Occurrence of antineutrophil cytoplasmic antibodies and associated vasculitis in patients with hyperthyroidism treated with antithyroid drugs: A long-term followup study.

OBJECTIVE: To test whether antineutrophil cytoplasmic antibodies (ANCA) and ANCA-associated vasculitis (AAV) are not only induced during treatment with antithyroid drugs, but can also become evident when medication has been ceased, possibly after years. METHODS: Patients who visited our hospital for the treatment of hyperthyroidism were included (n = 207). Treatment consisted of antithyroid medications, radioactive iodide, thyroidectomy, or a combination of these treatment options. Patients were retested 3-6 years later to evaluate long-term effects of antithyroid drugs. Patients were tested

Steroids, sex hormone-binding globulin, homocysteine, selected hormones and markers of lipid and carbohydrate metabolism in patients with severe hypothyroidism and their changes following thyroid hormone supplementation.

Laboratory markers of thyroid function, selected steroid hormones, sex hormone-binding globulin (SHBG), homocysteine, prolactin, major markers of lipid- and glucose metabolism and of insular-growth hormone axes were investigated in fasting sera from 16 female patients with severe hypothyroidism after thyroidectomy because of thyroid cancer. The results obtained in severe hypothyroidism within 5-6 weeks after withdrawal of thyroid substitution therapy before control scintigraphy were compared with those obtained after correction of thyroid function. Elevated levels of homocysteine and prolactin

[Mediastinal metastasis of differentiated thyroid cancers. Treatment by total mediastinal curettage in 9 cases].

Nine patients with mediastinal lymph node metastasis in differentiated thyroid carcinoma (7 papillary carcinomas, 1 Hürtle's cells carcinoma, 1 insular carcinoma) have been treated by systematic mediastinal dissection, 2 cases arose 6 and 10 years after the initial surgery, 2 cases after 18 and 20 months, but 5 revealed their metastases at the moment of the diagnosis or 3 months later. Investigations caused by a rise in thyroglobuline level (4 cases) necessarily involve a mediastinal CT scan, which always proved conclusive. All the patients underwent a median total sternotomy associated with a

Adverse effects of thyroid hormone preparations and antithyroid drugs.

Thyroid hormone preparations, especially thyroxine, are widely used either at replacement doses to correct hypothyroidism or at suppressive doses to abolish thyrotropin (thyroid-stimulating hormone) secretion in patients with differentiated thyroid carcinoma after total thyroidectomy or with diffuse/ nodular nontoxic goitre. In order to suppress thyrotropin secretion, it is necessary to administer slightly supraphysiological doses of thyroxine. Possible adverse effects of this therapy include cardiovascular changes (shortening of systolic time intervals, increased frequency of atrial premature

Long-term thyroxine treatment and bone mineral density.

Studies of the effect of thyroxine replacement therapy on bone mineral density have given conflicting results; the reductions in bone mass reported by some have prompted recommendations that prescribed doses of thyroxine should be reduced. We have examined the effect of long-term thyroxine treatment in a large homogeneous group of patients; all had undergone thyroidectomy for differentiated thyroid cancer but had no history of other thyroid disorders. The 49 patients were matched with controls for age, sex, menopausal status, body mass index, smoking history, and calcium intake score; in all s

A prospective study of 110 Ethiopians with thyrotoxicosis.

One hundred and ten Ethiopian thyrotoxic patients were studied between February 1986 and January 1991 in a weekly endocrine clinic of Tikur Anbessa Teaching Hospital (TATH) in Addis Ababa, Ethiopia. This gives an incidence of 22 patients per year. There were 14 males and 96 females (M:F = 1:7). Fifty three patients (48.2%) had Graves' hyperthyroidism, 37 (33.7%) toxic multinodular goitre (TMNG), 14 (12.7%) toxic adenoma, 4 (3.6%) Jodbasedow phenomenon and 2 (1.8%) had thyroiditis. The mean age was 29.2 +/- 9.8 (+/- S.D.) years for Graves' cases, 49.0 +/- 10.9 years for TMNG cases and 46.4 +/-

Hypothyroidism and the influence on human blood rheology.

Viscoelastic properties of human whole blood were measured in 338 subjects: 110 persons (75 F, 35 M) following total thyroidectomy but without substitution therapy over three weeks as well as 228 healthy control subjects (124 F, 104 M) were investigated in this study. Subjects with other variable factors were excluded. The viscous and the elastic portion of apparent whole blood viscosity -the latter subdivided into red cell aggregation and red cell rigidity were determined by using a newly developed oscillating capillary rheometer and densitymeter at the real hematocrit value and at a computed

[Hyperthyroidism due to a TSH-secreting pituitary tumor].

A 28-year-old female with a 12-year history of goiter is presented. She had both clinical and laboratory evidence of hyperthyroidism, and her serum TSH was persistently and markedly elevated after treatment with antithyroid drugs. A TRH stimulation test resulted in no further rise in serum TSH after cessation of medication. Menses were regular and serum prolactin levels were normal. Serum LH and FSH responses to LHRH stimulation test were normal. No other evidence of pituitary or peripheral endocrine deficiencies existed. She underwent a subtotal thyroidectomy followed by 131I therapy three ye

Emerging Therapeutic Strategies for Graves' Disease: Current Advances and Future Perspectives.

Graves' Disease (GD) is the leading cause of autoimmune hyperthyroidism and is still considered a challenging clinical condition. Antithyroid drugs, radioactive iodine, and thyroidectomy have long been used as main treatments. These approaches are generally effective in restoring euthyroidism and controlling thyroid function, even if they cannot target the underlying autoimmune mechanisms implicated in the disease. As a result, relapses after medical therapy remain frequent, whereas definitive treatments often lead to permanent hypothyroidism and the need for a lifelong thyroid hormone replace

Civil medical liability in general surgery: A systematic review.

Civil medical liability is an important issue in modern surgical practice, affecting both medical practice and the doctor-patient relationship, and contributing to the increasing use of defensive medicine. General surgery, as a predominantly invasive specialty with a wide range of emergency and elective procedures, is at increased risk of legal claims. The purpose of this systematic review was to investigate the international literature on the civil medical liability of general surgeons, with an emphasis on the types of procedures involved, the causes of lawsuits, and the reported compensation

Low serum thyroglobulin level in A 15-year-old girl with papillary thyroid cancer and multiple neck lymph nodes: a case report.

BACKGROUND: Papillary thyroid carcinoma (PTC) is the most common form of differentiated thyroid cancer. Serum thyroglobulin (Tg) is a key biomarker used in postoperative surveillance. However, discrepancies between Tg levels and disease burden may occur, complicating clinical assessment. The report herein proposes to discuss such a case. CASE PRESENTATION: We present a case of a 15-year-old girl with a history of total thyroidectomy for PTC (age 13) who developed a new vascular neck mass. Sonographic and cytologic evidence confirmed metastasis. Serum Tg levels (evaluated using concurrent immu

Approach to the Patient: Hereditary Medullary Thyroid Carcinoma.

Medullary thyroid carcinoma is a rare neuroendocrine tumor originating from calcitonin-secreting parafollicular C cells of the thyroid gland. Approximately 25% of cases in adults are hereditary medullary thyroid carcinoma (hMTC), arising from activating, germline pathogenic variants in the REarranged during Transfection (RET) proto-oncogene and causing the syndromes multiple endocrine neoplasia (MEN) types 2A and 2B. A paradigmatic feature of MEN2 is its robust genotype-phenotype correlations, which predict the disease spectrum and age of onset of hMTC and other clinical manifestations. Advanc

Severe Phenotype of De Novo TSHR Activating Pathogenic Variants.

CONTEXT: The phenotypic spectrum of thyroid-stimulating hormone (TSH) receptor (TSHR) pathogenic variants is broad. Germline variants causing constitutive TSHR activation in the absence of TSH result in familial nonautoimmune hyperthyroidism (FNAH) or sporadic nonautoimmune hyperthyroidism (SNAH). This hyperthyroid state, if present in utero or early childhood, can impact multisystem development. The consequences of severe early-onset hyperthyroidism have not been well described. CLINICAL CASES: Here, we report two unrelated individuals, each with a distinct monoallelic de novo TSHR pathogeni

Thyroid Tuberculosis: Misdiagnosed as Papillary Thyroid Carcinoma - A Rare Case Report.

BACKGROUND: Thyroid tuberculosis (TTB), either in its primary or secondary form, is a rare occurrence, even in recent years, with a high incidence of tuberculosis in China. Nowadays, with the increasing morbidity of thyroid nodules, it is apt to be misdiagnosed with malignant tumors. In this case, a suspicious malignant lesion was found on thyroid ultrasound (US), but the result after the surgery showed TTB. CASE PRESENTATION: This article reported a 67-year-old woman who was found left thyroid nodules during a physical examination three month ago. Thyroid US showed two suspicious hypoechoic

Giant thyroid metastasis originating from renal carcinoma: a case report.

Thyroid metastases from clear cell renal cell carcinoma are rare. Although they often present in combination with primary thyroid tumors, they are difficult to distinguish from primary thyroid tumors on imaging and are easily misdiagnosed preoperatively. In this article, we report a case of clear cell renal cell carcinoma that metastasized to the thyroid gland. A 69-year-old woman had been diagnosed with a thyroid mass 8 years earlier, which gradually enlarged into a giant thyroid tumor. Tumor compression of the trachea caused intermittent wheezing, and the tumor protruded into the thoracic ca

[Integration of Molecular, Genetic, and Surgical Evidence Into the Indication Criteria for Preventive Thyroid Surgery].

Preventive thyroid surgery aims to prevent the occurrence of malignant diseases or improve their prognosis. The main indication is hereditary medullary thyroid carcinoma, which is caused by pathogenic RET germline variants. Other clinical manifestations can also occur in the context of multiple endocrine neoplasia. Early prophylactic thyroidectomy in the asymptomatic stage can make this tumour entity curable. The timing of surgery depends on the genotype and the associated risk classification and calcitonin levels, with particularly aggressive mutations requiring intervention as early as the f
