[Fever of intermediate duration].

Fever of intermediate duration (FID) is defined as non-localized fever occurring in the community, lasting from one to four weeks, and having no diagnostic orientation after basic clinical, analytical and radiological evaluations are completed. These may include careful recording of clinical history, physical examination, hemogram, plasma creatinine determination, urinalysis, and chest radiography. The etiology of FID is still not completely defined. In our country, 70% of cases are caused by systemic infectious diseases (rickettsiosis, brucellosis, and mononucleosis syndrome) and 7.7% by loca

Serum beta 2-microglobulin reflects disease activity in Behçet's disease.

Behçet's disease (BD) is a systemic remitting vasculitis characterized by orogenital ulceration and uveitis. The disease is not associated with specific laboratory abnormalities. Hence, the activity of BD is generally assessed by clinical findings and--to some extent--by nonspecific markers of inflammation. This study was performed to investigate the relative efficiency of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), serumamyloid A protein (SAA), and beta 2-microglobulin (beta 2-m) levels as markers of disease activity in patients with BD. The study population consisted of 2

Limited polyarteritis nodosa of the male and female reproductive systems: diagnostic and therapeutic approach.

BACKGROUND: Polyarteritis nodosa (PAN) is a multisystem necrotising small and medium sized vasculitis that when left untreated carries a grave prognosis, with a five year survival of 10-15%. Prolonged immunosuppressive treatment with cyclophosphamide and steroids leads to high remission rates while carrying the risk of life threatening complications. The diagnostic and therapeutic approach for patients with isolated genital tract PAN is not well defined. OBJECTIVE: To present the management and follow up of two patients with limited PAN localised to the male and female reproductive system. C

[Sensorineural hearing loss associated to Kawasaki Disease].

OBJECTIVE: Kawasaki disease is a systemic idiopathic self-limited vasculitis of small and medium-sized vessels. Thirteen cases of sensorineural hearing loss during the evolution of this disease have been described in the literature. We describe a case of an infant with Kawasaki disease who developed sensorineural hearing loss during the acute phase. This case report shows a complication of Kawasaki disease, with few citations in literature, enhancing the importance of careful evaluation of these patients. DESCRIPTION: Nineteen-month-old-boy, formerly healthy, with persistent daily fever seven

Human HSP 60 peptide responsive T cell lines are similarly present in both Behçet's disease patients and healthy controls.

Heat shock protein (HSP, 60/65 kDa) is investigated as a candidate autoantigen in Behçet's disease (BD), a systemic vasculitis of unknown origin, and a prominent response to 'disease-specific epitopes' of mycobacterial and human HSP60/65 is described in BD patients. In this study, long-term T cell lines from peripheral blood of BD patients (n=6) and controls (n=7) were stimulated with mycobacterial recombinant HSP and purified protein derivate (PPD) and expanded with IL-2. In the BD group, 15 out 27 and in the controls, 25 out of 35 PPD specific T cell lines have responded to the synthetic pep

[A case of Churg-Strauss syndrome after corticosteroid withdrawal manifesting mononeuritis multiplex and gastrointestinal perforations].

We report a case of a 70-year-old man who developed Churg-Strauss syndrome (CSS), diagnosed by characteristic histological findings, clinical presentations of mononeuritis multiplex, and multiple small intestinal ulcers with perforations, after discontinuation of corticosteroid therapy. The patient had developed asthma at age of 45, treated with oral prednisolone (10-20 mg daily) from that time. At the age of 70 he developed pneumonia. The condition responded to antibiotics, and the corticosteroid was quickly tapered and discontinued. He was never given leukotriene receptor antagonists during

Coexistent minocycline-induced systemic lupus erythematosus and autoimmune hepatitis.

OBJECTIVES: This study was performed to raise awareness among rheumatologists about two autoimmune disorders associated with long-term minocycline therapy that can coexist in the same patient. We provide an update on the occurrence of these disorders, their main characteristics, and the current knowledge of potential pathogenic mechanisms. METHODS: We searched the medical literature in English indexed in MEDLINE from 1966 through April 1998 for the term minocycline combined with each of the following: autoimmune hepatitis (AIH), chronic hepatitis, lupus, systemic lupus erythematosus (SLE), an

Paraneoplastic autoimmune phenomena in patients with myelodysplastic syndromes: response to immunosuppressive therapy.

We analysed the clinical features, course and response to immunosuppressive therapy in 30 patients with autoimmune disorders associated with myelodysplastic syndromes (MDS). 18 patients with MDS developed acute systemic autoimmune disorders. Common manifestations were skin vasculitis (n = 15) and arthritis (n = 11). Seven patients had an acute clinical syndrome of vasculitic skin rash, fever and arthritis with peripheral oedema in three and pulmonary infiltrates in five of these seven patients. Other acute manifestations included pericarditis, pleural effusions, skin ulceration, seizures, myos

[Antineutrophil cytoplasmic antibodies in human pathologies].

The prevalence of antineutrophil cytoplasmic antibodies (ANCAs) was verified in 338 patients with various rheumatic diseases using the internationally standardized indirect immunofluorescence method. The method was improved by using ethidium bromide as a nuclear counterstain. We observed three patterns of fluorescence: the classical diffuse cytoplasmic granular staining (C); the perinuclear one (P) and a yet undescribed atypical cytoplasmic distribution (A). We thus found ANCAs in 14/15 (93%) patients with active Wegener's granulomatosis: 13 were C-ANCAs, 1 was P-ANCA. We also found ANCAs in 6

Expression of VCAM-1 in the normal and diseased kidney.

We have conducted an immunocytochemical analysis to investigate the presence of the recently described vascular cell adhesion molecule-1 (VCAM-1) in human kidney, using the anti-VCAM-1 monoclonal antibody 1.4C3. In normal control tissue VCAM-1 was present on some (but not all) parietal epithelial cells lining Bowman's capsule. Forty-nine of fifty clinical biopsy specimens were characterised by the additional presence of VCAM-1 on proximal tubular cells. This was most marked in biopsies of patients with interstitial nephritis or systemic vasculitis with crescentic nephritis, but was also observ

Pleural effusions associated with ethchlorvynol lung injury result from visceral pleural leak.

Traditional thinking suggests that pleural fluid develops on the basis of systemic venous hypertension or a primary pleural process. Recent investigations, however, indicate that both acute lung injury and pulmonary venous hypertension can be important in the pathogenesis of pleural effusions. To evaluate the role of acute lung injury in the formation of pleural effusions, we developed a model of acute, reversible lung injury in NZW rabbits. Intravenous ethchlorvynol (ECV), known to produce permeability edema in humans, was used to produce permeability pulmonary edema in rabbits. The injury wa

Measurement of immune complexes with the liquid phase C1q binding assay: ten years experience in a routine diagnostic laboratory.

We describe our 10 years experience in assaying over 15,000 clinical specimens for immune complexes (IC) using the C1q binding assay. Normal ranges were initially established using a large panel of blood donor sera and precision of the assay was optimized by inclusion of heat aggregated IgG (HAGG) as standards. Nevertheless some variability was observed due to variation in C1q binding from batch to batch and with aging of this reagent. In an empirically selected 2 year period involving over 3,000 clinical specimens, 25% had elevated concentrations of IC. Of these the majority were from patient

Unraveling the Clinical Spectrum of DNASE1L3 Deficiency: Insights from Case Series and Systematic Literature Review.

BACKGROUND: DNASE1L3 deficiency is a rare monogenic cause of lupus and lupus-like autoimmunity resulting from impaired extracellular DNA clearance and sustained immune activation. Although most reported patients present with early-onset systemic lupus erythematosus (SLE), emerging evidence suggests broader phenotypic variability, including vasculitic and overlap manifestations. Whether these presentations represent distinct clinical entities or a continuum of DNASE1L3-associated immune dysregulation remains unclear. We aimed to define the clinical spectrum of DNASE1L3 deficiency and examine th

Antineutrophil cytoplasmic autoantibodies and associated diseases: a review.

Antineutrophil cytoplasmic autoantibodies (ANCA) are specific for constituents of neutrophil primary granules and monocyte lysosomes. There are different types of ANCA with different specificities. By indirect immunofluorescence microscopy using alcohol-fixed neutrophils as substrate, two major categories of ANCA can be recognized, one with cytoplasmic staining (C-ANCA) and the other with artifactual perinuclear staining (P-ANCA). Some C-ANCA have specificity for proteinase 3 (PR3-ANCA) and some P-ANCA have specificity for myeloperoxidase (MPO-ANCA), but there are additional C-ANCA and P-ANCA

Urinary exosomal microRNAs in Kawasaki disease and their changes after treatment.

BACKGROUND: Kawasaki disease (KD) is an acute systemic vasculitis of childhood characterized by intense inflammatory responses. Although circulating microRNAs have been investigated in KD, urinary exosomal microRNAs (miRNAs) and their dynamic changes following treatment remain poorly defined. METHODS: Urinary exosomes were isolated from children with KD and febrile control (FC) subjects. MicroRNA expression profiles were analyzed using TaqMan® human microRNA low-density arrays. Differentially expressed miRNAs were further evaluated in paired urine samples collected before and after intravenou

Dengue fever case report: a red flag for retinal vein occlusion.

BACKGROUND: Dengue is a prevalent arboviral infection in which ocular disease ranges from maculopathy and hemorrhage to vasculitis and retinal vascular occlusions, appearing during the acute phase or weeks later. Retinal vein and artery occlusions after dengue are uncommon but have been described in case reports and small series. Proposed mechanisms include thrombocytopenia with bleeding tendency, capillary leakage, and immune-mediated inflammation. The true frequency, risk factors, and patterns of bilateral or asymmetric ischemic involvement remain uncertain. We report serology confirmed mild

Reduction of monocyte activation and oxidative stress by phosphodiesterase 4 blockade in Behçet's syndrome.

OBJECTIVES: Behçet's syndrome (BS) is a systemic vasculitis characterized by multisystemic inflammatory manifestations driven by innate and adaptive immunity. Apremilast, an oral small-molecule selective inhibitor of phosphodiesterase 4 (PDE4), has shown effectiveness in treating oral ulcers of BS in both randomized clinical trials and real-world studies. This study aimed to explore the potential molecular mechanisms of apremilast in BS. METHODS: Bulk RNA sequencing of peripheral blood mononuclear cells (PBMCs) from 15 BS patients undergoing apremilast treatment was performed. We evaluated re

Association between Behçet disease and pregnancy-related outcomes: A systematic review and meta-analysis.

Behçet disease (BD) is a chronic autoinflammatory disorder characterized by systemic vasculitis. It is highly prevalent in women of reproductive age and may increase the risk of pregnancy-related complications. This study evaluated the impact of BD on maternal, obstetric, and fetal outcomes. A systematic search of PubMed, Scopus, Web of Science, and the Cochrane Library was conducted for studies comparing pregnancy-related outcomes between women with BD and controls, up to January 27, 2025. A random-effects model was used for the meta-analysis, and results were calculated as Mantel-Haenszel ri
