Role of Ayurvedic Principles in Addressing Malnutrition and Non-Communicable Diseases in Low-Resource Settings.

Non-communicable diseases (NCDs) and malnutrition constitute a double burden in low-resource settings, where high out-of-pocket expenditure, inconsistent supply chains, and poor long-term adherence reduce the sustained impact of conventional biomedical interventions. This review critically examines Ayurvedic principles as a complementary, systems-based framework for addressing shared nutritional and metabolic determinants underlying both conditions. Drawing on conceptual foundations and available empirical evidence, the analysis evaluates how dietary regulation, digestive optimisation, lifesty

Beyond the Pelvis: Brain Metastases in Cervical Cancer - A Case Series from a Low-Middle-Income Country.

Brain metastases from cervical cancer are exceedingly rare and pose significant clinical challenges, particularly in low- and middle-income countries (LMICs), where access to advanced diagnostics and therapies may be limited. We report two cases of cervical carcinoma with brain metastases managed at a tertiary center in Indonesia, each illustrating distinct histopathologic subtypes and metastatic trajectories. The first case involved a 31-year-old woman with a grade II neuroendocrine carcinoma who developed intracranial recurrence 11 months after achieving a complete response to concurrent che

Pseudohypoparathyroidism Presenting With Recurrent Twitching: Challenges Making a Diagnosis in a Low-Resource Environment.

Pseudohypoparathyroidism (PHP) is a metabolic disorder that occurs due to target end-organ resistance to parathyroid hormone (PTH). It is a rare cause of severe symptomatic hypocalcemia as it characteristically manifests with high phosphate and low calcium. The clinical presentation, biochemical features, and severity vary from patient to patient leading to delay in diagnosis. Reduced awareness and lack of recognition of this rare clinical syndrome coupled with limited resources in rural health facilities also contribute to missed or late diagnosis. Reported here is a case of a 13-year-old gir

Pediatric transplantation in Europe during the COVID-19 pandemic: Early impact on activity and healthcare.

The current pandemic SARS-CoV-2 has required an unusual allocation of resources that can negatively impact chronically ill patients and high-complexity procedures. Across the European Reference Network on Pediatric Transplantation (ERN TransplantChild), we conducted a survey to investigate the impact of the COVID-19 outbreak on pediatric transplant activity and healthcare practices in both solid organ transplantation (SOT) and hematopoietic stem cell transplantation (HSCT). The replies of 30 professionals from 18 centers in Europe were collected. Twelve of 18 centers (67%) showed a reduction i

Monotherapy in an era of combination therapy: is there a benefit? Experience in HIV-1-infected symptomatic South African children.

We investigated the benefit of treating HIV-1-infected children with monotherapy where resources are limited. A retrospective chart review was undertaken in 12 symptomatic HIV-1-infected children treated with zidovudine or didanosine for at least 2 months. The main outcome measure was the effect on hospitalization. Anti-retroviral therapy was commenced in nine children because of prolonged or frequent hospitalization. Of three whose primary indication was bleeding secondary to thrombocytopenia, two had been hospitalized owing to severe intercurrent illness. One child had failure to thrive and

Scale-up evaluation results of a multi-level approach using mailed immunochemical test (FIT) and patient navigation to increase colorectal cancer screening rates in rural clinic settings.

BACKGROUND: Colorectal cancer (CRC) screening effectively reduces CRC mortality and morbidity, yet screening rates remain below US national targets in rural and low-resourcedd settings. An effective intervention to improve screening is mailed fecal immunochemical tests (FIT) combined with patient navigation. However, these multicomponent interventions have been unevenly adopted across health systems and organizations, and studies of how to spread effective system-level approaches are limited. METHODS: We conducted a mixed-methods evaluation of a multi-component scale-up approach designed to i

Epidemiology of cesarean delivery: frequency, indications, related morbidity, and evidence-based strategies for optimal use.

Cesarean delivery is a complex issue, involving medical, social, economic, and ethical dimensions. Cesarean delivery rates have steadily increased worldwide since 1990. Yet, important disparities exist between and within countries, with cesarean delivery rates ranging from less than 3% to more than 55% between 2000 and 2015. These differences often reflect inequities in access to obstetric care-particularly in rural or low-resource settings-and socioeconomic divides. In many low-income areas, underuse is driven by limited access to surgical services and geographical and financial barriers. In

A systematic review of psychosocial interventions for children affected by armed conflict: mapping what works, for whom, and under what conditions.

BACKGROUND: Armed conflict exposes children to prolonged and multifaceted adversity, including violence, displacement, bereavement, family disruption, poverty, insecurity, and disrupted schooling. Although research on psychosocial interventions for conflict-affected children has grown, the evidence remains heterogeneous, and causal conclusions are often difficult to distinguish from feasibility, contextual, or implementation findings. OBJECTIVE: This systematic interpretive mapping review aimed to synthesize evidence on psychosocial interventions for children affected by armed conflict by map

Routine uptake of prenatal iron-folic acid supplementation and associated factors among pregnant women in peri-urban areas of Dodoma City, Tanzania: a cross-sectional study.

BACKGROUND: The physiological requirements for iron and folic acid in pregnancy are a significant challenge to achieve through normal dietary intake, especially in low resource settings. The World Health Organization recommends daily oral iron and folic acid supplementation (IFAS) to prevent maternal anaemia and related adverse effects in community settings where the prevalence of anaemia during pregnancy is > 40%. The objective of this study was to assess the routine uptake of prenatal iron-folic acid supplementation and associated factors among pregnant women at peri-urban areas of Dodoma Ci

Access to transcatheter aortic valve implantation: interregional variability and expert evaluation.

INTRODUCTION AND OBJECTIVES: Transcatheter aortic valve implantation (TAVI) has revolutionised the treatment of severe symptomatic aortic stenosis, providing an alternative to surgical valve aortic replacement, especially in high-risk patients. Despite its benefits, significant interregional variability in TAVI access persists within Spain. This study aimed to analyse disparities in TAVI implementation across different autonomous communities, identifying the key factors underlying this variability. METHODS: We conducted a retrospective observational study using data from the Spanish National
