Enhancing Cardiovascular Health in Southeastern United States for Underrepresented Racial and Ethnic Minorities With HIV: A Qualitative Inquiry Using the Health Belief Model.

People with HIV from historically underrepresented racial and ethnic minority (URM) groups are at increased risk of cardiovascular disease (CVD). Guided by the health belief model, we conducted in-depth interviews with URM with HIV, established CVD, and CVD risk factors at three academic medical centers in the Southern United States between November 2019 and March 2020. We analyzed the data using applied thematic analysis. A total of 27 participants were interviewed, and most were African American. Participants described their HIV status as a contributing factor to perceived severity of CVD. B

Excess cardiovascular mortality in men with non-alcoholic fatty liver disease: A cause for concern!

Non-alcoholic fatty liver disease (NAFLD) has emerged as the commonest cause of chronic liver disease worldwide in recent years. With time, our understanding of NAFLD has evolved from an isolated liver condition to a systemic disease with significant manifestations beyond the liver. Amongst them, cardiovascular diseases (CVDs) are the most important and clinically relevant. Recent research supports a strong independent link between NALFD and CVD beyond the shared risk factors and pathophysiology. Female sex hormones are well known to not only protect against CVD in pre-menopausal females, but

Cold stress, heart failure, and esophageal occlusion cause the death of a West Indian manatee Trichechus manatus in Alabama, USA.

Cold stress is the leading cause of mortality in West Indian manatees Trichechus manatus in the northcentral Gulf of Mexico. This report describes an adult male manatee that stranded alive with signs of cold stress in Alabama (USA) waters in January 2022 and died during the rescue intervention. Postmortem examination and histopathologic review revealed multiple contributors to death. While the animal had evidence of cold stress syndrome, there was aortic stenosis and concentric left ventricular hypertrophy as well as an esophageal foreign body (plastic bag). Main findings from blood analysis i

Xylazine: The Drug Taking the World By Storm: What You Need to Know.

Xylazine has taken the world by storm and proactive strategies are urgently needed to combat its negative impacts on population health. Xylazine is an unscheduled non-opioid indicated as a veterinary tranquilizer, also known as "Tranq." This drug is commonly used in combination with other drugs, such as heroin, fentanyl, and cocaine. Xylazine can be used orally, intranasally, sniffed, smoked, and injected, but is mostly used intravenously. Adverse effects of xylazine are secondary to central nervous system (CNS), cardiovascular, and respiratory function depression. When alpha-2-receptors in th

FGF23 from bench to bedside.

There is a strong association between elevated circulating fibroblast growth factor-23 (FGF23) levels and adverse outcomes in patients with chronic kidney disease (CKD) of all stages. Initially discovered as a regulator of phosphate and vitamin D homeostasis, FGF23 has now been implicated in several pathophysiological mechanisms that may negatively impact the cardiovascular and renal systems. FGF23 is purported to have direct (off-target) effects in the myocardium, as well as canonical effects on FGF receptor/α-klotho receptor complexes in the kidney to activate the renin-angiotensin-aldostero

Critical Appraisal of the Utility and Limitations of Animal Models of Scleroderma.

Scleroderma, also called systemic sclerosis (SSc), is an autoimmune connective tissue disease characterized by abnormality of the immune system, cardiovascular disorder, and fibrosis of the skin, lung, kidney, and other internal organs. Skin symptom and stiffness of the skin with accumulation of collagen cause severe disability, and cardiovascular and lung complications lead to high mortality. Despite recent progress, therapies for these complications in SSc are limited and the underlying mechanisms are poorly understood. Animal models are one strategy to identify molecular mechanisms and path

Phosphate is a uremic toxin.

Hyperphosphatemia is one of the more prevalent metabolic disturbances in kidney failure. Phosphate can be considered a uremic toxin based on the accumulation of phosphate during chronic kidney disease, the effects of phosphate on biological systems, and the adverse effects of hyperphosphatemia. The renal clearance of phosphate is maintained until later stages of chronic kidney disease, when the remaining nephrons are no longer able to excrete sufficient phosphate to offset dietary phosphate absorption. Clearance of phosphate by conventional forms of dialysis is insufficient to prevent hyperpho

Sleep-disordered breathing and cardiovascular risk.

Sleep-disordered breathing, broadly characterized by obstructive sleep apnea (OSA) and central sleep apnea (CSA), is an increasingly recognized public health burden. OSA, consisting of apneas or hypopneas associated with respiratory efforts in the face of upper airway narrowing or collapse, is a common disorder that can be effectively treated with continuous positive airway pressure (CPAP). OSA not only results in daytime sleepiness and impaired executive function, but also has been implicated as a possible cause of systemic disease, particularly of the cardiovascular system. CSA, which may co

Serotonin and the heart.

Serotonin is a naturally occurring vasoactive substance that has diverse cardiophysiological effects. These effects can be explained by the existence of serotonin receptor subtypes which mediate different biological actions. The vasoconstrictive actions of serotonin are mediated by 5-HT2 serotonergic receptors, and serotonin also amplifies the release and activities of other vasoconstrictors, such as angiotensin and norepinephrine. Abnormalities in the serotonergic system may play an important role in the pathophysiology of multiple cardiovascular disease states such as systemic hypertension,

Blood pressure, heart rate and arterial blood gas reactions to acute hypoxia in carotid body denervated spontaneously hypertensive rats.

The role of the peripheral arterial chemoreceptors in the reflex control of respiration and the cardiovascular systems was studied in spontaneously hypertensive rats (SHR). In carotid body denervated and in sham-operated control rats mean arterial blood pressure, heart rate, respiratory rate and arterial blood gases were measured under normoxic conditions and in acute normobaric hypoxia. Under normoxia the carotid body denervated SHR differ from the sham-operated ones only in significantly lowered arterial pO2 and pH and in significantly increased pCO2 values. The carotid body denervated SHR r

Lipoprotein(a) and panvascular disease.

Panvascular disease (PVD) is an emerging clinical concept that encompasses a spectrum of atherosclerotic conditions involving multiple major vascular beds, including the coronary, cerebral, peripheral, and valvular arteries. Although not formally recognized as a nosological entity, in this review, PVD is adopted as a conceptual framework to reflect the systemic nature of atherosclerosis affecting vascular territories supplying the heart, brain, and peripheral circulation. This perspective enables a more integrated understanding of disease processes across organ systems that are often studied i

uPAR/suPAR Signaling and Organ Crosstalk in Cardiovascular-Kidney-Metabolic Syndrome.

Cardiovascular-kidney-metabolic syndrome is driven by inflammatory mechanisms that propagate injury across organ boundaries, yet the molecular mediators converting systemic inflammation into end-organ damage remain incompletely defined. Across multiple prospective cohorts, suPAR (soluble urokinase plasminogen activator receptor) is associated with incident cardiovascular events, heart failure, diabetes, and kidney disease progression, and genetic and experimental studies support a causal role. How a glycosylphosphatidylinositol-anchored receptor without a transmembrane domain initiates intrace

Case-matched Comparison of Cardiovascular Outcome in Loeys-Dietz Syndrome versus Marfan Syndrome.

Background: Pathogenic variants in TGFBR1, TGFBR2 and SMAD3 genes cause Loeys-Dietz syndrome, and pathogenic variants in FBN1 cause Marfan syndrome. Despite their similar phenotypes, both syndromes may have different cardiovascular outcomes. Methods: Three expert centers performed a case-matched comparison of cardiovascular outcomes. The Loeys-Dietz group comprised 43 men and 40 women with a mean age of 34 ± 18 years. Twenty-six individuals had pathogenic variants in TGFBR1, 40 in TGFBR2, and 17 in SMAD3. For case-matched comparison we used 83 age and sex-frequency matched individuals with Mar

Post-traumatic stress disorder (PTSD) symptom clusters associated with an indicator of heart rate variability: The ADVANCE cohort study.

BACKGROUND: Heart rate variability (HRV) is governed by sympathetic and parasympathetic regulatory systems. Post-Traumatic Stress Disorder (PTSD) may influence these systems and consequently affect cardiovascular functioning. METHODS: The sample consisted of 860 UK male military personnel approximately half of whom had sustained physical combat injuries in Afghanistan. HRV was measured via Root-Mean Square of Successive Differences (RMSSD) in normal heart beats and PTSD using a self-report questionnaire (Posttraumatic Checklist-Civilian version (PCL)). Associations between probable PTSD statu

Systemic impacts of metabolic dysfunction-associated steatotic liver disease (MASLD) and metabolic dysfunction-associated steatohepatitis (MASH) on heart, muscle, and kidney related diseases.

Metabolic dysfunction-associated steatotic liver disease (MASLD), previously known as non-alcoholic fatty liver disease (NAFLD), is the most common liver disorder worldwide, with an estimated global prevalence of more than 31%. Metabolic dysfunction-associated steatohepatitis (MASH), formerly known as non-alcoholic steatohepatitis (NASH), is a progressive form of MASLD characterized by hepatic steatosis, inflammation, and fibrosis. This review aims to provide a comprehensive analysis of the extrahepatic manifestations of MASH, focusing on chronic diseases related to the cardiovascular, muscula

Associations of cardiac function and arterial stiffness with cerebrovascular disease.

BACKGROUND: White matter hyperintensities (WMHs) represent diffuse small vessel disease implicating the cardiac, systemic, and cerebral vasculatures. As the brain may be the end-organ of cumulative vascular disease, and higher education is protective of both cardiovascular and brain health, we aim to clarify their intertwining relationships. METHODS: We evaluated participants (mean age = 64) from the UK Biobank with neuroimaging measures of WMHs, left ventricular ejection fraction (LVEF) quantified using cardiovascular MRI, and arterial stiffness index (ASI) quantified using finger photopleth

The potential link between acromegaly and risk of acute ischemic stroke in patients with pituitary adenoma: a new perspective.

Acromegaly is an endocrine disorder due to the excess production of growth hormone (GH) from the anterior pituitary gland after closed epiphyseal growth plates. Acromegaly is mainly caused by benign GH-secreting pituitary adenoma. Acute ischemic stroke (AIS) is one of the most common cardiovascular complications. It ranks second after ischemic heart disease (IHD) as a cause of disability and death in high-income countries globally. Thus, this review aimed to elucidate the possible link between acromegaly and the development of AIS. The local effects of acromegaly in the development of AIS are

Moving from Evidence to Implementation of Breakthrough Therapies for Diabetic Kidney Disease.

Diabetic kidney disease is the most frequent cause of kidney failure, accounting for half of all cases worldwide. Moreover, deaths from diabetic kidney disease increased 106% between 1990 and 2013, with most attributed to cardiovascular disease. Recommended screening and monitoring for diabetic kidney disease are conducted in less than half of patients with diabetes. Standard-of-care treatment with an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker is correspondingly low.
