First Use of a Novel System for Allograft Preservation in a Donation After Cardiac Death Heart Transplantation.

Heart transplantation (HTx) from controlled donation after circulatory death (c-DCD) is a promising alternative to donation after brain death (DBD), though warm ischemia remains a concern. Thoraco-abdominal normothermic regional perfusion (TANRP) enables in situ functional assessment, but preserving grafts during transport under optimal hypothermic conditions is still a challenge. We describe the first clinical application of a novel temperature-controlled cold static preservation system-Pertravel (Aferetica s.r.l., Bologna, Italy)-in a c-DCD heart transplant. A 62 year old male donor underwen

Cuproptosis Inhibition by C-Phycocyanin Confers Neuroprotection against Copper-Induced Mitochondrial Damage and Cognitive Impairment.

Copper (Cu) is an essential trace element, but its dyshomeostasis has been increasingly linked to neurotoxicity and neurodegenerative diseases. The specific contribution of cuproptosis, a novel copper-dependent regulated cell death pathway, to neuronal damage remains poorly defined. This study evaluated the neuroprotective potential of C-phycocyanin (C-PC), a natural compound derived from Spirulina, against Cu-induced neurotoxicity, with a focus on cuproptosis. In vitro, C-PC treatment markedly attenuated Cu-induced cell death in mouse neuronal cells. It restored mitochondrial membrane potenti

PANoptosis in neurological disorders: from inflammatory cell death mechanisms to neuroprotective strategies.

PANoptosis is now regarded as an inflammatory form of programmed cell death (PCD). It reflects the coordinated involvement of apoptosis, pyroptosis, and necroptosis, usually through the PANoptosome in a shared pathological environment. This concept may be especially useful in neurological diseases. It helps explain why neuronal death, sustained inflammatory activation, and tissue injury often develop together and reinforce one another. Neural tissue is particularly sensitive to oxidative stress, mitochondrial dysfunction, immune-mediated inflammation, and blood-brain barrier disruption. These

TGR5 and FXR receptors in motor degeneration: Molecular mechanism, crosstalk pathways and therapeutic prospects.

Motor neuron degeneration in disorders such as amyotrophic lateral sclerosis, spinal muscular atrophy, and Parkinson's disease is increasingly recognized as a consequence of disrupted metabolic, mitochondrial, and inflammatory balance. There is emerging data that bile acid receptors - Takeda G-protein-coupled receptor 5 (TGR5) and Farnesoid X receptor (FXR) are key regulators that combine systemic metabolism with neuronal survival. These receptors modulate the mitochondrial biogenesis, oxidative stress responses, and glial inflammatory signaling and coordinate gut-liver-brain crosstalk. Their

Current evidence on histopathological and immunohistochemical markers useful for the forensic diagnosis of death in decomposed bodies.

Determining the cause of death in decomposed bodies is a major challenge in forensic pathology, as autolysis and putrefaction severely compromise tissue morphology and limit the value of conventional macroscopic and histological examinations. In this context, histopathological special stains and immunohistochemical (IHC) techniques have been explored as complementary tools capable of detecting residual markers of ante-mortem pathophysiological processes. This review evaluates current evidence on the applicability, diagnostic contribution, and limitations of histopathological and IHC markers in

Digging deeper into NINJ1: its multifaceted role in central nervous system diseases.

Ninjurin1 (NINJ1) is a cell-surface molecule that has gained considerable attention for its role in mediating plasma membrane rupture (PMR). Originally identified as an adhesion molecule induced after nerve injury, NINJ1 is now recognized as a common terminal executor of PMR across multiple forms of lytic cell death, including pyroptosis, necroptosis, and ferroptosis. This function positions NINJ1 as a key link between cell death and inflammatory activation. However, the precise role of NINJ1 in the central nervous system (CNS) remains unclear. This review systematically outlines the molecular

Treatment of a severe vascular disease using a bespoke CRISPR-Cas9 base editor in mice.

Pathogenic missense mutations in the alpha actin isotype 2 (ACTA2) gene cause multisystemic smooth muscle dysfunction syndrome (MSMDS), a genetic vasculopathy that is associated with stroke, aortic dissection and death in childhood. Here we perform mutation-specific protein engineering to develop a bespoke CRISPR-Cas9 enzyme with enhanced on-target activity against the most common MSMDS-causative mutation ACTA2 R179H. To directly correct the R179H mutation, we screened dozens of configurations of base editors to develop a highly precise corrective A-to-G edit with minimal deleterious bystander

The weight of multiple hits: how TBI and infectious encephalitis co-modulate adverse outcomes.

BACKGROUND: Chronic neurologic deficits from traumatic brain injury (TBI) and subsequent infectious encephalitis are poorly characterized. METHODS: Using TriNetX database we queried patients 18 years or older with a confirmed diagnosis of encephalitis between 2016 and 2024. Patient cohorts included those with a diagnosis of TBI at least one month before encephalitis (N = 1,038), those with a diagnosis of a TBI anytime before encephalitis (N = 1,886), and those with encephalitis but no TBI, (N = 45,210; N = 45,215). Traumatic brain injury-related ferroptosis: current perspectives.

Traumatic brain injury (TBI) is a leading cause of mortality and long-term disability worldwide, with secondary injuries amplifying neuronal damage through mechanisms such as ferroptosis-a regulated iron-dependent cell death driven by lipid peroxidation. This review synthesizes current evidence on ferroptosis in TBI, highlighting its core pathways: (1) iron metabolism dysregulation (e.g., TfR1 upregulation, ferritin suppression), (2) lipid peroxidation (mediated by ACSL4/LPCAT3/PUFA oxidation), and (3) glutathione depletion (via System Xc-/GSH/GPX4 axis disruption). Ferroptosis intersects with

POU4F2 overexpression promotes the genesis of retinal ganglion cell-like projection neurons from late progenitors.

Retinal ganglion cells (RGCs) are the projection neurons of the retina, and their death promotes an irreversible blindness. Several factors were described to control their genesis during retinal development. These include Atoh7, a major orchestrator of the RGC program, and downstream targets of this transcription factor, including Pou4f factors, that in turn regulate key aspects of terminal differentiation. The absence of POU4F family genes results in defects in RGC differentiation, aberrant axonal elaboration and, ultimately, RGC death. This confirms the requirement of POU4F factors for RGC d

Role of innate immune and inflammatory signaling in West Nile virus tropism and neuronal and glial cell death.

West Nile virus (WNV) is a mosquito-borne virus that causes severe neurological disease in humans. Despite substantial advances, our knowledge of the mechanisms involved in damaging the human brain is still limited. To address this gap, we developed a physiologically relevant in vitro model using human neuronal/glial cells and aimed to determine WNV tropism, assess whether the virus induces innate immune and inflammatory responses, and elucidate the resulting pathophysiological consequences. We found that WNV productively infected glial cells, whereas neurons exhibited a remarkable and unexpec

The clinical characteristics of Streptococcus equi ssp. zooepidemicus causing acute death in pigs and its prevention with chimeric monoclonal antibodies.

Streptococcus equi subsp.zooepidemicus (SEZ) causes acute death in pigs and severely impacts the pig industry; however, comprehensive records of the clinical signs of SEZ from a bacteriological perspective are lacking. In this study, we found that piglets developed fever and a reduced appetite within 24 hours-post-infection (hpi) with SEZ and that sudden death usually started at 36 hpi. The mortality rate reached 100 % within 48 hpi. The moribund piglets presented prominent gross lesions, including swollen lungs with peripheral consolidation, enlarged and congested spleens, turbid cerebrospina

In Vitro Models of Amyotrophic Lateral Sclerosis.

Amyotrophic Lateral Sclerosis (ALS) is one of the commonest neurodegenerative diseases of adult-onset, which is characterized by the progressive death of motor neurons in the cerebral cortex, brain stem and spinal cord. The dysfunction and death of motor neurons lead to the progressive muscle weakness, atrophy, fasciculations, spasticity and ultimately the whole paralysis of body. Despite the identification of several genetic mutations associated with the pathogenesis of ALS, including mutations in chromosome 9 open reading frame 72 leading to the abnormal expansion of GGGGCC repeat sequence,

Tissue Injury Protection: The Other Face of Anticoagulant Treatments in the Context of Ischemia and Reperfusion Injury with a Focus on Transplantation.

Organ transplantation has enhanced the length and quality of life of patients suffering from life-threatening organ failure. Donors deceased after brain death (DBDDs) have been a primary source of organs for transplantation for a long time, but the need to find new strategies to face organ shortages has led to the broadening of the criteria for selecting DBDDs and advancing utilization of donors deceased after circulatory death. These new sources of organs come with an elevated risk of procuring organs of suboptimal quality. Whatever the source of organs for transplant, one constant issue is t

Dengue Encephalitis Versus Japanese Encephalitis in Cases of Dengue Fever With Altered Sensorium: A Diagnostic Dilemma.

Dengue and Japanese encephalitis (JE) are diseases that often conquer the top headlines in the leading newspapers during epidemics. Although recovery is the rule in most dengue cases, some unfortunately land up with multiple organ dysfunction syndromes, get critical, and even succumb to death. The main risk here is bleeding due to thrombocytopenia and platelet dysfunction. On the other hand, JE often presents with acute encephalitis syndrome (AES). We report a confirmed case of dengue (NS1 reactive, IgM dengue positive) by enzyme-linked immunosorbent assay (ELISA) who developed sudden onset al

Donation After Circulatory Death in Heart Transplantation: History, Outcomes, Clinical Challenges, and Opportunities to Expand the Donor Pool.

Heart transplantation remains the gold-standard therapy for end-stage heart failure; the expected median survival range is 12-13 years. More than 30,000 heart transplants have been performed globally in the past decade alone. With advances in medical and surgical therapies for heart failure, including durable left ventricular assist devices, an increasing number of patients are living with end-stage disease. Last year alone, more than 2500 patients were added to the heart-transplant waitlist in the United States. Despite recent efforts to expand the donor pool, including an increase in transpl

Photoreceptor Cells Constitutively Express IL-35 and Promote Ocular Immune Privilege.

Interleukin-27 is constitutively secreted by microglia in the retina or brain, and upregulation of IL-27 during neuroinflammation suppresses encephalomyelitis and autoimmune uveitis. However, while IL-35 is structurally and functionally similar to IL-27, the intrinsic roles of IL-35 in CNS tissues are unknown. Thus, we generated IL-35/YFP-knock-in reporter mice (p35-KI) and demonstrated that photoreceptor neurons constitutively secrete IL-35, which might protect the retina from persistent low-grade inflammation that can impair photoreceptor functions. Furthermore, the p35-KI mouse, which is he

iPSC-Derived Neurons from Patients with POLG Mutations Exhibit Decreased Mitochondrial Content and Dendrite Simplification.

Mutations in POLG, the gene encoding the catalytic subunit of DNA polymerase gamma, result in clinical syndromes characterized by mitochondrial DNA (mtDNA) depletion in affected tissues with variable organ involvement. The brain is one of the most affected organs, and symptoms include intractable seizures, developmental delay, dementia, and ataxia. Patient-derived induced pluripotent stem cells (iPSCs) provide opportunities to explore mechanisms in affected cell types and potential therapeutic strategies. Fibroblasts from two patients were reprogrammed to create new iPSC models of POLG-related
