Screening for prostate cancer. Role of ultrasonography.

Over the next 10 years, prostate cancer will be the cause of death in a large number of men. Perhaps the most feasible approach to reducing this mortality is early detection. The initial studies with transrectal ultrasonography demonstrate that this test is capable of detecting prostate cancers that are not detected by the digital rectal examination. A reasonable conclusion from these data is that every man over age 50 should have a sonogram as part of his annual check-up. Unfortunately, routine screening may also lead to serious morbidity, yet it may not lower the mortality rate from this dis

Creatine kinase isoenzyme (CK-BB) by RIA as a follow-up marker in stage D cancer of the prostate: comparison with PAP RIA.

Fifty-nine patients with stage D carcinoma of the prostate under different modalities of treatment were studied for creatine phosphokinase isoenzyme BB (CK-BB) and prostatic acid phosphatase (PAP) levels in serum by radioimmunoassay (RIA) technique, in order to study the possible use of CK-BB as a follow-up marker compared to PAP. Thirty-three patients were in stable, 19 in progressive, and seven in regressive clinical state. CK-BB was above normal level in 52 (88%) out of 59 patients with no statistically significant difference between the three clinical states. On the other hand, PAP was abo

Androgen-STAT3 activation may contribute to gender disparity in human simply renal cysts.

BACKGROUND: Simple renal cysts (SRC) are a common urological disease mostly in elderly, however the male-to-female ratio was 2.81. Androgen receptor (AR) activation was initially proposed as a vital signaling pathway in prostate cancer and consequent signal transducer and activator of transcription 3 (STAT3)-AR complex led an important putative mechanism by which prostate cells are sensitized with growth factor signals. However, in SRC disease, no related study emerged. METHODS: 30 patients with SRC and 20 age-matched healthy controls were recruited. Puncture biopsy was performed to acquire c

Unraveling 50-Year-Old Clues Linking Neurodegeneration and Cancer to Cycad Toxins: Are microRNAs Common Mediators?

Recognition of overlapping molecular signaling activated by a chemical trigger of cancer and neurodegeneration is new, but the path to this discovery has been long and potholed. Six conferences (1962-1972) examined the puzzling neurotoxic and carcinogenic properties of a then-novel toxin [cycasin: methylazoxymethanol (MAM)-β-d-glucoside] in cycad plants used traditionally for food and medicine on Guam where a complex neurodegenerative disease plagued the indigenous population. Affected families showed combinations of amyotrophic lateral sclerosis (ALS), parkinsonism (P), and/or a dementia (D)

The regulatory mechanism and clinical significance of RNA editing in prostate cancer.

Prostate cancer (PCa) is one of the most common malignancies in men and remains particularly challenging as it progresses to the castration-resistant stage. Increasing evidence indicates that dysregulated RNA editing plays an active role in PCa progression and resistance to therapy. Among these modifications, adenosine-to-inosine editing catalyzed by adenosine deaminase acting on RNA 1 (ADAR1) alters transcripts, such as androgen receptor (AR) and antizyme inhibitor 1, promoting androgen-independent growth, metastasis, and immune evasion. Conversely, cytidine-to-uridine editing mediated by apo

Assessment of Presence and Metastatic Involvement of Lymph Nodes in Anterior Periprostatic Fat (APPF) in Prostate Cancer Patients Treated with Robotic and Laparoscopic Radical Prostatectomy.

Introduction: Lymph nodes (LN) in the anterior periprostatic fat (APPF) may harbour metastatic disease in patients with Prostate Cancer (PCa). We investigated the incidence and significance of LN in APPF tissue removed during robotic and laparoscopic radical prostatectomy (RP). Patients and Methods: We retrospectively analysed RP performed by a single surgeon from 2013 to 2023. A total of 670 patients underwent RP, with 407 procedures conducted laparoscopically and 263 robotically. Histological results were available for 509 patients, who were examined for the presence of LN and any evidence o

PSMA Therapy Grey Zones: Actinium, Lutetium, or Terbium for the Future of Radioligand therapy?

BACKGROUND: Radioligand therapy targeting the prostate-specific membrane antigen (PSMA) has been established as a key treatment for PSMA-positive metastatic castration-resistant prostate cancer following the approval of [177Lu]Lu-PSMA-617 (177Lu). However, variable responses, resistance, uncertain treatment sequencing, and limited efficacy in selected biological settings remain major clinical challenges. This narrative perspective reviews the rationale for radionuclide selection in PSMA-targeted therapy. EVIDENCE ACQUISITION: A nonsystematic review of PubMed/MEDLINE and EMBASE was performed,

Contextualizing the Utility of Polygenic Risk Scores using Absolute Risk Models in Diverse Ancestry Populations.

Polygenic risk scores (PRSs) are emerging as powerful tools for quantifying inherited risk for common diseases and, in some cases, are approaching clinical implementation. A major concern for PRS implementation is their limited accuracy in non-European populations, particularly in those of African ancestry. However, past evaluations have focused on metrics such as relative risk or AUC, which do not capture background risk arising from contextual factors. We introduce a novel measure of variable importance, the conditional average derivative estimator (CADE), to evaluate PRS utility across dive

Unmasking prostate cancer risk factors leveraging organoid technology.

Organoid technology is rapidly refining our understanding of organ development, physiology, and disease. The generation of patient-derived organoid cultures is paving the way for effective personalized treatments, defined based on specific genetic and molecular criteria. Moreover, the possibility of testing in vitro the response of tumor cells to the most appropriate therapeutic regimes helps to anticipate the emergence of resistance, to define the molecular mechanisms involved and, in turn, to tailor treatment through a bed-to-bench-to-bed co-clinical strategy. In addition, the creation of or

Pro-neuropeptide Y as a circulating biomarker for poor prognosis in prostate cancer.

Prostate cancer (PCa) is common world-wide. Current diagnostics based on testing for circulating levels of prostate specific antigen (PSA) is unspecific, and novel prognostic markers are needed for personalized therapeutic strategies. Pro-neuropeptide Y (pro-NPY) has been reported as a tissue marker for PCa related to poor prognosis. This study explored the prognostic value of circulating pro-NPY in PCa. Plasma samples were obtained from two patient cohorts: (1) men examined due to increased PSA levels in 2003-2011 (n = 796) and (2) patients treated for PCa in 2013-2016 (n = 92). Cohort 2 also

TMPRSS2: A Key Host Factor in SARS-CoV-2 Infection and Potential Therapeutic Target.

The transmembrane serine protease 2 (TMPRSS2) gene plays a crucial role in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection by priming the viral spike protein for membrane fusion and facilitating viral entry into host cells. This review aims to explore the molecular function of TMPRSS2, its genetic variations, and its potential as a therapeutic target in corona virus disease 2019 (COVID-19) and other respiratory viral infections. TMPRSS2 is highly expressed in lung and prostate tissues and is regulated by androgens, which may contribute to sex-based differences in COVID-1

Understanding Racial Disparities in Prostate Cancer: A Multifaceted Approach.

Prostate cancer (PCa) remains a significant public health challenge in the United States, disproportionately affecting African American (AA) men, who face higher incidence rates, more aggressive disease, and elevated mortality compared to Caucasian American (CA) men. This review explores the multifactorial underpinnings of these disparities, integrating genomic, socioeconomic, environmental, and systemic contributors. Genomic analyses reveal that AA men harbor distinct molecular alterations, including higher frequencies of FOXA1, BRAF, and CHD1 mutations, as well as DNA damage repair defects,
