The shipboard Beirut terrorist bombing experience: a historical account and recommendations for preparedness in events of mass neurological injuries.

On a Sunday morning at 06:22 on October 23, 1983, in Beirut, Lebanon, a semitrailer filled with TNT sped through the guarded barrier into the ground floor of the Civilian Aviation Authority and exploded, killing and wounding US Marines from the 1st Battalion 8th Regiment (2nd Division), as well as the battalion surgeon and deployed corpsmen. The truck bomb explosion, estimated to be the equivalent of 21,000 lbs of TNT, and regarded as the largest nonnuclear explosion since World War II, caused what was then the most lethal single-day death toll for the US Marine Corps since the Battle of Iwo J

Leukotrienes in nociceptive pathway and neuropathic/inflammatory pain.

The purpose of this review is to summarize the recent studies examining the expression of leukotrienes (LTs) and their receptors in nociceptive pathways, and their crucial roles in pathological pain conditions. LTs belong to a large family of lipid mediators, termed eicosanoids, which are derived from arachidonic acids and released from the cell membrane by phospholipases. LTs are known to be important factors in a variety of local and systemic diseases and allergic/inflammatory diseases. We examined whether LTs were implicated in neuropathic pain following peripheral nerve injury. Using the S

Vascular injuries in compound fractures of the leg with initially adequate circulation.

We carried out a randomised, controlled trial in 157 patients who had isolated severe Gustilo type-IIIa and type-IIIb fractures of the metaphysis or diaphysis of the leg to determine the prevalence of vascular injuries and the role of vascular repair. All patients had stable vital signs and clinically adequate circulation in their legs before operation. In a control group of 64 patients we performed conventional surgery with systematic debridement and primary stabilisation of the fractures. In the trial group of 93 patients the major vessels and nerves adjoining the compound fracture were rout

Lycopene protects against central and peripheral neuropathy by inhibiting oxaliplatin-induced ATF-6 pathway, apoptosis, inflammation and oxidative stress in brains and sciatic tissues of rats.

The fact that oxaliplatin (OXL), a platinum-based chemotherapeutic drug, causes severe neuropathy greatly limits its clinical use. This study investigated the effects of lycopene, a potent antioxidant, on OXL-induced central and peripheral neuropathy. In this study, 30 min after oral administration of LY at a dose of 2 mg/kg b.w./day and 4 mg/kg b.w./day on 1 st, 2nd, 4th and 5th days, rats were given 4 mg/kg b.w./day of OXL intraperitoneally. It was detected that LY decreased OXL-induced lipid peroxidation and increased the activities of superoxide dismutase (SOD), catalase (CAT), and glutath

Large-scale reorganization at multiple levels of the somatosensory pathway follows therapeutic amputation of the hand in monkeys.

Reorganization of somatosensory cortex after peripheral nerve damage typically has been attributed to cortical plasticity. Here we provide evidence that much of the large-scale cortical reorganization that occurs after a major loss of peripheral inputs reflects the sprouting or expansion of afferents from the remaining forelimb into deprived territories of the spinal cord and brainstem. We examined sensory afferent terminations in the spinal cord and brainstem, and determined the somatotopic organization of cortical area 3b in three adult monkeys with previous hand or forearm amputation, as ve

Neuromuscular Properties of the Human Wrist Flexors as a Function of the Wrist Joint Angle.

The joint angle dependence of voluntary activation and twitch properties has been investigated for several human skeletal muscles. However, although they play a key role for hand function and possess a unique neural control compared to muscles surrounding other joint complexes, little is known about the wrist flexors innervated by the median nerve. Therefore, isometric voluntary and electrically evoked contractions of the wrist flexors were analyzed at three wrist joint angles (extension: -30°, neutral: 0°, flexion: 30°) to quantify the joint angle dependence of (i) voluntary activation (asses

The multifaceted role of B cells in traumatic spinal cord injury: Antibody-dependent and antibody-independent mechanisms.

A traumatic spinal cord injury (SCI) is caused by an acute insult to the spinal cord that inflicts severe damage to the nerve tissue and leads to impaired motor, sensory, and autonomic functions. After the initial injury, a secondary injury phase occurs, which is characterized by an excessive inflammatory response mediated by resident and infiltrating peripheral immune cells that accumulate within the spinal cord lesion. Increasing evidence supports the involvement of B cells in this secondary injury phase post-SCI. Elevated B cell numbers have been reported following experimental SCI, both wi

Jun N-terminal kinase's role in neuropathic pain: C-fiber remodeling and growth-associated protein-43 phosphorylation.

INTRODUCTION: Peripheral nerve injury (PNI) often cause neuropathic pain and induces morphological changes in C-fiber terminals. Concurrently, PNI increased growth-associated protein-43 (GAP-43) levels and activated JNK in the injured dorsal root ganglion. A previous study demonstrated that JNK phosphorylates GAP-43 at S96 in growth cones, which are often considered a model for axon elongation. OBJECTIVES: The aim of this study was to investigate whether PNI-induced JNK is involved in the phosphorylation of GAP-43 and in the morphological changes of injured C-fiber terminal in dorsal horn. Pre-Injection of Small Interfering RNA (siRNA) Promotes c-Jun Gene Silencing and Decreases the Survival Rate of Axotomy-Injured Spinal Motoneurons in Adult Mice.

Brachial plexus injury is a common clinical peripheral nerve trauma. A series of genes in motoneurons were activated in the corresponding segments of the spinal cord after brachial plexus roots axotomy. The spatial and temporal expression of these genes directly affects the speed of motoneuron axon regeneration and precise target organ reinnervation. In a previous study, we observed the overexpression of c-Jun in motoneurons of the spinal cord ventral horn after brachial plexus injury in rats. However, the relevance of c-Jun expression with respect to the fate of axotomy-induced branchial plex

Brachial plexus palsy following proximal humerus fracture in patients who are skeletally immature.

From 1998 to 2006, 578 patients who were skeletally immature with proximal humerus fractures were treated at our institution. During that time period, 4 patients (0.7%) had associated brachial plexus and major peripheral nerve palsies. Average age at the time of injury was 12.3 years (range 10-14 years). Two fractures were physeal and 2 were metaphyseal. In all patients, the distal fracture fragment was displaced into the axilla, with resultant adduction and valgus malalignment at the time of injury. Two patients were treated with closed reduction and sling and swathe immobilization. One patie

Microglial Activation of GLP-1R Signaling in Neuropathic Pain Promotes Gene Expression Adaption Involved in Inflammatory Responses.

BACKGROUND: Neuropathic pain is a common chronic pain, which is related to hypersensitivity to stimulus and greatly affects the quality of life of patients. Maladaptive gene changes and molecular signaling underlie the sensitization of nociceptive pathways. We previously found that the activation of microglial glucagon-like peptide 1 receptor (GLP-1R) could potently relieve formalin-, bone cancer-, peripheral nerve injury-, and diabetes-induced pain hypersensitivity. So far, little is known about how the gene profile changes upon the activation of GLP-1R signaling in the pathophysiology of neu

Post-injury delivery of rAAV2-CNTF combined with short-term pharmacotherapy is neuroprotective and promotes extensive axonal regeneration after optic nerve trauma.

Recombinant adeno-associated viral (rAAV) vectors expressing neurotrophic genes reduce neuronal death and promote axonal regeneration in central nervous system (CNS) injury models. Currently, however, use of rAAV to treat clinical neurotrauma is problematic because there is a delay in the onset of transgene expression. Using the adult rat retina and optic nerve (ON), we have tested whether rAAV gene therapy administered at the time of injury combined with short-term pharmacotherapy has synergistic effects that enhance neuronal survival and regeneration. The ON was transected and a 1.5 cm segme
