The effect of minimally displaced posterior malleolar fractures on decision making in minimally displaced lateral malleolus fractures.

PURPOSE: Despite the high incidence of cases of minimally displaced lateral or posterior malleolus ankle fractures, treatment guidelines are still an issue of controversy. The purpose of this study was to delineate treatment preferences among orthopaedic surgeons in these fractures with and without concomitant posterior malleolus fractures. We hypothesized that concomitant minimally displaced fractures of the posterior malleolus can shift treatment preference towards operative intervention. METHODS: A questionnaire-based study was conducted among orthopaedic surgeons attending the 2012 Europe

A systematic approach for diagnosing subscapularis tendon tears with preoperative magnetic resonance imaging scans.

PURPOSE: To determine and propose a systematic approach to evaluating magnetic resonance imaging (MRI) scans for subscapularis tears and compares preoperative MRI interpretations with findings of the same shoulders at arthroscopy. METHODS: The study was composed of 202 patients who underwent shoulder arthroscopy by 1 of 5 orthopaedic surgeons during a 3-month period. All patients had MRI scans performed within 6 months before arthroscopy. RESULTS: Of the 202 patients, 82 had subscapularis tendon tears confirmed at the time of arthroscopy. The orthopaedic surgeons correctly diagnosed 60 of 82

The development of cardiac surgery in West Africa--the case of Ghana.

West Africa is one of the poorest regions of the world. The sixteen nations listed by the United Nations in this sub-region have some of the lowest gross domestic products in the world. Health care infrastructure is deficient in most of these countries. Cardiac surgery, with its heavy financial outlay is unavailable in many West African countries. These facts notwithstanding, some West African countries have a proud history of open heart surgery not very well known even in African health care circles. Many African health care givers are under the erroneous impression that the cardiovascular su

Volume-outcome analysis of colorectal cancer-related outcomes.

BACKGROUND: Significant associations between caseload and surgical outcomes highlight the conflict between local cancer care and the need for centralization. This study examined the effect of hospital volume on short-term outcomes and survival, adjusting for the effect of surgeon caseload. METHODS: Between 1998 and 2002, 8219 patients with colorectal cancer were identified in a regional population-based audit. Outcomes were assessed using univariable and multivariable analysis to allow case mix adjustment. Surgeons were categorized as low (26 or fewer operations annually), medium (27-40) or h

Efficacy of neck dissection: are surgical volumes important?

UNLABELLED: OBJECTIVES/HYPHOTHESIS: Surgical volumes have been shown in many fields to reflect expertise and to be associated with improved clinical outcomes. We sought to test similar hypotheses that neck dissection is a volume-sensitive procedure wherein the number of lymph nodes harvested and clinical outcomes are correlated with the number of neck dissections performed. METHODS: We conducted a retrospective analysis of 375 neck dissections. The number of nodes harvested was examined by multiple linear regression before and after mathematical correction for pathologists' variation over tim

Factors influencing laparoconversions during the learning curve of laparoscopic myomectomy.

BACKGROUND: To assess the probability of conversion of a laparoscopic myomectomy to an open procedure, we only found the score developed by Dubuisson et al. (2001) based on four preoperative risk factors. Routinely this score is not appropriate, as realized by the most skilled laparoscopic surgeons. METHODS: The aim of this study was to identify the preoperative factors affecting the risk of conversion in data collected in different centers among a population of surgeons at the beginning of their experience in laparoscopic myomectomy. We collected preoperative clinical and ultrasonography dat

Axillary dissection and ductal carcinoma in situ of the breast: a change in practice.

BACKGROUND: Axillary dissection may be associated with significant morbidity and, while it is necessary in the treatment of invasive breast cancer, is not indicated for the treatment of pure ductal carcinoma in situ (DCIS), although it is being performed in a significant number of cases. The present study examined the incidence of elective axillary dissection in the treatment of DCIS cases detected in a mammographic screening programme over a 4-year period, and whether surgeons have changed their practice in this respect. METHODS: BreastScreen Victoria records were examined retrospectively fo

Ophthalmic Trauma-Related Instruments-Critical Tools for Winning the Case.

Ophthalmic trauma surgery is complex and often requires utilization of skills not commonly used in routine ocular surgery. Ophthalmic surgeons should have a comprehensive understanding of trauma related instruments to maximize clinical outcomes in these difficult cases. Here we review surgical instruments, techniques, tips, and tricks that are helpful during ophthalmic trauma surgery with special consideration for cases of open globe injuries and intraocular foreign bodies. We hope that this review can serve as a resource for both novice and experienced surgeons when planning for ophthalmic tr

Pharyngeal Arches, Chapter 2: Ear and Neck Abnormalities.

The pharyngeal arches are the foundation of face and neck development. Impaired development of these embryologic structures can result in craniofacial abnormalities. Surgeons who manage head and neck pathology will invariably encounter conditions associated with aberrant pharyngeal arch anatomy, and a thorough understanding of the normal development of these structures is paramount to accurate diagnosis and treatment. This manuscript is the second of a four-part series written for plastic surgeons, focusing on the abnormal development of pharyngeal arches leading to pathologic ear and neck ano

[Research progress of autofluorescence imaging of parathyroid glands].

The main causes of hypoparathyroidism are unintentional parathyroidectomy and/or impaired blood supply. Therefore, accurate identification and preservation of parathyroid glands in situ during thyroid or parathyroid surgery has become one of the problems that plague endocrine surgeons. In recent years, near-infrared autofluorescence imaging technology has gradually attracted more and more attention from surgeons because of its simplicity, safety, accuracy, real-time, no-contrast agent, and non-invasiveness. This article reviews the development history, clinical application, and application pro

Anomalous Course of Facial Artery Through the Submandibular Gland and its Redundant Loop at the Base of Mandible.

Knowledge of variant course and branching pattern of the facial artery gains importance in the surgeries of upper neck and face. The authors report a unilateral anomalous course and a redundant loop of right facial artery as seen in an adult male cadaver. The right facial artery had its origin from the external carotid artery. It pierced through the submandibular salivary gland and formed a redundant loop at the base of the mandible. Its further course and branching pattern was normal. This case could be important for radiologists, plastic surgeons and craniofacial surgeons.

Multidisciplinary Care and the Standards of Care for Transgender and Gender Nonconforming Individuals.

In providing care to transgender patients, surgeons interact with health care providers of other disciplines, including medical and mental health providers. Mental health or medical providers often see a patient first, when hormones are initiated. The Standards of Care recommend that mental health professionals assess patients for surgery according to set criteria and send surgeons their evaluations prior to surgery. Open communication is essential between surgical, medical, and mental health providers as well as with patients, who must give informed consent. A patient's multidisciplinary team

Anomalous radiological and clinical demonstration of gallbladder duplication in an asymptomatic pregnant female.

Gallbladder duplication is an uncommon congenital anatomical deviation encompassing a number of variants. The morphological difference occurs during budding of the hepatic diverticulum. We report the case of an asymptomatic pregnant woman who was incidentally found to have a large subhepatic cyst on ultrasound. The case highlights the difficulty in diagnosing large gallbladder duplication and the different radiological imaging available to surgeons to aid in mapping such anatomical variants. We propose that this case holds valuable lessons for both radiologists and surgeons when considering th

The anatomical relations of the sphenoid sinus and their implications in sphenoid endoscopic surgery.

Alongside the past development of endoscopic sinus surgery, knowledge about the anatomy of the sinuses has become crucial for surgeons. The sphenoid sinus is one of the most variable of all sinuses. Its relations to vital vascular and nervous elements make its approach a challenge for endoscopic surgeons. These relations include the internal carotid artery, the optic nerve, the pituitary gland, the Vidian nerve, and other vascular and nervous elements depending on the size of the sinus. This paper will make a review of these anatomical relations and their importance in endoscopic sphenoid surg

Proximal forearm fractures.

Unfortunately, the literature has little guidance for revision elbow surgery. This article attempts to supplement what is known in the literature with the author's anecdotal experience. With this article, it is the author's hope that the reader may learn from his or her successes and his or her failures without having to discover them first hand. There is good reason for angst to overcome surgeons looking at radiographs depicting a traumatized proximal ulna or radius. Surgeons know that there is a good chance they will be seeing these patients for a long time.

Rectal prolapse.

Optimal management of rectal prolapse requires multiple clinical considerations with respect to treatment options, particularly for surgeons who must counsel and give realistic expectations to rectal prolapse patients. Rectal prolapse outcomes are good with respect to recurrence. Although posterior rectopexy remains most popular in the United States, increasingly surgeons perform ventral rectopexy to repair rectal prolapse. Functional outcomes vary and are fair after rectal prolapse repair. Although incarceration with rectal prolapse is rare, it is potentially life threatening and requires imm

The use of acellular dermal matrices in chest wall reconstruction.

Surgeons are faced with increasingly complex and larger chest wall defects as a result of a variety of pathologies, the majority of which are oncologic. Skeletal reconstruction of these resulting defects and subsequent soft-tissue coverage remain a challenge for thoracic and plastic and reconstructive surgeons. A variety of techniques and grafts have been utilized to support the thoracic cage. This review focuses on the use of acellular dermal matrices in thoracic skeletal reconstruction, with a focus on the indications, published data, and surgical techniques for utilizing acellular dermal ma
