Congenital tricuspid incompetence simulating pulmonary atresia with intact ventricular septum: a report of two cases.

Two infants with isolated congenital tricuspid incompetence appeared to have associated right ventricular outflow tract obstruction at diagnostic evaluation, including catheterization and cineangiography. One infant died during surgery; the other improved rapidly and survived with medical therapy. We discuss the problem of establishing the presence of patency of the right ventricular outflow tract in infants with massive tricuspid incompetence and outline an approach to the management of infants whose diagnosis remains unclear even after careful evaluation.

[State of the coronary bed and certain indicators of hemodynamics and myocardial contraction in auricular fibrillation].

Selective coronary angiography was performed and cardiac catheterization was employed to study the indices of the myocardial contractility and the state of central haemodynamics in 20 patients with ischaemic heart disease, 24 patients with myocarditic cardiosclerosis complicated by cardiac fibrillation, 6 patients with "idiopathic" atrial fibrillation, and 20 patients with extracardiac pathology (controls). The data obtained by the author indicate that selective coronary angiography helps significantly in the differential diagnosis between myocarditic cardiosclerosis and ischaemic cardiomyopat

[Cardiovascular malformations in embryofetal alcohol syndrome (author's transl)].

17 infants and children with embryofetal alcohol syndrome were examined of cardiovascular malformations. In 8 cases heart catheterization revealed malformations, mainly atrial septum defects (in 5 of 8 cases), only in one case a ventricular septum defect, in another case an aplasia of the right pulmonary artery and in one case a mild outflow tract obstruction of the left ventricle. Congenital heart diseases can be expected in nearly 50% of the cases in embryofetal alcohol syndrome.

Modern Approach to Vascular Access in the Critically Ill Patient.

Vascular access is a key intervention in the management of critically ill emergency department patients. This can include peripheral or central venous access and intraosseous access. In combination with arterial catheterization, central venous access can provide key information to guide volume and vasoactive medication titration in isolation as well as in combination with dynamic measures of fluid responsiveness. This article will review the indications and contraindications as well as decision-making for the use of these modalities in light of the current evidence.

Protocol for inducing cystitis via direct instillation of acrolein into the bladder of mice.

Mouse models of bladder inflammation, termed cystitis, can be used to study urinary dysfunction and visceral pain in both sexes. Here, we present a protocol for a mouse model of cystitis that is induced by instilling acrolein directly into the urinary bladder using a transurethral catheter. We provide instructions for preparing the acrolein solution and catheterizing female and male mice. We then detail procedures for confirming the voiding dysfunction phenotype using the void spot assay.

Practice patterns for patients with ST-elevation myocardial infarction during the early phase of the COVID-19 pandemic-Valuable lessons learned.

During the early phase of the COVID‐19 pandemic in the United States, there was a significant reduction in cardiac catheterization lab activations and numbers of PPCIs for STEMI as well as an increase in door‐to‐balloon time. The decrease in STEMI volume is likely multifaceted involving factors at the patient and health system levels. Longitudinal data on STEMI care beyond the early phase of the pandemic is needed to better understand how different health systems have been adapting to the ongoing pandemic.

Infantile Scimitar Syndrome With Contralateral Pulmonary Vein Stenosis and Refractory Pulmonary Hypertension.

Infantile scimitar syndrome is associated with pulmonary hypertension which can be difficult to manage. We present a three-month-old infant with scimitar syndrome, who eventually developed refractory pulmonary hypertension, posing a significant management challenge. Further workup demonstrated contralateral pulmonary vein stenosis, which is rarely described in scimitar syndrome. Our index case highlights the importance of follow-up cardiac catheterizations in these patients with severe pulmonary hypertension.

Urinary Catheter Dependent Loops as a Potential Contributing Cause of Bacteriuria: An Observational Study.

Urologic studies suggest that urinary catheter dependent loops (tubing low points) may be a contributing cause of bacteriuria and urinary tract infection among catheterized patients. The means by which this type of contaminant transmission occurs, however, remains poorly understood. An observational, cross-sectional study was conducted to provide a foundational look at catheter dependent loops and their possible role in catheter-acquired urinary tract infections, and as a building block for further research.

[Scrotal mass presenting as giant urethral diverticulum].

Urethral diverticulum in the male is a rare entity that may be congenital or acquired. They are common in paraplegic patients, who are prone to developing this disorder on an acquired basis because of prolonged catheterization. The most common diseases in patients with spinal cord injury are stricture, fistula and diverticula. Patients with diverticula typically present with symptoms of urinary incontinence, dysuria, perineal pain, or a mass on the ventral aspect of the genitalia or perineum. Treatment of choice is always surgical and a complete extirpation should be performed.

Prolonged postpartum urinary retention: the importance of asking the right questions at the right time.

The pathophysiology of postpartum urinary retention (PUR) is still unclear. The prevalence rate varies with the definition used and the literature discusses various risk factors. Bladder catheterization is the treatment of choice, but the best way to do it is not ascertained. Long-term sequelae seem to be rare. There are no widely accepted preventive measures. Our 2 cases illustrate that it is important to prevent PUR not only by avoiding the known risk factors, but also by additionally asking for signs of voiding dysfunction postpartum.

[Ultrasound diagnosis of vascular complications following transfemoral puncture].

Percutaneous catheter interventions are performed increasingly often for the purposes of both diagnosis and therapy. The incidence of local complications reported after transfemoral catheterization in the literature is 0.1-9%. Such complications should be detected as early and reliably as possible, and sonography is a good means of finding them during follow-up, not least because it is suitable for use in examinations at the bedside. The clinical value of sonography compared with other examination techniques is made clear with reference to specimen cases.

[Pelvic phlebitis presenting as paradoxical arterial embolism].

The authors report the case of a 24-year-old woman with paradoxical embolism of the right arm subsequent to childbirth. Patent foramen ovale was diagnosed by right cardiac catheterization and contrast echocardiography. On the basis of clinical, immunoscintigraphic and radiologic data, the pelvic region was the only possible origin of the embolic process. The main interest of this case is that arterial ischemic signs were indicative of an embolic process of pelvic origin diagnosed by scintigraphy using radiolabeled antifibrin antibody.

[Virilizing tumor in a woman. Management].

Androgen secreting ovarian or adrenal neoplasms represent a rare cause of hirsutism in women. Because of their small volume and episodic secretion, a number of these tumors may be difficult to diagnose and treat. The authors report a case of lipoid cell tumor of the ovary, which was only detected, because of its small volume, by catheterization of adrenal and ovarian veins. This latter procedure should be performed when strong clinical and biological suspicion is faced by the negativity of other imaging techniques.

[A method for correcting dislocation in hemorrhages into the cerebral hemispheres].

Hypertension consequent upon increasing brain edema, and intercerebral pressure gradient which is the cause of transverse dislocation diminish with the use of a method which provides for hydrodynamic equilibrium. The method consists in connecting the ventricle of the intact cerebral hemisphere to a balloon located in the cavity of the removed hematoma according to the principle of communicating vessels. The craniospinal pressure gradient is removed by additional catheterization of the spinal subarachnoid space and its connection to the ventricle and balloon according to the same principle.

Thermodilution cardiac output measurement with a large left-to-right shunt.

Cardiac output was measured by the thermodilution method in a patient with a left-to-right shunt undergoing cardiac catheterization. It appeared that the thermodilution method measured systemic rather than pulmonary blood flow. This occurred because of the slow injection of injectate in the presence of a large left-to-right shunt. Theoretical thermodilution cardiac output curves are provided to illustrate the interaction of these two factors when three different durations of injection and four different shunt sizes are used.

[Iatrogenic injuries of the venous femoroiliac axis].

The occurrence of iatrogenous vascular accidents is underestimated. The authors describe 14 venous lesions to the femoro-iliac axis out of 60 iatrogenous vascular traumatisms. They consider the different aetiologies, underlining the disadvantages of femoral catheterisms and the main surgicalfactors contributing to venous traumatism. They emphasize the need for preventive measures in each discipline, and also the need for rapid diagnosis, thorough treatment, and careful post-operative after-care, bearing in mind secondary thrombo-embolic complications.

Sacral agenesis and neurogenic bladder dysfunction. A case report and review of the literature.

A case of partial sacral agenesis resulting in mixed upper and lower motor neurogenic bladder dysfunction is reported. Although the patient also has a concomitant congenital absence of one forearm she is being managed successfully on intermittent clean self-catheterization, illustrating the value of this modality in the management of neurogenic bladder dysfunction and that it can quite easily be managed by a young woman with only one functional hand. A review of the literature on this uncommon condition is also presented.

Echocardiography in congenital heart disease.

It has been demonstrated that echocardiography is a useful and accurate noninvasive technique for the study of congenital cardiovascular diseases. This method provides significant information in that invasive cardiac catheterization and angiocardiography can be planned with greater facility. This is especially important in severely ill babies in whom invasive diagnostic methods are risky. It is particularly attractive in neonates in whom the clinical differentiation of lethal heart disease from septic shock is difficult, since in the latter situation the echographic findings are normal.

[Iatrogenic hypospadias a preventable rare complication with two-stage repair with buccal mucosa graft: a case presen- tation].

Urethral catheterization is a common procedure, but it is associated with a number of complications. Iatrogenic hypospadias can rarely occur. There is a limited literature dedicated to this condition. We report a young patient with COVID-19 with iatrogenic hypospadias of grade 3. He was undergone to a two-stage procedure with acceptable outcome. Surgical repair should be offered and performed for young patients to ensure good function with acceptable penile appearance. A surgical treatment will improve psychological, sexual and social outcomes.

Macedo procedure: A continent catheterizable ileum-based reservoir. Step-by-step video.

This video provides a case report of a 16-year-old male patient who underwent bladder enlargement with a catheterized conduit under Macedo's technique. This technique involves the use of a segment of the distal ileum with a flap that is used for confection of the conduit. Key points include: 1) skin incision planning; 2) bladder release to allow a tension-free anastomosis; 3) reservoir and a conduit creation with a distal ileum; and 4) creation of a continence mechanism.
