The prevalence and risk factors of pulmonary embolism in patients with chronic obstructive pulmonary disease: a systematic review and meta-analysis.

BACKGROUND: An increased prevalence of PE has been found in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Early identification of risk factors for the development of PE in patients with AECOPD and intervention is important. Therefore, we comprehensively pool and analyze the prevalence and risk factors of PE among patients experiencing AECOPD, aiming to provide valuable insights for clinical-based diagnostic determination and prevention of PE in the AECOPD patient population. METHODS: A systematic literature search was conducted for studies reporting the in

Waste to worth: diagnostic accuracy of Xpert MTB/XDR on contaminated liquid cultures to salvage the detection of drug-resistant tuberculosis.

UNLABELLED: Mycobacterium Growth Indicator Tube (MGIT) 960 culture is critical for tuberculosis (TB) drug susceptibility testing (DST) but is vulnerable to contamination. We evaluated the accuracy of Xpert MTB/XDR, a molecular DST for isoniazid, fluoroquinolone, amikacin, and ethionamide, on to-be-discarded contaminated growth. Xpert MTB/XDR was applied to acid-fast-bacilli-negative, contaminated cultures from sputum from people with rifampicin-resistant TB when Xpert MTB/XDR on sputum was unsuccessful (not resistant or susceptible for all drugs), either at diagnosis (Cohort A) or during treat

Clinical evaluation of a commercial culture-free targeted next-generation sequencing test for diagnosis of drug-resistant tuberculosis.

UNLABELLED: Rapid molecular diagnostics have significantly improved access to tuberculosis (TB) drug resistance detection and reduced turnaround times. However, these tools remain limited in their capacity to generate comprehensive resistance profiles, particularly for newer and repurposed anti-TB drugs. Targeted next-generation sequencing (tNGS) offers a promising alternative, enabling a broader resolution of Mycobacterium tuberculosis genomes and faster reporting compared to phenotypic drug susceptibility testing (DST). We conducted a prospective, cross-sectional, multicenter evaluation of t

Evaluation of Serum Iron Profile in Patients with Pulmonary Tuberculosis.

Approximately 10.0% of tuberculosis (TB) Infected individuals develop clinical disease in the absence of immune suppression suggests that individual factors may play a role in the response to infection. Body's immune function is boosted by micronutrient and also plays a major role in response to tubercular infection. Someone, may argue that cell mediated immunity is compromised in iron deficiency before anemia becomes apparent. This descriptive observational study intended to assess serum iron profile in patients suffering from pulmonary tuberculosis. This study included 56 newly diagnosed spu

Effect of patient-delivered household contact tracing and prevention for tuberculosis: A household cluster-randomised trial in Malawi.

BACKGROUND: Household contact tracing provides TB screening and TB preventive therapy (TPT) to contacts at high risk of TB disease. However, it is resource intensive, inconvenient, and often poorly implemented. We investigated a novel model aiming to improve uptake. METHODS: Between May and December 2014, we randomised patient with TB who consented to participate in the trial to either standard of care (SOC) or intervention (PACTS) arms. Participants randomised to PACTS received one screening/triage tool (adapted from WHO integrated management of adolescent and adult illnesses [IMAI] guidelin

Significance of monitoring plasma concentration of voriconazole in a patient with liver failure: A case report.

RATIONALE: Invasive pulmonary aspergillosis is associated with significant morbidity and mortality in patients with liver failure. Voriconazole (VRCZ) is recommended as a primary therapeutic agent for the treatment of invasive aspergillosis and metabolized in the liver. Now, data are still lacking on the safety and appropriate dosage of VRCZ in patients with liver failure. Here, we report a representative case of invasive pulmonary fungal infection in a patient with liver failure who was treated with low-dose VRCZ. PATIENT CONCERNS: A 21-year-old man, presented with subacute liver failure cau

The associations of human genetic variations with airway microbiome, environmental exposures, and respiratory health.

UNLABELLED: The intricate interactions between environmental exposures, the respiratory microbiome, and host genetic variations remain inadequately understood in the context of respiratory health. This study utilized sputum metagenomic data from 1,651 individuals in our previous cohort to elucidate these associations. Mendelian randomization indicated that air pollutants (e.g., SO2, CO, and PM2.5) were associated with lung function, which is potentially mediated by microbes, such as Actinomyces, Haemophilus influenzae, and Veillonella spp. Several genetic loci associated with respiratory micro

Comparative Evaluation of Fluorescein Diacetate/Ethidium Bromide Viability Microscopy and Ziehl-Neelsen Staining for Rapid Diagnosis of Pulmonary Tuberculosis.

BACKGROUND: Ziehl-Neelsen (ZN) staining is routinely used for tuberculosis (TB) diagnosis, but cannot differentiate viable from nonviable bacilli. This limitation is particularly relevant in smear-positive pulmonary TB patients, where assessment of treatment response is essential. Fluorescein diacetate/ethidium bromide (FDA/EtBr) viability microscopy detects live Mycobacterium tuberculosis based on metabolic activity. This study compared the diagnostic performance of FDA/EtBr microscopy with ZN staining, using culture and drug susceptibility testing (DST) as the reference standard. METHODS: A
