OBJECTIVES: There is increasing evidence that both immune and neurochemical alterations are involved in the pathogenesis of bipolar disorder; however, their precise role remains unclear. In this study, we aimed to evaluate neuro-immune changes in a prospective study on children of patients with bipolar disorder.
METHODS: Bipolar offspring, from the prospective Dutch bipolar offspring study (n = 140), were evaluated cross-sectionally within a longitudinal context at adolescence, young adulthood, and adulthood. We examined the expression of 44 inflammation-related genes in monocytes, the cytoki

BACKGROUND: Migration is a risk factor for the development of schizophrenia.
AIMS: To examine whether migration is also a risk factor for bipolar affective disorder, unipolar depressive disorder and mood disorders in general.
METHOD: Medline was searched for population-based incidence studies concerning mood disorders among migrants and mean relative risks were computed using a mixed-effects statistical model.
RESULTS: Only a few studies of unipolar depressive disorder were retrieved. The mean relative risk of developing bipolar affective disorder among migrants was 2.47 (95% CI 1.33-4.59).

Adolescent mania is often misdiagnosed. This case study describes the clinical course and diagnostic reclassification from schizophrenia to bipolar disorder in a 15-year-old girl. This case study also describes the pedigree of the siblings, familial aggregation, and anticipation of mood disorders. In addition, we present the successful use of topiramate, a new antiepileptic drug, which is increasingly being used as a mood stabilizer in paediatric bipolar disorder. The efficacy of topiramate in this case supports its role as a promising agent in treatment-resistant adolescent mania associated w

A 22-year-old man with a long history of cannabis-use presented with a full blown mania. According to recent population-based studies, someone who starts using cannabis early in life runs the risk of developing not only psychosis but also manic symptoms and bipolar disorder. Further literature research showed that the use of cannabis influences both the onset and the course of bipolar disorder. When a patient presents with mania it might be advisable for the psychiatrist to take cannabis-use into account when making a diagnosis and prescribing treatment.

BACKGROUND: Family therapy is sometimes used as adjunctive treatment to pharmacotherapy to help patients recover from mood episodes of bipolar I disorder. However, the efficacy of this practice is not known.
METHODS: Ninety-two patients meeting criteria for a current bipolar I mood episode were randomly assigned to family therapy plus pharmacotherapy, multifamily psychoeducational group therapy plus pharmacotherapy, or pharmacotherapy alone. Time to recovery was analyzed with survival analysis.
RESULTS: The proportion of subjects within each treatment group who recovered did not significantl

Several dimensions of psychological functioning of bipolar manic-depressives in remission were investigated. The following hypotheses were examined: (1) During remission, these individuals exhibit psychological maladjustment or a lack of positive mental health. (2) Bipolar manic-depressives are characterized by strong external orientations. The Personal Orientation Inventory, Marlowe-Crowne Social Desirability Scale, Levenson's Internal and Powerful Others Locus of Control Scales, and the Embedded Figures Test were administered to matched groups of 35 bipolar manic-depressives and 35 normal co

A substantial portion of patients with juvenile bipolar disorder (BD) have a comorbid panic disorder (PD). The aim of our study was to analyze the cross-sectional and longitudinal implications of such comorbidity in children and adolescents with BD. The sample comprised 224 referred children and adolescents with BD, 140 males (62.5%) and 84 females (37.5%), mean age 13.8+/-2.8 years, diagnosed with a clinical interview (K-SADS-PL), and followed up naturalistically for 6 months. Fifty-one BD patients (22.8%) had a lifetime diagnosis of comorbid PD. Subjects with BD+PD and those without BD (BD-n

BACKGROUND: The direct causal relationships between common mental disorders (anxiety disorders, broad depression, major depressive disorder (MDD), bipolar disorder, and insomnia) and miscarriage or recurrent spontaneous abortion (RSA) are unclear. Therefore, this study aimed to explore these, using Mendelian randomization.
METHODS: Genome-wide association studies (GWAS) meta-analyses with the largest sample size possible and selected independent single individuals of European ancestry were selected. Inverse variance weighted (IVW) was the main analysis method. The heterogeneity of the instrum

BACKGROUND: The timings of reproductive life events have been examined to be associated with various psychiatric disorders. However, studies have not considered the causal pathways from reproductive behaviors to different psychiatric disorders. This study aimed to investigate the nature of the relationships between five reproductive behaviors and twelve psychiatric disorders.
METHODS: Firstly, we calculated genetic correlations between reproductive factors and psychiatric disorders. Then two-sample Mendelian randomization (MR) was conducted to estimate the causal associations among five repro

PURPOSE OF REVIEW: Structural brain changes are apparent in some magnetic resonance imaging studies of patients with mood disorders, but results are inconsistent. The focus of this review is to examine whether there are demographic or clinical characteristics of people with mood disorders that are associated with regional brain volume changes. A systematic search of the literature in English, from January 2004 to July 2005, was performed on MEDLINE. References cited in all reports were searched iteratively to identify missing studies.
RECENT FINDINGS: Recent studies have focused on factors th

OBJECTIVE: Current pharmacologic treatments for a depressive episode in unipolar major depressive disorder (MDD) and bipolar depression are limited by low rates of remission. Residual symptoms include a persistent low mood and neurovegetative symptoms such as fatigue. The objective of this study was to examine the efficacy and tolerability of augmentation of first-line therapies with the novel stimulant-like agent modafinil in MDD and bipolar depression.
DATA SOURCES: MEDLINE/PubMed, PsycINFO, 1980-April 2013 were searched using the following terms: (modafinil or armodafinil) and (depressi* o

fMRI measures beyond zero-lag functional connectivity could serve as useful tools for understanding the distinct spatio-temporal dynamics characterizing psychiatric conditions. Therefore, the primary objective was to investigate whether and how state-dependence influences lag-structure in healthy controls (n = 95). Moreover, the study aimed to explore clinical-behavioral correlates of state-dependent lag-structure in three groups of psychiatric patients (35 ADHD, 38 Bipolar Disorder and 23 Schizophrenia patients, diagnosed according to DSM-IV-TR). Lag-structure was computed from cross-correlat

OBJECTIVES: There is a paucity of evidence on bipolar I acute symptoms' presentation in the elderly individuals compared to younger patients. The current literature provides little, and at times conflicting, information on age-related bipolar disorder (BD) symptom presentation. This article aims to compare symptom profile by age group among patients with bipolar I in an acute affective episode as evaluated in outpatient settings.
METHODS: The current analyses include all Systematic Treatment Enhancement Program for Bipolar Disorder (STEP-BD) participants with a lifetime diagnosis of bipolar I

Although most treatment research on bipolar disorder has focused on mania, depressive episodes occur more frequently among patients with bipolar disorder. Here, we report the results of 2 identically designed, 8-week, multicenter, randomized, double-blind, placebo-controlled studies (CN138-096 and CN138-146) to evaluate the efficacy and safety of aripiprazole monotherapy in outpatients with bipolar I disorder experiencing a major depressive episode without psychotic features. Patients were randomized to placebo or aripiprazole (initiated at 10 mg/d, then flexibly dosed at 5-30 mg/d based on cl

INTRODUCTION: Impaired executive and memory function is a putative genetic trait marker of bipolar I disorder (BPD I). Although executive/memory function has been posited to be an endophenotype of BPD I, it is unclear whether this extends to bipolar spectrum illness. It is also unclear to what extent non-genetic factors such as childhood abuse, alcoholism and medication influence neurocognitive function. We assessed the neuropsychological performance of a large cohort of bipolar disorder probands and their affectively ill and healthy family members, while controlling for self-reported childhoo

CONTEXT: Recent genome-wide association studies have revealed that common variations and rare copy-number variations contribute to the risk of mental disorders. Rare recurrent microdeletions at 1q21.1 were reported to be associated with schizophrenia, and the BCL9 gene at 1q21.1 was also a functional candidate gene for mental disorders.
OBJECTIVES: To investigate and validate whether common variations exist in a functional candidate gene in the copy-number variation region, and, if so, to determine whether these variations confer risk of schizophrenia or other mental disorders.
DESIGN: A 3-s
