Aim: We aimed to provide a structured ex vivo protocol for cardiopulmonary micro-CT that combines gelatin-barium sulfate (gelatin-BaSO4) contrast medium with agar embedding in neonatal canine cardiopulmonary specimens. STUDY OBJECTIVE: To characterize the physiologic response to, and safety of, intravenacaval membrane oxygenation and carbon dioxide removal.
DESIGN: Interventional before-after study.
SETTING: University teaching hospital ICU.
PATIENTS: Twenty-two patients with severe acute respiratory distress syndrome (ARDS).
INTERVENTIONS: Implantation of a hollow-fiber membrane oxygenator (IVOX; CardioPulmonics; Salt Lake City, Utah) into the superior and inferior venae cavae by venotomy of the right femoral or right internal jugular vein for a duration of up to 20 days.

OBJECTIVE: Our objective was to assess survival, need for pacemaker insertion, and rejection frequency with a new surgical technique of orthotopic heart transplantation using bicaval and pulmonary venous anastomoses.
METHODS: We retrospectively reviewed 100 consecutive patients who had orthotopic heart transplantation with this technique between July 1991 and September 1995.
RESULTS: The mean age was 57.0 +/- 11.1 years, with 51 patients being 60 years or older. The mean donor/recipient weight ratio was 0.92, and in 28 patients the ratio was less than 0.8. The early (30-day) survival was 100

We developed technique similar to transcatheter treatment for superior sinus venosus defects to treat a patient with a Partial Anomalous Pulmonary Venous Connection of the right upper pulmonary veins (RUPV) without an atrial septal defect. A double transseptal puncture was performed, and the left atrium (LA) was connected with the RUPV using a covered stent. The blood flow from the superior vena cava was directed to the right atrium (RA) using a second covered stent.

Budd-Chiari syndrome (BCS) is defined by the obstruction of the hepatic venous outflow between the small hepatic veins and the junction of the inferior vena cava (IVC) with the right atrium. BCS with IVC obstruction occasionally progresses to hepatocellular carcinoma (HCC). Here, we report the case of a patient with HCC arising from a cirrhotic liver with BCS, in whom the hepatic portion of the IVC was obstructed, and who had a favorable outcome with a multidisciplinary approach and IVC balloon angioplasty.

AngioVac suction embolectomy is recommended by the manufacturer to be performed with a centrifugal pump due to safety considerations. However, roller head pumps are significantly cheaper to use, and thus may be more readily available during shortages and in resource-poor settings. We present the technique of Angiovac suction embolectomy being successfully performed with a roller pump to evacuate a clot-in-transit in the inferior vena cava and right atrium, along with discussion of important safety caveats.

A 61-year-old man presented with dysphagia, weight loss and shortness of breath. On examination, he had reduced lung sounds on the right, and obvious neck vein distention. Chest X-ray raised a suspicion of lung tumor with possible superior vena cava syndrome. Subsequent CT scan of chest confirmed the presence of markedly dilated and tortuous esophagus (sigmoid megaesophagus) extending to the right hemithorax and pressing on the trachea. The patient was referred for surgery.

Ruptured thoracoabdominal aneurysms (rTAAAs) are rare and carry a significant rate of morbidity and mortality. Aortocaval fistula secondary to rTAAA is even more infrequent. We describe an urgent and staged endovascular treatment of a ruptured extent III thoracoabdominal aortic aneurysm with an aortocaval fistula by performing vena cava stenting to treat aortocaval fistula as a damage control maneuver prior to transfer and subsequent TAAA repair with a physician-modified endograft at a quaternary level hospital.

A 76-year-old male presented with a submassive pulmonary embolism despite having an inferior vena cava (IVC) filter. Imaging demonstrated pulmonary artery emboli and a deep vein thrombosis in the left common femoral vein. Venography revealed the IVC filter with struts extending into the left and right renal veins. A new IVC filter was deployed below the prior filter. This case demonstrates IVC filter migration complicated by a submassive pulmonary embolism.

Femoral venous route is routinely used for percutaneous closure of atrial septal defects (ASDs). However, a situation may arise where transfemoral approach is not feasible. We describe a successful transjugular closure of a moderate-sized ASD in a 49-year-old symptomatic man with interrupted inferior vena cava, using a novel deployment technique, which helped in overcoming difficulties such as maintaining stable sheath position and minimizing risk of air embolism.

Iliac vein aneurysms are rare, even among venous aneurysms. We report the first case of an external iliac vein aneurysm presenting with paradoxical embolism to bilateral lower extremities. Workup after embolectomy demonstrated a patent foramen ovale along with the aneurysm and an inferior vena cava obstruction, which was treated with balloon angioplasty and ultimately stenting. The patient then underwent interval lateral aneurysmectomy. At 4-year follow-up, she is free of recurrence.

This report describes an infant with heterotaxy syndrome and severe hypoplasia of the left heart who presented with profound cyanosis at birth despite a large patent ductus arteriosus. Pulmonary venous return was difficult to demonstrate by echocardiography. Angiography showed total anomalous pulmonary venous return via a plexus that drained through the paravertebral veins and bilateral superior vena cavae. Autopsy confirmed these findings, and histopathology demonstrated severe occlusive changes within the pulmonary veins.

We describe the case of a 17 year old male who presented with severe groin pain leading to inability to weight bear on his left leg. Investigation revealed extensive bilateral and proximal deep vein thrombosis, in association with an absent inferior vena cava and anomalous venous drainage system. We present a review of the literature surrounding this association, summarise the typical clinical presentation and common characteristics in this group of patients and discuss its management.

We describe a patient after liver transplantation with a preexisting surgical splenorenal shunt close to the confluence of the splenic and superior mesenteric veins and a wide, short communication between the splenic and renal veins. To close the shunt, an inferior vena cava filter was inserted in the shunt and a vascular plug was placed in the splenic vein inside the filter. When this failed to stop the flow through the shunt, a covered stent was deployed at the superior mesenteric vein-portal vein junction.

Behçet's disease is a systemic inflammatory disorder characterized by vasculitis, which affects all types and sizes of blood vessels. A thrombotic tendency with a predilection for the venous side of the vasculature is well known for Behçet's disease. However, central vein involvement is a rare complication. The authors report an unusual case of complete thrombosis of the supraazygos superior vena cava, brachiocephalic veins, as well as right internal jugular vein in a patient who is a young woman with Behçet's disease.

Herniation of the caecum and the ascending colon in the lesser sac through the Winslow's hiatus is an uncommon cause of intestinal occlusion. The clinical presentation can be acute or chronic. The diagnosis often remains unclear before surgical operation, even with the assistance of abdominal CT, providing an image of a digestive structure in the lesser sac and the presence of mesentery between the portal triad and the vena cava. The treatment consists of reducing the internal hernia, which could be considered under laparoscopy.

We present a case of a nonfunctioning intrapericardial paraganglioma that presented as a typical chest pain in a 51-year-old woman. The tumor was initially diagnosed on coronary angiography where it had direct irrigation from the right coronary artery. Further computed tomography and magnetic resonance imaging scans showed significant compression of the superior vena cava by the tumor. This was excised through median sternotomy and extracorporeal circulation. Histopathological examination of the mass was characteristic of a paraganglioma.

We report a case of atrioventricular (AV) reentrant tachycardia with ostial atresia of the coronary sinus (CS). On levophase coronary venogram, dual anomalous venous drainage into both atria was seen; no ostium of the CS and no persistent left superior vena cava were noted. Based on mapping, radiofrequency (RF) energy was applied at the mitral ring and the bypass tract was eliminated. Eight months after the procedure, the patient has remained completely symptom-free.

A 30-years-old Japanese woman with a liver tumor was found to have congenital absence of the portal vein (CAPV). Both three-dimensional CT and angiography revealed that the superior mesenteric vein and splenic vein flowed into inferior vena cava and there was us portal vein, CAPV is an extremely rare congenital anomaly and liver tumor. Most cases on diagnosed in childhood, although this case was found in on adult. We reviewed the literature on reported CAPV cases.

The percutaneous closure of ostium secundum (OS) atrial septal defect (ASD) is a well-established procedure and is today considered the treatment of choice due to its good results and low morbidity and mortality. The procedure is routinely performed through the inferior vena cava (IVC). However, this route of access is not always available due to the obstruction or congenital absence of the IVC. We will present a case of ASD closure by means of transhepatic puncture due to the impossibility of using the IVC.
