[Physiological approach to peripheral neuropathy. Conventional nerve conduction studies and magnetic motor root stimulation].

In this communication, I first show some points we should mind in the conventional peripheral nerve conduction studies and later present clinical usefulness of motor root stimulation for peripheral neuropathy. CONVENTIONAL NERVE CONDUCTION STUDIES (NCS): The most important point revealed by the conventional NCSs is whether neuropathy is due to axonal degeneration or demyelinating process. Precise clinical examination with this neurophysiological information leads us to a diagnosis and treatment. Poor clinical examination makes these findings useless. Long standing axonal degeneration sometimes

Regeneration of acutely and chronically injured descending respiratory pathways within post-traumatic nerve grafts.

Central respiratory neurons, which are acutely axotomized by peripheral nerve grafts implanted at the level of the descending respiratory pathways within the C2 spinal cord, can regenerate their axons within the grafts and still transmit normal physiological messages [Decherchi et al., 1996. Exp. Neurol. 137, 1-14]. The present work investigated the extent to which mature central neurons, acutely or chronically axotomized by a spinal lesion, still maintain the potential to regenerate an axon following post-traumatic nerve grafting within supra-lesional spinal structures and remained functional

Imaging findings for severe traumatic brain injury.

Traumatic brain injury (TBI) is the leading cause of morbidity and mortality in young patients. The Marshall classification predicts six-month mortality and divides severe TBI patients into six groups based on CT findings in the acute phase of trauma. MRI also has prognostic value because it detects 30% more traumatic lesions, especially brainstem injury and diffuse axonal injury. Diffuse axonal injury occurs in three different anatomical areas, graded according to severity, and the greater the trauma, the deeper the brain involvement extends. Traumatic brainstem injuries with the worst progno

A new player in the game: identification of C1ql1 as a novel factor driving OPC differentiation.

Oligodendrocytes (OLGs) are the myelin-producing cells in the central nervous system (CNS). Following injury, these cells are prone to death, leading to demyelination and, eventually, axonal loss and neurodegeneration. Upon injury, the damaged CNS repopulates the lesion with oligodendrocyte precursor cells (OPCs) that consequently mature into OLGs to repair the myelin damage and prevent further axonal loss. In this issue, Altunay et al. identified that complement component 1, q subcomponent-like-1 (C1ql1), a factor known to play a role in neuron-neuron synapses, is also expressed by OPCs and d

Changes in integrins αv and α5 with Nogo-A in the rat retina after optic nerve injury.

OBJECTIVE: The purpose of this study was to investigate whether integrin levels are associated with axon regeneration after central nervous system (CNS) injury. MATERIALS AND METHODS: By using immunohistochemistry, we performed a detailed investigation of the changes in and colocalization of integrins αv and α5, with Nogo-A in the retina after optic nerve injury. RESULTS: We confirmed that integrins αv and α5 were expressed in the rat retina and colocalized with Nogo-A. After optic nerve transection, we found that integrin α5 levels increased over 7 days, but integrin αv levels remained unch

Myelin-associated glycoprotein inhibits neurite outgrowth through inactivation of the small GTPase Rap1.

Rap1 is a small GTPase that has been implicated in dendritic development and plasticity. In this study, we investigated the role of Rap1 in axonal growth and its activation in response to neurotrophins and myelin-associated inhibitors. We report that Rap1 is activated by brain-derived neurotrophic factor and that this activation can be blocked by myelin-associated glycoprotein (MAG) or central nervous system myelin, which also induced increases in Rap1GAP1 levels. In addition, we demonstrate that adenoviral overexpression of Rap1 enhances neurite outgrowth in the presence of MAG and myelin, wh

Communication between pathogenic T cells and myeloid cells in neuroinflammatory disease.

Clinical and experimental data suggest that T helper (TH) cells are involved in the pathogenicity of experimental autoimmune encephalomyelitis (EAE) and multiple sclerosis (MS), but it is unlikely that they are directly responsible for the observed demyelination and axonal loss. Instead, the cell population that targets the destruction of oligodendrocytes and axons, and the mechanism exploited by central nervous system (CNS)-invading encephalitogenic TH cells to instruct these cells to mediate tissue damage, are still under debate. Mature myeloid cells form a prominent component of the neuroin

Idiopathic hypocomplementemic urticarial vasculitis-linked neuropathy.

Hypocomplementemic urticarial vasculitis (HUV) is a rare form of cutaneous small-vessel vasculitis characterized by recurrent episodes of urticaria and painful, tender, burning or itchy skin lesions, often associated with extracutaneous involvement but usually with no significant peripheral nerve damage. We describe a patient with an HUV of undetermined cause that developed a progressive multifocal sensory neuropathy whose symptoms were temporarily relieved by intravenous immunoglobulin treatment. Sural nerve biopsy showed asymmetrical multifocal nerve fiber loss and axon degeneration in nerve

The cell adhesion molecule L1 controls growth cone navigation via ankyrin(B)-dependent modulation of cyclic AMP.

During development, asymmetric Ca(2+) signals across the growth cone mediate bidirectional axon guidance depending on intracellular levels of cyclic AMP: Ca(2+) signals trigger attractive or repulsive turning when cyclic AMP levels are high or low, respectively. Here, we report that the cell adhesion molecule L1 elevates cyclic AMP levels in neurons via ankyrin(B), a protein that links the L1 cytoplasmic tail with the spectrin network. We also show that the loss of ankyrin(B) expression converts Ca(2+)-triggered attraction to repulsion when the growth cone migrates via an L1-dependent mechanis

Neurotransmitter receptors in the life and death of oligodendrocytes.

Oligodendrocytes are crucial to the function of the mammalian brain: they increase the action potential conduction speed for a given axon diameter and thus facilitate the rapid flow of information between different brain areas. The proliferation and differentiation of developing oligodendrocytes, and their myelination of axons, are partly controlled by neurotransmitters. In addition, in models of conditions like stroke, periventricular leukomalacia leading to cerebral palsy, spinal cord injury and multiple sclerosis, oligodendrocytes are damaged by glutamate and, contrary to dogma, it has rece

Cellular and subcellular localization of gamma-aminobutyric acidB receptors in the rat olfactory bulb.

Olfactory nerve axons terminate in rounded regions of the olfactory bulb, termed glomeruli, where they make excitatory synapses with the dendrites of second-order neurons. Neurotransmission from the olfactory nerve to the postsynaptic targets is negatively regulated by gamma-aminobutyric acid (GABA), and there is evidence that inhibition of sensory input is mediated, at least in part, by GABA(B) receptors. Using an antiserum that recognizes two GABA(B) receptor splice variants (GBR1a and GBR1b), we show here that GABA(B) receptors are located on the axon terminals of the olfactory nerve, where

Regeneration of dorsal column fibers into and beyond the lesion site following adult spinal cord injury.

Regeneration is abortive following adult mammalian CNS injury. We have investigated whether increasing the intrinsic growth state of primary sensory neurons by a conditioning peripheral nerve lesion increases regrowth of their central axons. After dorsal column lesions, all fibers stop at the injury site. Animals with a peripheral axotomy concomitant with the central lesion show axonal growth into the lesion but not into the spinal cord above the lesion. A preconditioning lesion 1 or 2 weeks prior to the dorsal column injury results in growth into the spinal cord above the lesion. In vitro, th

Electromyographic findings in shoulder dislocations and fractures of the proximal humerus: comparison with clinical neurological examination.

There is no consensus of opinion about the frequency of associated nerve lesions in anterior shoulder dislocations and fractures of the proximal humerus. We undertook a prospective study to assess the incidence, the severity of the nerve injury and the diagnostic value of electromyographic examination; 215 patients were included. We performed neurological examination and needle electromyography (EMG). Nerve injury was graded according to a denervation score at the EMG. EMG disorders were seen in 133 patients (62%). Testing of sensibility and clinical reflexes proved not to be a reliable indica

Sciatic neuropathies--a retrospective review of electrodiagnostic features in 29 patients.

Sciatic neuropathy is an uncommonly diagnosed focal mononeuropathy. We reviewed the aetiology and electrodiagnostic features of 29 patients studied at the Neurodiagnostic Laboratory, Tan Tock Seng Hospital from January 1989 to April 1995. External nerve compression was the most common cause (38%) followed by trauma (21%). Other rare causes in this series included intragluteal injections, hip surgery and diabetic mononeuropathy while 24% had uncertain aetiology. Electrodiagnostic studies showed preferential involvement of the peroneal division in 51%. Axonal loss was found in 97%. We conclude t

Electrophysiological evaluation of the effects of naftidrofuryl on skeletal muscle reinnervation in the rat.

The effects of naftidrofuryl on the reinnervation of the rat gastrocnemius muscle after its denervation by localized freezing of the sciatic nerve were tested with electrophysiological techniques. Daily intraperitoneal injections of 30 mg/kg of naftidrofuryl do not increase the rate of axonal regeneration since early signs of reinnervation appeared as in controls around the 10th day after surgery. However, axonal sprouting is markedly increased since the percentage of muscle fibers with polyneuronal innervations was almost twice as high as in controls at the 15 and 21 day postoperative stages.

GM1 reduces injury-induced metabolic deficits and degeneration in the rat optic nerve.

This study demonstrates the earliest reported effects of GM1 treatment on crush-injured axons of the mammalian optic nerve. GM1, administered intraperitoneally immediately after injury, was found to reduce the injury-induced metabolic deficit in nerve activity within 2 hr of injury, as measured by changes in the nicotine-amine adenine dinucleotide redox state. After 4 wk, transmission electron microscopy 1 mm distal to the site of injury revealed a sevenfold increase in axonal survival in GM1-treated compared to untreated injured nerves. These results emphasize the beneficial effect of GM1 on

[Polyneuropathy associated with human immunodeficiency virus infection].

We report five patients with human immunodeficiency virus (HIV) infection in stages IV-C1, IV-C2 and IV-D who developed peripheral polyneuropathy (PNP). Two patients had invalidating PNP with a demyelinating predominance, while in the remaining three axonal component predominated. However, both components were present in different degrees in all patients. We have observed two cases with transition from one type of PNP to the other during the course of the disease. Inflammatory polyradiculoneuropathy usually develops in the initial stages of the infection, while the distal axonal type is more c

[Ultrastructure of the synapses of the anterior limbic cortex in schizophrenia].

Autopsied anterior limbic cortex (field 24) was investigated in order to provide an electron microscopic characterization of spine and dendrite synapses. Major changes of synapses were detected in I-II layers of anterior limbic cortex. Heterogeneity of axon terminals increased as the active zone increased in length. In a part of synapses terminal swelling was accompanied by a reduction of the synaptic vesicles count. Other synapses increased the vesicles quantity with mitochondria collapsing and disappearing. Rare synaptic contacts had patterns similar to Waller degeneration. "Membrane structu
