Surgical treatment strategies for intracranial fusiform aneurysms secondary to cardiac myxomas: illustrative case.

BACKGROUND: Intracranial myxomatous aneurysms secondary to cardiac myxomas are rare. Fusiform aneurysms, often multiple and predominantly involving distal vessels, represent the predominant manifestation. To date, there remains no consensus on the optimal management strategy for these fusiform aneurysms. OBSERVATIONS: The authors present the first case of a patient who underwent resection of a middle cerebral artery aneurysm and M3-superficial temporal artery (STA)-M3 end-to-end anastomosis bypass surgery 10 years following cardiac myxoma resection. They also review case reports on the treatm

Florid Pseudosarcomatous Polypoid Fibroplasia: An Unusual Cutaneous Manifestation of Chronic Lymphedema.

Bizarre, reactive fibroblastic proliferations associated with localized lymphedema may arise with chronic condom catheter use. We report a unique presentation of innumerable, cutaneous nodules occurring in chronic lymphedema of the right lower extremity due to remote pelvic lymph node dissection and radiation therapy for squamous carcinoma of the uterine cervix. Histologically, the cutaneous nodules consisted of several bizarre, enlarged polygonal to stellate, multinucleated fibroblasts having variably vacuolated cytoplasm in a background of markedly edematous dermis with prominent vascular ec

An international standardization to study the clinical use of lung ultrasound to discriminate viral, bacterial and atypical pneumonia in children with community acquired pneumonia.

BACKGROUND: Community-acquired pneumonia (CAP) is a leading cause of morbidity and mortality in the pediatric population worldwide. Establishing a definitive etiological diagnosis in children with CAP remains challenging, as clinical, laboratory, and radiologic findings are often insufficient. Consequently, empirical and frequently unnecessary antibiotic treatments are commonly prescribed. MAIN BODY: Lung ultrasound (LUS) has demonstrated diagnostic accuracy comparable to chest X-ray (CXR) for CAP, while eliminating radiation exposure. Emerging evidence suggests that LUS may also differentiat

Surgical management of a pulmonary neuroendocrine tumor causing epidural spinal cord compression and transdural intramedullary invasion: illustrative case.

BACKGROUND: Intramedullary spinal cord metastasis from local transdural invasion is an exceedingly rare event. Therefore, specific guidance on the timing, type, and extent of surgical intervention is lacking. OBSERVATIONS: A 60-year-old male with a known pulmonary neuroendocrine tumor presented with 2 days of progressive lower extremity paraplegia rendering him nonambulatory. MRI demonstrated transpleural and paravertebral invasion by the mass with extension through the left T1-2 and T2-3 neural foramina causing significant epidural compression. There was also transdural invasion at T2 extend

A Marine-Derived Sterol, Ergosterol, Mitigates UVB-Induced Skin Photodamage via Dual Inhibition of NF-κB and MAPK Signaling.

BACKGROUND: Ultraviolet B (UVB) radiation induces oxidative stress, inflammation, and collagen degradation in skin, leading to photodamage. Ergosterol (ERG)-a sterol widely distributed in fungi and algae, including numerous marine species-possesses antioxidant and anti-inflammatory activities, but its photoprotective mechanisms remain unclear. METHODS: Using integrated in vitro (UVB-irradiated human keratinocytes) and in vivo (topical ERG in a murine UVB model) approaches, combined with transcriptomic and network pharmacology analyses, we evaluated ERG's effects on oxidative stress, inflammat

Neuroinflammation in Radiation Maculopathy: A Pathophysiologic and Imaging Perspective.

BACKGROUND: Radiation maculopathy (RM) is a delayed, sight-threatening complication of ocular radiotherapy. Traditionally regarded as a pure microvascular disease, emerging evidence points to the central role played by retinal neuroinflammation, driven by microglial activation and cytokine dysregulation affecting both the retina and the choroid. Hyperreflective retinal foci, neuroinflammatory in origin (I-HRF), visualized through advanced imaging modalities such as spectral domain optical coherence tomography (OCT), have been identified as early and critical biomarkers of both preclinical and

Pulsed field ablation as a bailout for ventricular tachycardia ablation: a case report.

BACKGROUND: Catheter ablation of ventricular tachycardia (VT) using radiofrequency energy has proven to be an effective therapy. The outcome in patients presenting with an intramural septal substrate has however been disappointing. New tools and energy such as pulsed field ablation (PFA) might improve our efficacy to treat arrhythmia in this setting. CASE SUMMARY: A 64-year-old patient with valvular and ischaemic cardiomyopathy had recurrence of arrhythmic storm despite previous endocardial ablation and stereotactic body radiation therapy targeting the interventricular septum. A new attempt u

Microsurgical Management of Complex Meningiomas Involving Dural Venous Sinuses: Operative Techniques, Case Illustrations and Long-Term Outcomes in a Series of 70 Cases.

BACKGROUND: Meningiomas involving the dural venous sinuses present unique therapeutic challenges. While gross total resection remains the mainstay of treatment for meningiomas, involvement of critical vascular structures may limit extent of resection and increase the risk of recurrence. Optimal management of meningiomas with venous sinus involvement has been discussed in the literature, with some advocating for subtotal resection with postoperative surveillance and radiation, if necessary, while others recommend total resection with reconstruction of resection of the involved sinus. METHODS:

Alpelisib and Immunotherapy: A Promising Combination for Recurrent and Metastatic Squamous Cell Carcinoma of the Head and Neck.

BACKGROUND: Recurrent squamous cell carcinoma (SCC) of the head and neck (SCCHN) remains a formidable clinical challenge despite available treatments. The phosphatidylinositol 3-kinase (PI3K) pathway has been identified as a potential therapeutic target, and alpelisib, a selective PI3Kα inhibitor, has demonstrated efficacy in certain malignancies. Combining this targeted therapy with immunotherapy has been suggested in previous studies as a promising strategy to bolster the immune response against cancer. CASES: A 69-year-old woman with locoregional recurrence of PIK3CA-mutated SCC of the lef

Detecting radioactive particles in complex environmental samples using real-time autoradiography.

Radioactive particles often contain very high radioactivity concentrations and are widespread. They pose a potential risk to human health and the environment. Their detection, quantification, and characterization are crucial if we are to understand their impact. Here, we present the use of a real-time autoradiography gaseous detector (using parallel ionization multiplier) to expedite and improve the accuracy of radioactive particle screening in complex environmental samples. First, standard particles were used to assess the detector capabilities (spatial resolution, spectrometry, and artefact
