Coronavirus disease 2019 respiratory failure: what is the best supportive care for patients who require ICU admission?

PURPOSE OF REVIEW: Currently, there is no cure for SARS-CoV-2 infection, yet hospital mortality rates for COVID-19 have improved over the course of the pandemic and may be due in part to improved supportive care in the ICU. This review highlights the evidence for and against various ICU supportive therapies for the treatment of critically ill patients with COVID-19. RECENT FINDINGS: Early in the pandemic, there was great interest in novel ICU supportive care, both for the benefit of the patient, and the safety of clinicians. With a few exceptions (e.g. prone ventilation of nonintubated patien

Mortality and clinical outcomes in patients with COVID-19 pneumonia treated with non-invasive respiratory support: A rapid review.

INTRODUCTION AND AIM: Non-invasive ventilation (NIV) and continuous positive airway pressure (CPAP) have been widely employed to treat acute respiratory failure secondary to COVID-19 pneumonia, but their role in terms of efficacy and safety are still debated. The aim of this review was to analyse mortality and intubation rates in COVID-19 patients treated with NIV/CPAP. METHODS: Rapid review methodology was applied to include all the studies published since December-2019 until November-2020 with available data on in-hospital mortality in COVID-19 patients treated with NIV or CPAP. RESULTS: 2

Neurally Adjusted Ventilatory Assist in Newborns.

Patient-ventilator asynchrony is very common in newborns. Achieving synchrony is quite challenging because of small tidal volumes, high respiratory rates, and the presence of leaks. Leaks also cause unreliable monitoring of respiratory metrics. In addition, ventilator adjustment must take into account that infants have strong vagal reflexes and demonstrate central apnea and periodic breathing, with a high variability in breathing pattern. Neurally adjusted ventilatory assist (NAVA) is a mode of ventilation whereby the timing and amount of ventilatory assist is controlled by the patient's own n

Acute effects of ambient air pollution on outpatients with chronic rhinitis in Xinxiang, China.

Air pollution exposure leads to increased mortality and morbidity rates of respiratory diseases. Most of the evidence was founded on acute diseases such as acute lower respiratory diseases. However, limited studies have been conducted to evaluate the effects of air pollution on chronic respiratory diseases. This time-series study was conducted to examine the acute effects of 6 criteria ambient air pollutants on hospital outpatients with chronic rhinitis (CR) in Xinxiang, China. We retrieved 223,826 outpatient records of patients with respiratory diseases, of which 62,901 were those of patients

Percutaneous nocturnal oximetry in amyotrophic lateral sclerosis: periodic desaturation.

Percutaneous nocturnal oximetry (PNO) is useful to screen respiratory function in amyotrophic lateral sclerosis (ALS). PNO recordings of some patients disclose a periodical pattern of O(2) desaturation (PP), whose significance is unknown. We aimed to characterize PP pattern, and we used a prospective study enrolling 261 consecutive ALS patients. Clinical, pulmonary and neurophysiological tests performed included: ALS functional rating scale, forced vital capacity (FVC), maximal inspiratory pressure (PImax), mouth occlusion pressure (MOP), phrenic nerve motor response, needle electromyography o

Review of ECMO (extra corporeal membrane oxygenation) support in critically ill adult patients.

Mechanical circulatory support has evolved markedly over recent years. ECMO (extra corporeal membrane oxygenation) is instituted for the management of life threatening pulmonary or cardiac failure (or both), when no other form of treatment has been or is likely to be successful. Most commonly, it is instituted in an emergency or urgent situation after failure of other treatment modalities. It is used as temporary support, usually awaiting recovery of organs, or can be used as a bridge to a more permanent device or cardiac transplantation. ECMO can be deployed in a veno-arterial configuration (

Interpretable Machine Learning Model Using Oxygenation and Respiratory Variability to Predict Hemorrhagic Stroke Mortality: A Multicenter Validation Study.

BACKGROUND: Hemorrhagic stroke has high in-hospital mortality, but early risk prediction remains suboptimal. While prior studies have focused on ventilation strategies and mechanical support, limited research has explored the prognostic significance of oxygenation status or hyperoxia. This may be because single-point measurements often fail to capture the dynamic instability of oxygenation and respiratory status during early ICU care. To address this gap, we aimed to evaluate the prognostic value of oxygenation and respiratory variability and incorporate it into an interpretable machine learni

Targeted next-generation sequencing analysis of pathogens and microecology in pediatric lower respiratory tract infections identifies risk factors for severe community-acquired pneumonia.

BACKGROUND: Traditional diagnostic methods have inherent limitations in the comprehensive assessment of the etiological spectrum and microecological characteristics of pediatric lower respiratory tract infections (LRTIs), particularly community-acquired pneumonia (CAP). Against this backdrop, the present study seeks to delineate the pathogen profile of children with LRTIs via targeted next-generation sequencing (tNGS), and further explore the associations between clinical manifestations, upper respiratory microbiome signatures and disease severity in pediatric CAP cases. METHODS: A retrospect

Cardiac Mitochondrial Dysfunction Following Bleomycin-Induced Acute Lung Injury in Rats.

BACKGROUND: Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are frequently associated with cardiac complications, including myocardial injury and right ventricular dysfunction. However, the mechanisms linking pulmonary injury to cardiac dysfunction remain incompletely understood. In this study, we investigated ventricular mitochondrial respiratory function during the acute phase of bleomycin-induced ALI. METHODS: ALI was induced in male and female rats by intratracheal bleomycin (2.5 mg/kg); saline served as a control. Circulating cardiac troponin I (cTnI) was measured

Aminopyridines Restore Ventilation and Reverse Respiratory Acidosis at Late Stages of Botulism in Mice.

Botulinum neurotoxin (BoNT) is a potent protein toxin that causes muscle paralysis and death by asphyxiation. Treatments for symptomatic botulism are intubation and supportive care until respiratory function recovers. Aminopyridines have recently emerged as potential treatments for botulism. The clinically approved drug 3,4-diaminopyridine (3,4-DAP) rapidly reverses toxic signs of botulism and has antidotal effects when continuously administered in rodent models of lethal botulism. Although the therapeutic effects of 3,4-DAP likely result from the reversal of diaphragm paralysis, the correspon
