We studied 8 symptomatic patients aged 32-56 years with isolated stenosis of the middle cerebral artery who had TIAs or minor strokes. Angiography showed MCA stenosis in the M1 segment in 7 patients, and proximal M1 occlusion that later recanalized with persistent stenosis in 1 patient, without evidence of more widespread atherosclerotic disease. Seventeen patients with disabling claudication resulting from multilevel arteriosclerotic disease were treated by combined intraoperative iliac transluminal angioplasty and femoropopliteal arterial reconstruction. Clinical improvement or total relief of ischaemic symptoms was observed in 15 out of 17 patients. Iliac pressure gradients were reduced with balloon dilatation to < 2 mmHg in all cases. In a series of 252 consecutive patients who underwent 282 carotid endarterectomies, we conducted clinical and angiographic follow-up for 2 to 6 years (mean, 3.2 years). Digital subtraction angiography (DSA) was done postoperatively in 95% of cases. Clinical follow-up was achieved in 97% of cases, and DSA follow-up was obtained in 66% of cases.
