Bioengineering Innovations for Personalized Care in Low Back Pain: From Sensors to Smart Therapeutics.

Low back pain (LBP) remains one of the most prevalent and disabling musculoskeletal conditions worldwide, shaped by interacting mechanical, neurophysiological, inflammatory, vascular, and behavioral factors. Conventional care often relies on generalized exercise programs and episodic, predominantly subjective assessment, which can underrepresent inter-individual heterogeneity and longitudinal change. Recent bioengineering advances enable continuous, multimodal monitoring of objective correlates of function-neuromuscular activation and coordination (sEMG/polyEMG), movement patterns and activity

Prediction of mild cognitive impairment progression using time-sensitive multimodal biomarkers.

Alzheimer's disease (AD) develops silently for years before symptoms emerge, making early identification of at-risk individuals essential for prevention trials and early intervention. We tested whether combining neurophysiological, imaging, and blood biomarkers improves prediction of progression to mild cognitive impairment (MCI) in cognitively unimpaired older adults with a family history of AD (n = 102; 31 progressors; mean follow-up of 5.9 years). Magnetoencephalography, magnetic resonance imaging, plasma biomarkers, and amyloid and tau positron emission tomography each captured complementa

The current status of foundation models in decoding inner speech from non-invasive brain signals: a mini review.

Inner speech (IS), or imagined speech without overt articulation, is a promising target for brain-computer interfaces (BCIs) aimed at restoring communication in individuals with severe speech impairments, such as locked-in syndrome. Foundation models (FMs), typically trained using self-supervised learning (SSL) on large-scale datasets, offer new opportunities for learning transferable and robust representations from neural signals. This mini review provides an overview of FM-based approaches for IS decoding using non-invasive neuroimaging modalities, including functional magnetic resonance ima

EEG microstates and dynamic functional connectivity reveal stage-specific brain networks in subjective tinnitus.

Tinnitus with normal hearing suggests central mechanisms, yet stage-specific brain network dynamics remain unclear. This study investigated stage-specific brain network patterns in normal-hearing tinnitus patients using electroencephalography (EEG) microstate and dynamic functional network (DFN) analyses. Resting-state EEG was recorded from 45 participants (15 acute tinnitus, <6 months; 15 chronic tinnitus, ≥6 months; and 15 healthy controls). Results showed that acute tinnitus involved enhanced salience network engagement and reduced central executive network participation, with transitions s

Electroencephalographic responses before, during, and after upper limb paired associative stimulation.

Paired associative stimulation (PAS) is a non-invasive protocol involving repeated stimulus pairs to activate two cortical areas alternately, inducing Hebbian-like plasticity. However, its neurophysiological impacts remain unclear. To determine the changes that occur in the brain during PAS, brain activity during PAS must be measured and distinguished from the electromagnetic artifacts produced by the stimulation. Here, we present a novel dataset of electroencephalography (EEG) measurements during PAS with an inter-stimulus-interval of 25 ms (PAS25, expected to induce long-term potentiation-li

CNN-based framework for Alzheimer's disease detection from EEG via dynamic mode decomposition.

Alzheimer's disease (AD) and frontotemporal dementia (FTD) are major neurodegenerative disorders with characteristic EEG alterations. While most prior studies have focused on eyes-closed (EC) EEG, where stable alpha rhythms support relatively high classification performance, eyes-open (EO) EEG has proven particularly challenging for AD, as low-frequency instability obscures the typical spectral alterations. In contrast, FTD often remains more discriminable under EO conditions, reflecting distinct neurophysiological dynamics between the two disorders. To address this challenge, we propose a CNN

Differential modulation of movement speed with state-dependent deep brain stimulation in Parkinson's disease.

Subthalamic deep brain stimulation (STN-DBS) provides unprecedented spatiotemporal precision for the treatment of Parkinson's disease (PD), allowing for direct real-time state-specific adjustments. Inspired by findings from optogenetic stimulation in mice, we hypothesized that STN-DBS can mimic dopaminergic reinforcement of ongoing movement kinematics during stimulation. To investigate this hypothesis, we delivered DBS bursts during particularly fast and slow movements in 24 patients with PD. Our findings reveal that DBS during fast movements enhanced future movement speed more than DBS during

A review of transcranial pulse stimulation: innovations in neuromodulation.

INTRODUCTION: Non-invasive brain stimulations (NIBS), such as transcranial magnetic stimulation, transcranial direct current stimulation, transcranial alternating current stimulation, transcranial focused ultrasound, and transcranial pulse stimulation (TPS), employ electric currents or acoustic waves to induce and modulate neuroplasticity in humans. Thus, NIBS have the capacity to modify pathological plasticity while promoting plasticity in neuropsychiatric disorders, helping to ameliorate symptoms and enhance rehabilitation. AREAS COVERED: The efficacy of TPS in treating neuropsychiatric dis

Hyperarousal, Dissociation, Emotion Dysregulation and Re-Experiencing-Towards Understanding Molecular Aspects of PTSD Symptoms.

Approximately 70% of people will experience a traumatic event in their lifetime, but post-traumatic stress disorder (PTSD) will only develop in 3.9% and complex post-traumatic stress disorder (CPTSD) in 1-8% of the population worldwide, although in some countries (e.g., Poland and Northern Ireland) it will develop in a much higher percentage. Stress-related disorders have a complex pathogenesis involving neurophysiological, genetic, epigenetic, neuroendocrine and environmental factors. This article reviews the current state of knowledge on the molecular aspects of selected PTSD symptoms: hyper

End of Apnea Event Prediction Leveraging EEG Signals and Interpretable Machine Learning.

Obstructive sleep apnea is a prevalent sleep disorder with serious health implications. While previous studies focused on detecting apnea events, little is known about the factors that determine whether an apnea episode continues or terminates. Understanding these mechanisms is crucial for optimizing treatment strategies. In this study, we analyzed 30-s brain activity segments during continuous and ending apnea events to identify neurophysiological markers of event termination, with particular emphasis on the most influential EEG features. Frequency-domain and complexity features were extracte

Music-based intervention as a new therapeutic treatment for executive dysfunction in Parkinson's disease.

Executive dysfunction is a prominent feature of cognitive impairment in Parkinson's disease (PD), affecting approximately 42% of patients. Due to the lack of effective treatment options for the executive dysfunction, it may lead to more severe gait disorder, dementia and be difficult to recuperate. Current studies indicate that music-based intervention (MBI) is an innovative and vital treatment for executive dysfunction. This review aims to describe the ideas-MBI for treating executive dysfunction in PD. We summarize the existing protocols and clinical trials of MBI for treating executive dysf

Virtual reality and psychedelics: new perspectives and new possibilities in the treatment of alcohol use disorder.

Psychedelic-assisted therapy is a remarkably promising treatment for substance use disorders and for alcohol use disorder (AUD) in particular. Research supporting psychedelics as a safe and effective pharmacotherapy for AUD and its comorbid psychiatric conditions dates from the late1940s and includes over 2,000 published studies. There are, however, challenges to the clinical implementation of psychedelics on a scale appropriate to a highly prevalent disease. Virtual reality applications can aid in meeting these challenges. Studies have shown that virtual reality simulations of psychedelic phe

Gamma entrainment induced by deep brain stimulation as a biomarker for motor improvement with neuromodulation.

Finely tuned gamma (FTG) oscillations from the subthalamic nucleus (STN) and cortex in Parkinson's disease (PD) patients undergoing deep brain stimulation (DBS) are often associated with dyskinesia. Recently it was shown that DBS entrains gamma activity at 1:2 of the stimulation frequency; however, the functional role of this signal is not yet fully understood. We recorded local field potentials from the STN in 19 chronically implanted PD patients on dopaminergic medication during DBS, at rest, and during repetitive movements. Here we show that high-frequency DBS induced 1:2 gamma entrainment

Assessing Exercise and Fitness-Related Brain Health Using Multi-modal Magnetic Resonance Imaging Outcomes.

Several magnetic resonance imaging (MRI)-based modalities have provided insight into how acute and chronic exercise and physical activity impact brain health. Structural MRI-based measures provide information regarding brain volume and cortical thickness, diffusion-weighted imaging measures provide indices of white matter and gray microstructure, and cerebrovascular effects that may be assessed through measures of regional cerebral blood flow, and brain activation and neural networks can be indexed using task-based and connectivity-based functional MRI, respectively. In this chapter, a series

Eculizumab as a Disease-Modifying Therapy in Chronic Inflammatory Demyelinating Polyneuropathy (CIDP): A Case Report.

BACKGROUND AND AIMS: Chronic inflammatory demyelinating polyneuropathy (CIDP) is a rare immune-mediated disorder; about 20%-30% of patients do not adequately respond to first-line treatments (immunoglobulins, therapeutic plasmapheresis, and corticosteroids) posing diagnostic and therapeutic challenges. CASE REPORT: We report the case of a 58-year-old man diagnosed with CIDP. During follow-up, he progressively became refractory to all first-line treatments. Therefore, 20 months after the diagnosis, an add-on therapy with Rituximab was tried. Despite previous works supporting the use of Rituxim

Can Antidromic and Orthodromic Stimulation Both Be Used for Correct Carpal Tunnel Syndrome Staging by J. D. Bland and L. Padua?

Background and Objectives: Padua (1997) and Bland (2000) have already proposed neurophysiological classification scales for patients with carpal tunnel syndrome (CTS), where the absence of orthodromic sensory response is used as a criterion of a severe stage. We hypothesized that antidromic values cannot be used equally for correct staging. Materials and Methods: We performed a consecutive investigation with nerve conduction studies in 60 arms of patients with CTS and prolonged distal motor latency. Results: In 11 out of 60 arms (18.3% of cases), orthodromic sensory nerve action potential (SNA
