A case of pulmonary nodules in a patient living with HIV diagnosed with secondary syphilis.

We discuss a case of secondary syphilis with pulmonary involvement in a 45-year-old man who tested positive for HIV. He presented with dyspnoea, chest pain and a rash on his limbs and torso. A CT showed multiple bilateral necrotic lung nodules. A diagnosis of pulmonary syphilis was made due to his respiratory symptoms and imaging, his serological, histopathology findings, and the resolution of symptoms on treatment with benzathine penicillin.

Spontaneous splenic rupture: case report and review of literature.

Splenic rupture is a potentially life-threatening condition, often associated with chest or abdominal trauma. Spontaneous rupture is very rare and is usually reported as being secondary to underlying pathological conditions. We report a case of atraumatic splenic rupture in a patient with no underlying disease pathology. This case should remind the emergency physician spontaneous splenic rupture should be considered in the differential diagnosis of unexplained acute abdominal pain.

Takotsubo cardiomyopathy triggered by wasabi consumption: can sushi break your heart?

Takotsubo cardiomyopathy is a left ventricular dysfunction that typically occurs after sudden intense emotional or physical stress and mimics myocardial infarction. We describe a case of a 60-year-old woman that presented to the emergency department with chest pain after she attended a wedding and ate a large amount of wasabi, assuming it to be an avocado. To the best of our knowledge, this is the first report of takotsubo cardiomyopathy triggered by wasabi consumption.

Spontaneous splenic rupture in a patient with infectious mononucleosis.

We present a case of spontaneous, atraumatic splenic rupture secondary to Epstein-Barr virus (EBV) infection, in a young, female patient. Splenic rupture is a rare complication of EBV infection, but is associated with the highest mortality. Additionally, this case illustrates the diagnostic challenge in a patient presenting in atypical manner, with only left-sided pleuritic chest pain, and lacking any of the classical tonsillitis symptoms associated with EBV infection.

[Multiple abdominal venous aneurysms revealed by pulmonary embolism].

Venous aneurysms are rare lesions, they have been reported in most venous territories. Abdominal location is unusual. A 61-year-old man presented with chest pain and dyspnea attributed to pulmonary embolism. Computed tomography of the abdomen showed multiple thrombosed venous aneurysms of the left external iliac vein and the inferior vena cava. There were no signs of rupture. The patient received anticoagulant treatment. He had no surgical treatment.

Diffuse Increased Sodium Fluoride Gastric Activity in a Patient With Hypercalcemia.

Increased extraosseous uptake of sodium fluoride can be visualized because of many different etiologies. A 62-year-old man with a history of osteoporosis had an F-sodium fluoride PET/CT scan for accessing chest pain caused by an accident. The images showed multiple fractures. Unexpectedly, diffuse activity was noted in the gastric wall, which was suggestive of possible hypercalcemia. The serum calcium level confirmed the patient had hypercalcemia.

Ineffective Esophageal Motility Progressing into Distal Esophageal Spasm and Then Type III Achalasia.

The clinical significance of minor esophageal motility disorders is unclear, though they typically carry a benign course. Distal esophageal spasm progressing to achalasia has been reported, although it appears to be rare. We report a case of a patient with dysphagia and chest pain who was found to have ineffective esophageal motility on high-resolution manometry, which developed into distal esophageal spasm and then progressed to type III achalasia.

Blunt traumatic dissection of right coronary artery presenting with acute inferior wall myocardial infarction: Dilemma in management.

Thirty-nine year male had a history of road traffic accident with polytrauma. At emergency room he started having chest pain with ventricular tachycardia. He was subsequentially diagnosed with right coronary artery dissection secondary to blunt trauma which is an extremely rare cause of inferior wall myocardial infarction. After some dilemmas, he was ultimately treated with intravascular ultrasound guided coronary angioplasty with stenting and had an uneventful recovery.

"Killer coronary artery" and aortic valve stenosis: A tricky case.

Anomalous origin of the left main coronary artery from the right coronary sinus is rarely diagnosed in elderly patients. We report such an anomaly in a 75-year-old lady presenting with chest pain and syncope. Preoperative screening revealed that her aortic valve was moderately stenotic. The patient underwent a successful unroofing procedure combined with aortic valve replacement. The outcome was uncomplicated and the patient remained asymptomatic at one year postoperatively.

Management of anomalous circumflex coronary artery from the neo-pulmonary artery in an adolescent following neonatal arterial switch operation.

A 15-year-old male with transposition of the great arteries presented with exertional chest pain. He was found to have a circumflex coronary artery from the neo-pulmonary artery that had not been transferred during his arterial switch operation. The circumflex coronary artery, fed through collaterals from a re-implanted single coronary artery, resulted in coronary steal. This report describes a management pathway to treat this rare anomaly.

Ruptured thymoma managed via thoracotomy.

Thymomas rarely present with chest pain due to hemorrhage. This could cause shortness of breath if it ruptures into the pleural space, and is best managed surgically. We describe the case of an 83-year-old woman who presented with such symptoms. Computed tomography showed a ruptured mediastinal mass with pleural effusion. She was managed successfully by thoracotomy with excision of the mass and drainage of the effusion. Histopathology revealed a ruptured thymoma with infarction and necrosis.

Vitello-intestinal duct injury after transabdominal preperitoneal inguinal hernia repair.

A 71-year-old patient underwent a transabdominal preperitoneal right inguinal hernia repair and presented to the emergency department 48 hours postoperatively with nonspecific abdominal and chest pain and was diagnosed an abdominal abscess at the umbilicus by computed tomography. Laparotomy showed a vitello-intestinal duct injury caused by the umbilical trocar at his previous laparoscopic surgery. We will discuss this rare complication and how to avoid it in future practice.

Spontaneous coronary dissection in polycystic kidney disease.

We report a case of a 46-year-old woman with hypertension and autosomal dominant polycystic kidney disease who presented with chest pain and was found to have spontaneous coronary artery dissection (SCAD) on diagnostic catheterization. We review the pathogenesis, management and prognosis of SCAD. We conclude that in patients with polycystic kidney disease who present with angina pectoris and positive cardiac biomarkers, coronary artery dissection should be considered.

Simultaneous Esophageal and Gastric Ulceration Due to Doxycycline Ingestion: Case Report and Review of the Literature.

Doxycycline induced mucosal injury to the esophagus and, rarely, stomach is an under appreciated cause of odonyphagia and chest pain. The diagnosis is dependent on a comprehensive exposure history and endoscopic evaluation. Once recognized, doxycycline induced ulceration is easily treated with cessation of the medication and gastric acid suppression. Our case report describes a classic description of doxycyline induced esophageal ulceration with the unusual endoscopic finding of a simultaneous antral ulcer.

Symptomatic pericardial cyst: a case series.

Pericardial cysts are most commonly located at the cardiophrenic angle or, rarely, in the posterior or anterior superior mediastinum. The majority of pericardial cysts are asymptomatic and are found incidentally. Symptomatic pericardial cysts present with dyspnoea, chest pain, or persistent cough. We describe four patients with symptomatic pericardial cysts who were treated with either echocardiographically guided percutaneous aspiration or video-assisted thoracoscopic surgery, or both; thoracotomy; or conservative therapy.

Anomalous origin of right coronary artery associated with hypertrophic obstructive cardiomyopathy.

A 29-year-old man with anginal chest pain and recurrent syncopal attacks was observed with invasive and noninvasive cardiodiagnostic techniques, which disclosed an anomalous origin of right coronary artery from the left coronary cusp and hypertrophic obstructive cardiomyopathy. The authors report a very rare coexistence of these 2 clinical entities, both of which are well known to independently increase the likelihood of sudden cardiac death under strenuous physical stress.

Acute aortic dissection mimics acute inferoposterior wall myocardial infarction in a Marfan syndrome patient.

A 30-year old man with acute chest pain was diagnosed with acute inferoposterior wall myocardial infarction following electrocardiography. After a failed coronary angiography, an echocardiogram revealed an aortic intimal flap after which acute aortic dissection was diagnosed. The patient received a successful Bentall operation without immediate complication. Retrospective examination then confirmed the diagnosis of Marfan syndrome. This case demonstrates acute aortic dissection may mimic acute myocardial infarction.

[Pulmonary artery compression by a mediastinal tumor].

Acquired arterial pulmonary stenosis is rarely found in adults. Reviews and case reports suggest that tumours of the mediastinum (teratomatas and Hodgkin's disease) are the most frequent culprits leading to compression of the main pulmonary artery. We present the case of a young patient with chest pain. Compression of the main and left pulmonary artery by an anterior mediastinal nonseminomatous germ cell tumors (GCTs) was diagnosed by transthoracic echocardiography and CT scan.
