course of her illness. Following he

Frequent POCUS and auscultation for an earlier diagnosis of takotsubo syndrome and unraveling of its pathophysiology: The possible crucial role of LVOTO.

There is ample literature associating LVOTO with hypertension, AMI, LV hypertrophy, sigmoid septum, HCM, and TTS, particularly in midde aged/elderly/postmenopausal women, suggestive of a causal role for LVOTO in the pathophysiology of TTS. Although there is significant evidence that TTS is triggered by a sudden autonomic sympathetic nervous system surge and/or elevated blood-ridden catecholamines, the exact pathophysiologic trajectory leading to the clinical expression of the disease is still being debated. This review expounds on the possibility that LVOTO is a causal early component of this

Diagnostic Modalities in the Detection of Cardiac Amyloidosis.

Cardiac amyloidosis (CA) results mainly from the infiltration of the myocardium by either immunoglobulin light-chain fibrils (AL) or transthyretin fibrils (ATTR), causing restrictive cardiomyopathy and eventually death if untreated. AL derives from monoclonal immunoglobulin light chains produced by plasma cell clones in the bone marrow, while ATTR is the misfolded form of hepatically derived transthyretin (TTR) protein and can be hereditary (ATTRv) or wild-type (ATTRwt). Over the last decade, improvements in diagnostic imaging and better clinical awareness have unleashed a notable presence of

Intramyocardial calcification in apical hypertrophic cardiomyopathy assessed using multimodality imaging: a case series.

Apical hypertrophic cardiomyopathy (ApHCM) is an HCM variant, affecting frequently males in midlife. It is characterized by apical obliteration and persistent diastolic contraction, often resulting in microvascular ischaemia. We report five cases of ApHCM, with evidence of intramyocardial calcification on echocardiogram. On cardiac magnetic imaging (MRI), a hypointense component at early gadolinium enhancement (EGE) sequences, compatible with calcium, and a deep layer, with hyperintensity at late gadolinium enhancement (LGE) sequences, referable to fibrosis, suggest an endomyocardial fibrosis

Tissue Iron in Friedreich Ataxia.

Heart, dentate nucleus, and dorsal root ganglia (DRG) are targets of tissue damage in Friedreich ataxia (FA). This report summarizes the histology and histopathology of iron in the main tissues affected by FA. None of the affected anatomical sites reveals an elevation of total iron levels. In the myocardium, a small percentage of fibers shows iron-reactive granular inclusions. The accumulation of larger iron aggregates and fiber invasion cause necrosis and damage to the contractile apparatus. In the dentate nucleus, the principal FA-caused tissue injury is neuronal atrophy and grumose reaction

Inter- and Intracellular Signaling Pathways.

Cardiovascular diseases, both congenital and acquired, are the leading cause of death worldwide, associated with significant health consequences and economic burden. Due to major advances in surgical procedures, most patients with congenital heart disease (CHD) survive into adulthood but suffer from previously unrecognized long-term consequences, such as early-onset heart failure. Therefore, understanding the molecular mechanisms resulting in heart defects and the lifelong complications due to hemodynamic overload are of utmost importance. Congenital heart disease arises in the first trimester

A DenseNet-based Abnormal Ventricular Potentials Onset Delineation: A Feasibility Study.

Abnormal ventricular potentials (AVPs) are fractionated and complex electrograms (EGMs), typically associated with slow conduction areas in the myocardium. As such, in ventricular tachycardia (VT), their identification supports the localization of the myocardial areas sustaining the arrhythmia in the electroanatomic mapping. Their latency can be related to their spatial localization, suggesting the entrance or the exit of a reentry pathway. In this work, we explored a novel approach to delineate the onset of the pathological deflections in AVPs. For this preliminary study, an expert cardiologi

Intraoperative Takotsubo Syndrome.

Takotsubo cardiomyopathy is a rare condition that presents a diagnostic challenge due to its close resemblance to acute myocardial ischemia and other cardiac disorders. The excessive production of catecholamines triggers abnormal and severe changes in the myocardium, typically resulting in significant dyskinesia of the left ventricle's apex, reduced ejection fraction, hypotension, and pulmonary edema. Recent reports suggest that potential risk factors may include postmenopausal syndrome and intense, unexpected stress, whether related to life events or medical conditions. Complications such as
