Dental anxiety--the development of a measurement model.

A more detailed measurement instrument for dental anxiety has been developed. To investigate the dimensionability of the response, two instruments were administered to 452 persons. The first set of items (33) concerned stimuli which might prove fearsome to patients. Some of these were dental specific, others were not. A second instrument (20 items) elicited the anxiety symptoms experienced. By comparing separate Kaiser-Caffrey Alpha Factor Analyses two factors were identified within each instrument. These were labelled "generalized" phobias and "dental specific" phobias; and "endogenous" and "

Local anaesthetic adverse effects and other emergency problems in general dental practice.

This paper has three parts: local anaesthetic adverse effects, haemorrhage control and altered immune response. Primary emphasis is placed on the problems with anaesthetics since their use is widespread. Every day, nearly 2 million injections of local anaesthetic are given in dental practice. From 2.5 to 10 per cent of patients experience adverse reactions. This adverse effect rate is 10 times higher than that in medicine for the same drugs. Allergic, toxic, idiosyncratic and psychogenic effects are discussed. New data on the role of local anaesthetic agents, vasoconstrictors and preservatives

An epidemiological study of mental disorders at pune, maharashtra.

BACKGROUND: The WHO Global Burden of Disease study estimates that mental and addictive disorders are among the most burdensome in the world, and their burden will increase over the next decades. The mental and behavioral disorders account for about 12% of the global burden of disease. However, these estimates and projections are based largely on literature review rather than cross-national epidemiological surveys. In India, little is known about the extent, severity and unmet need of treatment mental disorders. Thus, there was a need to carry out rigorously implemented general population surve

Dynamic changes in metacognitive mechanisms and symptoms during the attention training technique: Insights from ecological momentary assessment.

BACKGROUND: Anxiety disorders are prevalent and associated with substantial distress and impairment. Within the metacognitive model of emotional disorders, the cognitive attentional syndrome (CAS) and associated metacognitions maintain anxiety. The Attention Training Technique (ATT) targets the CAS and related metacognitions. However, less is known about how symptoms and metacognitive mechanisms evolve across treatment phases. OBJECTIVE: The study aimed to explore temporal dynamics in emotional symptoms and metacognitive mechanisms during group ATT by estimating mean levels, variability, and

What is the effect of homework engagement in group cognitive behavioral therapy for anxiety disorders and depression?

BACKGROUND: Homework is integral to cognitive behavioral therapy (CBT) treatment programs. Previous research has reported mostly positive findings regarding the effect of homework adherence on CBT outcomes, but only limited research has evaluated the effect in transdiagnostic CBT (tCBT). METHODS: This secondary study used data from 164 patients with major depressive disorder, social phobia, agoraphobia, or panic disorder, randomized to 14 sessions of diagnosis-specific CBT (dCBT) in groups or group tCBT (Unified Protocol). The number of times patients engaged in homework assignments was measu

Evaluation of mechanism of change in transdiagnostic cognitive behaviour therapy using single case experimental design.

BACKGROUND AND OBJECTIVES: Transdiagnostic mechanisms of change (txMOC) specific to cognitive behaviour therapy are poorly understood. Salkovskis (1996) proposed one such mechanism in terms of the shift towards an alternative, less negative view of their problems or cognitive flexibility. This hypothesis has been described as involving a shift in beliefs, from "theory A″ to "theory B". The objective of this research was to evaluate this hypothesis. METHODS: Effectiveness of a novel txCBT and temporal changes in process and symptom measures were evaluated using a non-concurrent multiple baseli

Prevalence, onset and course of anxiety disorders during pregnancy: A systematic review and meta analysis.

BACKGROUND: Anxiety disorders during pregnancy are associated with various adverse outcomes. Previous reviews of anxiety disorders during pregnancy have methodological limitations and were conducted without a meta-analysis. The present study is a systematic review and meta-analysis of the published research on the prevalence, onset and course of all anxiety disorders during pregnancy plus obsessive-compulsive disorder (OCD) and posttraumatic stress disorder. METHODS: A comprehensive literature search was performed on a wide range of databases. A random effects model was used for the meta-anal

Migraine-Like Visual Auras Among Traumatized Cambodians with PTSD: Fear of Ghost Attack and Other Disasters.

This article profiles visual auras among traumatized Cambodian refugees attending a psychiatric clinic. Thirty-six percent (54/150) had experienced an aura in the previous 4 weeks, almost always phosphenes (48% [26/54]) or a scintillating scotoma (74% [40/54]). Aura and PTSD were highly associated: patients with visual aura in the last month had greater PTSD severity, 3.6 (SD = 1.8) versus 1.9 (SD = 1.6), t = 10.2 (df = 85), p < 0.001, and patients with PTSD had a higher rate of visual aura in the last month, 69% (22/32) versus 13% (7/55), odds ratio 15.1 (5.1-44.9), p < 0.001. Patients often

Personality functioning in anxiety disorders.

BACKGROUND: The Alternative DSM-5 Model for Personality Disorders as well as the upcoming IDC-11 have established a new focus on diagnosing personality disorders (PD): personality functioning. An impairment of self and interpersonal functioning in these models represents a general diagnostic criterion for a personality disorder. Little is known so far about the impairment of personality functioning in patients with other mental disorders than PD. This study aims to assess personality functioning in patients with anxiety disorders. METHODS: Ninety-seven patients with the diagnosis of generaliz

Lessons learned from an in-flight medical event.

Air transportation, although safe, exposes passengers to distinct physical, psychological, and physiological stressors that may be well tolerated in a healthy passenger but may trigger or exacerbate a medical event in a passenger with underlying medical problems. This article describes how a medical provider on the flight assisted with a medical emergency on a transatlantic flight.

Neonatal maternal separation opposes the facilitatory effect of castration on the respiratory response to hypercapnia of the adult male rat: Evidence for the involvement of the medial amygdala.

Respiratory manifestations of panic disorder (PD) include a greater respiratory instability and enhanced responsiveness to CO2 compared to normal individuals. Although the prevalence of PD is approximately three times greater in women compared to men, the origins of this sexual dimorphism remain poorly understood. Similar to PD patients, adult female rats previously subjected to neonatal maternal separation (NMS) show an increase in their ventilatory response to CO2 . Because this effect of NMS is not observed in males, we hypothesised that testosterone prevents NMS-induced hyper-responsivenes

Internet-delivered cognitive behavioural therapy for children with anxiety disorders: A randomised controlled trial.

BACKGROUND: Cognitive behaviour therapy (CBT) has been shown to be an effective treatment for anxiety disorders in children, but few affected seek or receive treatment. Internet-delivered CBT (ICBT) could be a way to increase the availability of empirically supported treatments. AIMS: A randomised controlled trial was conducted to evaluate ICBT for children with anxiety disorders. METHOD: Families (N = 93) with a child aged 8-12 years with a principal diagnosis of generalised anxiety disorder, panic disorder, separation anxiety, social phobia or specific phobia were recruited through media a

Predictors of the application of exposure in vivo in the treatment of agoraphobia in an outpatient clinic: An exploratory approach.

OBJECTIVE: Although exposure in vivo is considered to be the most effective therapy component in the treatment of agoraphobia (AG), there is a remarkable lack of its application in psychotherapeutic routine care. We examined the severity of anxiety, psychological distress/comorbidity, therapeutic process/alliance, and sociodemographic status as potential predictors of in vivo exposure. METHOD: We applied correlational analyses and logistic regression analyses in a sample of N = 92 patients (main diagnosis AG) in an outpatient setting. RESULTS: Logistic regression analyses did not yield any s

Mediators of transdiagnostic group cognitive behavior therapy.

The efficacy of cognitive-behavioral therapy (CBT) for anxiety is well established. Investigations into the mechanisms of change in CBT report changes in cognitive vulnerabilities mediating improvements over the course of treatment. As anxiety disorders share certain risk factors, there is a trend toward CBT emphasizing these vulnerabilities, including negative affectivity (NA) and also more specific constructs such as anxiety sensitivity (AS) and intolerance of uncertainty (IU). The purpose of this investigation was to analyze potential mediators of anxiety reduction over the course of transd

Through the eyes of anxiety: Dissecting threat bias via emotional-binocular rivalry.

An extensive body of research has demonstrated that anxious individuals abnormally process threat-related content. Yet, the manner in which clinical anxiety affects the selection of threatening signals and their maintenance within consciousness is yet to be explored. The present study used an emotional binocular rivalry (e-BR) procedure, in which pictures of faces depicting either fearful or neutral expressions competed with pictures of a house for conscious perception. We assumed that first- or cumulative-preferred perception of faces with fearful over neutral expression (i.e., initial or sus

Chronic subjective dizziness.

PURPOSE OF REVIEW: In 1986, the German neurologists Thomas Brandt and Marianne Dieterich described a syndrome of phobic postural vertigo (PPV) based on clinical observations of patients with nonvertiginous dizziness that could not be explained by then-known neuro-otologic disorders. Subsequent research by an American team led by Jeffrey Staab and Michael Ruckenstein confirmed the core physical symptoms of PPV, clarified its relationship to behavioral factors, and streamlined its definition, calling the syndrome chronic subjective dizziness (CSD). This article reviews the 26-year history of PPV

Single-unit muscle sympathetic nervous activity and its relation to cardiac noradrenaline spillover.

Recent work using single-unit sympathetic nerve recording techniques has demonstrated aberrations in the firing pattern of sympathetic nerves in a variety of patient groups. We sought to examine whether nerve firing pattern is associated with increased noradrenaline release. Using single-unit muscle sympathetic nerve recording techniques coupled with direct cardiac catheterisation and noradrenaline isotope dilution methodology we examined the relationship between single-unit firing patterns and cardiac and whole body noradrenaline spillover to plasma. Participants comprised patients with hyper

CBT for pharmacotherapy non-remitters--a systematic review of a next-step strategy.

BACKGROUND: Non-remission rates to pharmacotherapy for anxiety disorders are related to higher relapse rates, decreased quality of life and greater functional impairment. Here we sought to investigate the efficacy of cognitive-behavior therapy (CBT) as a next-step strategy in the treatment of patients with anxiety disorders who did not remit after a pharmacological intervention. METHOD: We carried out a systematic review in the ISI, Pubmed and PsycINFO/PsychLit databases. Studies that did not use CBT and that did not focus on resistance to drug therapy were excluded. We considered resistant p
