Neuroendocrine neoplasms (NENs) are rare and heterogeneous malignancies requiring multidisciplinary management. Large language models (LLMs) are emerging as decision-support tools, but their role in therapeutic decision-making is largely unexplored. ARTEMIS was a pilot cross-sectional study comparing three configurations-a baseline GPT, a customised GPT with static domain knowledge (GPTs), and a retrieval-augmented GPT (RAG)-against a panel of nine Italian NEN experts using twenty simulated, non-surgical cases. The primary endpoint was non-inferiority for systemic therapy recommendations; seco

Philadelphia chromosome-negative myeloproliferative neoplasms (Ph-MPNs) are clonal disorders marked by high morbidity and mortality, driven by uncontrolled myeloid proliferation from hematopoietic stem/progenitor cells (HSCs) and associated with a significant risk of thrombosis. This study explored the relationship between JAK2V617F and protease-activated receptor 1 (PAR1) by examining PAR1 expression and activation across various hematopoietic stem/progenitor cell (HSPC) subgroups, assessing their contribution to the hypercoagulable state in Ph-MPNs. We investigated the effects of thrombin, a

Pancreatic cystic lesions (PCLs) are often benign; however, a subset carries malignant potential, making accurate diagnosis essential to avoid both missed malignancies and overtreatment. Conventional tools, such as cyst fluid cytology and biochemical analysis, have limited sensitivity, often leading to misdiagnosis. This review summarizes the role of endoscopic ultrasound-guided through-the-needle biopsy (EUS-TTNB) for PCL diagnosis, incorporates recent ESGE recommendations, and discusses future directions. EUS-TTNB enables direct histologic sampling of pancreatic cysts, improving tissue adequ

BACKGROUND AND AIMS: Plasma cell-free DNA (cfDNA) has been used to monitor gene mutations and diagnose tumors. Discriminating parathyroid carcinoma (PC) from parathyroid adenoma (PA) before surgery is difficult because of the overlap in clinical features between parathyroid neoplasms. We aimed to detect cfDNA mutations in plasma samples from PC and PA patients before surgery to predict the CDC73 status in tumor tissue and help in the differential diagnosis of parathyroid neoplasms.
MATERIALS AND METHODS: Eighteen PC patients and 13 PA patients were enrolled. Plasma cfDNA was detected using ne

CONTEXT: Nodular fasciitis (NF), myositis ossificans/fibro-osseous pseudotumor of the digits (MO/FP) and aneurysmal bone cyst (ABC) are grouped under the category of 'USP6-associated neoplasms' (UANs) due to their shared characteristic of an underlying USP6 rearrangement.
OBJECTIVE: Our aim in this study is to investigate the diversity of USP6 rearrangements in UANs.
DESIGN: We performed a clinicopathological and molecular investigation using targeted RNA sequencing on 124 cases of NF, 19 cases of MO/PF and 32 cases of ABC. Additionally, NF cases were scored for the presence of typical morph

The accurate distinction between primary and secondary mucinous ovarian cancers is a crucial tool for effective surgical and systematic treatment. Mucinous ovarian metastases of appendiceal origin are a special group of tumors because they appear even in half of female patients with primary appendiceal mucinous carcinomas and demonstrate pathological similarity to primary ovarian mucinous neoplasms. The current literature review focuses on the differences based on pre-operative symptoms, radiological findings, the spectrum of microscopic features, and the significance of the immunophenotype of

BACKGROUND: Neuroendocrine neoplasms (NENs) represent a relatively rare yet heterogeneous group of neoplasms arising from diverse anatomical origins. The inherent variability in clinical manifestations and gradations of biological aggressiveness pose substantial challenges in diagnostic and therapeutic management. This report presents a diagnostically complex case of postpartum non-functional NENs.
CASE PRESENTATION: A 39-years-old female with childhood history of acute pancreatitis presented with intermittent abdominal pain and considered pancreatitis during pregnancy. Initial laboratory eva

The origin of mucinous cystic neoplasms (MCNs) remains a major challenge in hepato-pancreato-biliary pathology. These cystic tumors are defined by their mucinous epithelium and ovarian-like stroma, with an estimated 10% risk of progression to invasive carcinoma. The origin of the ovarian-like stroma remains a subject of debate. In this study, we conducted immunohistochemical profiling, targeted DNA sequencing, and genome-wide DNA methylation analysis on a cohort of 15 pancreatic MCNs (MCN-P) and six hepatic MCNs (MCN-L). Using immunohistochemistry and targeted DNA sequencing, we unequivocally

PURPOSE: This study aimed to construct a nomogram based on novel ultrasound vascularity features to differentiate follicular thyroid carcinoma (FTC) from follicular benign neoplasms (FBN).
METHODS: A total of 153 patients were included. We evaluated various sonographic features of all nodules and analyzed the diagnostic efficiency of different vascularity patterns for FTC. Furthermore, independent predictors for FTC were screened using multivariable logistic regression. A nomogram was established to predict FTC, and various methods were used to evaluate the performance of the nomogram.

Therapy-related myeloid neoplasms (t-MN), which include acute myeloid leukemia (t-AML), myelodysplastic syndrome (t-MDS), and myelodysplastic/myeloproliferative neoplasms are secondary malignancies occurring as a late complication following chemotherapy or radiation therapy for an antecedent disorder. Allogeneic hematopoietic stem cell transplant (allo-HCT) is a potentially curative treatment option in t-MN patients. This systematic review and meta-analysis aimed to explore the outcomes of allo-HCT in t-MN. Following PRISMA guidelines, a comprehensive literature search was performed on PubMed,

Myeloid/Lymphoid Neoplasms with eosinophilia and involvement of Tyrosine Kinase gene fusions (MLN-TK) is a WHO disease category including a diverse group of malignancies characterised by recurrent genomic rearrangements of tyrosine kinase (TK) genes such as PDGFRA, PDGFRB, FGFR1, JAK2, ETV6 and FLT3. Identification of these TK rearrangements is important for the accurate diagnosis of MLN-TK and allows targeted therapy with TK inhibitors. In this study, we validated the use of optical genome mapping (OGM) retrospectively by analysing 11 samples from 10 cases with suspected or known TK rearrange

Multiple primary malignancies are not uncommon in daily oncology practice, even though their frequency in the same or different organ systems varies. Regardless, early detection and proper planning of therapeutic approaches are essential for successful management. Here, we present a 73-years-old male with adenocarcinoma of the sigmoid who was referred for initial staging with 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT). 18F-FDG PET/CT revealed two metabolically active formations in the sigmoid and ascending colon and a large, heterogeneous tumor l

BACKGROUND: Endoscopic submucosal dissection (ESD) for superficial non-ampullary duodenal epithelial tumors (SNADETs) is technically challenging and is associated with a risk of adverse events, particularly when lesions are located near the major papilla. Pancreatic duct (PD) stent may reduce the risk of post-ESD pancreatitis; however, no standard strategy has been established. This study aimed to evaluate the effectiveness and safety of PD stent placement combined with ESD for SNADETs near the major papilla.
METHODS: This was a retrospective study of duodenal ESD after prophylactic PD stent

Background/Objectives: Thymic neuroendocrine neoplasms (t-NENs) are rare, biologically aggressive malignancies of the anterior mediastinum. Due to their low incidence, clinical evidence remains limited, and treatment recommendations are primarily based on expert opinion and extrapolation from other neuroendocrine tumors. This study aimed to analyze the clinicopathological features, treatment patterns, and survival outcomes of patients with t-NENs treated at a single comprehensive cancer center over 25 years. Methods: A retrospective review was performed on 19 adult patients diagnosed with t-NE

The rising prevalence of pancreatic cystic lesions (PCLs), particularly intraductal papillary neoplasms (IPMNs), has been attributed to increased utilization of advanced imaging techniques. Incidental detection of PCLs is frequent in abdominal CT and MRI scans, with IPMNs representing a significant portion of these lesions. Surveillance of IPMNs is recommended due to their malignant potential; however, their overlapping imaging features with benign entities can lead to misdiagnosis, overtreatment, and overutilization of healthcare resources. This paper aims to highlight and differentiate lesio
