Total elbow arthroplasty for the treatment of insufficient distal humeral fractures. A retrospective clinical study and review of the literature.

Treatment of complex distal humeral fractures in older patients with osteopenic bone remains a major surgical challenge. We report the results of 11 patients over 75 years of age who underwent semiconstrained sloppy-hinge total elbow arthroplasty (TEA) due to comminuted intraarticular fractures of the distal humerus. There were 9 women and 2 men with a mean age of 79.6 years. The mean duration of follow up was 2.8 years. According to AO classification, there were 8 type C3 and 3 type C2 fractures. The mean time from injury to operation was 4.3 days and the mean length of hospital stay was 9.8

Functional outcome of en bloc excision and osteoarticular allograft replacement with the Sauve-Kapandji procedure for Campanacci grade 3 giant-cell tumor of the distal radius.

PURPOSE: Multiple options have been reported for reconstruction of Campanacci grade 3 giant-cell tumor of the distal radius after resection. In 1990, the senior author described an allograft reconstruction combined with a Sauve-Kapandji procedure after distal radius resection and reported preliminary results in 3 patients. The purpose of this study was to assess with validated patient outcome surveys the intermediate to long-term outcomes of all patients treated with this surgery and to analyze their functional results and document tumor control. METHODS: All cases of distal radius osteoartic

Comminuted fractures of the radial head: comparison of resection and internal fixation. Surgical technique.

BACKGROUND: Satisfactory internal fixation of comminuted radial head fractures is often difficult to achieve, and radial head resection has been the accepted treatment. In this study, we compared the results of radial head resection with those of open reduction and internal fixation in patients with a comminuted radial head fracture. METHODS: Twenty-eight patients with a Mason type-III radial head fracture (some with associated injuries) were enrolled in the study. Fifteen patients underwent radial head resection as the initial treatment (Group I), and thirteen patients underwent open reducti

[Recent fractures of the radial head associated with elbow instability treated with floating Judet prosthesis].

PURPOSE OF THE STUDY: Fracture of the radial head associated with elbow instability is infrequent. We report a retrospective series of floating Judet prostheses implanted for comminutive fractures of the radial head associated with elbow laxity caused either by dislocation or rupture of the medial collateral ligaments. MATERIAL AND METHODS: The series included ten patients who underwent surgery from October 1996 to September 2002 at the Amiens University Hospital. The indication for radial head prosthesis was established in the emergency setting for fracture unamendable by osteosyntheis and e

Linking compression mechanics and pressure redistribution in hybrid support surfaces for pressure ulcer prevention.

Pressure ulcers are a major concern for individuals with limited mobility. This study developed six hybrid textile-based support surfaces as low-cost, non-electric alternatives for pressure redistribution. Each surface was constructed by layering cotton pile and spacer fabrics to achieve mechanical gradation and improved body conformity. Compression behavior and interface pressure distribution were systematically analyzed in supine and lateral postures using a proof-of-concept model. To ensure reliability, triplicate measurements were averaged after a 30-second stabilization period. The result

Sleeping position affects the hairline recession in male patients with androgenetic alopecia.

Clinical observations indicate that some male patients with androgenic alopecia show varying degrees of hairline recession in the frontal and temporal regions on both sides, often with noticeable asymmetry between the left and right, which suggests a potential link to certain lifestyle habits. In this study, 161 male patients with androgenic alopecia were included, all exhibiting an "M"-shaped hairline. Patients' sleeping positions and combing directions were recorded and the areas of hairline recession in the frontal and temporal regions were calculated. Analysis of variance showed a signific

[Diabetic Autonomic Neuropathy].

Diabetic autonomic neuropathy is a clinically important component of diabetic neuropathy involving the cardiovascular, gastrointestinal, genitourinary, sudomotor, pupillary, and glucose counterregulatory systems. Cardiovascular autonomic neuropathy (CAN) has been extensively studied and is clinically relevant because of its association with adverse cardiovascular and renal outcomes. Early CAN is frequently silent and may only be recognized by reduced heart rate variability or abnormal cardiovascular autonomic reflex testing. As CAN progresses, patients develop resting tachycardia, reduced exer

Too high and too loose: dysautonomia and the hypertensive paradox in hypermobility disorders.

PURPOSE OF REVIEW: Hypertension in hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD) represents an increasingly recognized but poorly understood clinical phenomenon. This review addresses the unique pathophysiology, diagnostic considerations, and management challenges of hypertension occurring in the context of dysautonomia - a comorbidity affecting up to 70% of patients with hEDS/HSD. RECENT FINDINGS: Emerging evidence identifies distinct hypertensive endotypes in hEDS/HSD, including hyperadrenergic dysautonomia, paroxysmal hypertension, cervicogenic hypert

Review of How the Advice to Reduce Sudden Infant Death Syndrome Was Developed, Including Its Dramatic Impact on Sweden.

AIM: This paper describes sudden infant death syndrome (SIDS) from a medical historical perspective and explores how risk-reducing advice was developed. A particular focus is the dramatic reduction in the incidence of SIDS in Sweden. METHODS: A mini review of the literature was carried out. RESULTS: Sudden unexpected deaths in infants have been recognised since biblical times and were attributed to overlaying. These are assumed to equate to modern-day SIDS cases. SIDS became an established entity in the late 1960s, and the incidence increased in Sweden in the 1970s and the 1980s, when prone

Variability of Renal Ultrasound Measurements: How Physician Experience and Patient Position Affect Measurement Accuracy?

This study was designed to investigate the variability of renal ultrasound measurements, focusing on the impact of physician experience and patient position. Background: Since decreased kidney length is considered an indicator for chronic renal disease, understanding measurement repeatability and reproducibility is crucial for establishing effective diagnostic guidelines. Methods: Fifty healthy young adults underwent renal ultrasound scans performed by three examiners with varying levels of experience (12 years, 5 years, and 4 weeks). Renal length was measured at the level of the hilum in thre

Transitioning barium enema preparation protocols to CT colonography in the modern imaging era.

PURPOSE: To evaluate the feasibility of applying a barium enema-style bowel preparation protocol to CT colonography (CTC) in clinical practice. MATERIALS AND METHODS: 11 patients underwent CTC using a simplified bowel preparation protocol based on magnesium citrate, similar to that used for barium enema. Two radiologists assessed the homogeneity of fluid tagging, volume of residual fluid, and degree of colonic distension in six colonic segments (cecum, ascending, transverse, descending, sigmoid, rectum) in both supine and prone positions. Each parameter was rated on a 4-point Likert scale (0

Advances in the delivery of cardiopulmonary resuscitation in a diving bell.

This commentary discusses the provision of cardiopulmonary resuscitation to casualties in a diving bell. This single resource consolidates recent advances in the field, published in different medical journals, to support dissemination across the wider diving industry. It summarises the evaluation of techniques for the provision of manual cardiopulmonary resuscitation (CPR) to a seated casualty, including head-to-chest, knee-to-chest, and prone knee-to-chest compression delivery, and concludes that the only safe and potentially effective approach in a diving bell setting without room for a supi

Multivariate relationships between graft detachment after DMEK and twelve pre/perioperative factors.

This study aimed to identify pre/perioperative risk factors for graft detachment after Descemet membrane endothelial keratoplasty (DMEK). Hierarchical multivariate analysis was conducted on consecutive adults with Fuchs endothelial corneal dystrophy or pseudophakic bullous keratopathy who underwent pseudophakic- or triple-DMEK in 2014-2022 and were followed for 3 months. Significant graft detachment was defined as exceeding 20% of the graft or affecting the graft center. Twelve pre/perioperative factors were included. Of the 170 eyes (125 patients, median age 73 years, 65% women), 15% displaye

A safe method for rapid therapeutic pancreatic cooling.

Acute pancreatitis (AP) has no targeted therapy. Previously, pancreatic cooling to 31°C-33°C and 24°C-27°C, respectively, ameliorated mild and severe AP in rats. Here, Yucatan pigs (40-50Kg) whose abdominal size and anatomy are like humans underwent pancreatic cooling. This was via a gastric cooling balloon placed endoscopically with catheters exteriorized on the abdominal wall. Laparoscopically placed wireless transmitters monitored pancreas tail, head, and urinary bladder temperatures. Controls included un-perfused water filled balloons, and sedation-only groups. Tap water perfusion (375 mL/
