[Reinnervation in obstetrical brachial plexus paralysis].

Post-paralytic associated movements ("co-contractions") in 16 cases with obstetrical brachial plexus palsy (OPP) were studied by a neurographic method. This consisted of stimulating a nerve and searching for heterogeneous motor axon reflexes (HAR) in the muscles of the upper limb and shoulder. HAR can be elicited when branches of a single axon reach two different muscles, even very distant from each other. The anatomy of the plexus and the fact that regenerating axons maintain direction determine the frequency with which two muscles are connected. Most HAR are recorded in muscles innervated by

Pediatric vascular trauma. Manifestations, management, and sequelae of extremity arterial injury in patients undergoing surgical treatment.

Twenty pediatric patients with a mean age of 9.2 years exhibited 21 injuries to arteries of an extremity and underwent operation. Included were 12 iatrogenic injuries and nine non-iatrogenic penetrating injuries. Femoral, brachial, popliteal, radial, axillary, iliac, and tibial arteries were sites of vascular trauma. Long-term follow-up, averaging 36 months, was possible in 14 patients. Postoperative scanograms, digital impedance plethysmographic studies, and bone age roentgenograms were routinely obtained. Limb length disparities greater than 5 mm, extremity blood pressure differentials great

Functional Outcomes of Anatomic Anterior Cruciate Ligament Reconstruction Using Retrograde Femoral Socket Drilling via a Far Anteromedial Portal Combined with a Conventional Antegrade Tibial Tunnel: A Retrospective Cohort Study.

Background/Objectives: Anterior cruciate ligament (ACL) reconstruction remains the accepted standard of care for ACL ruptures in physically active individuals. Various surgical techniques have been developed to achieve anatomic reconstruction and optimize functional outcomes. The aim of this study was to descriptively report the early functional outcomes of anatomic ACL reconstruction performed using retrograde femoral socket drilling using a FlipCutter through a far anteromedial portal combined with a conventional antegrade tibial tunnel, without claiming superiority over alternative techniqu

Bier block regional anesthesia and casting for forearm fractures: safety in the pediatric emergency department setting.

BACKGROUND: Bier block regional anesthesia was first described in 1908; however, it is uncommonly used for fears of cardiac and neurological complications. Although recent studies have documented safe usage in an adult population, no study to date has investigated its use in a pediatric setting. In addition, most emergency departments feel that splint placement is safer than casting after acute forearm fracture reduction in the pediatric population. However, to our knowledge there is no such study that documents the complication rates associated with immediate casting. The goal of this study w

Painful and nonpainful phantom and stump sensations in acute traumatic amputees.

BACKGROUND: The formation, prevalence, intensity, course, and predisposing factors of phantom limb pain were investigated to determine possible mechanisms of the origin of phantom limb pain in traumatic upper limb amputees. METHODS: Ninety-six upper limb amputees participated in the study. A questionnaire assessed the following question: side, date, extension, and cause of amputation; preamputation pain; and presence or absence of phantom pain, phantom and stump sensations or stump pain or both. RESULTS: The response rate was 84%. Sixty-five (81%) participants returned the questionnaire. In

Effects of hand cycle training on wheelchair capacity during clinical rehabilitation in persons with a spinal cord injury.

PURPOSE: To evaluate the effects of a structured hand cycle training programme on physical capacity in subjects with spinal cord injury (SCI) during clinical rehabilitation. METHOD: Twenty subjects with SCI who followed hand cycle training were compared with matched control subjects from a Dutch longitudinal cohort study, who received usual care. Primary outcomes of physical capacity were peak power output (PO(peak)), peak oxygen uptake (VO₂(peak)) and oxygen pulse during a hand rim wheelchair test. Secondary outcome measures were isometric peak muscle strength of the upper extremities and pu

Persistent sensitivity disorders at the radial artery and saphenous vein graft harvest sites: a neglected side effect of coronary artery bypass grafting procedures.

OBJECTIVE: The use of radial artery conduits in coronary artery bypass grafting (CABG) surgery is associated with improved long-term patency and patient survival rates as compared with saphenous vein conduits. Despite increasing popularity, relative incidence of local harvest-site complications and subjective perception of adverse long-term sequelae remain poorly described. METHODS: To allow for direct comparison, we investigated a consecutive series of patients in whom both the radial artery and the saphenous vein had been harvested for isolated CABG during a 36-month period. Patients were i

Brachial plexopathy due to massive swelling of the neck associated with craniotomy in the park bench position.

BACKGROUND: During prolonged neurosurgical procedures, anesthetized patients are at risk for position-related complications. We report a rare combination of neck swelling and brachial plexopathy as operative position-related complications. CASE DESCRIPTION: This 56-year-old woman was placed in the left park bench position for removal of a tentorial meningioma in the right posterior fossa. At 2 hours after the 10-hour procedure, her left neck began to swell with progression during the next 10 hours to involve the face on the same side and the face and neck on the opposite side. Primary wound closure in war amputations of the limbs.

We have reviewed 75 amputees, 58 in the lower limb and 18 in the upper. Closed amputation was performed in 69 patients (92%) and an open procedure in six (8%). Wound infection occurred in six (8%) and one patient died from thromboembolism. Wound healing was normal in the remainder. The results of closed amputation in war conditions are very good, if performed within 6 hours of injury.

Effects of aerobic exercise on integrated cardiovascular health and energy metabolism in patients with type 2 diabetes mellitus: study protocol for a randomized controlled trial.

OBJECTIVE: Type 2 diabetes mellitus (T2DM) induces integrated cardiovascular and metabolic impairments. The comprehensive effect of aerobic exercise on these systemic alterations remains unclear. Therefore, we designed a randomized controlled trial to investigate its impact on integrated cardiovascular health - specifically assessed by nitroglycerin-mediated dilation (NMD) and myocardial global work efficiency (GWE) - and systemic energy metabolism in patients with T2DM. METHODS: This study is a randomized, single-assessor-blind, parallel-group, two-arm controlled trial that will enroll 74 pa

Sustained functional recovery one year after TFCC debridement with adjuvant umbilical cord-mesenchymal stem cells and secretome therapy: a case report.

INTRODUCTION AND IMPORTANCE: Triangular fibrocartilage complex (TFCC) tears can significantly impair wrist stability and daily function. Regenerative medicine, particularly mesenchymal stem cell-based therapy, may complement traditional surgical approaches such as debridement with synovectomy to enhance recovery. PRESENTATION OF CASE: This report presents the 1-year follow-up of a 41-year-old male with a TFCC tear previously treated with debridement synovectomy followed by intra-articular umbilical cord-derived mesenchymal stem cells (UC-MSCs) and UC-MSC-derived secretome. UC-MSCs were select

Percutaneous Screw Fixation for Distal Oblique Scaphoid Fractures: A Computed Tomography-Based Evaluation of the Dorsal and Volar Approach.

BACKGROUND: The scaphoid height-to-length (H/L) ratio assesses reduction quality, with lower values indicating better anatomic restoration and reduced malunion risk. We hypothesized that dorsal percutaneous fixation of acute, unstable distal oblique fractures yields a lower postoperative H/L ratio compared with volar fixation. METHODS: We retrospectively reviewed all Herbert B1 fractures treated with percutaneous screw fixation at a single, tertiary referral center over 10 years. Dorsal and volar approaches followed standardized protocols, with guidewire placement under fluoroscopy and headle

Percutaneous screw fixation of pubic symphysis diastasis.

OBJECTIVE: Describe percutaneous screw fixation of symphyseal disruption as an alternative to open reduction and plate fixation. INDICATIONS: Pelvic ring fractures with symphyseal disruption ≥ 25 mm (anteroposterior compression [APC] type II, lateral compression [LC] type I and II). CONTRAINDICATIONS: Open reduction is required if closed reduction fails. Obesity or hernia may hinder screw insertion. SURGICAL TECHNIQUE: Reduction with a Weber clamp placed percutaneously. Using C‑arm imaging, the first guide wire is introduced percutaneously from one pubic tubercle across the symphysis. The n

Tools and Techniques of Endoscopic Radiofrequency Ablation in Pancreaticobiliary Diseases: A Narrative Review.

BACKGROUND: Endoscopic radiofrequency ablation (RFA) has become an established minimally invasive option for patients with pancreaticobiliary tumors who are not candidates for resection or who have obstructing lesions of the bile duct or pancreas. This narrative review summarizes the devices, technique, and supporting clinical evidence for endoscopic RFA, organized by clinical indication rather than by device alone. OBJECTIVE: RFA delivers high-frequency alternating current that produces localized thermal injury and coagulative necrosis. Two endoscopic delivery routes are in use: an ERCP-guid

Transoral robotic surgery using CO2 laser in oropharyngeal squamous cell carcinoma: a clinical case series.

PURPOSE: Transoral robotic surgery (TORS) is an established surgical approach for oropharyngeal squamous cell carcinoma (OPSCC). Carbon dioxide (CO₂) laser offers high cutting precision and minimal collateral thermal injury, yet its use in TORS remains underreported. The purpose of this study is therefore to describe the first UK case series detailing the use of CO₂ laser via hollow waveguide in TORS. METHODS: Eight patients (six male, two female; mean age 60 years) presenting with either left tonsillar (seven cases) or right tongue base (one case) OPSCC underwent primary TORS resection with
