OBJECTIVES: Clavicular fractures account for 2.6% of all fractures, and more than 80% involve the middle third of the clavicle. Plate fixation has been the most common method of fixation reported but has been associated with complications such as infection, wound breakdown, nonunion, implant failures, poor cosmetic outcome, and local skin numbness. We report on a series of cases receiving minimally invasive insertion of titanium elastic nails (TEN) to fix the displaced midclavicular fractures.
DESIGN: Prospective, clinical study.
SETTING: Regional referral center.
PATIENTS/PARTICIPANTS: Fro

BACKGROUND: The functional results associated with nonoperative treatment of severely impacted valgus fractures of the proximal part of the humerus are poor, and these injuries are difficult to treat with minimally invasive percutaneous fixation techniques. The aim of this study was to review the functional and radiographic results and complications of a new operative technique in a series of twenty-five patients.
METHODS: Over a two-year period, we treated twenty-nine patients with a severely impacted valgus fracture of the proximal part of the humerus. Three patients were lost to follow-up

Complex tibial plateau fractures continue to challenge orthopaedic surgeons because of the difficulty in obtaining an anatomic reduction of the joint, axial alignment of the articular segment, while avoiding complications. Despite overall good results, significant complications persist with internal fixation, particularly in cases with compromised soft tissues, open fractures, bone loss, extensive comminution, compartment syndrome, extensor mechanism disruption, or significant osteopenia. Circular external fixation offers advantages in these situations because of its unique biomechanics, adapt

Purpose. Impaction fractures of the anterior aspect of the humeral head, the reversed Hill-Sachs lesion, are common in posterior shoulder dislocation. We present a new technique to address these lesions arthroscopic-assisted with the use of a bone substitute. Methods. We report the case of a 45-year-old male with a reversed Hill-Sachs lesion after posterior shoulder dislocation. Initially a glenohumeral arthroscopy is performed to address concomitant intra-articular injuries. Guided by the k-wire a cannulated sizer was inserted for reduction of the fracture under arthroscopic visualization. Fo

High-resolution imaging, based on CT, has become the expected standard of imaging in severe sinofacial trauma. Imaging must include both bone and soft-tissue detail. Intracranial complications such as hematoma, contusion, and dural tear must be noted and followed appropriately. Early orbital assessment must be included to allow surgical decompression of hematoma or fracture reduction before irreversible changes to the visual pathway occur. Clinical assessment and initial, limited plain films offer an invaluable overview to set up priorities in resuscitation and subsequent direction for more de

Spinocerebellar ataxia type 1 (SCA1) is one of the autosomal dominant spinocerebellar degeneration (SCD) diseases characterized by progressive cerebellar ataxia, muscle atrophy, and peripheral neuropathy. We report the management of a 43-year-old man with SCA1 who underwent general anesthesia for open reduction and internal fixation of a mandibular fracture. Although anesthesia-induced vocal cord paralysis has been reported in patients with SCD, nasotracheal intubation was performed uneventfully with video laryngoscope. After taking into consideration the increased risk of postoperative respir

A 60-year-old female sustained a distal radius fracture and underwent open reduction internal fixation with a volar locking plate. The patient had an uneventful recovery until four months postoperatively when the patient clinically regressed, and an expansile, radiolucent metaepiphyseal lesion was found. Further workup revealed this was a giant cell tumor of bone (GCTB). Definitive management consisted of extensive curettage, cryoablation, and cementation of the lesion, and the hardware was left intact. The current case presents an uncommon presentation of GCTB. The case illuminates the import

Arthroscopy of the wrist has gained popularity among hand and wrist surgeons for diagnosing and treating wrist pathologies. In general, it is a minimally-invasive procedure which can be performed in day care surgery. It has a higher diagnostic accuracy compared to conventional imaging modalities such as CT or MRI. Simultaneous therapeutic intervention can be performed such as ligament repair, fracture reduction, ganglion cyst removal or synovectomy of the wrist. Recent studies have shown better functional outcomes and fewer complications compared to open surgical interventions. Prospective stu

Charcot arthropathy is a progressive condition primarily affecting the lower limbs in patients with diabetes mellitus. It is a rare complication of diabetic neuropathy and if left untreated can lead to severe limb destruction necessitating major amputation. Here, we report the case of a 41-year-old female who presented with rapidly progressive Charcot foot over a 10-day period, necessitating open reduction and internal fixation of Lisfranc-type fracture dislocations. Her presentation with a rapidly progressing red, swollen foot with a blister on the plantar aspect prompted initial treatment on

Reverse Hill-Sachs lesion is a defect caused by the anterior compression fracture of the humeral head in posterior shoulder dislocation. We present a 34-year-old male patient with pain and limited joint mobility in the right shoulder due to a fall following electric shock 2 days prior to admission. Radiography and CT of the shoulder revealed posterior dislocation of the shoulder with a reverse Hill-Sachs lesion. The patient underwent open reduction of the humeral head, elevation of the depressed osteochondral surface, reconstruction using autograft, and repair of the posterior surface of the g

A 5-year-old boy with Williams syndrome received open reduction of fracture of the antebrachium twice. He had been diagnosed as having Williams syndrome with some characteristic symptoms, including elfin face, mental retardation and primary pulmonary hypertension. Williams syndrome has a tetrad of cardiovascular disease, elfin face, mental retardation and hypercalcemia. Operations were performed twice under general anesthesia. Airway management with mask technique was easily performed. Tracheal intubation was accomplished successfully. Anesthesia was induced with propofol, fentanyl, and vecuro

A bioresorbable syndesmotic screw was used successfully for fixation in 4 patients, 3 with a bimalleolar Weber type-C ankle fracture and 1 with a Maisonneuve-type injury. The 5-mm screw consisted of a polyglycolic acid/polylactic acid copolymer placed in standard fashion at the time of open reduction and internal fixation. The patients healed without difficulty, and follow-up radiographs showed anatomic maintenance of the syndesmotic space. There were no resorption problems. Signs of minimal irritation were noted at the screw-head site in 2 patients at 3 to 4 months after surgery, but the irri

Three patients were treated with external fixation with iliofemoral distraction as an alternative treatment of comminuted central fracture-dislocations of the hip. The patients were mobilized on the first postoperative day and discharged from the hospital an average of 10 days postaccident. The frame was left in place for 6 weeks while the traction was maintained. Full weight bearing was started 3 months after the injury. A full range of motion in the injured hip was achieved at 4 months postoperation. No signs of avascular bone necrosis were seen during follow-up for up to 2 years. The clinic

Four cases of unreduced (2-11 months postinjury) anterior hip dislocation are reported. One public dislocation 2 months postinjury was treated by open reduction with a fair result. A unique case of iliac dislocation 11 months postinjury, overlooked due to an associated femoral shaft fracture, was treated by valgus/derotation osteotomy. Two obturator dislocations 7.5 and 9.5 months postinjury were treated by a subcapital osteotomy and displacement of the femoral neck into the acetabulum (modified excision arthroplasty). Both of these patients had fairly stable, painless, and mobile hips at 2.1

This review addresses the neurovascular complications associated with the surgical treatment of clavicle fractures through open reduction and internal fixation (ORIF). Despite being a generally safe procedure, it can lead to severe complications including brachial plexopathy, pseudoaneurysm, arteriovenous fistulas (AVF), deep vein thrombosis (DVTs), and thoracic outlet syndrome (TOS). One significant observation, not often highlighted in previous literature, is that neurovascular complications are more common in cases involving delayed fixation, nonunion, or malunion, compared to those treated

Surgical treatment of distal tibia fractures can present as a difficult task. Intramedullary nailing (IMN), external fixation, and open reduction internal fixation (ORIF) have displayed various complications ranging from a high rate of knee morbidity with IMN to wound complications and infection with ORIF. Minimally invasive plate osteosynthesis (MIPO) has been used to decrease development of these complications. MIPO respects the soft tissue envelope along with maintaining the biological environment needed for proper osseous healing. Favorable results have been shown with MIPO in regard to fu

UNLABELLED: Chronic sports-related injuries involving the hallux are well known in adult populations. However, they are less frequently described in adolescents. We present 2 cases of elite-level gymnasts with neglected Salter-Harris IV fractures involving the proximal phalanx of the great toe following a severe hyperextension injury to the metatarsophalangeal joint. Both were successfully treated with open reduction and internal fixation with return to pre-injury level of activity, improvement in outcome scores, and demonstration of no disability from injury 2 years after surgical interventio

Management of lower limb fractures in children involves many approaches, spanning from conservative treatment to open reduction and internal fixation. A number of intermediate treatments have also been shown to be effective, notably skeletal traction. However, each of these techniques has its own advantages and disadvantages in terms of complications. In this report, we describe two new cases of chronic osteomyelitis that manifested (many years) after childhood skeletal traction. After analysing these two cases, we proposed measures that can be implemented to avoid such complications, along wi

Bony avulsion fractures of the posterior cruciate ligament of the tibia have commonly been treated by open reduction and internal fixation using the posterior approach. However, this approach, using the prone position, makes it difficult to investigate and treat other combined injuries of the knee joint. We report a case of posterior cruciate ligament avulsion of the tibia that was arthroscopically reduced and firmly fixed with two cannulated screws. The posterior sag was absent after the operation and the result was excellent. By arthroscopy, we got rigid fixation of the avulsed fragment for

The article analyses 41 fractures of the medral epicondyle of the humerus in children with incarceration of the fragment in the cavity of the elbow joint. The clinical and X-ray signs of this complication are described in detail. Open reduction is the optimal method of treatment. During the operation the ulnar nerve is examined, the cavity of the elbow joint is cleaned, and stable osteosynthesis of the medial epicondyle is performed. Restorative treatment can be started in the early postoperative period if a screw-awl has been applied. The function of the elbow joint is restored completely in
