Reduction of mortality in rural Haiti through a primary-health-care program.

Deaths and their causes in a rural Haitian population of 8820 were studied through hospital records, death registration, a disease survey, and health surveillance. The results were used in selecting eight diseases for the delivery of health services by village-level health workers. The impact of the services was measured by monitoring annual age-specific and disease-specific mortality rates and by comparing them with officially estimated national mortality rates. Mortality rates fell progressively during five years, to levels only one fourth as high as the national estimates. The fall in morta

Nurse-perceived workload, staffing, and patient turnover: An analysis of longitudinal routine data.

BACKGROUND: Nursing workload is a critical factor in ensuring safe, high-quality patient care and maintaining nurse well-being. While high workload has been linked to adverse outcomes for both patients and staff, its assessment remains challenging. Common approaches rely on activity- or dependency-based measures or professional judgement, yet these often overlook important subjective and contextual factors. Nurse-perceived workload, particularly at the shift level, offers valuable insights but has received limited attention in research and is rarely used to guide staffing decisions. OBJECTIVE

Work-Related Stress Among Neglected Work Forces in Tertiary Hospitals, Eastern Ethiopia: A Cross-Sectional Study.

BACKGROUND: Hospital cleaners are an often-neglected workforce routinely exposed to hazardous working conditions that increase their risk of work-related stress (WRS). However, evidence on WRS among hospital cleaners in low- and middle-income countries, including Ethiopia is limited. This study aimed to determine the prevalence of WRS and identify associated factors among hospital cleaners working in eight tertiary hospitals in eastern Ethiopia. METHODS: A hospital-based cross-sectional study was conducted among 741 cleaners from May to August 2023. Work-related stress (WRS) was assessed usin

Global consensus framework for the safe and equitable implementation of telesurgery and remote surgical education: a society of robotic surgery supported accelerated Delphi study.

An estimated 160 million additional surgical procedures are required annually to address unmet surgical need globally, with the greatest deficits occurring in low- and middle-income countries. While telesurgery has re-emerged as a feasible model for delivering remote operative expertise, its broader implementation remains constrained by unresolved technical, ethical, regulatory, educational, and governance challenges. We aimed to develop an international multidisciplinary consensus framework to guide the safe, equitable, and scalable implementation of telesurgery and remote surgical education.

A Systematic Review of Nutrition Interventions for Shift Workers, With Subgroup Analysis of First Responder Populations.

INTRODUCTION: Shift work is an increasingly prevalent feature of the modern workforce, with first responders constituting a critical occupational group required to sustain 24-h operations. Modifiable health behaviour interventions, such as improving diet quality, may be effective in preventing or mitigating the non-communicable disease onset linked to shift work. This review evaluated evidence supporting nutritional interventions in shift workers with first responder populations considered as a pre-specified subgroup of interest. METHODS: A systematic review (PROSPERO CRD42023421400) evaluate

DEVELOPMENT AND EVALUATION OF A NURSE-LED SELF-DIRECTED PEDIATRIC REHABILITATION MODEL: COMPARATIVE ANALYSIS OF INTERNATIONAL PRACTICES.

BACKGROUND: Pediatric rehabilitation has become a global health priority due to increasing survival among children with neurodevelopmental disorders and medical complexity. Although nurse-led rehabilitation models are increasingly recognized as effective approaches for improving continuity of care and rehabilitation outcomes, considerable international variation persists in nursing roles, organizational structures, and integration of digital health strategies, particularly in transitional health systems. AIM: To systematically map and compare international nurse-led pediatric rehabilitation m

Regional inequities in myocardial infarction burden in Brazil's public healthcare system.

BACKGROUND: Myocardial infarction (MI) remains a leading cause of morbidity and mortality, with an uneven burden across Brazil. We evaluated trends in emergency MI hospitalizations, in-hospital mortality, and standardized costs within Brazil's Unified Health System (SUS) from 2008 to 2023, with emphasis on regional disparities and the COVID-19 period. METHODS: We performed a retrospective observational analysis using SUS Hospital Information System (SIH/SUS) data. Age- and sex-standardized rates were calculated for emergency MI hospitalizations and in-hospital mortality; standardized costs we

Fairness of a Professionalism-Oriented Situational Judgment Test for Residency Selection in the United States: A Predictive Bias Analysis by Gender, URiM Status, and International Medical Graduate Status.

Phenomenon: Residency selection has traditionally emphasized cognitive measures such as Unites States Medical Licensing Examination (USMLE) Step 1, Step 2 Clinical Knowledge (CK), and clerkship grades, even though physician performance encompasses cognitive and noncognitive competencies. Situational judgment tests (SJTs) have emerged as a promising complement, demonstrating criterion-related validity and producing smaller subgroup score differences than cognitive measures. However, smaller mean differences do not establish fairness. The Standards for Educational and Psychological Testing disti

The State of Primary Care in the United States.

UNLABELLED: Policy Points Primary care, when done well, is the country's most cost-effective solution to runaway costs and widening health outcome gaps. Primary care is increasingly starved for resources, with associated reductions in relational continuity and comprehensiveness that collectively increase the risk of losing patient trust. The 2021 National Academies of Sciences, Engineering, and Medicine report, Implementing High-Quality Primary Care: Rebuilding the Foundation of Health Care, supported a cascade of federal initiatives to strengthen, coordinate, and create accountability for pri

Safety and equity in scaling minimally invasive surgery worldwide in 109 countries using cholecystectomy as a tracer procedure: a prospective cohort study.

BACKGROUND: Minimally invasive surgery is rapidly expanding globally, yet there is insufficient knowledge of how to scale this technology safely and equitably across diverse health systems. We aimed to identify health-system factors associated with safe implementation of minimally invasive surgery globally, using minimally invasive cholecystectomy as a tracer procedure. METHODS: We conducted a multicentre, prospective cohort study of consecutive adults undergoing cholecystectomy between July 31 and Nov 19, 2023, in 1218 hospitals across 109 countries. Data were collected by more than 10 000 h

Advancing AI and data science for health in Africa: education, collaboration, and applications for global health priorities.

BACKGROUND: Africa faces a shortage of health data scientists. Despite bearing 25% of the global disease burden, it has only 3% of the world's healthcare workforce and even less health data science expertise. As artificial intelligence and data science transform global healthcare, from disease surveillance to precision medicine, this capacity gap poses a significant threat to Africa's ability to address its health challenges and harness its growing young population for health innovation. METHODS: We describe the WASHA Takwimu program, a multi-institutional capacity-building initiative funded

From development to implementation through a RE-AIM lense: lessons learned from a qualitative process evaluation of workplace health promotion workshops in long-term care.

BACKGROUND: Workplace Health Promotion (WHP) in long-term care facilities is essential to support staff wellbeing and sustain workforce capacity, yet implementing such interventions remains challenging. Within the pilot project "Healthy Care Cares for Health", WHP workshops were developed and implemented. To strengthen the feasibility of future WHP projects in long-term care facilities, this study pursued two objectives: first, to present the development and implementation of the workshops to ensure traceability and enable adaptation in future setting-specific WHP projects; second, to derive i

Trauma-Informed Support in a Dementia Helpline: Retrospective Mixed Methods Study.

BACKGROUND: People with dementia and their caregivers experience significant psychological distress, which may increase their vulnerability to trauma across the dementia diagnostic and caregiving trajectory. Specialist dementia helplines offer immediate emotional support, information, and signposting; however, little empirical evidence exists about how call handlers account for potential trauma in their responses. OBJECTIVE: This study aimed to examine the extent to which call handlers' responses on the United Kingdom's only 24-hour dementia helpline reflected trauma-informed (TI) principles
