Emerging pathogen: a case and review of Raoultella planticola.

Raoultella planticola has been considered a relatively harmless Gram-negative bacteria, rarely associated with clinical infection. However, in recent years, the frequency at which severe infection by R. planticola and drug-resistant strains are reported in literature has increased. Here, we present one case of acute cholecystitis caused by R. planticola, and review all previously reported cases of the infection in an attempt to identify new trends in biological and clinical features of R. planticola infections.

[Cetuximab improved the poor performance status of a colorectal cancer patient].

A 60-year-old man with unresectable colon cancer required treatment for hydronephrosis, acute cholecystitis, and obstructive jaundice before chemotherapy. His performance status (PS) gradually deteriorated to PS 4. Cetuximab monotherapy was initiated instead of intensive chemotherapy. His general condition improved and mFOLFOX7 therapy was then continued in addition to cetuximab. A computed tomography (CT) scan after 6 months of chemotherapy revealed a partial response (PR). Cetuximab monotherapy may contribute to the treatment of patients with a poor PS.

Evaluation of the biliary tree and gallbladder with hepatocellular MR contrast agents.

Newer hepatobiliary magnetic resonance (MR) contrast agents allow the ability to combine the anatomical information from T2-weighted magnetic resonance cholangiopancreatography with functional information derived from contrast-enhanced T1-weighted magnetic resonance cholangiography for off-label biliary imaging. Potential applications include determination of biliary anatomy, functional assessment of biliary obstruction, and evaluation for acute cholecystitis. Through MR protocol optimization, it is also possible to achieve this within the standard 30-minute imaging window of liver MR.

[Acute gastrointestinal involvement in dengue disease by serotype 4: a case report and literature review].

Dengue fever is the world's most important arboviral disease, presenting a wide clinical spectrum. We report for the first time in Peru, a case caused by dengue virus serotype 4 with significant gastrointestinal involvement (acute acalculous cholecystitis and acute hepatitis). In addition we carried out a review of the literature atypical presentation illustrating the importance of the characteristics of abdominal pain (right upper quadrant); presence of Murphy's sign, ultrasound, and liver enzymes levels, for appropriate diagnosis and clinical management.

Gallbladder pseudolithiasis caused by ceftriaxone in young adult.

Ceftriaxone is a commonly used antibiotic due to some of its advantages. Reversible gallbladder (GB) sludge or stone has been reported after ceftriaxone therapy. Most of these patients have no symptom, but the GB sludge or stone can sometimes cause cholecystitis. We experienced two patients who had newly developed GB stones after ceftriaxone therapy for diverticulitis and pneumonia, and this resolved spontaneously 1 month after discontinuation of the drug. Awareness of this complication could help to prevent unnecessary cholecystectomy.

Conservative management of segmental infarction of the greater omentum: a case report and review of literature.

Segmental omental infarction (SOI) is a rare cause of acute abdominal pain. Depending on the site of infarction, it mimics conditions like appendicitis, cholecystitis, and diverticulitis. Before the widespread use of Computed Tomography (CT), the diagnosis was usually made intraoperatively. SOI produces characteristic radiological appearances on CT scan; hence, correct diagnosis using this form of imaging may prevent unnecessary surgery. We present the case of a young woman who was treated conservatively after accurate radiological diagnosis.

Segmental omental infarction.

Acute right-sided abdominal pain is a common presenting symptom in the emergency department. Acute cholecystitis and acute appendicitis are the most likely etiologies for right upper and lower quadrant pain, respectively. However, other differential possibilities include right-sided diverticulitis and perforated colon carcinoma. This case report of an 18-year-old man with segmental omental infarction highlights a much less frequent, self-limited cause of right-sided abdominal pain, which is increasingly identified on computed tomography scans.

[A case of general anesthesia combined with epidural anesthesia in a pregnant woman undergoing laparoscopic cholecystectomy].

We report a case of general anesthesia for laparoscopic cholecystectomy at 12 weeks of gestation. A 20-year-old woman weighing 123 kg was admitted with epigastralgia. She was diagnosed as pregnancy of 6 weeks of gestation and acute cholecystitis. Percutaneous trans-gallbladder drainage was performed to delay operation until 12 weeks of gestation. Laparoscopic cholecystectomy was performed uneventfully under general anesthesia combined with epidural anesthesia. There were no clinical signs of fetal distress during the perioperative period.

[Emergency laparoscopy in the examination of middle-aged and elderly patients with acute surgical diseases of the abdominal cavity].

Urgent laparoscopies were performed on 566 elderly and senile patients suspected of acute surgical diseases of abdominal organs. Laparoscopic examinations proved to be highly informative in 90.4%, little informative in 6.6%, erroneous in 3% of cases. Wide use of urgent laparoscopies in elderly and senile patients allowed to avoid laparotomies in 62.3% of the cases. High curative-diagnostic efficiency of laparoscopies added by laparoscopic cholecystostomy and antegrade cholecystocholangiography in patients with acute cholecystitis, cholecystopancreatitis, mechanical jaundice has been shown.

[Surgical therapy of intestinal complications in abdominal typhus].

At our clinic 185 patients with severe typhoid fever were treated. In 51 cases severe complications required a surgical therapy: 24 times bleeding of small or large intestine, 14 times perforation of the bowel and 13 times acute typhoid cholecystitis. Lethality of typhoid peritonitis with perforation is nowadays--inspite of specific therapy with antibiotics--still between 40% and 50%; lethality at our clinic was 37.5%. Dangerous complications are bleeding and re-perforation, which require an aggressive surgical procedure.

Laboratories to Scrutinize.

Isolated laboratory values should not be a driving force when it comes to diagnosing patients. There are many causes for most laboratory values to be abnormal, and the entire clinical picture, including history and physical examination, are important when caring for patients. If a provider has a high enough clinical suspicion for a certain diagnosis, then a normal laboratory value should not falsely reassure a provider. Prime examples are a normal lactate in a patient with mesenteric ischemia, or a normal white blood cell count in a patient with acute cholecystitis.

Metastatic Colon Cancer Presenting as Gallbladder Perforation: Highlighting the Diagnostic Challenges and Clinical Implications.

Gallbladder perforation is an uncommon occurrence that demands prompt surgical intervention, typically observed in the context of acute cholecystitis. In this article, we present an extraordinary case of gallbladder gangrene and perforation, originating from metastasis of colon cancer. The patient's presentation included an incidental discovery of colon cancer, which was indicated by histopathology of the gall bladder. This case report aims to shed light on the intricate relationship between gallbladder pathology and metastatic colon cancer, emphasizing the need for vigilant evaluation and com

Two distinct episodes of life-threatening hemobilia due to a lesion of common bile duct and delayed intrapancreatic arteriobiliary fistula managed by emergency pancreatoduodenal resection.

Hemobilia is an extremely rare cause of upper gastrointestinal bleeding. It often has intermittent manifestation, which may lead to significant diagnostic delay. In 65% of the cases, the causes are iatrogenic, in 7% the cause is malignancy, in 5% - gallstones, in 8% it is inflammation (cholecystitis, parasites, reflux cholangitis), vascular abnormality is the cause in 7% (most commonly pseudoaneurysm of the hepatic artery), and pancreatic pseudocyst causes hemobilia in 1%. In almost all cases, the bleeding originates from intrahepatic or extrahepatic bile ducts, and rarely from the pancreas.

Outcomes and limitations in EUS-guided gallbladder drainage.

EUS-guided gallbladder drainage (EUS-GBD) is gaining popularity as an option for drainage of the gallbladder in patients suffering from acute cholecystitis but at high risk for cholecystectomy. It allows internal drainage of the gallbladder and avoidance of the external tube as used in percutaneous cholecystostomy (PT-GBD). It may also provide additional benefits, including reduced re-admissions and re-interventions. In this chapter, we review the indications and outcomes of EUS-GBD. Furthermore, the follow-up management of patients that received EUS-GBD would be outlined.

Rare presentation of haemobilia and Loeffler's pneumonia in a child by ascaris lumbricoides.

Ascariasis is a soil-sourced, second most common parasitic infection worldwide. Because of its wandering nature, it migrates from the intestine to other organs of the body like the lungs and biliary system. This results in complications such as biliary colic, acute cholecystitis, pyogenic cholangitis, liver abscesses, pancreatitis and loeffler's pneumonia. We report a unique case of an 8-year-old boy who presented with upper gastrointestinal bleed and chest infection. He was diagnosed as haemobilia and loeffler's pneumonia caused by ascaris lumbricoides.

The Pucker sign: an operative and radiological indicator of impending operative difficulty due to severe chronic contractive inflammation in cholecystectomy.

INTRODUCTION: Marked inflammatory contraction of the gallbladder at the time of cholecystectomy is associated with biliary injury. METHODS: The evolution of contraction is traced in two patients who developed acute cholecystitis. RESULTS: Within months the gallbladder became fibrotically contacted and in doing so pulled the adjacent liver down, puckering the surface of the liver. This puckering could be observed preoperatively on computed tomography and also visually at the time of surgery. CONCLUSIONS: Puckering of the liver ("Pucker sign") can alert the surgeon to the presence of severe c

Case Report of Ectopic Liver on Gallbladder Serosa with a Brief Review of the Literature.

This case describes an intraoperative incidental finding and surgical removal of ectopic liver tissue attached to the gallbladder during a standard laparoscopic cholecystectomy for acute cholecystitis. These anomalies are rare, with interesting associations and possible clinically relevant complications. The details of the case, along with a brief literature review of embryology, common ectopic sites, and associations/complications, are presented in this paper. Since laparoscopic cholecystectomy is a very common procedure, it is important to increase vigilance of ectopic liver tissues during s

Intraluminal Bowel Erosion: A Rare Complication of Retained Gallstones after Cholecystectomy.

Laparoscopic cholecystectomy for acute cholecystitis and cholelithiasis is one of the most common operations performed in the United States. Inadvertent perforation and spillage of gallbladder contents are not uncommon. The potential impact of subsequent retained gallstones is understated. We present the case of an intraperitoneal gallstone retained from a previous cholecystectomy eroding into the bowel and leading to intraluminal mechanical bowel obstruction requiring operative intervention. This case illustrates the potential risks of retained gallstones and reinforces the need to diligently

Intraabdominal focal fat infarction in a 75-year-old woman presenting as acute abdomen.

Epiploic appendagitis is a rare, self-limiting inflammation of the epiploic appendices or omental appendices. It presents as abdominal pain often misdiagnosed as appendicitis, cholecystitis, or diverticulitis. Epiploic appendagitis can be treated conservatively with anti-inflammatory and pain medications. It is important to diagnose this etiology of abdominal pain in order to avoid long-term hospital stay and other medical expenses including surgery. In this case report we present a rare case of epiploic appendagitis that presents in a 75 year old female patient.
