Evaluation and Management of Young Febrile Infants: An Overview of the New AAP Guideline.

The American Academy of Pediatrics released a clinical practice guideline for the management of febrile infants in August 2021 to compile nearly 40 years of research into a cohesive text that would provide a framework for the clinician in safely managing these patients in a variety of settings. (1) This guideline incorporates shared decision-making with the caregiver to guide treatment when appropriate and provides algorithms for 3 age groups: 8 to 21 days, 22 to 28 days, and 29 to 60 days. This guideline applies to previously healthy, well-appearing infants born at 37 weeks' gestation or late

Frontal plane knee moment in clinical gait analysis: A systematic review on the effect of kinematic gait changes.

INTRODUCTION: The frontal plane knee moment (KAM1 and KAM2) derived from non-invasive three-dimensional gait analysis is a surrogate measure for knee joint load and of great interest in clinical and research settings. Many aspects can influence this measure either unintentionally or purposely in order to reduce the knee joint load to relieve symptoms and pain. All these aspects must be known when conducting a study or interpreting gait data for clinical decision-making. METHODS: This systematic review was registered with PROSPERO (CRD42020187038). Pubmed and Web of Science were searched for p

Comparing the activity of novel antibiotic agents against carbapenem-resistant Enterobacterales clinical isolates.

OBJECTIVE: We compared the activity of 8 novel β-lactam and tetracycline-derivative antibiotics against a cohort of clinical carbapenem-resistant Enterobacterales (CRE) isolates and investigated the incremental susceptibility benefit of the addition of an aminoglycoside, fluoroquinolone, or polymyxin to the β-lactam agents to assist with empiric antibiotic decision making. METHODS: A collection of consecutive CRE clinical isolates from unique patients at 3 US hospitals (2016-2021) was assembled. Broth microdilution was performed to obtain antimicrobial susceptibility testing results. Mechanis

The Implications of Genetic Testing on Radiation Therapy Decisions: A Guide for Radiation Oncologists.

The advent of affordable and rapid next-generation DNA sequencing technology, along with the US Supreme Court ruling invalidating gene patents, has led to a deluge of germline and tumor genetic variant tests that are being rapidly incorporated into clinical cancer decision-making. A major concern for clinicians is whether the presence of germline mutations may increase the risk of radiation toxicity or secondary malignancies. Because scarce clinical data exist to inform decisions at this time, the American Society for Radiation Oncology convened a group of radiation science experts and clinici

Increased in-hospital complications after primary posterior versus primary anterior cervical fusion.

BACKGROUND: Although anterior (ACDF) and posterior cervical fusion (PCDF) are relatively common procedures and both are associated with certain complications, the relative frequency and severity of these complications is unclear. Since for some patients either approach might be reasonable it is important to know the relative perioperative risks for decision-making. QUESTIONS/PURPOSES: The purposes of this study were to: (1) characterize the patient population undergoing ACDF and PCDF; (2) compare perioperative complication rates; (3) determine independent risk factors for adverse perioperativ

Towards a neuro-symbolic approach for precision anti-reflux surgery.

Surgical management of gastroesophageal reflux disease (GERD) is limited by non-technical challenges, including variability in patient selection, incomplete physiological assessment, imprecise procedure choice, and heterogeneity of intraoperative judgment. Artificial intelligence (AI) offers a promising approach to address these limitations. To enhance decision reproducibility and explainability, we advocate for the integration of AI models based on machine learning with formal logic-based reasoning. This neuro-symbolic approach enables the formal encoding of clinical knowledge, the management

Artificial Intelligence-Based Analysis of Laparoscopic Imaging for Intraoperative Surgical Decision Support.

Across surgical specialties, minimally invasive (laparoscopic) surgery has become a standard technique, as it is associated with less trauma, reduced postoperative complication rates, and quicker recovery for patients as compared with open surgery. Due to the limited field of view and limited haptic feedback, the surgical decision-making process in laparoscopic surgery is currently guided solely by surgeons' visual interpretation of the laparoscopic video stream. Modern artificial intelligence (AI) methods excel at the interpretation of visual data and find applications in clinical routine in

Responding to Parental Requests for Potentially Nonbeneficial Treatment in Life-Threatening Situations: Technical Report.

Although consensus on a medical treatment plan is ideally achieved through a process of shared decision making, disagreements about whether life-sustaining interventions should be initiated or continued can occur and are sometimes difficult to resolve. When parents (or other appropriate surrogate decision makers) desire treatments for their child that are not medically recommended, these "potentially nonbeneficial" interventions must be systematically considered in the context of contemporary ethical principles and relevant laws and regulations while keeping in mind the historical and cultural

Algorithmic Prognostication in Female Oncofertility Counseling: Ethical Challenges of Bias, Autonomy, and Predictive Uncertainty.

Advances in machine learning, predictive analytics, and clinical prediction modeling have accelerated the development of algorithmic tools for estimating reproductive outcomes after cancer treatment. In female oncofertility counseling, these models may support individualized assessment of treatment-related amenorrhea, premature ovarian insufficiency, and fertility risk, thereby improving risk communication and timely fertility-preservation referral. However, their use raises ethical concerns beyond predictive accuracy. This narrative review examines algorithmic prognostication in female oncofe

Guidance for chest-CT in children and adults with cystic fibrosis: A European perspective.

The European Cystic Fibrosis Society-Clinical Trials Network (ECFS-CTN) herein proposes guidance for the use of chest CT-scans for the regular monitoring of lung disease in CF. Statements were completed in a 3-step process: the questions were identified via an anonymous online survey, followed by a comprehensive literature search, and a final Delphi process. The guidance recommends the use of ultra-low dose CT scans (effective radiation dose, 0.08 mSv; equivalent to 2 to 4 chest X-rays), tracking of patients' cumulative radiation and effective communication strategies using "de-medicalized" in

Decision Tree for the Treatment of Multiple Gingival Recession Defects When Utilizing MCAT or MCAF Based on Evidence and Clinical Experience.

The multiple coronally advanced flap (MCAF) and the modified coronally advanced tunnel technique (MCAT) are the most commonly used methods for treating multiple gingival recessions. However, treating multiple defects is very complex due to various biologic and anatomical factors, and there is no clear guideline on the major or minor determinants that influence surgical decisions. The aim of the present article is to discuss a decision tree to suggest to clinicians the most relevant anatomical factors to consider when evaluating the choice between an MCAT and an MCAF. In the proposed decision-m

Navigating Shared Decision-Making in Non-Operative Management of Locally Advanced Rectal Cancer.

The management of locally advanced rectal cancer has evolved significantly with the introduction of total neoadjuvant therapy, which increases rates of clinical complete response and enables select patients to pursue non-operative management through a watch-and-wait strategy. While total mesorectal excision remains the standard curative approach, it is associated with considerable morbidity and long-term impacts on bowel, urinary, and sexual function. In contrast, non-operative management offers organ preservation and improved functional outcomes but carries oncologic uncertainty and requires

AI-Augmented Fundus Disease Screening by Non-Ophthalmologist Physicians: A Paired Before-After Study.

Screening for retinal disease is increasingly performed by general practitioners and other non-ophthalmologist clinicians in primary care, especially where access to ophthalmology is limited and diagnostic accuracy may be suboptimal. To investigate the role of an automated fundus-interpretation support solution in improving general physicians' screening accuracy and referral decisions, we conducted a paired before-after study evaluating an AI-based decision support tool. Four non-ophthalmologists who have been involved in screen fundus images in clinical practice reviewed 500 de-identified col
