What makes patient navigation work? Identifying functions and forms and conducting causal loop diagramming to specify components of a successful colorectal cancer patient navigation program.

BACKGROUND: We implemented a centralized colorectal cancer (CRC) screening program with navigation to follow-up colonoscopy for community health center (CHC) patients with positive stool-based test screening results. Navigation increased six-month colonoscopy completion by 24 percentage points compared with usual care. Here, we describe how we applied a functions and forms framework alongside causal loop diagramming (CLD) to understand the effectiveness of our navigation program and explore its potential for implementation in other settings. METHODS: We first identified barriers to colonoscop

Medication therapy management in Pakistan: a cross-sectional evaluation of pharmacists' knowledge, attitudes, practices, and barriers.

BACKGROUND: Medication therapy management (MTM) is a patient-centered clinical service designed to optimize therapy and enhance patients’ quality of life. The published research has shown that pharmacists’ MTM services improve health outcomes. Collaboration among pharmacists, patients, and other healthcare providers is essential to ensure the safe and effective use of medicines. Despite its benefits, countries like Pakistan still face challenges in raising awareness of and implementing MTM. This study aimed to assess the current knowledge, attitudes, and practices of community and hospital pha

The telemedical platform MyaLink for remote monitoring in myasthenia gravis - rationale and protocol for a proof of concept study.

RATIONALE: Myasthenia gravis (MG) is a rare, chronic neurological disorder leading to fluctuating muscle weakness and potentially life-threatening crises. Patients often require life-long specialized treatment, but timely interventions are frequently hindered by the limited availability of specialists. Telemedical solutions at specialized centers enabling patient-physician interaction hold promise in bridging this gap, but are not yet available for MG. We developed 'MyaLink,' a remote monitoring platform tailored for MG, and outline the study design assessing the platform and clinical outcomes

National Trends and Predictors of Waiting Times for Cancer Surgery in the US.

IMPORTANCE: As cancer care becomes increasingly complex, patients may experience prolonged delays between diagnosis and definitive surgical management. While consolidation and expansion of health systems, along with centralization of care, may further amplify these delays, a comprehensive assessment of contemporary waiting times within cancer surgery care pathways in the US is lacking. OBJECTIVE: To evaluate national trends and predictors of delays from diagnosis to initiation of first-course therapy among patients ultimately undergoing cancer surgery. DESIGN, SETTING, AND PARTICIPANTS: This

BACKGROUND: Cisgender women (subsequently referred to as women) account for 23% of new HIV diagnoses in the United States. There are significant sociostructural barriers to engagement and retention in the pre-exposure prophylaxis (PrEP) cascade, particularly for Black women. OBJECTIVE: In response to the lack of evidence-based interventions (EBIs) to improve PrEP initiation, adherence, and persistence among women in the United States, we developed a clinic-based, sociostructural intervention focused on engagement and retention in the PrEP cascade for women. METHODS: We used the Assessment, D

Geographic Dialysis Facility Density and Early Dialysis Initiation.

IMPORTANCE: The decision of when to start maintenance hemodialysis may be affected by health system-level support for high-intensity care as manifested by area dialysis facility density. Yet an association between early hemodialysis initiation and higher area density of dialysis facilities has not been shown. OBJECTIVE: To examine whether there is an association between area dialysis facility density and earlier dialysis initiation. DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional analysis was conducted of publicly reported claims and geographic-based population data collected in the Medic

A Cross-Sectional Study of the Trends in Cardiovascular Mortality Among African Americans With Hypertension.

Background and Objective In the United States, hypertension remains a significant cause of cardiovascular disease mortality and morbidity, affecting various racial and ethnic groups. High blood pressure is a common health concern, given its high frequency among all populations and racial groups in the United States; nevertheless, the condition remains untreated in most individuals. It affects a significant number of individuals in the African American community and contributes to a notable proportion of deaths. Arguably more prevalent, severe, and tends to occur earlier in African Americans co

Comparative host feeding patterns of the Asian tiger mosquito, Aedes albopictus, in urban and suburban Northeastern USA and implications for disease transmission.

BACKGROUND: Aedes albopictus is an invasive species which continues expanding its geographic range and involvement in mosquito-borne diseases such as chikungunya and dengue. Host selection patterns by invasive mosquitoes are critically important because they increase endemic disease transmission and drive outbreaks of exotic pathogens. Traditionally, Ae. albopictus has been characterized as an opportunistic feeder, primarily feeding on mammalian hosts but occasionally acquiring blood from avian sources as well. However, limited information is available on their feeding patterns in temperate re

Sleep Trajectories and All-Cause Mortality Among Low-Income Adults.

IMPORTANCE: Short and long sleep durations are adversely associated with cardiovascular disease (CVD), type 2 diabetes, and mortality. It remains unclear how sleep duration trajectories over time are associated with mortality and whether these associations vary by well-documented sex, race, and socioeconomic sleep disparities. OBJECTIVE: To investigate the association of 5-year sleep duration trajectories with all-cause and cause-specific mortality among US adults, predominantly those in low-income groups. DESIGN, SETTING, AND PARTICIPANTS: The Southern Community Cohort Study included partic

National surveillance for radiological exposures and intentional potassium iodide and iodine product ingestions in the United States associated with the 2011 Japan radiological incident.

BACKGROUND: In March of 2011, an earthquake struck Japan causing a tsunami that resulted in a radiological release from the damaged Fukushima Daiichi nuclear power plant. Surveillance for potential radiological and any iodine/iodide product exposures was initiated on the National Poison Data System (NPDS) to target public health messaging needs within the United States (US). Our objectives are to describe self-reported exposures to radiation, potassium iodide (KI) and other iodine/iodide products which occurred during the US federal response and discuss its public health impact. METHODS: All

Smart Speaker-Based Applications to Support Social Connectedness in Older Adult Residents in Affordable Housing: User-Centered Design Study.

BACKGROUND: Older adults in affordable housing face heightened risks of social isolation and loneliness due to limited social networks, transportation barriers, chronic conditions, and inadequate technology access. Smart speakers offer potential for enhancing social connectedness in this underserved population, yet technology interventions are rarely designed with meaningful input from older adults themselves. User-centered design (UCD) approaches can address this gap by engaging end users throughout the development process to ensure technology solutions align with their needs and living conte
