Avascular necrosis of the first metatarsal head following Austin osteotomy: a follow-up study.

A prospective study is described of 12 consecutive patients with hallux abducto valgus deformity in whom an Austin osteotomy without lateral release was performed. All patients had similar postoperative care and magnetic resonance images (MRI) studies of their first metatarsals. The MRI was performed in T1- and T2-weighted images with algorithms programmed to image changes in the bone consistent with avascular necrosis of the first metatarsal head. One of the 12 patients following Austin osteotomy without lateral release had changes on MRI consistent with avascular necrosis of the first metata

The natural history of hip deformity in myelomeningocele.

We studied 1061 children with myelomeningocele, reviewing 3184 pelvic radiographs from 802 patients. Hip dislocation had occurred by the age of 11 years in 28% of children with a thoracic neurosegmental level, 30% of those with an L1/2 level, 36% of L3, 22% of L4, 7% of L5 and only 1% of those with sacral levels. Hip dislocation was not inevitable even when there was maximal muscle imbalance about the hip. The average hip flexion contracture in children aged 9 to 11 years was significantly greater in those with thoracic (22 degrees) and L1/2 (33 degrees) levels than in those with L4 (9 degrees

Digital "V" arthrodesis.

Numerous procedures have been proposed through the years for the treatment of digital hammertoes and other related structural deformities. Experience has determined that many of these procedures are structurally incorrect and result in secondary deformity, which are often worse than the pathology initially addressed. Phalangeal base resections were found to destroy the structural stability provided by the intrinsic musculature. Phalangeal head resections make the toe impressionable to the extrinsic forces of the adjacent toes often leading to structural mal-alignment, or additional contracture

Gunshot wounds to the face. Current concepts.

The primary goal in reconstructing a gunshot wound to the face is identical to that for a major injury of the hand or lower extremity: primary healing of the soft tissue. This prevents the cascade of soft-tissue infection, further delay in healing, bone graft infection, and soft-tissue contracture. Current craniofacial techniques and methods of fixation allow for concomitant bony reconstruction without compromising primary healing of the soft tissues. The result is not only that the soft tissue is healed but also that the original contour, stretch, and pliability are maintained. Once the soft

[Compartment syndrome of the hand. Diagnosis, therapy, results, late sequelae].

Ischemic necrosis of the intrinsic muscles of the hand has been known about for decades, as has the resulting local ischemic contracture in the hand. As isolated compartment syndrome of the hand is a rare condition, this term has not yet become established. The etiopathology, diagnosis, therapy and results of treatment in 13 patients treated for compartment syndrome of the hand are presented. At the time of our follow-up study, 3 patients had died and 2 did not attend. In 1 patient the upper limb had meanwhile had to be amputated. Thus, late results were examined in 7 patients, but 2 of these

Non-invasive evaluation of malignant hyperthermia susceptibility with phosphorus nuclear magnetic resonance spectroscopy.

Using in vivo 31P NMR spectroscopy, the authors compared the NMR spectra of the flexor muscles of the forearm from 13 humans characterized as MH susceptible on the basis of in vitro caffeine/halothane contracture tests with those from 25 normal controls. The levels of phosphocreatine (PCr), inorganic phosphate (Pi), and ATP during rest, graded exercise, and post-exercise recovery were measured in their forearms. MH susceptible subjects had significantly (P less than 0.001) higher Pi/PCr values (0.222 +/- 0.009) at rest than did normal controls (0.140 +/- 0.004). In addition, a significantly (P

Muscle energy metabolism in McArdle's syndrome by in vivo phosphorus magnetic resonance spectroscopy.

Five patients with McArdle's syndrome were examined by phosphorus magnetic resonance spectroscopy (31P-NMR). Adenosine triphosphate (ATP) levels at rest were reduced by 22%, but did not fall further during exercise or contracture. The slope of work rate versus inorganic phosphate/phosphocreatine (Pi/PCr) was 42 +/- 8 joules/min/Pi/PCr in three patients without muscle wasting, compared with 13 and 16 in patients with atrophy (normal, 30 to 50 joules/min/Pi/PCr). Recovery from exercise showed similar rates in patients (postischemic exercise 1.03 +/- 0.17, post-aerobic 1.63 +/- 0.17 PCr/Pi units

Erythrocyte fragility screening is not a tool for diagnosis of human malignant hyperthermia.

The fragility of erythrocytes from 21 patients undergoing in vitro skeletal muscle contracture testing for malignant hyperthermia (MH) susceptibility was examined. Osmotic fragility was determined by the degree of hemolysis in buffered saline solutions of decreasing osmotic strength. Halothane-induced fragility was determined in an isotonic solution containing increasing percentages of halothane saturated solution. The effects of six different incubation conditions prior to performing fragility tests were examined in an attempt to optimize discrimination of MH susceptible patients, including t

Functional status in ulnar deficiency.

Function is the most important consideration in the evaluation of patients with congenital ulnar deficiency. The upper extremity function of 8 patients with ulnar deficiency was evaluated. None of these patients had been treated with surgical procedures directed to the elbow, forearm, or wrist. The functional criteria included: (a) active ranges of elbow, forearm, and wrist motion, (b) power grip, (c) prehension, (d) dexterity, and (e) a patient's activities questionnaire. The averaged total active range of joint motion was 229 degrees or 46% of predicted normal active motion. Power grip avera

[Orthopaedic examination of 1 500 infants in maternity (author's transl)].

1 500 infants have been examined by two orthopaedic surgeons within forty eight hours. The examination of the feet has showed that the usual position in utero was the dorsiflexion. A new classification of deformities of the feet is proposed. The examination of the knees has permitted to discover a rotatory instability between the femur and the tibia in 2% of the infants. Some of them were actual dislocations. During the examination of the hips, it has been researched an instability; the passive abduction, the tension of the adductors and an abduction contracture have been analysed. Some instab

Modification of fibronectin distribution pattern in solid human tumours.

Using indirect immunofluorescence, the distribution pattern of fibronectin has been studied in a variety of solid human tumours and corresponding normal tissues. Fibronectin could be detected on normal epithelial cell membranes, in periglandular and perivascular basement membranes and in the intercellular stroma. No modification of this distribution pattern was detected in a variety of benign tumours or in certain pathological, non-malignant disorders (Dupuytren's contracture). In infiltrating mammary carcinomas, cell membrane staining disappeared even before invasion, the staining pattern of

[Essential deformans osteolysis. Report of a case with progressive and generalized evolution (author's transl)].

The authors report a clinical and radiographic course of a 16 years old caucasian girl with a progressive deformation of her skeleton. The primary lesions occur in knees and wrists. This disease was at first considered as a rheumatoid arthritis, then as a dysplasia epiphysialis multiplex, and now as essential deformans osteolysis (with carpal lysis, shortening of the forearm bones, dislocation of the elbows, disparition of the humeral and femoral heads, contracture of hips and knees, posterior tarsal lysis, and kypho-scoliosis). The clinical (particularly ophtalmologic), biological (including

Hemifacial spasm and the facial nucleus.

Spontaneous and associated hyperkinetic facial movements and contracture which follow injury to the seventh cranial nerve (postparalytic hemifacial spasm) or arise without known previous injury (cryptogenic hemifacial spasm) are pathological motor phenomena not found in the distribution of other cranial or somatic motor nerves. The commonly expressed hypotheses of pathogenesis--aberrant regeneration and fiber excitation by false synapse formation (ephapses) at the site of injury--cannot account for all aspects of these phenomena or for the uniqueness of such movements to the distribution of th

Juvenile diabetic cheiroarthropathy.

A peculiar involvement of the interphalangeal joints of both hands with palmar flexion of the fingers has been observed in 11 insulin-treated, nonrheumatoid, juvenile diabetics. The onset of diabetes occurred between 1 and 12 years of age. Painless deformities of the fingers with progressive stiffness and impaired extension started 4 to 10 years later. One patient complained of articular pain and swelling. X-ray and circulatory changes were absent or minimal. Prepubertal patients showed delayed puberty and stunted growth, adult patients had normal sexual development. Rheumatic or rheumatoid si

From learned helplessness to motor recovery: integrating intensive neurorehabilitation in poststroke spastic paresis-clinical insights from over 10 years of practice.

BACKGROUND: Poststroke spastic paresis represents a dual pathology combining neurological impairment and secondary muscle contracture, often perpetuated by learned non-use. Current rehabilitation approaches frequently address these components separately, limiting functional recovery potential. METHODS: This perspective synthesizes over 10 years of clinical experience treating poststroke spastic paresis across European and sub-Saharan African settings, integrating constraint-induced movement therapy with progressive muscle-lengthening protocols. Clinical insights are drawn from over 300 patien

Case Report: Frostbite in the tropics: abdominal pedicled flap reconstruction to prevent the metacarpal hand.

The reconstructive salvage of a severely frostbitten hand in a tropical setting presents a unique challenge. We report a 59-year-old male who sustained extensive frostbite to the face, ears and bilateral hands following a Mount Everest ascent. Hyperbaric oxygen therapy (HBOT) was initiated during demarcation to optimise tissue viability. Following demarcation, dry gangrene of the right hand required amputation, revealing viable 1.5-2 cm proximal phalangeal stumps in all five digits. To avoid a non-functional metacarpal hand, reconstruction aimed to preserve digital length and metacarpophalange

Botulinum Toxin Type A as an Early Intervention for Traumatic Oculomotor Nerve Palsy: A Pediatric Case Report.

Traumatic third cranial nerve palsy is a rare complication of head injury, with an incidence of approximately 1% and a characteristically poor prognosis. Conventional management remains conservative, often yielding unsatisfactory outcomes. We report the case of a 13-year-old girl who developed complete right third cranial nerve palsy following a 15-meter fall, presenting with exotropia (35Δ), ptosis, complete ophthalmoplegia, and pupillary dysfunction. Brain CT revealed hemorrhage in the right cavernous sinus, and subsequent MRI demonstrated focal nerve damage. Thirty-eight days post-injury, a

A rhesus macaque model of α-dystroglycanopathy caused by a POMT1 splice altering variant.

BACKGROUND: Biallelic mutations in genes associated with α-dystroglycan glycosylation manifest in a spectrum of conditions referred to as α-dystroglycanopathies, including Walker-Warburg syndrome, which primarily disrupt brain, eye, and muscle development. While small-animal models for the disease have been developed and described, the condition has not been documented in non-human primates. METHODS: Three spontaneous cases of severe lissencephaly, microphthalmia, and congenital muscular contracture in infant rhesus macaques were investigated. Histological analysis was performed on brain, ret

Cast-rod construct for gradual correction of fixed knee flexion deformities in young children with arthrogryposis: Technical note and preliminary results.

INTRODUCTION: Fixed knee flexion deformities (FKFDs) are common in children with arthrogryposis, yet their management in younger children is underreported. MATERIAL AND METHOD: This study describes a non-invasive cast-rod construct for the gradual correction of FKFDs secondary to arthrogryposis. RESULTS: This study presents preliminary results from 18 knees in 12 patients under four years old (median age at 30 mo, 18-48). The median knee flexion contracture angle improved from 54.5 degrees (27-90) to 10 degrees (0-30) after cast removal, (p < 0.05). Treatment lasted for a median of 32 days (

Diagnosis and treatment of early eosinophilic fasciitis: a case report.

BACKGROUND: Eosinophilic fasciitis is an infrequent connective tissue disorder that often delays treatment owing to misdiagnosis or omission. In advanced stages, severe skin sclerosis can lead to joint contracture and tendon retraction, which can severely affect the quality of life. Therefore, it is important to make a clear diagnosis and intervene in the early stages of the disease. CASE PRESENTATION: We report a case of a 52-year-old Chinese patient with marked symmetric swelling of her extremities, especially forearms and calves, sclerosis of the subcutaneous tissue, limited joint mobility
