The dual role of the orexin/hypocretin system in modulating wakefulness and respiratory drive.

PURPOSE OF REVIEW: Today, numerous studies show that orexin peptides act as regulators of many functions including the control of sleep-wake states, breathing, and central chemosensitivity. However, little is known on neuronal mechanisms by which orexin regulates breathing in a state-dependent manner. This review summarizes recent data on the control of neuronal circuits by orexin, with a special emphasis on breathing, central chemosensitivity, and obstructive sleep apneas. RECENT FINDINGS: Activity of hypothalamic orexinergic neurons is subjected to maturation and is mandatory to maintain lo

Efficacy of portable sleep monitoring device in diagnosing central sleep apnea in patients with congestive heart failure.

INTRODUCTION: Central sleep apnea (CSA) is a common and serious comorbidity mainly occurring in patients with heart failure (HF), which tends to be underdiagnosed and has not been widely studied. Overnight polysomnography (PSG) is the gold standard for diagnosing CSA; however, the time and expense of the procedure limit its applicability. Portable monitoring (PM) devices are convenient and easy to use; however, they have not been widely studied as to their effectiveness in detecting CSA in patients with HF. In the current study, we examined the diagnostic value of PM as a screening tool to ide

Evaluation of Sleep Apnea in Stroke Patients Using a Portable Smartphone-Based System.

Sleep apnea is common but often underdiagnosed after stroke, hindering rehabilitation and functional recovery. Access to sleep studies remains limited for stroke patients, partly due to the inconvenience of current diagnostic tools. The widespread availability and built-in sensors of smartphones make them powerful solutions for mobile health applications, including sleep monitoring. This study aims to detect and evaluate sleep apnea in post-stroke patients using a smartphone and extract multimodal digital biomarkers to characterize their sleep patterns. For that, overnight sleep tests were con

Sleep apnea is common in patients with coronary artery disease.

Sleep-disordered breathing (SDB) has a prognostic impact in patients with cardiac diseases. We included 257 patients with preserved left ventricular function and angiographically proven coronary artery disease (CAD). All patients underwent cardiorespiratory polygraphy. In 251 patients high-sensitive C-reactive protein and fibrinogen were measured. SDB was documented in 188 patients (apnea-hypopnea-index [AHI] 16.4+/- 1.9/h): 58 patients presented central sleep apnea (CSA) and 130 patients obstructive sleep apnea (OSA). All patients (73%) with SDB had higher blood fibrinogen levels than those w

Reduction of sleep apnea and bradycardia in preterm infants on oscillating water beds: a controlled polygraphic study.

The sleep and respiratory patterns of eight apneic preterm infants were polygraphically recorded for 24 hours. This polygraphic study was designed to test and extend our previous finding that gently oscillating water beds reduce apnea in premature infants. The infants who ranged in gestational age from 27 to 32 weeks and in birth weight from 1,077 to 1,650 gm served as their own controls, off and on the water bed. The 24-hour recordings were divided into four time blocks with the infant being placed on the water bed during alternate six-hour periods. Apnea was significantly reduced while the i

Adaptive servoventilation (ASV) in patients with sleep disordered breathing associated with chronic opioid medications for non-malignant pain.

BACKGROUND: Adaptive servoventilation (ASV) can be effective therapy for specific types of central apnea such as Cheyne-Stokes respiration (CSR). Patients treated chronically with opioids develop central apneas and ataxic breathing patterns (Biot's respiration), but therapy with CPAP is usually unsuccessful. There are no published studies of ASV in patients with sleep apnea complicated by chronic opioid therapy. METHODS: Retrospective analysis of 22 consecutive patients referred for evaluation and treatment of sleep apnea who had been using opioid medications for at least 6 months, had an apn

[Cheyne-Stokes respiration in patients with chronic heart failure: only a diagnostic marker or also a cardiovascular risk factor?].

Sleep disordered breathing with predominant obstructive or central apnea is an under-recognized but highly prevalent comorbidity in patients with chronic heart failure. As the severity of heart failure increases the prevalence of central sleep apnea (CSA) and Cheyne-Stokes respiration (CSR) is also much more frequent. Cheyne-Stokes respiration is characterized by alternating periods of crescendo and decrescendo respiration followed by central apnea. Present data indicate that CSA-CSR is not only a compensatory response to severe heart failure but also a predictor of worse prognosis. However th

PLMS is associated with low arousal threshold and favorable upper airway patency in patients with OSA.

OBJECTIVES: Periodic limb movements during sleep (PLMS) in patients with obstructive sleep apnea (OSA) increases the complexity of disease management. However, pathological mechanisms linking OSA and PLMS have remained unclear. This study aims to compare the endotypic traits of sleep apnea in patients with combined PLMS (OSA-PLMS) versus those with OSA alone. METHODS: A total of 1008 adults with moderate-to-severe OSA were prospectively recruited from a single sleep center at which they underwent in-laboratory full-night polysomnographic studies. Periodic limb movements were manually scored,

Sleep Disordered Breathing in Children.

Sleep-disordered breathing (SDB) refers to a group of disorders characterized by abnormalities of respiration or ventilation during sleep. It encompasses obstructive sleep apnea (OSA), central sleep apnea (CSA) syndromes, sleep-related hypoventilation and sleep-related hypoxemia disorders. This review will concentrate on the disorder most prevalent in pediatrics, i.e., OSA, highlighting the most recent developments in our understanding of the etiology, pathophysiology and treatment options of this condition. OSA morbidities primarily involve the neurocognitive, cardiovascular and metabolic sys

Screening for sleep-disordered breathing with Pediatric Sleep Questionnaire in children with underlying conditions.

The Pediatric Sleep Questionnaire described by Chervin et al. (Sleep Medicine, 2000, 1, 21-32) was originally validated for children with obstructive sleep apnoea syndrome but without other disorders. The aim of our study was to check the applicability of this questionnaire in children with underlying chronic medical conditions. Children aged 2-18 years who underwent a diagnostic sleep study at Great Ormond Street Hospital were recruited over a 10-month period. The Pediatric Sleep Questionnaire completed by their parents and cardiorespiratory polygraphy were scored. Sensitivities and specifici
