A review of lower extremity assistive robotic exoskeletons in rehabilitation therapy.

The rapid advancement of robotics technology in recent years has pushed the development of a distinctive field of robotic applications, namely robotic exoskeletons. Because of the aging population, more people are suffering from neurological disorders such as stroke, central nervous system disorder, and spinal cord injury. As manual therapy seems to be physically demanding for both the patient and therapist, robotic exoskeletons have been developed to increase the efficiency of rehabilitation therapy. Robotic exoskeletons are capable of providing more intensive patient training, better quantit

[Case of NMO (neuromyelitis optica) spectum disorder triggered by interferon alpha, which involved extensive pyramidal tract lesion of the brain].

A 65-year-old woman developed left optic neuritis during the course of peg-interferon alpha (PEG-IFN-α) and ribavirin combination therapy for chronic hepatitis C. Brain T(2)W-MRI disclosed hyperintense lesions in the corpus callosum and white matter. We diagnosed neuromyelitis optica spectrum disorder (NMOSD) on the basis of anti-aquaporin-4 antibody seropositivity. PEG-IFN-α was discontinued, and she received steroid pulse therapy (intravenous high dose methylprednisolone). Two weeks later she also developed right optic neuritis. Repetitive steroid pulse therapy improved the left optic neurit

[Nephrotoxicity--proteinuria and hypertension--].

Targeted and biological therapies have been investigated as methods of improving anticancer therapy. One approach to targeted therapy is to inhibit tumor angiogenesis, which has a critical role in the development of cancer. However, the potential for further improvement in outcomes is likely to be limited by its nephrotoxic side effects such as urinary protein and hypertension. Among the anti-angiogenesis inhibitors, the humanized monoclonal antibody, bevacizumab, directed against VEGF(vascular endothelial growth factor), is the first anti-angiogenic agent to be approved for cancer therapy, bu

Eruptive papules during efalizumab (anti-CD11a) therapy of psoriasis vulgaris: a case series.

BACKGROUND: Newer biological therapies for moderate-to-severe psoriasis are being used more frequently, but unexpected effects may occur. CASE PRESENTATIONS: We present a group of 15 patients who developed inflammatory papules while on efalizumab therapy (Raptiva, Genentech Inc, anti-CD11a). Immunohistochemistry showed that there were increased CD11b+, CD11c+ and iNOS+ cells (myeloid leukocytes) in the papules, with relatively few CD3+ T cells. While efalizumab caused a decreased expression of CD11a on T cells, other circulating leukocytes from patients receiving this therapy often showed inc

State of the art therapy in multiple myeloma and future perspectives.

Treatment for multiple myeloma (MM) has changed beyond recognition in the past decades. While until the early 1980s, MM caused a slow progressive decline in quality of life until death after about two years, today's patients can expect a 50% chance of achieving a complete remission, a median survival time of five years and a 20% chance of surviving longer than ten years. State of the art therapy comprises: evidence-based supportive care; highly effective and well tolerated chemotherapeutic regimens; and for patients qualifying for intensive high-dose conditioning, autologous haematopoietic ste

Development of selective androgen receptor modulators and their therapeutic applications.

Androgens control a broad range of physiological functions. The androgen receptor (AR), a steroid receptor that mediates the diverse biological actions of androgens, is a ligand inducible transcription factor. Abnormalities in the androgen signaling system result in many disturbances ranging from changes in gender determination and sexual development to psychiatric and emotional disorders. Androgen replacement therapy can improve many clinical conditions including hypogonadism and osteoporosis, but is limited by the lack of efficacious and safe therapeutic agents with easy delivery options. Conservative management of testicular endodermal sinus tumor in childhood.

Endodermal sinus tumor is the most common testicular neoplasm in childhood. The management of children with this neoplasm remains controversial. We have treated prospectively 5 children with stage I endodermal sinus tumor with limited surgery and no adjuvant therapy. The median patient age at diagnosis was 21 months (range 5 to 24 months). All children underwent an inguinal orchiectomy with high ligation of the spermatic cord. Retroperitoneal node dissection was not performed in any case and no child received adjuvant chemotherapy or radiation therapy. All patients were well without evidence o

Editor's Highlights-December 2026.

The effects of climate change, unequal access to biologic therapies, and the limited representation of Indigenous communities and diverse skin tones in educational materials underscore an important point: dermatology does not operate independently of social and environmental factors. The five papers in this issue not only identify these gaps but also call for specific measures to strengthen climate-resilient preparedness, expand safety-net services, and promote truly inclusive education. Equitable care is no longer a nice-to-have option; it is a fundamental standard of clinical quality.

Efficacy and safety of risankizumab in patients with erythrodermic and sub-erythrodermic psoriasis: a case series.

Erythrodermic psoriasis (EP) is a severe and complex form of psoriasis. While risankizumab, an interleukin (IL)-23 inhibitor, has demonstrated efficacy in patients with moderate-to-severe plaque psoriasis, its effectiveness in patients with EP remains less explored. The literature contains several small studies and case reports that provide evidence for the potential use of biologic therapies in the treatment of erythrodermic psoriasis. This case series suggests that risankizumab may represent an effective and sustainable treatment option for patients with a history of EP.

Severe, Resistant Generalized Pustular Psoriasis-Associated Inflammatory Syndrome Successfully Treated With Adalimumab: A Case Report.

Generalized pustular psoriasis (GPP) is a rare and severe variant of psoriasis, especially in pediatric patients, characterized by recurrent episodes of fever, malaise, and sterile pustules. We present the case of an eight-year-old girl with genetically confirmed GPP, carrying heterozygous mutations in IL36RN and CARD14, successfully treated with adalimumab in combination with methotrexate. Despite previous therapies, sustained remission was achieved only with this regimen. This case highlights the importance of genetic testing and the potential role of biologic agents in the management of ped

Neurosarcoidosis.

Sarcoidosis affects the nervous system in 5% of cases. 60% of cases involve the cranial and peripheral nerves, the remainder the central nervous system, in which a leptomeningitis, a pachymeningitis and a vasculitis may arise. Stroke and cerebral haemorrhage may occur, and certain infections in the brain are more likely with sarcoidosis. Patients respond well to treatment but oftentimes with residual neurological impairments which may be severe. A greater understanding of the disease and the need for early treatment and use of biological therapies have improved treatment outcome in recent time

[Current therapy of polyarticular forms of juvenile idiopathic arthritis].

Juvenile idiopathic arthritis (JIA) is the most common rheumatic disease in infancy and childhood. Approximately 20 % of patients with JIA suffer from the polyarticular form of the disease, which causes a substantial disease burden and long-term sequelae. Therapeutic approaches have used steroids and conventional disease modifying antirheumatic drugs (DMARD) but over the last decade new drugs have become available for the treatment of JIA, in particular biologic DMARD. This article summarizes the current therapy options for polyarticular JIA.

Inflammatory Mechanisms in Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a chronic relapsing disease of follicular occlusion that causes immense clinical and psychosocial morbidity when refractory to treatment. HS is no longer considered a disease of primary infectious etiology, although bacteria play a role. There is increasing evidence that HS is associated with immune dysregulation, based on its clinical association with other immune-mediated disorders, by its response to biologic therapy in the clinical arena, and from molecular research. This article summarizes what is known in relation to the inflammatory pathways in HS.

Emergent treatments based on the pathophysiology of bipolar disorder: A selective review.

Bipolar disorder is a chronic psychiatric disorder that is a cause of significant symptomatology even in the setting of optimal treatment. Most current treatments are developed from serendipity, and not based on known pathophysiology. In this review we examine a number of somatic and pharmacologic therapies that are poised to become part of the armamentarium of interventions to treat bipolar illness. As a group, these interventions are derived from a growing understanding of the biological underpinnings of bipolar disorders. We will look at emergent treatments based on our understanding of the

Severe gram positive bacterial infection in an ulcerative colitis patient treated with Infliximab.

Tumour necrosis factor-alpha (TNF-α) inhibitors represent an important treatment advance for a number of inflammatory conditions, including inflammatory bowel disease. Since their introduction in 1999, it has become clear that some biological therapies may be associated with an increased risk for bacterial infections. Herein we present the first case of septic arthritis and spine empyema, caused by S. aureus, in a 63 year-old patient with ulcerative colitis, who was under treatment with Infliximab and with 40mg per day of prednisolone.

How do mesenchymal stromal cells exert their therapeutic benefit?

In recent years mesenchymal stromal cells (MSC) have emerged as a major new form of cell therapy. While the original perception was that MSC were stem/progenitor cells with the potential to contribute to the regeneration of tissue, more recent data suggest that the principal mechanism of MSC activity is through the release of soluble mediators that elicit the observed biologic response. Future studies are needed to identify more completely the spectrum of therapeutic applications and delineate better the associated molecular and cellular mechanisms.

Duodenal carcinoid: a rare cause of melaena in a cirrhotic patient.

Small intestinal neuroendocrine tumours are relatively rare. Laboratory tests and diagnostic imaging are of help in diagnosis. Surgical resection is the standard approach. Metastatic disease has a poor prognosis. These are indolent tumours and hence role of chemotherapy is limited. Radionuclide and biological therapies are emerging. We report a 29-year-old man presenting with melaena and diagnosed as having a neuroendocrine tumour of the duodenum together with liver cirrhosis. Standard Whipple's procedure was done and he is doing well at follow-up.

[18-year-old patient with hypercalcemia, hypophosphatemia, nephrocalcinosis and normal iPTH values. Primary hyperparathyroidism].

We report an 18-year old woman with surgically proven primary hyperparathyroidism (pHPT) and normal intact parathyroid hormone (iPTH) serum levels. The reason for this rare biochemical presentation are possible biologically active amino-terminal parathyroid hormone polypeptide fragments not detected by the widely used two-site immunoradiometric parathyroid hormone assay (PTH IRMA). Diagnosis and therapy of primary hyperparathyroidism therefore should not exclusively rest on the finding of hypercalcemia coupled with an elevated iPTH level.

Oral hairy leukoplakia: diagnosis and management.

Oral hairy leukoplakia (HL) is a remarkable lesion associated with Epstein-Barr virus (EBV) found in persons infected with the human immunodeficiency virus. The clinical and histologic features of HL are characteristic and distinctive. However, none of these features are entirely specific for HL, and we consider that the presence of EBV is required for diagnosis in questionable cases. EBV can be demonstrated by means of electron microscopy, immunocytochemistry, or molecular biologic techniques. Therapy directed toward the HL lesion is sometimes indicated; acyclovir is the current drug of prefe

A Vanadium(II) Photosensitizer Delivered by Human Serum Albumin for Photodynamic Therapy.

Photodynamic therapy (PDT) is an established clinically approved treatment; however, the development of next-generation photosensitizers remains constrained by the reliance on scarce precious-metal scaffolds and poor pharmacological properties. Earth-abundant first-row transition metals offer an attractive alternative, but their application in PDT remains underexplored. Herein, we report the first vanadium(II) photosensitizer for PDT and show that its biological activity can be unlocked through loading into human serum albumin (HSA), a strategy intended to improve its pharmacological propertie
