Persistent fluid leakage following epidural catheter removal is rare in infants and may indicate cerebrospinal fluid leakage, with poorly described clinical manifestations. We report a case of a 10-month-old infant who developed persistent fluid leakage from the epidural insertion site and position-dependent changes in consciousness after the removal of a thoracic epidural catheter following right hepatectomy. Brain computed tomography excluded acute intracranial pathology, and the clinical course suggested intracranial hypotension secondary to suspected cerebrospinal fluid leakage. Bed rest,

Proximal lymphaticovenous anastomosis (LVA) achieves superior limb decompression; however, recipient veins in this region are often deep and disproportionately large in caliber relative to small lymphatic vessels (LVs). The resulting size mismatch frequently forces surgeons to forgo anastomoses with several LVs, undermining drainage, and no current technique reliably resolves this constraint. We developed the Lymphatico-Lymphatic Bridge (LLB), a sequential end-to-side method that uses an adjacent LV as an autologous conduit, linking otherwise unreachable LVs to a single recipient vein. The LLB

INTRODUCTION: Acute type A aortic dissection (ATAAD) and aortic arch aneurysms are life-threatening conditions requiring complex surgical intervention, often involving circulatory arrest. Cerebral ischemia and neurological complications remain significant challenges in aortic arch surgery. This narrative review focuses on retrograde cerebral perfusion (RCP) techniques used to mitigate these risks.
AREAS COVERED: This review article examines the evolution of cerebral protection strategies, with a focus on the techniques, parameters, and monitoring of RCP. It also compares RCP with antegrade ce

OBJECTIVE: Managing severe biventricular dysfunction with venoarterial extracorporeal membrane oxygenation (ECMO) and Impella requires a strategic approach. The ramp-down/ramp-up protocol standardizes care for such complex cases.
KEY STEPS: Key monitoring tools include pulmonary artery catheter echocardiography (transesophageal echocardiography/transthoracic echocardiography), end-tidal CO2, invasive blood pressure, and blood gases. The primary goal is to ensure adequate oxygen delivery with mixed venous oxygen saturation (>65%). After stabilization (6-12 hours), ventricular function is evalu

Hydrocephalus is a dynamic neurosurgical entity, with cerebrospinal fluid (CSF) diversion methods continually evolving to strike a balance between long-term efficacy and the risk of complications. We describe a 19-year-old man who was treated with endoscopic third ventriculostomy (ETV) for congenital aqueductal stenosis, with initial relief of symptoms. Two years later, he experienced recurring headaches and papilledema, but neuroimaging showed stable ventricles and patency of the ETV stoma. Magnetic resonance venography (MRV) ruled out venous sinus thrombosis, while lumbar CSF manometry revea

In the field of wound medicine and surgery significant attention is devoted to well-defined hard-to-heal (chronic) wounds, such as diabetic foot ulcers (DFUs), venous leg ulcers (VLUs) and pressure ulcers (PUs). These conditions dominate research, clinical guidelines and resource allocation, due to their clear aetiologies and high prevalence among specific patient populations. However, a substantial category of wounds-often labelled as 'other' open wounds under International Classification of Diseases (ICD)-10 codes-remain underappreciated and frequently excluded from analyses. These include n

Wound healing represents a dynamic process centered on the temporally coordinated inflammatory, proliferative, and remodeling phases. The inflammatory response exhibits a double-edged role: a moderate response is essential for normal healing, but excessive or persistent response impedes repair processes. In normal wound healing, histone modifications precisely regulate inflammatory responses from initiation to resolution, mainly manifested through early-stage modulation of NETs and late-stage intervention in macrophage polarization. In chronic non-healing wounds such as diabetic wounds, venous

PURPOSE: To describe a case of choroidal detachment secondary to cavernous sinus thrombosis.
METHODS: Case report.
RESULTS: A 73-year-old man with a history of primary open-angle glaucoma and pseudophakia presented with retrobulbar pain, blurred vision, and diplopia of the right eye. Examination revealed severe nonhemorrhagic bullous chemosis, tortuous retinal vessels, and elevated intraocular pressure in the right eye. Although computed tomography venography findings initially raised suspicion of a carotid-cavernous fistula, the diagnostic angiogram showed no such evidence, instead raising

Post-thrombotic syndrome (PTS), a chronic complication of deep vein thrombosis, causes pain, swelling, and ulcers, impairing quality of life. This study explores the role of lipid peroxidation and chronic inflammation in PTS. Oxidative stress and chronic inflammation damage the endothelium, increase venous pressure, and induce fibrosis, driving PTS progression. A biomarker-based ELISA approach is proposed to measure plasma lipid peroxidation products (e.g. MDA) and pro-inflammatory cytokines (e.g. IL-6, TNF-α). This method detects early biochemical changes in post-DVT patients and tracks PTS r

This case report describes a unique occurrence of rheumatic heart disease (RHD) with severe mitral, tricuspid, and pulmonary stenosis in a 17-year-old female. The patient presented with progressive exertional dyspnea and dependent edema. She had a history of acute rheumatic fever and joint pain 5 years ago. Evidence of congestive heart failure, such as pitting pedal edema, hepatomegaly, and elevated jugular venous pressure, was noted on presentation. The electrocardiogram (ECG) revealed important diagnostic information and signs consistent with right ventricular and right atrial enlargement. C

Renal vein entrapment, especially concerning the right renal vein, represents a scarcely explored anatomical aberration. The right renal vein's pivotal role in renovascular renal hemodynamics underlines the clinical significance of its compression, which can precipitate an elevated renal venous pressure gradient in relation to the inferior vena cava. This report delineates a unique instance of right renal vein entrapment in a 92-year-old male cadaver, identified during routine dissection. This entrapment is due to an unusual course of the right middle suprarenal artery that originates from the

Cerebral venous thrombosis (CVT) is a rare stroke with multiple risk factors. One rare risk factor is Behçet's disease (BD). Out of around 3000 cases at our center in the past 10 years, two cases of BD with CVT were seen. Herein, we report on their clinical symptoms, course, and management. Case 1 was a 18-year-old girl with a history of recurrent skin lesions presenting with encephalopathy syndrome due to CVT, requiring decompression. Despite our best efforts, she developed complications and expired due to sepsis. Case 2 was a 22-year-old male with raised intracranial pressure syndrome and a

Portal hypertension is a complex medical condition characterized by elevated blood pressure in the portal venous system. The conventional diagnosis of such disease often involves invasive procedures such as liver biopsy, endoscopy, or imaging techniques with contrast agents, which can be uncomfortable for patients and carry inherent risks. This study presents a deep neural network method in support of the non-invasive diagnosis of portal hypertension in patients with chronic liver diseases. The proposed method utilizes readily available clinical data, thus eliminating the need for invasive pro

Endoscopic ultrasound (EUS) has numerous advanced applications as a diagnostic and therapeutic modality in contemporary medicine. Through intraluminal placement, EUS offers a real-time Doppler-guided endoscopic visualization and access to intra-abdominal vasculature, which were previously inaccessible using historical methods. We aim to provide a comprehensive review of key studies on both current and future EUS-guided vascular applications. This review details EUS-based vascular diagnostic techniques of portal pressure measurements in the prognostication of liver disease and portal venous sam

Obesity is a chronic disease with high prevalence and associated comorbidities, making it a growing global concern. These comorbidities include type 2 diabetes, hypertension, ventilatory dysfunction, arthrosis, venous and lymphatic circulation diseases, depression, and others, which have a negative impact on health and increase morbidity and mortality. GLP-1 agonists, used to treat type 2 diabetes, have been shown to be effective in promoting weight loss in preclinical and clinical studies. This review summarizes numerous studies conducted on the main drugs in the GLP-1 agonists class, outlini

Neonatology pioneer Mildred (Millie) T. Stahlman celebrated her 100th birthday on July 31, 2022. Her distinguished career at Vanderbilt University Medical Center in Nashville, TN, is reviewed to commemorate this milestone. Stahlman was arguably the first to establish a modern neonatal intensive care unit in 1961, successfully utilizing negative pressure ventilation and umbilical arterial and venous catheters to monitor blood gasses and pH levels. She received early invaluable training in newborn physiology at the Karolinska Institute in Stockholm, Sweden, under John Lind and Petter Karlberg, a

Right atrial pressure (RAP) is an important prognostic criterion in pulmonary hypertension (PH). The main goals were to evaluate the following: (i) the accuracy of Doppler assessment of common femoral vein flow waveform to detect elevated RAP and (ii) the diagnostic accuracy of RAP assessed by echocardiography (eRAP). Fifty-seven patients, addressed for right heart catheterization, were included in a retrospective cross-sectional study during a 6-month period. Forty-five patients (78.9%) had PH confirmed by RHC. Elevated RAP was defined by RAP ≥ 10 mmHg. Femoral venous stasis index (FVSI) was

Heart failure (HF) is a complex disease associated with multisystem organ failure, recurrent hospital admissions, and increased mortality. Acute decompensated heart failure (ADHF) increases central venous pressure (CVP) with resultant hepatic congestion, and this relationship has prognostic significance. The gold standard method of measuring CVP, right heart catheterization, is invasive and costly, prompting further investigation into more accurate non-invasive assessments in HF patients, including liver elastography. Liver elastography relies on imaging techniques to assess liver stiffness me

Keratinocyte migration is vital in the re-epithelialisation of the skin during wound healing. Multiple factors conspire to impair closure of chronic wounds such as diabetic foot ulcers, venous leg ulcers and pressure wounds. Despite deep mechanistic understanding of microRNA (miRNA) biogenesis and function, the translational potential of these small genetic molecules has not been exploited to promote wound repair. In this review, I focus on miRNAs whose importance for wound healing stems from their impact on epidermal keratinocyte behaviour. These include miR-21-5p, miR-31-5p, miR-132-3p, miR-
