Leveraging paired germline and somatic analysis to improve the classification of DDX41 variants.

Constitutional pathogenic variants in DDX41 predispose to myelodysplasia and acute myeloid leukaemia. Acquisition of subsequent somatic hits in the second allele is frequent, with notable recurrent variants at key hotspots. Sequencing of Deoxyribonucleic acid from blood/marrow of 239 patients with suspected/confirmed haematological malignancies at a single centre within a 4-year period identified 136 unique DDX41 variants. Among those with co-occurring somatic and likely/confirmed germline variants, 54.8% of likely/confirmed germline variants were pathogenic when classified according to curren

A novel in vitro permeability and efflux classification system for proteolysis targeting chimeras.

Proteolysis targeting chimeras (PROTACs) have emerged as a promising modality over the past decade, with only a few advancing to clinical trials. As "beyond rule-of-five" compounds, PROTACs pose significant challenges for absorption, distribution, metabolism, and excretion, often necessitating iterative in vitro absorption, distribution, metabolism, and excretion assay optimization or reliance on in vivo pharmacokinetic studies, particularly to understand oral absorption. In this study, in vitro-in vivo correlations from clinically advanced PROTACs were leveraged to develop a novel classificat

Traditional serrated adenoma of the colorectum: Evolving concepts in histological classification and terminology.

Traditional serrated adenoma (TSA) is the least common of the currently recognized serrated colorectal polyps and, thus, elucidating its pathogenesis, spectrum of morphological features and biological risk has been challenging. Most TSAs harbour either KRAS or BRAF mutations and may progress to adenocarcinoma via distinct carcinogenic pathways. TSAs with KRAS mutation are more commonly located in the distal colorectum, whereas BRAF-mutant TSAs typically arise proximal to the transverse colon, may be associated with a background sessile serrated lesion and represent putative precursors of biolo

An update in the MRI features, classification, and associated brain abnormalities in hypothalamic hamartomas.

OBJECTIVES: Hypothalamic hamartomas (HHs) are rare, non-neoplastic malformations associated with gelastic seizures and, less frequently, precocius puberty. Recent classifications have linked HH subtypes with clinical features, and emerging evidence suggests that HH may co-occur with other brain abnormalities. This study investigates the imaging characteristics of HH, the prevalence of concurrent structural abnormalities, and metabolic alterations using magnetic resonance spectroscopy (MRS). METHODS: We retrospectively analyzed MRI and MRS findings in 18 patients with HH (mean age, 9.24 ± 8 ye

Complications of Laser and Energy-Based Procedures in Dermatology: Classification, Management, and Prevention.

Laser and energy-based devices (EBDs) are firmly established in dermatology and widely used for both medical and aesthetic purposes. Even with advanced safety features such as integrated cooling and real-time monitoring, these procedures still carry inherent risks. Complications may occur as a result of inappropriate device choice, suboptimal parameter settings, insufficient patient evaluation, or limited operator experience. This CME article provides an overview of the spectrum of adverse events associated with dermatologic laser and EBD treatments, ranging from expected short-term reactions

[Imaging assessment and treatment of Charcot arthropathy of the shoulder assisted by NGH classification].

Charcot arthropathy of the shoulder is a rare destructive joint disorder that is most commonly secondary to syringomyelia and often leads to osteolysis of the shoulder joint and severe upper limb dysfunction. The diagnostic and therapeutic challenges of this condition lie not merely in identifying joint destruction, but in the nonspecific nature of its early clinical manifestations, which may mimic infection, tumor, or inflammatory arthropathy. In addition, the extent of bone destruction in the glenoid and humeral head may vary considerably, making conventional imaging descriptions insufficien

The ischium as a key anatomical landmark in the classification of persistent urogenital sinus anomalies.

BACKGROUND: Congenital adrenal hyperplasia is an endocrine disorder that leads to external virilization in affected females, often accompanied by an abnormal vaginal insertion that joins the urethra, forming a common urogenital sinus. OBJECTIVE: We propose a new anatomical classification for common urogenital sinus anomalies based on the relationship between the distal vagina and the ischial bony landmark on lateral genitography. MATERIALS AND METHODS: The study included girls with virilized external genitalia secondary to congenital adrenal hyperplasia who underwent feminizing genitoplasty

Assessment, diagnosis, severity, and considerations for phenotype classification of trismus: a systematic review.

BACKGROUND: Trismus is a prevalent and functionally debilitating consequence of head and neck cancer (HNC) and its treatments, yet variability in assessment practices limits accurate incidence estimation, cross-study comparison, and development of targeted interventions. This systematic review maps current methodologies used to assess, diagnose, and classify trismus across clinical contexts. METHODS: Following Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines, eight databases and gray literature sources were searched (March 2025). On the impact of input resolution on CNN-based gastrointestinal endoscopic image classification.

Gastric cancer (GC) remains a significant global health issue, and convolutional neural networks (CNNs) have shown their high potential for detecting precancerous gastrointestinal (GI) conditions on endoscopic images [1] [2]. Despite the need for high resolution to capture the complexity of GI tissue patterns, the impact of endoscopic image resolution on the performance of these models remains underexplored. This study investigates how different image resolutions affect CNNs classification of intestinal metaplasia (IM) using two datasets with different resolutions and imaging modalities. Our r

Validation of clinicopathologic features of a genetic myelodysplastic syndrome classification in an independent cohort.

BACKGROUND: Current classification systems for myelodysplastic syndromes (MDS) incorporate morphologic findings, blast percentage, and some genetic features such as del(5q) and SF3B1 and TP53 mutations. A recent comprehensive molecular taxonomy proposed by the MDS-International Working Group (MDS-IWG) categorizes MDS into 16 molecular groups and two residual groups and describes associations with various clinicopathological features and differing overall survival among groups. PURPOSE: In this study, we attempt to validate the findings described in the MDS-IWG classification in an independent

Machine learning for early detection and severity classification in people with Parkinson's disease.

Early detection of Parkinson's disease (PD) and accurate assessment of disease progression are critical for optimizing treatment and rehabilitation. However, there is no consensus on how to effectively detect early-stage PD and classify motor symptom severity using gait analysis. This study evaluated the accuracy of machine learning models in classifying early and moderate-stages of PD based on spatiotemporal gait features at different walking speeds. A total of 178 participants were recruited, including 103 individuals with PD (61 early-stage, 42 moderate-stage) and 75 healthy controls. Parti
