Fungal granuloma of the sphenoid sinus and clivus in a patient presenting with cranial nerve III paresis: case report rand review of the literature.

OBJECTIVE AND IMPORTANCE: Isolated fungal granulomas originating within the sphenoid sinus are extremely rare in immunocompetent patients. In their symptoms and morphological appearance, these lesions may be mistaken for pituitary tumors. We report such a case and review the literature. CLINICAL PRESENTATION: A 74-year-old man presented with a 3-week history of Cranial Nerve III paresis. The patient had a long-term history of snuff abuse. Computed tomography demonstrated a space-occupying lesion of the sellar and sphenoid sinus region with displacement of the cavernous sinus. INTERVENTION: T

Recovery of hearing after removal of a large jugular foramen schwannoma: report of two cases.

BACKGROUND: Although hearing loss is a common presenting symptom of jugular foramen schwannoma (JFS), recovery of hearing after tumor removal has rarely been reported. We report two cases of a large JFS presenting with severe hearing loss and recovering normal hearing after tumor removal. METHODS AND RESULTS: Two patients complaining of progressive hearing loss each proved to have a large JFS involving the posterior fossa. The hearing losses, which exceeded 90 dB, were not associated with dysfunction of the 9th, 10th, or 11th cranial nerves. Schwannomas were removed by a suboccipital retrosig

Pattern of malformations in the axial skeleton in human trisomy 13 fetuses.

The purpose of this study was to analyse the development of the axial skeleton in human trisomy 13 fetuses and to define which fields in the axial skeleton are affected in this condition. We investigated nine human fetuses with trisomy 13 and gestational ages of 14-19 weeks. Whole body radiographs and radiographs of midsagittal tissue blocks of the cranial base and the spine were studied. In the youngest fetus, 14 w GA, no malformations were observed. In eight fetuses, 17-19 weeks GA, malformations occurred in the lumbosacral spine. In four fetuses additional malformations were observed in the

Image-stitching artefacts and distortion in CCD-based cephalograms and their association with sensor type and head movement: ex vivo study.

OBJECTIVES: To assess presence and severity of image-stitching artefacts and distortion in lateral cephalograms acquired by CCD-based sensors and their association with movement. METHODS: A human skull was mounted on a robot simulating five head movement types (anteroposterior translation/lifting/nodding/lateral rotation/tremor), at three distances (0.75/1.5/3 mm), based on two patterns (skull returning/not returning to the initial position, except for tremor). Three cephalometric units, two ProMax-2D (Planmeca Oy, Finland), one with Dimax-3 (D-3) and one with Dimax-4 (D-4) sensor, and one Or

Comparison between MR and CT imaging used to correct for skull-induced phase aberrations during transcranial focused ultrasound.

Transcranial focused ultrasound with the InSightec Exablate system uses thermal ablation for the treatment of movement and mood disorders and blood brain barrier disruption for tumor therapy. The system uses computed tomography (CT) images to calculate phase corrections that account for aberrations caused by the human skull. This work investigates whether magnetic resonance (MR) images can be used as an alternative to CT images to calculate phase corrections. Phase corrections were calculated using the gold standard hydrophone method and the standard of care InSightec ray tracing method. MR bi

A retrospective study of more than 400 feline nasal biopsy samples in the UK (2006-2013).

OBJECTIVES: The main objective of this study was to utilise a large database from a UK-based, commercial veterinary diagnostic laboratory to ascertain the prevalence of different forms of nasal disease within the feline population. Further objectives included using this database to detect any breed, sex or age predilections, or associations between the degree of brachycephalism, and the different conditions diagnosed. METHODS: Records from the laboratory were searched for feline submissions received between 31 May 2006 and 31 October 2013. For all samples taken from the nasal cavity, the diag

A facial growth analysis based on FEM employing three dimensional surface measurement by a rapid laser device.

Although there have been several studies of finite element method (FEM) analysis on two dimensional (2D) facial growth with cephalometric X-rays, there has been little FEM analysis on three dimensional (3D) facial growth of long term observation. Therefore the objective of our study is to use FEM model by 3D surface measurement of rapid laser device from human dried skull and to analyze the changes of facial growth based on FEM by the volume and the direction of strain in each stage. Samples were taken from each human dried skull for 5 stages; about 0, 3, 6, 9, 12 and 18 years of age, a total

Prediction of personalized therapeutic response in pediatric clivus chordoma: chemoresistance test.

Precision medicine has expanded therapeutic options for rare central nervous system tumors, yet actionable molecular targets remain limited in pediatric cases. Functional ex vivo drug-response assays may complement genomic approaches by identifying tumor-specific sensitivities. Clival chordoma is a rare, locally aggressive tumor generally considered chemotherapy-refractory, particularly in children. We report the prospective use of a functional drug-response platform (Bioverso Test) to guide treatment in a pediatric patient with recurrent metastatic clival chordoma. A four-year-old patient wit

Collision Tumor of the Clivus: Chordoma and Chronic Lymphocytic Leukemia.

Collision tumors, defined as the simultaneous occurrence of two distinct neoplasms within the same anatomical location, are exceptionally rare in the clivus. The coexistence of chordoma and chronic lymphocytic leukemia (CLL) within the clivus has not been previously reported, making this case particularly noteworthy. We present the case of a 69-year-old woman with a known history of stable CLL who presented with a 3-month history of progressive right-sided horizontal diplopia and hemianopsia. Imaging revealed a large sellar/suprasellar lesion with significant involvement of both cavernous sinus

Total Resection of Clival Chordoma with Brainstem Invasion via Endoscopic Transnasal Approach: Technical Note.

OBJECTIVE: Clival chordomas with brainstem invasion are extremely rare; only 5 cases have been reported in the literature. In such cases total tumor resection is associated with high intraoperative risks and is a notable challenge for the neurosurgeon. However, it is the quality of surgical treatment that influences the future prognosis, despite the efficacy of proton therapy. METHODS: We describe the clinical case of a 39-year-old male patient with the sudden onset of double vision and left trochlear nerve paresis, who was diagnosed with a clival chordoma with brainstem invasion. RESULTS: T

Using the anterior cranial base to provide a reliable reference plane for patients with or without facial asymmetry.

INTRODUCTION: This study aimed to investigate the midsagittal reference plane (MSP) reliability derived from the 3-dimensional characteristics of patients with or without facial asymmetry in the anterior cranial base (ACB). METHODS: We divided the cone-beam computed tomography (CBCT) images of 60 adult patients into maxillofacial symmetry and asymmetry groups. The ACB models were 3-dimensionally constructed, and then symmetrical characteristics were evaluated with surface asymmetry for each group. The reliability of the MSP derived from the symmetry of the anterior cranial base (MSPACB) was a

The Vascularized Occipital Fascial Flap (OFF): A Novel Reconstructive Technique for Posterior Fossa Surgery.

BACKGROUND: Posterior fossa surgery is particularly prone to cerebrospinal fluid (CSF) leakage. Several methods have been introduced to address and/or prevent this complication. However, to the best of our knowledge, the use of a vascularized fascial flap based on the occipital artery for the purpose of reconstruction has not been reported. We introduce the occipital fascial flap (OFF) for reconstruction of a craniectomy defect after the retrosigmoid approach. METHODS: A 57-year-old woman with a large cerebellar metastasis underwent gross total resection of the mass followed by reconstruction

Anterior Transpetrosal Approach for Petroclival Meningioma: Operative Nuances.

Petroclival meningiomas are unique neurosurgical challenges that require planning an approach to the retrosellar and upper and mid retroclival locations. The classical subtemporal-transtentorial approach went into disrepute due to excessive brain retraction. To resolve this challenge Buchenek and Kukwa introduced extradural approach with drilling of the petrous bone and called it the extended middle fossa approach. It was Kawase et al who modified the extended middle fossa approach initially for dealing with basilar trunk aneurysms. This anterior transpetrosal approach has been described below

Application of the Scale for the Assessment and Rating of Ataxia (SARA) in pediatric oncology patients: A multicenter study.

Posterior cranial fossa (PCF) tumors in childhood are often associated with ataxia as well as other motor, neurobehavioral and linguistic impairment. The use of a reliable outcome measure is mandatory to evaluate the severity of impairment and monitor rehabilitation effectiveness. The aim of this work is to explore the validity of the Scale for the Assessment and Rating of Ataxia (SARA) in pediatric subjects with ataxia secondary to PCF tumor resection and evaluate the influence of age and comorbidities. Seventy eight patients (3-18 years) were recruited in 5 centers from 2016 to 2018. The age

Comparison between VII-to-VII and XII-to-VII coaptation techniques for early facial nerve reanimation after surgical intra-cranial injuries: a systematic review and pooled analysis of the functional outcomes.

The surgical injury of the intracranial portion of the facial nerve (FN) is a severe complication of many skull base procedures, and it represents a relevant issue in terms of patients' discomfort, social interactions, risk for depression, and social costs. The aim of this study was to investigate the surgical and functional outcomes of the most common facial nerve rehabilitation techniques. The present study is a systematic review of the pertinent literature, according to the PRISMA guidelines. Two different online medical databases (PubMed, Scopus) were screened for studies reporting the fun

Beyond Tympanomastoidectomy: A Review of Less Common Postoperative Temporal Bone CT Findings.

Postoperative temporal bone imaging after surgical procedures such as ossiculoplasty, tympanomastoidectomy, cochlear implantation, and vestibular schwannoma resection is often encountered in clinical neuroradiology practice. Less common otologic procedures can present diagnostic dilemmas, particularly if access to prior operative reports is not possible. Lack of familiarity with the less common surgical procedures and expected postoperative changes may render radiologic interpretation challenging. This review illustrates key imaging findings after surgery for Ménière disease, superior semicirc
