Broader autism phenotype, the default mode network, and brain entropy: A network-level hypothesis.

Despite well-documented disparities by sexual and gender minority (SGM) status in suicide attempt and mortality rates, few studies have investigated the lived experiences that contribute to SGM people's disproportionate risk of suicide. Having a history of at least one near-fatal (or medically serious) suicide attempt serves as a proxy for suicide mortality, but no known study has involved SGM people who have made such an attempt. Ideation-to-action theories of suicide posit that individuals acquire the capability for suicide through repeated exposure to painful and provocative events - namely

Co-expressed MicroRNAs Associated with An Elevated Psychometabolic Risk Phenotype in Women during Midlife.

INTRODUCTION: The bidirectional relationship between depression and type 2 diabetes (T2D) is well-established. Women are disproportionately affected by their co-occurrence, particularly during midlife, yet sex- and age-specific studies on phenotypic and mechanistic factors underlying risk for their co-occurrence are limited. The purpose of this study was to identify combined risk profiles (i.e., depression, T2D) in women during midlife and to determine if microRNAs (miRs) that are associated with high-risk profiles provide mechanistic insights into multimorbidity. MATERIALS AND METHODS: This

A Phenotype-Driven Discovery of Pro-Revascularization Chalcone Derivatives Using Zebrafish and CAM Models.

Angiogenesis is crucial for tissue repair and the treatment of ischemic diseases, yet effective small-molecule proangiogenic agents are lacking in clinical practice. Chalcone-based compounds are of interest due to their diverse biological activities. This study aimed to synthesize and evaluate the proangiogenic potential of a series of novel chalcone derivatives and to elucidate their structure-activity relationship (SAR) using computational chemistry. We synthesized and tested six chalcone derivatives (1a-1f), with compound (E)-1-(3,4-dimethoxyphenyl)-3-(3-hydroxy-4-methoxyphenyl)prop-2-en-1-

Age-Associated YBX1 Phosphorylation Regulates the Keratinocyte Senescence-Associated Secretory Phenotype Through Translational Control.

Skin aging is characterized by epidermal atrophy, reduced keratinocyte proliferation, and the accumulation of senescent cells that sustain a chronic, low-grade inflammatory secretome known as the senescence-associated secretory phenotype (SASP). We previously showed that Y-box binding protein 1 (YBX1) limits keratinocyte senescence in human epidermis by acting as a translational repressor of SASP chemokines, including CXCL1 and IL8. How this brake is regulated during skin aging, however, remains undefined. Here we demonstrate that, although total YBX1 protein is reduced in keratinocytes from a

Expanding the Neurological Phenotype of ZTTK Syndrome: A Case of Recurrent Acute Confusional Migraine.

Acute confusional migraine (ACM) is an uncommon pediatric migraine variant characterized by transient confusion and agitation, nonspecific electroencephalographic changes, and normal bloodwork and neuroimaging. We present the first case of ACM in a child with Zhu-Tokita-Takenouchi-Kim (ZTTK) syndrome (OMIM: 617140), a rare multisystem disorder caused by a pathogenic variant in the SON gene, clinically characterized by global developmental delay, intellectual disability, hypotonia, distinctive craniofacial dysmorphic features, and structural brain anomalies. Emerging evidence suggests that SON

Functional Data Strengthen Clinical Validation of PhenoScore Phenotype-Guided AI for ANKRD11 Missense Variants.

KBG syndrome (KBGS, OMIM #148050) is a rare genetic disorder caused by heterozygous truncating or missense variants in the ANKRD11 gene or a deletion of 16q24.3 involving ANKRD11. While truncating variants clearly disrupt protein function, the interpretation of missense variants is more challenging, as many remain variants of uncertain significance (VUS). To address this, we evaluated PhenoScore, an open-source AI-based phenomics framework integrating facial recognition and medical data analysis, for predicting the pathogenicity of ANKRD11 missense variants and providing supporting evidence fo

Novel associations of VPS13C with phenotype and conversion of idiopathic REM sleep behavior disorder.

While VPS13C is a recessively inherited Parkinson's disease (PD) gene, its potential dominant effects in idiopathic Rapid-eye movement (REM) sleep behavior disorder (iRBD) remain unexplored. The relation between its monogenic form and the onset of PD suggested that subtype specificity may need to be considered. We examined the presence of likely pathogenic VPS13C variants in 150 iRBD and 180 α-synucleinopathy patients (iRBD-first and movement disorder-first). VPS13C variants were significantly enriched in iRBD patients, and ten iRBD risk variants have been identified. iRBD risk VPS13C variant

Biallelic XPR1 Variants Are Linked to Brain Calcifications, Developmental Delay, Hypophosphatemia, and Cardiopulmonary Phenotype.

XPR1 encodes the only known phosphate exporter in human cells and regulates the export of inorganic phosphate (Pi). To date, heterozygous pathogenic variants have been reported to be associated with autosomal-dominant idiopathic basal ganglia calcification-6 (IBGC6; MIM: 616413). We collate clinical information on individuals with biallelic XPR1 variants and use in silico and cell-based studies to evaluate pathogenicity. Four consanguineous families have been documented, with a severe neonatal phenotype presenting with persistent pulmonary hypertension, chronic lung disease, cardiomyopathy, an

Further delineation of the phenotype and genotype in a newly identified PAN2-related disorder.

Regulation of mRNA decay is a critical mechanism for effective gene expression. Poly(A) tail length is directly associated with mRNA stability, and deadenylation, which shortens poly(A) tails, plays a central role in mRNA regulation. PAN2 encodes a key component of the deadenylation machinery. Recently, loss-of-function variants in PAN2 have been associated with neurodevelopmental delay and multiple congenital anomalies in seven patients from five unrelated families. Here, we report two siblings presenting with neurodevelopmental delay and multiple congenital anomalies consistent with previous

Neonatal Presentation of 49,XXXXY (Fraccaro) Syndrome with Ventriculomegaly: Expanding the Early Neuroimaging Phenotype.

49,XXXXY syndrome (Fraccaro syndrome) is a rare sex chromosome pentasomy, historically considered a severe variant within the Klinefelter spectrum. It is characterized by intellectual disability, craniofacial dysmorphism, skeletal anomalies, hypogonadism, and congenital cardiac defects. Although neuroimaging abnormalities have increasingly been recognized in 49,XXXXY syndrome, neonatal diagnosis prompted primarily by ventriculomegaly remains rare. We report a neonate with prenatally detected ventriculomegaly in whom postnatal evaluation revealed cleft palate, congenital cardiac defects, bilate

Characterization of Kidney and Liver Cystic Phenotype Associated with GANAB Using Advanced Imaging Biomarkers.

BACKGROUND: Monoallelic pathogenic variants in GANAB cause autosomal dominant cystic kidney and liver disease, but quantitative imaging phenotypes remain incompletely defined. METHODS: We performed a retrospective study of 16 individuals with GANAB variants and available abdominal imaging. Deep learning-based cyst segmentation quantified kidney and liver volumes and cyst metrics, including height-adjusted total kidney volume (htTKV), height-adjusted total liver volume, total cyst number (TCN), and height-adjusted total cyst volume. RESULTS: Hepatic involvement was common, with polycystic liv

GLP-1 receptor agonists in ADPKD: from metabolic rationale to phenotype-enriched translational testing.

Autosomal dominant polycystic kidney disease (ADPKD) remains therapeutically anchored to vasopressin V2-receptor antagonism, yet progression heterogeneity and persistent unmet need increasingly suggest residual disease biology beyond cAMP-centered control. Converging experimental and observational human phenotype data suggest that metabolic reprogramming, mitochondrial dysfunction, impaired fatty-acid oxidation, obesity, and visceral adiposity may modify cyst growth, kidney-volume expansion, eGFR decline, or treatment-response heterogeneity, although causal and therapeutic evidence remains inc

Expanded Phenotype Associated With an Intronic PPP1R12A Variant: A Case Report and Literature Review.

Autosomal dominant PPP1R12A-related genitourinary and/or brain malformation syndrome is a recently described multisystem disorder caused by loss-of-function variants in the protein phosphatase 1 regulatory subunit 12a (PPP1R12A) gene. To date, 22 affected individuals have been reported with variable brain malformations and genitourinary anomalies, including differences of sexual development. Of these cases, four probands had anomalies involving both organ systems. Here, we report a 12-year-old individual with global developmental delay, semi-lobar holoprosencephaly, microcephaly, spastic quadr

Excessive erythrocytosis and the hypertensive phenotype at high altitude: emerging evidence and unresolved questions.

Systemic hypertension at high altitude is clinically important but mechanistically heterogeneous. Excessive erythrocytosis (EE), commonly discussed in relation to high-altitude polycythemia (HAPC) or the erythrocytotic component of chronic mountain sickness (CMS), is one of the most visible maladaptive responses to chronic high-altitude hypoxia. Emerging human studies suggest that erythrocytosis burden may be associated with the hypertensive phenotype at high altitude. Here, we critically evaluate the evidence linking EE and systemic hypertension and discuss the principal mechanistic pathways

Identifying a Refractory Shock Phenotype in Pediatric Sepsis Using a Vasoactive-Adjusted Shock Index.

OBJECTIVES: This study aimed to: 1) develop a cardiovascular index that includes hemodynamic status and adjusts for vasoactive medication use; 2) identify pediatric patients with refractory septic shock (RSS) using early trends of the adjusted index, and 3) assess the heterogeneity of treatment effect (HTE) to common adjuvant therapies in RSS versus non-RSS patients. METHODS: Children with sepsis across 13 US pediatric intensive care units between 2012 and 2018 were included. The vasoactive-adjusted shock index (VASI) was derived using the shock index (heart rate/systolic pressure) and adjust

Catastrophic acute haemolytic reaction: The discovery of first Bombay phenotype in Iraq /case report.

BACKGROUND: The Bombay (Oh) phenotype results from homozygous FUT1 loss-of-function (hh), causing complete absence of H antigen on red blood cells and preventing expression of A and B antigens. Because routine forward and reverse grouping can mimic group O, affected individuals may be transfused with standard group O units, risking severe hemolysis due to naturally occurring anti-H. CASE: A 31-year-old man in Baghdad presented with massive traumatic hemorrhage and received an uncrossmatched leukoreduced group O RhD-negative packed red cell unit. Within 10 min he developed fever, rigors, hypot

Concussion pathophysiology: From biomechanical insult to clinical phenotype - or is injury truly the beginning?

Mild traumatic brain injury (mTBI) is associated with substantial morbidity worldwide. Emerging evidence demonstrates that both impact- and blast-related mTBI produce diffuse microstructural and functional alterations, e.g., diffuse axonal injury, astroglial and microglial activation, cerebrovascular dysfunction, and neurometabolic disturbance. Rotational acceleration in impact injury preferentially induces white matter shear. Blast overpressure exerts prominent effects at cerebrovascular interfaces, particularly within perivascular spaces (PVS). These biomechanical differences may yield disti
