[Bilateral Medial Medullary Infarction with Meningovascular Neurosyphilis].

Case report A 31-year-old previously healthy man who had sex with men presented with a two-day history of acute-onset dizziness, nausea, and vomiting. On admission, he was afebrile with generalized lymphadenopathy and a truncal and lower-limb rash. Neurological examination revealed no motor weakness or hyperreflexia, but he could not walk unassisted due to dizziness. Brain MRI diffusion-weighted imaging (DWI) showed characteristic V-shaped hyperintense lesions in the bilateral medial medulla (Fig. 1A) and at the pontomedullary junction (Fig. 1B). MR angiography (MRA) showed no stenosis or diss

Opioid-free anesthesia in enhanced recovery after surgery for gastrointestinal surgery: current status, challenges, and prospects.

The enhanced recovery after surgery (ERAS) protocol has been increasing implementation in gastrointestinal surgeries to optimize perioperative management, mitigate surgical stress responses, and accelerate patient recovery. Although opioid-based anesthesia effectively alleviates pain, it is associated with significant adverse effects, including postoperative nausea and vomiting (PONV), respiratory depression, and intestinal paralysis, which can impeder early recovery. Opioid-free anesthesia (OFA) is designed to alleviate these concerns. This article examines the pharmacological agents and regi

Acute Non-Hemorrhagic Adrenal Infarction in a Diabetic Patient: A Rare but Critical Consideration in Acute Abdominal Pain.

Acute non-hemorrhagic adrenal infarction (ANHAI), though rare, should be taken into account in the differential diagnosis of patients presenting with unexplained abdominal pain, especially those with underlying hypercoagulable conditions or risk factors for thrombosis. Timely recognition and diagnosis are paramount to prevent complications, including adrenal insufficiency. Imaging plays a crucial role in diagnosing ANHAI, with CT scans typically showing an enlarged, hypo-attenuated adrenal gland and the presence of the capsular sign offering valuable diagnostic clues. Here, we present a middle

A Tough Nut to Crack: Navigating a Rare Case of Bouveret Syndrome.

Bouveret syndrome, a rare complication of cholelithiasis (1%-3%), leads to high mortality (12%-30%). It occurs when a gallstone passes through a cholecystoduodenal fistula, causing gastric outlet obstruction. We report a 66-year-old woman with abdominal pain, nausea, and vomiting. Imaging showed acute cholecystitis, pneumobilia, cholecystoduodenal fistula, and a 5-cm gallstone obstructing the gastric outlet. Initial treatment with antibiotics and endoscopic lithotripsy relieved the obstruction but did not remove the entire stone. Surgery with open gastrotomy successfully extracted the stones,

The Influence of Region-of-interest Selection on Ejection Fraction Calculation in a Septate Gallbladder.

A 62-year-old woman with chronic nausea and epigastric pain underwent hepatobiliary scintigraphy. Gallbladder ejection fraction (GBEF) calculation suggested normal function. However, upon further review, a septate gallbladder was identified. This promoted a re-evaluation of the selected region of interest (ROI). The initial ROI included only the gallbladder fundus, leading to GBEF overestimation. Recalculation with a comprehensive ROI that encompassed both compartments yielded a lower GBEF, consistent with biliary dyskinesia or chronic cholecystitis. This case highlights the importance of ROI

Resolved posterior fossa neurocysticercosis mimicking a fourth ventricular tumour and cervical cyst, with a postoperative course complicated by very low-pressure hydrocephalus.

A female in her late 40s presented with progressively worsening headaches, nausea, vomiting and gait imbalance and was ultimately found to have a fourth ventricular mass and cervical cyst. Initially presumed to be a neoplastic process, histopathology revealed longstanding neurocysticercosis and reactive changes. The patient underwent suboccipital craniotomy, C1 laminectomy and external ventricular drain placement. Postoperative complications included persistent very low-pressure hydrocephalus (VLPH), venous sinus thrombosis and fluctuating mental status requiring prolonged active cerebrospinal

Dengue fever maculopathy: case report and brief review.

A 45-year-old woman with a 7-day history of fever and nausea sought assistance at the emergency department complaining of blurred vision in her right eye. She was recently diagnosed with dengue fever. Fundus showed a discrete well-circumscribed, round yellow-orange lesion localized in the foveal region, retinal hemorrhages, macular oedema, soft exudates, and adjacent perivasculitis in the affected eye. Spectral domain optical coherence tomography (OCT) was compatible with dengue maculopathy. She was closely followed, with complete improvement after 1 week. The increasing incidence of dengue ma

Axonal Guillain-Barré Syndrome After Leptospirosis Contracted in a Cave or While Packrafting-A Case Report.

A 64-y-old male went caving and packrafting in Chiapas, Mexico. On return, he developed fever, headaches, and nausea and was treated empirically for leptospirosis with ceftriaxone and doxycycline. Convalescent IgM titers returned positive for leptospirosis. Other work-up was negative. As the symptoms of leptospirosis abated, the patient developed an areflexic quadriparesis. Nerve conduction velocities confirmed acute motor axonal neuropathy consistent with Guillain-Barré syndrome. Serologic work-up disclosed anti-GD1a antibodies. He was treated with intravenous immune globulin for 5 d and then

Tirzepatide-Associated Colonic Ischemia.

The use of the glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) receptor agonist tirzepatide has markedly increased for the treatment of weight loss and management of diabetes mellitus. Gastrointestinal adverse effects of GLP-1/GIP agonist therapy, including nausea, vomiting, and constipation, are common. We report a case of colonic ischemia in a 62-year-old woman which developed in association with the use of tirzepatide for weight loss. This report highlights a potential risk relationship between GLP-1/GIP agonist therapy and colonic ischemia and identif

Molecular mechanisms of enteric neuropathies in high-fat diet feeding and diabetes.

BACKGROUND: Obesity and diabetes are associated with altered gastrointestinal function and with the development of abdominal pain, nausea, diarrhea, and constipation among other symptoms. The enteric nervous system (ENS) regulates gastrointestinal motility. Enteric neuropathies defined as damage or loss of enteric neurons can lead to motility disorders. PURPOSE: Here, we review the molecular mechanisms that drive enteric neurodegeneration in diabetes and obesity, including signaling pathways leading to neuronal cell death, oxidative stress, and microbiota alteration. We also highlight potenti

Drug effects on neuropeptides and their receptors: Big hopes but moderate success in the treatment of chronic pain.

Neuropeptides, including tachykinins, CGRP, and somatostatin, are localized in a peptidergic subgroup of nociceptive primary afferent neurons. Tachykinins and CGRP are pronociceptive, somatostatin is an antinociceptive mediator. Intensive drug research has been performed to develop tachykinin and CGRP antagonists, and somatostatin agonists as analgesics. CGRP receptor antagonists are efficacious and well-tolerated drugs in migraine. Monoclonal antibodies against CGRP or its receptor are used for the prophylactic treatment of migraine. Tachykinin NK1 receptor antagonists failed as analgesics bu

Severe Hemorrhagic Cholecystitis in the Absence of Common Predisposing Factors.

Hemorrhagic cholecystitis is a rare and potentially life-threatening complication of acute cholecystitis. It lacks specific diagnostic and management guidelines due to its rarity. We present the case of a 70-year-old male with no typical risk factors who developed hemorrhagic cholecystitis. The patient presented with acute right upper quadrant pain radiating to the back and right lower abdomen, accompanied by nausea and fatigue. A CT scan revealed acute hemorrhagic cholecystitis, prompting an emergent open subtotal fenestrated cholecystectomy. Pathology confirmed hemorrhagic cholecystitis with

Eosinophilic enteritis presenting as intestinal obstruction: case report and review of the literature.

Eosinophilic enteritis (EE) is characterized by intense eosinophilic infiltrate of the gastrointestinal tract. Clinical manifestations depend on the affected segment and intestinal layer. First-line treatment is systemic corticosteroids; surgery is reserved for complications. 84-year-old male patient with a history of right hemicolectomy and two episodes of intestinal obstruction presented to the ED with abdominal pain, distension, nausea, and vomiting. CBC showed leukocytosis and no eosinophilia. Contrast-enhanced CT revealed stenosis with thickening of the distal intestinal wall and partial

Capecitabine-induced posterior reversible encephalopathy syndrome (PRES) in a patient with gastric adenocarcinoma.

Posterior reversible encephalopathy syndrome (PRES) is characterised by encephalopathy, visual disturbances and seizures, accompanied by radiological parieto-occipital oedema. Immunosuppressive and immunomodulatory drugs are risk factors. While capecitabine-induced PRES cases are rare, this report details a young woman with advanced gastric adenocarcinoma on capecitabine. She exhibited symptoms of nausea, vomiting and abdominal pain before developing hypertension, drowsiness and a seizure. Brain MRI revealed parieto-occipital hyperintense areas indicative of PRES. Suspending capecitabine led t

Surgical management of a rare giant ovarian serous cystadenoma with distinctive clinical manifestations.

Giant ovarian cysts are categorized with sizes >10 cm. After reaching huge diameters, these rare tumors cause clinical symptoms such as nausea, vomiting or abdominal pain. We present a 29-year-old woman with a giant unique cystadenoma represented with unusual clinical features like low back pain and progressive constipation. Specific imaging methods demonstrated an adnexal lesion in the form of an enormous ovarian cyst; afterward, an open laparotomy was recommended to access the abdominal cavity. The critical role of timely diagnosis and accurate workups for giant ovarian cysts in increasing p

Cervicogenic visual dysfunction: an understanding of its pathomechanism.

Atypical symptoms of cervical spondylosis include headache, nausea, gastrointestinal discomfort, blurred vision, tinnitus, hypomnesia, and palpitations. Successful treatment of these atypical symptoms has been achieved after conservative non-invasive and surgical spinal treatments, although the role of these interventions in mitigating atypical symptoms of cervical spondylosis is unclear. Our study introduces and elaborates on the visual dysfunction caused by cervical spondylosis. Although there are reports in the literature that spinal manipulation and surgery can improve visual dysfunction,

Empirical intravenous aciclovir therapy in a suspected case of acute encephalitis.

Herpes simplex encephalitis is the most common cause of sporadic viral encephalitis worldwide but presents as a diagnostic challenge at many settings due to its non-specific symptoms, which can be easily mistaken for systemic infection or metabolic encephalopathy. It has diverse range of presentations from fever, altered sensorium, nausea, vomiting, meningismus to seizures, neurological deficits and coma in advanced stages. It is associated with significant morbidity and mortality if treatment is delayed or inadequate. We here discuss a case of Herpes simplex virus (HSV) encephalitis which rap

Emphysematous gastritis: Impressive radiological findings.

Emphysematous gastritis is a rare form of phlegmonous gastritis, characterized by the formation of air in the gastric wall by gas-forming microorganisms. Early diagnosis and treatment are important because of its fatal consequences (high morbidity and mortality). A 64-year-old woman presented to our emergency department with abdominal discomfort, nausea, and general weakness. An abdominal examination revealed severe epigastric tenderness, and laboratory findings revealed elevated inflammatory markers.
