Treatment of somatization in primary care: evaluation of the Personal Health Improvement Program.

The objective of this evaluation is to examine the clinical utility of a behavioral medicine intervention for treatment of prevalent, ill-defined physical symptoms in primary care. Commonly known as somatization, these symptoms have no known organic or psychiatric cause, and are poorly treated by biomedicine alone. Designed to treat these mood-related physical symptoms, the Personal Health Improvement Program (PHIP) is a 6-week structured group intervention which includes classroom videos, exercises and home study assignments. Data were collected from 21 PHIP courses offered at both staff and

Outcome assessment in epilepsy: comparative responsiveness of quality of life and psychosocial instruments.

PURPOSE: Few data exist on the ability of instruments to detect within-patient change over time in epilepsy, a property referred to as responsiveness. Our aim was to compare the responsiveness of three instruments [i.e., Epilepsy Surgery Inventory-55 (ESI-55) and Washington Psychosocial Inventory (WPSI), both epilepsy specific, and Symptom Checklist-90-Revised (SCL-90-R), non-epilepsy specific]. METHODS: Instruments were administered at baseline and at 1 year in a prospective cohort of surgically (43) and medically (14) treated patients with temporal lobe epilepsy. Coefficient of Responsivene

Caffeine consumption in hospitalized psychiatric patients.

A total of 98 consecutively admitted psychiatric inpatients were asked for their daily consumption of coffee, tea and other products containing caffeine. Calculation of the corresponding daily caffeine intake was performed using data from the literature and from caffeine measurements carried out in different coffee and tea preparations in the hospital. Of the patients 13% presented a high (> or = 750 mg daily) caffeine consumption before hospitalization. The average caffeine consumption per day decreased from 405 mg before to 332 mg during hospitalization (P < 0.04), but the before and during

Scales, diagnoses, and child psychopathology: I. CBCL and DISC relationships.

OBJECTIVE: To clarify the relationship between scales and structured diagnostic interview diagnoses, the authors used a two-stage screening method to study 201 military families with one or more children ages 5 to 17. METHOD: Parents and children were interviewed with the Diagnostic Interview Schedule for Children (DISC 2.1); parents also completed the Child Behavior Checklist (CBCL) while the children completed other self-report symptom scales. RESULTS: Results indicate only a modest ability of scales to discriminate among discrete DISC-derived DSM-III-R diagnoses. Inclusion of diagnostic i

Who gets treated? Factors associated with referral in children with psychiatric disorders.

There is evidence from several industrialized countries that only a small proportion of children with psychiatric disorders receive specialist treatment. It is unclear, however, why some disturbed children are brought for treatment while others are not. To examine this issue in one community, children aged 7 through 11 were screened for behavioral problems using the Child Behavior Checklist (CBCL); 89 treated and 126 nontreated children, all of whom scored in the clinical range (above the 90th percentile) on the CBCL, were compared on measures of psychopathology, environmental factors and adap

Instability and falling in elderly patients.

Falls and instability in elderly patients may result from a single disease process or from the accumulated effect of multiple diseases and impairments. Therefore the first step in evaluating an elderly patient with a history of falling or instability is to identify single, potentially treatable diseases, such as normal pressure hydrocephalus, cervical spondylosis, or lumbar stenosis. The next step, regardless of whether or not a single disease process is identified, is to determine all factors possibly contributing to instability. This step involves a careful history and examination using the

Open assessment of the safety and efficacy of thioridazine in the treatment of patients with borderline personality disorder.

A 12-week open study was conducted to assess the possible efficacy and safety of low-dose thioridazine in 11 outpatients (8 women, 3 men) with borderline personality disorder (BPD) diagnosed according to DSM-III-R (American Psychiatric Association 1987) and the Diagnostic Interview for Borderline Patients (DIB; Gunderson et al. 1981) criteria. Mean thioridazine dose averaged 92 mg per day across the duration of this study. At endpoint, there was a significant reduction in Brief Psychiatric Rating Scale (BPRS; Overall & Gorham 1988) scores, and patients appeared to be less symptomatic on the im

Cellular Crosstalk at the Gut-Liver Interface in Liver Cirrhosis: Epithelial Barrier Failure, Bacterial Translocation and Immune Activation as a Bidirectional Network.

The gut-liver axis is central to the systemic complications of liver cirrhosis, but it is usually described as a linear cascade in which barrier failure releases microbial products that activate a downstream immune response. This review proposes instead a four-compartment framework, in which the epithelial/barrier, innate immune, adaptive effector, and stromal-endothelial compartments-the last including the gut-vascular barrier-are treated as reciprocally interacting nodes, and pathological bacterial translocation is treated as both an input to and an output of the network rather than as its e

Fear-related psychological experiences in people with Parkinson's disease: a systematic review of qualitative studies.

BACKGROUND: Fear is a multifaceted and pervasive phenomenon among people with Parkinson's disease. Although some qualitative studies have explored fear-related psychological experiences in this population, there remains a lack of comprehensive integration of these insights. This study aimed to synthesize existing qualitative research on fear-related emotional experiences in people with Parkinson's disease, and to provide a theoretical basis for developing effective psychological interventions in clinical practice. METHODS: A comprehensive computer-based search was conducted across 11 database

Skin-to-skin contact in neonatal intensive care units: a narrative review of implementation challenges and strategies within infant- and family-centered care.

Skin-to-skin contact (SSC) is a key component of infant- and family-centered developmental care in neonatal intensive care units (NICU) and is associated with improved clinical and neurodevelopmental outcomes. However, SSC implementation in NICUs remains highly variable, often limited by perceived barriers and uncertainties regarding safety. This review aims to identify these barriers and summarize strategies to support the safe and consistent integration of SSC into routine neonatal care. Current evidence identifies barriers at multiple levels, including clinical factors (extreme prematurity,
