Development and validation of a user-friendly smartphone imaging and telemedicine platform for remote diagnosis of anterior segment eye disease.

BACKGROUND: Cataract and anterior segment diseases are leading causes of blindness in low-resource settings. Eye camp screenings remain the primary mode of community outreach but are constrained by cost, logistics, and dependence on highly trained specialists. We designed and validated a low-cost, user-friendly smartphone-based anterior segment imaging and teleophthalmology platform to enable community health workers (CHWs) to perform diagnostic-quality eye screening. METHODS: We designed a portable imaging device (Scout TM ) paired with an accessible Android smartphone and mobile application

Integrating accompanying patients into clinical oncology teams: limiting and facilitating factors.

OBJECTIVES: Since 2018, four establishments in Quebec have been instrumental in implementing the PAROLE-Onco program, which introduced accompanying patients (APs) into healthcare teams to improve cancer patients' experience. APs are patient advisors who have acquired specific experiential knowledge related to living with cancer, using services, and interacting with healthcare professionals. They are therefore in a unique and reliable position to be able to provide emotional, informational, cognitive and navigational support to patients who are dealing with cancer. We aimed to explore APs' pers

Identifying prioritised actions for improving dysphagia services in Singapore: Insights from concept mapping with patients and caregivers.

BACKGROUND: Person-centred services are inherently related to patients/caregivers' needs. However, the extent to which dysphagia services within Singapore meet consumer needs is largely unknown. Involving patients/caregivers in service evaluation/redesign deepens the understanding of consumer needs. This process also increases the success of implementing service enhancements through relevant and meaningful change. AIM: This study aimed to identify aspects of dysphagia services, within acute and subacute settings in Singapore, perceived as highly important and easy to change by patients/caregi

The Occupational Health and Safety of Sign Language Interpreters Working Remotely During the COVID-19 Pandemic.

INTRODUCTION: Rates of human papillomavirus (HPV) vaccine are lower in rural counties than in urban areas. The objective of the minigrants evaluation was to assess HPV vaccination uptake, factors related to implementation success, and barriers to implementation. METHODS: We conducted a concurrent, mixed-methods evaluation guided by the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) and CFIR (Consolidated Framework for Implementation Science) frameworks. Four health departments received funding, a toolkit with implementation strategies, and monthly technical assistanc

Simple Mobile technology health management tool for people with severe mental illness: a randomised controlled feasibility trial.

BACKGROUND: Severe mental illness (SMI) is associated with care delivery problems because of the high levels of clinical resources needed to address patient's psychosocial impairment and to support inclusion in society. Current routine appointment systems do not adequately foster recovery care and are not systematically capturing information suggestive of urgent care needs. This study aimed to assess the feasibility, acceptability, and potential clinical benefits of a mobile technology health management tool to enhance community care for people with severe mental illness. METHODS: This random

The epidemiology of traumatic brain injuries sustained by children under 10 years of age presenting to a tertiary hospital in Soweto, South Africa.

BACKGROUND: Traumatic brain injury (TBI) in the paediatric population is a significant contributor to death and disability worldwide. In sub-Saharan Africa, death and disability from TBI are still superseded by infectious disease. Mechanisms of injury differ by region and socioeconomics, but in general, falls, road traffic collisions (RTCs), being 'struck by/against objects' and non-accidental injuries (NAIs) are responsible for most cases. OBJECTIVES: To: (i) quantify the burden of TBI in terms of demographics, causes and severity; (ii) explore resource utilisation regarding length of stay,

Atopic Dermatitis in Indigenous Communities in Canada: A Scoping Review and Environmental Scan of Assessment Instruments, Access Barriers, and Teledermatology Interventions.

Indigenous communities in Canada experience higher prevalence and disease burden of atopic dermatitis (AD) alongside systemic barriers to dermatologic care. This scoping review synthesizes evidence on AD prevalence, assessment approaches, barriers to access, and intervention models for Indigenous populations, an area with a critical dearth of rigorous research. A PROSPERO-registered, PRISMA-compliant systematic search of 5 databases identified limited rigorous research: No randomized trials specific to AD management exist, and Indigenous participants remain underrepresented in clinical trials

Preventing Medication Errors in Paediatrics: A Narrative Review.

Paediatric medication errors remain a major and preventable cause of patient harm worldwide. Children are particularly vulnerable to medication-related adverse events because of weight-based dosing requirements, age-dependent pharmacokinetics, off-label medication use, communication limitations, and the need for complex drug preparation and administration processes. High-risk settings such as neonatal intensive care units, emergency departments, oncology services, and transitions of care are associated with particularly high rates of prescribing and administration errors. This narrative review

Peer-led recruitment for equitable engagement of people who inject drugs: Evidence from the HEPCO cohort in Montreal, Canada.

BACKGROUND: Engaging people who inject drugs (PWID) in research is challenging due to stigma, distrust, and structural barriers. Peer-led recruitment may enhance reach and equity, yet few studies have quantitatively evaluated its outcomes. METHODS: Between Feb 2021-Dec 2023, a Partnerships Lead with Lived Experience implemented three new recruitment strategies within the HEPCO cohort study of PWID in Montreal, Canada. Strategies extended research activities into the community through a flexible engagement model comprising pop-up research clinics (n = 109 events), street outreach (n = 29 event

Cardiovascular risk reduction through a peer-led digital health lifestyle intervention in a low-income community: outcomes from the MYCardio-PEER program.

BACKGROUND AND AIMS: Low-income communities face a disproportionately high burden of cardiovascular disease (CVD), yet often lack access to preventive care. Peer-based interventions delivered through digital platforms offer a potentially scalable approach to address these disparities, but evidence remains limited. Our study evaluated the preliminary impact of an 8-week peer-led digital health lifestyle intervention (MYCardio-PEER) in improving knowledge, lifestyle behaviours and biomarkers related to cardiovascular health among a low-income population. METHODS AND RESULTS: Adults from the low

Impact of non-technical skills training on patient outcomes in surgery: a systematic review and meta-analysis.

BACKGROUND: Non-technical skills (NTS) such as teamwork, communication and leadership are essential for surgical safety, but their impact on patient outcomes is uncertain. This systematic review evaluated whether NTS interventions improve operative and patient outcomes in surgical settings. METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines, seven databases were searched for randomised and non-randomised studies of NTS interventions reporting patient or operative outcomes. Eligible interventions included team training, simulation, Cr

Nurse Practitioner Roles and Contributions in a Mobile Health Clinic for People Experiencing Homelessness.

AIM: To present a descriptive, practice-based account of a nurse practitioner-led mobile health clinic serving individuals experiencing homelessness, guided by the PRIME-NP framework. BACKGROUND: Homelessness is associated with health inequities and limited access to care. Individuals face barriers including transportation, stigma, and displacement. Although mobile outreach may help address these barriers, little work has examined competencies for nurse practitioners in unstable, resource-limited environments. In this clinic, nurse practitioners provide field-based care, including wound manag

"I liked having my voice heard:" a feasible, rapid approach for codesigning a contraceptive counseling curriculum.

Few youth- and family-engaged codesigned models of medical education programs exist. One area that may benefit from community perspectives is designing education to improve counseling on sensitive topics. The authors' codesigned an adolescent-focused contraceptive counseling continuing medical education training program for practicing primary care clinicians in partnership with community advisory boards and assessed the feasibility and acceptability of their codesign approach. From September 2023 to January 2024, the authors recruited female teens, caregivers, and clinicians residing in North

Delays in tuberculosis care among the top-ten-tuberculosis-high burden countries in the world: a scoping review.

BACKGROUND: Delayed diagnosis and treatment can be detrimental not only to patients but also to the community, as such delays can perpetuate TB transmission. This presents a significant challenge in the ongoing efforts to eliminate TB from the community. The primary objectives of this scoping review were to map out research surrounding delays in TB care across the ten countries with the highest TB burden globally and to identify research gaps in this area. METHODS: The guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews (PRISMA-Sc

Behavioral telehealth in low-resource primary care settings for anxiety and depression in youth: protocol for a randomized effectiveness-implementation study.

BACKGROUND: Anxiety and mood disorders in youth are prevalent and impairing, with high current and lifetime comorbidity. Untreated, these disorders lead to sustained functional impairment and increased risk for recurrent disorder and suicide. In addition, there are notable disparities in care, with both families experiencing increased social risk and youth of color significantly less likely to receive mental health services. In previous research, a primary-care-based transdiagnostic brief behavioral therapy (BBT) was developed and found to be effective for a broad population of youths with anx

Understanding the Unmet Needs of People with MS at Diagnosis and Throughout Their Care Journey: Insights from a Survey-Based Study.

INTRODUCTION: Multiple sclerosis (MS) has a broad range of symptoms and heterogenous trajectory that requires individualised care. To optimise shared decision-making between healthcare professionals (HCPs) and people with MS (PwMS), it is important to understand communication needs from the patient perspective at diagnosis and throughout their care journey. METHODS: Two multinational online surveys were conducted to explore (1) communication needs around the time of diagnosis, and (2) PwMS empowerment in communicating their specific needs and symptoms. Questionnaires included ten close-ended
