A Geriatric Assessment Score Predicting 1-Year Mortality and Functional Decline After TAVR: The GASS-TAVR Study.

BACKGROUND: A significant proportion of older adults undergoing transcatheter aortic valve replacement (TAVR) fail to achieve meaningful clinical benefit at 1 year, highlighting the need for improved risk stratification. OBJECTIVES: The aim of this study was to develop and validate a geriatric score to determine 1-year mortality or functional decline. METHODS: A multicenter cohort study was conducted across 3 high-volume academic centers in Italy from January 2020 to December 2022. Consecutive patients ≥75 years of age with severe symptomatic aortic stenosis evaluated for TAVR were enrolled.

Serological detection of Leptospira interrogans serovar Wolffi and clinical management of a crab-eating raccoon (Procyon cancrivorus) from the Brazilian Cerrado.

The crab-eating raccoon (Procyon cancrivorus) is a widely distributed South American mesocarnivore that frequently inhabits human-modified landscapes, where it may play a role in the transmission of zoonotic pathogens. Leptospirosis is an important but underreported disease in free-ranging wildlife, particularly in South America. This report describes the clinical management of a free-ranging crab-eating raccoon rescued after a road accident in southeastern Brazil, presenting systemic, hepatic, renal, and neurological alterations. Given the epidemiological context and compatible clinical findi

Concentration and Specialty Pair Patterns of Interdepartmental Consultations in Hospitalized Patients Using Real-World Data: Retrospective Cohort Study.

BACKGROUND: Interdepartmental consultations are essential for managing complex inpatient care but are often inefficient. Hospital-wide, data-driven analyses are needed to guide process improvements; yet, most existing studies have focused on single departments or specific diseases, leaving a gap in understanding hospital-level collaboration networks. Understanding these patterns is crucial for optimizing clinical workflows, reducing delays, and improving patient outcomes in large tertiary hospitals. OBJECTIVE: This study aimed to analyze the distribution and network characteristics of interde

Infants ≤24 weeks are not just smaller extremely preterm infants.

Among the most pressing topics in neonatal-perinatal medicine today is intensive care for infants born at ≤24 weeks' gestation. Infants born at 22-24 weeks comprise ~1 in 500 live births, with ~7500 liveborn infants annually in the U.S.-more common than Down syndrome or critical congenital heart disease-and make up 1 in 5 U.S. infant deaths. Major uncertainties exist about clinical decisions, including regarding obstetric care, delivery room procedures, incubator management, nutrition, respiratory support, and the optimal developmental environment. Partnering with families, we can develop a so

Therapeutic Effects of Fermented Soy Extract on Osteoporotic Changes in Ovariectomized Mice as a Model of Menopause.

The objective of this study was to evaluate the effects of fermented soy extract (FSE) on bone metabolism and neuroendocrine regulation in an ovariectomized (OVX) mouse model of postmenopausal conditions. FSE administration improved bone microarchitecture and modulated key markers associated with bone turnover, including reduced CTX-1 and RANKL levels and increased OPG expression, indicating suppression of osteoclastogenesis. In addition, FSE enhanced estrogen receptor-related activity and upregulated genes involved in bone metabolism. Notably, FSE also increased estradiol, serotonin, and nore

Minimally Invasive/Robotic Surgery for Gastric Cancer.

Minimally invasive and robotic gastrectomy have become integral components of contemporary gastric cancer management, supported by evidence demonstrating oncologic safety and faster postoperative recovery. Advances in multimodality treatment have improved outcomes for locally advanced gastric cancer. Adequate minimally invasive gastrectomy requires precise anatomic understanding, rigorous patient selection, and adherence to standardized D2 lymphadenectomy principles. Technical considerations include optimized port placement, safe lymph node dissection, and tailored reconstruction based on rese

Assessing essential needs expressed by breast and gynecological cancer patients with food insecurity.

Food insecurity poses a challenge for cancer patients, who must maintain adequate nutrition during treatment. This study analyzed data from 295 low-income participants in the NCI-funded Food to Overcome Outcome Disparities trial to examine the overlap between degree of food security and other essential needs. Patients with very low food security reported greater financial toxicity from cancer care and lived in more disadvantaged areas than those with low food security. Financial, supportive service, and housing needs were highly expressed among participants, particularly those with very low fo

A proposed protocol for combining core stability and pelvic floor muscle training to prevent stress urinary incontinence in pregnant women: a preventive framework.

Stress urinary incontinence is common during and after pregnancy and impacts women's health both physically and mentally. This protocol suggests a non-invasive preemptive training strategy which integrates pelvic floor muscle training and core stability exercises with nutrition counseling. This model provides a clear, systematic, evidence-based framework which can be incorporated into antenatal care arising from a literature and clinical guideline review. The objective is to equip healthcare professionals with tangible, multidisciplinary strategies for proactive urinary incontinence care durin

Workplace Ergonomic Adjustments and Supportive Policies During the Pregnancy of a Paint Sprayer: A Case Report.

A 29-year-old paint sprayer presented with mild lower back pain and fatigue related to prolonged standing and repetitive tasks in the first few weeks of pregnancy. An initial assessment included a review of her workstation, evaluation of posture, and standard prenatal tests such as routine blood work and ultrasound. These investigations showed normal fetal development and no complications. The employer modified her work environment, providing ergonomic seating, and reducing prolonged standing. She received counseling on safe physical activity and balanced nutrition to help maintain overall hea

The role of nutraceuticals in heart failure muscle wasting as a result of inflammatory activity. The International Lipid Expert Panel (ILEP) Position Paper.

Muscle wasting is one of the main causes for exercise intolerance and ventilatory inefficiency in patients with heart failure and a strong predictor of frailty and reduced survival. The prevalence of sarcopenia is at least 20% in patients with heart failure. Patients with heart failure often have subclinical systemic inflammation, which may exert sustained effects on skeletal muscle. Besides exercise, nutrition should also be carefully evaluated as an appropriate diet with selected nutraceuticals may be able to stimulate muscle anabolism and inhibit muscle catabolism. This review summarizes th

Management in severe dementia: recommendations of the Scientific Department of Cognitive Neurology and Aging of the Brazilian Academy of Neurology.

Alzheimer's disease (AD) and other neurodegenerative dementias have a progressive course, impairing cognition, functional capacity, and behavior. Most studies have focused on AD. Severe dementia is associated with increased age, higher morbidity-mortality, and rising costs of care. It is fundamental to recognize that severe dementia is the longest period of progression, with patients living for many years in this stage. It is the most heterogeneous phase in the process, with different abilities and life expectancies. This practice guideline focuses on severe dementia to improve management and

Cardiac rehabilitation: Effective yet underutilized in patients with cardiovascular disease.

Cardiac rehabilitation is a comprehensive program that treats patients with multiple cardiac conditions including post-myocardial infarction, stable angina, post-coronary artery bypass surgery, chronic heart failure, and peripheral vascular disease with structured exercise, and nutrition and risk factor counseling. It is an effective tool that has been shown to improve not only quality of life but also reduce adverse cardiac events, including death. While the value of cardiac rehabilitation is supported by a large body of evidence and its recommendation by the American Heart Association/Americ

Epigenetic mapping of the somatotropic axis in Nile tilapia reveals differential DNA hydroxymethylation marks associated with growth.

In vertebrates, the somatotropic axis comprising the pituitary gland, liver and muscle plays a major role in myogenesis. Its output in terms of muscle growth is highly affected by nutritional and environmental cues, and thus likely epigenetically regulated. Hydroxymethylation is emerging as a DNA modification that modulates gene expression but a holistic characterization of the hydroxymethylome of the somatotropic axis has not been investigated to date. Using reduced representation 5-hydroxymethylcytosine profiling we demonstrate tissue-specific localization of 5-hydroxymethylcytosines at sing

[The Metabolic Optimized Fast Track Concept: preoperative preparation for abdominal surgery].

In recent years the protocols of Enhanced Recovery After Surgery (E.R.A.S.) have been introduced into clinical practice and actively used. The goal of E.R.A.S. is to increase recovery, decrease complications and reduce length of stay after surgery through optimization of perioperative management. One of the key elements of E.R.A.S. is Metabolic Optimized Fast Track Concept (MOFA). It is aimed at the reduction of the period of preoperative fasting and at the activation of glucose transport inside the cell. It involves the administration of combined carbohydrate-protein-glutamine drinks which re

Agenda for Translating Physical Activity, Nutrition, and Weight Management Interventions for Cancer Survivors into Clinical and Community Practice.

Evidence supporting physical activity, diet, and weight management for cancer survivors has grown, leading to the development of guidelines and interventions. The next step is to identify necessary practice and policy changes and to develop a research agenda to inform how interventions can be delivered to survivors most effectively and efficiently in health care settings and by community-based organizations. Here, an agenda is proposed for research, practice, and policy that incorporates recommendations for a range of programming options, a patient-centered, tailored screening and referral app

To PEG or not to PEG: a review of evidence for placing feeding tubes in advanced dementia and the decision-making process.

Percutaneous endoscopic gastrostomy (PEG) has evolved into a common low-risk procedure in current medical practice. Clinical evidence supporting the use of tube feedings in patients with advanced dementia is clearly lacking, yet PEG procedures continue to be performed in a large number of these cases. In fact, multiple studies have shown that feeding tubes seldom are effective in improving nutrition, maintaining skin integrity by increased protein intake, preventing aspiration pneumonia, minimizing suffering, improving functional status, or extending life. The decision-making process is compli

Infection of a pancreatic pseudocyst due to Candida albicans.

A 40-year-old man with diabetes mellitus, congestive heart failure, alcoholic cirrhosis, and chronic pancreatitis had an exacerbation of pancreatitis due to alcohol abuse. His condition deteriorated rapidly with development of apparent sepsis; cultures were negative. He slowly improved with multiple antibiotic therapy and total parenteral nutrition. Serial imaging of the pancreas revealed edematous pancreatitis that evolved initially into a phlegmon and later into multiple pseudocysts. Intermittent fever prompted computed-tomography-directed percutaneous aspiration of the largest pancreatic fl

Interdisciplinary modelling approaches for One Health in agri-food systems: a scoping review and conceptual analysis.

BACKGROUND: Increasingly, the One Health approach must be applied to a changing food system to simultaneously support nutritional, infectious disease, and environmental outcomes. Successful interventions by public health authorities will require quantitative modelling for evaluating trade-offs and prioritization. Herein, we reviewed methodologies for One Health modelling relevant to the food system and proposed a framework for supporting international and national One Health initiatives within governmental institutions. METHODS: This study first identified an operational scope of One Health b

Effectiveness of structured nursing interventions in reducing complication related morbidity in leukemia patients undergoing hematopoietic stem cell transplantation.

BACKGROUND: Hematopoietic stem cell transplantation (HSCT) is a potentially curative treatment for leukemia but is associated with high rates of complication-related morbidity, including infections, oral mucositis, graft-vs. -host disease (GVHD), and prolonged hospitalization. Structured nursing interventions may reduce these complications, yet comprehensive prospective evidence in adult leukemia patients remains limited. OBJECTIVE: This study aimed to evaluate the effectiveness of a multifaceted structured nursing intervention program in reducing complication-related morbidity among leukemia
