A 24-year-old woman presented with a 1-year history of gradually increasing pain on eye movements, worse on the right. She had a significant background of undergoing intracranial correction of orbital hypertelorism with internal fixation of the lateral orbital rims at the age of 6 years. Imaging was consistent with bilateral metallic foreign bodies from both lateral orbital rims protruding into the globes. The patient underwent removal of bilateral plates and screws, with a significant improvement of symptoms and ocular motility postoperatively. This case demonstrates the need to consider orbi

BACKGROUND: Fixation of posterior malleolar fractures with plates or screws is under debate. A fatigue loading system and a spatial motion capture system were used in this study to evaluate a posterior malleolar fracture model.
METHODS: Thirty-six below-knee specimens with a single posterolateral fragment (Haraguchi I) type posterior malleolar fracture models were randomly divided into 2 groups. Two parallel-placed 3.5-mm partially threaded titanium alloy screws were used in Group A to fix the fractures, while anatomical plates were used in Group B. According to the ratio (S) of the area betw

First metatarsocuneiform joint arthrodesis has been used in foot and ankle surgery for the treatment of hallux abductovalgus deformity, among other pedal pathologic entities. The goal of the present retrospective study was to compare the fusion rates and complications of an intraplate compression screw fixation, crossing solid core screw fixation, and a single interfragmentary screw with a simple locking plate. All procedures were performed by a single surgeon, and all patients received an identical postoperative protocol. A medical record review was performed of 147 evenly distributed surgica

OBJECTIVE Subaxial cervical instability in very young or small-for-age children is uncommon and typically arises from trauma or skeletal dysplasia. Various operative techniques have been used to achieve stabilization in pediatric patients with evidence of instability, including anterior, posterior, and combined approaches. In this study, the authors report their results with subaxial cervical instability in this patient population treated using a static single-screw anterior cervical plate (ACP) system and allograft fusion. METHODS In a retrospective chart review, the authors identified all pa

We conducted a study to highlight areas of risk with distal radius fixation to prevent occurrence of extensor tendon injury without compromising the security and stability of the fixation. Twelve cadaveric forearms were used. The volar locking plate was placed to best anatomical and radiologic fit on the distal radius of each arm. All 7 holes in the plate were drilled, and bicortical length was measured using a depth gauge under fluoroscopy to estimate screw lengths. Screws were secured into place, and dorsal prominence was measured. The central screw positions had the least dorsal screw promi

PURPOSE: To evaluate whether the dorsal tangential view (DTV) allows for reliable and valid in vivo measurement of the distance between screw tips and the dorsal radial cortex (STCD) during volar plating of distal radius fractures.
METHODS: Subjects included 22 patients with displaced distal radius fractures who had volar plate osteosynthesis. We performed standardized DTV images intraoperatively. After surgery, we performed computed tomography (CT) scans with reconstructions parallel to the distal screws. Three independent observers blinded to the study protocol measured the STCD on the basi

INTRODUCTION: The potential value of proximal femoral locking compression plate (PFLCP) for extracapsular femoral fractures has been discussed in several case reports; however, clinical control studies are lacking. Therefore, we performed a case control study to assess: (1) If PFLCP offers better functional results and fewer complications than dynamic hip screws (DHS)? (2) Which kind of extracapsular femoral fractures would benefit from PFLCP fixation?
HYPOTHESIS: The PFLCP fixation offers better functional results and fewer complications than the DHS for the treatment of extracapsular fractu

BACKGROUND: Proximal humeral locking plates have significantly improved the treatment of proximal humeral fractures in recent years; however, they are not devoid of complications. Inadvertent screw penetration into the joint is a well-documented complication. Intraoperative 3-dimensional (3D) imaging may assist in detecting intra-articular implant penetration. This study compared the performance of a standard C-arm fluoroscope with a novel 3D imaging fluoroscope in detecting penetrating implants in a proximal humeral fracture model.
METHODS: Zinc-sprayed proximal humerus sawbones were affixed

PURPOSE: Pectus bar rotation is a major challenge in pectus repair. However, to date, no satisfactory technique to completely eliminate bar displacement has been introduced. Here, we propose a bar fixation technique using a bridge that makes the bar unmovable. The purpose of this study was to determine the efficacy of this bridge technique.
METHODS: A total of 80 patients underwent pectus bar repair of pectus excavatum with the bridge technique from July 2013 to July 2014. The technique involved connecting 2 parallel bars using plate-screws at the ends of the bars. To determine bar position c

BACKGROUND: There are various methods available to fix a calcaneal osteotomy, ranging from screws to plates and staples. It is not clear if one method is superior to the other. In this series we compare the complications and union rates of 3 different methods of fixation.
METHODS: A retrospective review of the records of a consecutive series of patients who had a calcaneal osteotomy was undertaken. All patients had their osteotomy by the same technique, however the subsequent fixation was performed using 3 different methods: a lateral locking plate, a headless, or a headed screw. The screws w

Intramedullary nailing is gradually emerging as the treatment of choice for pertrochanteric femoral fractures. Nevertheless, prospective randomised trials have failed to demonstrate the assumed superiority of cephalomedullary nails over the traditional treatment with the sliding hip screw. On the contrary, the gamma nail has been implicated in predisposing to secondary femoral fractures, although this seems to be rectified by newer techniques and nail designs. Sliding hip screw fixation remains the gold standard but can lead certain unstable pertrochanteric fracture subgroups to failure. Among

In the nineties, most spinal surgeons supported the validity of segmental spine instrumentation, but this procedure has progressively been abandoned because difficult and with a high risk of neurological complications, in favor of the Cotrel-Dobousset (CD). The CD instrumentation is based on segmentation of curves, thus improving the angular correction and actuates sagittal profile. Sublaminar acrylic loops (Universal Clamp) shows the same resistance to stress as steel or titanium alloy sublaminar wires. The simple procedure and the tensioning of the strips allows re-tensioning and progressive
