Alcohol Use Disorder Is Associated With Higher Rates of Adverse Events, Readmissions, and Costs of Care Within 90 Days of Arthroscopic Rotator Cuff Repair.

PURPOSE: To determine whether alcohol use disorder (AUD) patients undergoing arthroscopic rotator cuff repair (ARCR) have higher rates of (1) medical complications, (2) readmission rates, and (3) costs of care. METHODS: A retrospective review of a private insurance payor was performed. All patients who underwent ARCR were initially identified as the base population. All patients with a diagnosis of AUD were filtered from this base population and served as the study cohort. Conversely, those patients without AUD who underwent ARCR represented the comparison cohort. Patients who underwent ARCR

Mechanical stress-mediated ferritinophagy aggravates cartilage endplate degeneration via a PIEZO1-NCOA4-ZBP1 axis.

Abnormal mechanical stress is closely linked to intervertebral disc degeneration (IVDD). Iron homeostasis disorder occurs in various degenerative diseases, including IVDD. PIEZO1 serves as a mechanosensitive cation channel, involving in multiple physiological and pathological processes; however, its potential association with iron homeostasis and IVDD remain to be elucidated. Here, it is discovered that PIEZO1 accumulates in cartilage endplate (CEP) during IVDD, accompanied by intensive ferritinophagy. Specific activation of PIEZO1 or NCOA4 (nuclear receptor coactivator 4) aggravates CEP degen

Chronic low-grade inflammation drives skeletal aging and neurocognitive decline: inflammaging as a central hub coupling bone-brain aging.

BACKGROUND: Global population aging has driven a marked rise in the co-prevalence of osteoporosis and cognitive impairment, and a bidirectional epidemiological association between the two conditions is now supported by multiple meta-analyses. The shared biological mechanisms underlying this comorbidity, however, remain incompletely defined, and the two disorders continue to be managed within largely separate clinical disciplines. AIM AND SCOPE: This review consolidates existing evidence into an integrative immunopathological framework in which inflammaging-the chronic, low-grade, sterile syst

Analysis of Blame, Guilt, and Shame Related to Body and Body Weight and Their Relationship with the Context of Psychological Functioning Among the Pediatric Population with Overweight and Obesity: A Systematic Review.

Background/Objectives: There is scientific evidence showing that body- and/or body weight-related blaming, guilting, and shaming continue to be both promoted and tolerated in many societal contexts, including schools and healthcare settings. A deeply ingrained belief still prevails among many individuals that inducing these negative emotions can serve as a motivator for children and adolescents to engage in obesity treatment. Therefore, the aim of this systematic review is to examine these emotions (blame, guilt, shame) in relation to body weight and their impact on psychological functioning w

The associations among serum vitamin D concentration, systemic immune-inflammation index, and lifestyle factors in Chinese adults: a cross-sectional analysis.

INTRODUCTION: Vitamin D is a crucial lipid-soluble hormone that has been demonstrated to be closely associated with systemic chronic inflammation and various diseases, including metabolic disorders, cardiovascular conditions, autoimmune diseases, cancer, and aging. The pathological underpinnings of these diseases are intricately linked to systemic chronic inflammation. The systemic immune-inflammation index (SII), an emerging biomarker, offers a more comprehensive reflection of the state of systemic inflammation and the immune response by integrating interactions among diverse immune cell type

Evaluating Risk Factors for Lethality in Posterior Reversible Encephalopathy Syndrome following Hematopoietic Stem Cell Transplantation in Pediatric Patients: A Systematic Review and Individual Participant Data Meta-analysis.

Hematopoietic stem cell transplantation (HSCT) is frequently the sole curative treatment for a range of hematologic and nonhematologic disorders. One of the most notable neurological complications associated with HSCT is posterior reversible encephalopathy syndrome (PRES), which affects approximately 1 to 10% of pediatric recipients. Although usually reversible, PRES can lead to serious morbidity and lethality. This systematic review and individual participant data (IPD) meta-analysis aims to evaluate risk factors for lethality and characterize the clinical course of PRES in pediatric HSCT pat

Inflammatory Signatures in VEXAS Syndrome, Myelodysplasia Cutis, and Sweet Syndrome.

IMPORTANCE: VEXAS syndrome (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) is a monogenic disease caused by UBA1 somatic variants in hematopoietic progenitor cells, mostly involving adult men. It is associated with inflammatory-related symptoms, frequently involving the skin and hematological disorders. Recently described myelodysplasia cutis (MDS-cutis) is a cutaneous manifestation of myelodysplasia in which clonal myelodysplastic cells infiltrate the skin. In both cases, skin lesions are driven by the infiltration of clonal mutated myeloid cells and may clinically and histological

E. B. Hershberg Award: Taming Inflammation by Tuning Purinergic Signaling.

The author presents his personal story from early contributions in purinergic receptor research to present-day structure-guided medicinal chemistry. Modulating purinergic signaling (encompassing pyrimidine nucleotides as well) and other nucleoside targets with small molecules is fruitful for identifying new directions for therapeutic intervention. Purinergic signaling encompasses four adenosine receptors, eight P2Y receptors that respond to various extracellular nucleotides, and trimeric P2X receptors that respond mainly to ATP. Each organ and tissue in the body expresses some combination of t

Parthenolide Exerts Protection against Hypoxic-Ischemic Brain Damage in Neonatal Rats by Inhibiting Perk-Mediated Endoplasmic Reticulum Stress.

Perinatal hypoxia causes hypoxic-ischemic brain damage (HIBD), which can result in neurological sequelae of varying severity. This syndrome is a major cause of neonatal mortality and pediatric impairment. Although research is ongoing to better understand its origins and develop targeted treatments, endoplasmic reticulum stress (ERS)-mediated apoptosis via the unfolded protein response (UPR) is crucial for the progression of cerebral ischemia injury. Parthenolide (PN), a sesquiterpene lactone derived from the Asteraceae family, exhibits anti-inflammatory, anti-apoptotic, and antioxidant propert

The Yin and Yang of Microglia-Derived Extracellular Vesicles in CNS Injury and Diseases.

Microglia, the resident immune cells of the central nervous system (CNS), play a crucial role in maintaining neural homeostasis but can also contribute to disease and injury when this state is disrupted or conversely play a pivotal role in neurorepair. One way that microglia exert their effects is through the secretion of small vesicles, microglia-derived exosomes (MGEVs). Exosomes facilitate intercellular communication through transported cargoes of proteins, lipids, RNA, and other bioactive molecules that can alter the behavior of the cells that internalize them. Under normal physiological c
