[Bilateral pneumothorax during awake fiberoptic intubation : A rare complication of bronchoscopy].

An 86-year-old woman was scheduled for surgical treatment of a periprosthetic femoral fracture after a fall. Because of a known difficult airway due to multiple surgeries and radiation for cancer in the orofacial area, awake fiberoptic intubation was planned. During the fiberoptic maneuver the patient experienced a massive bout of coughing which resulted in large soft tissue emphysema and double-sided pneumothorax. This case reports a rare complication of fiberoptic intubation and airway management.

Pseudomesotheliomatous carcinoma due to pleural metastasis from renal pelvic cancer.

A 78-year-old man was referred to our department with a one-week history of dyspnea and coughing. A chest X-ray showed massive left pleural effusion. Computed tomography revealed diffuse irregular thickening of the left pleura similar to malignant mesothelioma and multiple nodules in both lungs. The patient died of respiratory failure nine days after hospitalization. An autopsy revealed metastasis to the pleura and lungs from urothelial carcinoma of the left kidney.

Massive bronchogenic cyst mimicking ischaemic chest pain.

The presentation of the bronchogenic cyst is variable making preoperative diagnosis difficult. The majority are either asymptomatic or discovered incidentally. The most common presenting symptoms are cough, fever and dyspnoea. We discuss the case of a large bronchogenic cyst in the posterior mediastinum mimicking ischaemic cardiac pain in a patient with known heart disease. This case demonstrates the need for detailed investigations prior to the treatment of an assumed acute coronary syndrome as a bronchogenic cyst may be the rare cause of such symptoms.

Cyst vallecula unusual presentation.

Laryngeal cysts, particularly cyst vallecula are generally benign lesions which cause mild dysphagia or change in voice. We report here a case of cyst vallecula that caused soreness of throat, dysphagia and change in voice. A 22-year male presented with progressive dysphagia and muffled voice secondary to a large cystic mass involving entire vallecula and during coughing presented in oral cavity. Per oral marsupilization done and the problem resolved.

Avian influenza A virus causing an outbreak of contagious interstitial pneumonia in mink.

An outbreak of contagious respiratory disease in mink occurred in October 1984 on the south-east coast of Sweden. High morbidity with coughing, sneezing and dullness was reported. Post mortem examination showed interstitial pneumonia in most examined mink. An avian influenza A virus was isolated and shown to belong to serotype H10N4. Serological studies established that this virus was the most probable cause of the outbreak and also that this new viral infection seemed to be limited to the south-east coast of Sweden.

Inhibition of bipolar demand pacemaker by diaphragmatic myopotentials.

This report describes inhibition of a normally functioning bipolar demand pulse generator by diaphragmatic myopotentials. Transient pacemaker suppression occurred repeatedly with deep respiration, straining, the Valsalva maneuver, coughing, sneezing and laughing. When the magnet was applied, none of these maneuvers inhibited the pacemaker. Extensive investigations ruled out an intermittent electrode problem such as a wire fracture or insulation break. Sensing of diaphragmatic myopotentials should be considered in the differential diagnosis of unexplained pacemaker pauses.

Leveraging large language models on free-text symptoms from participatory surveillance enhances pertussis forecasting in the United States.

BACKGROUND: Declines in childhood vaccination in the U.S. have contributed to a resurgence of vaccine-preventable diseases, including a notable increase in pertussis cases. Traditional pertussis surveillance is limited by underdiagnosis and underreporting. Participatory surveillance systems such as Outbreaks Near Me (ONM) provide an additional population-level data stream by capturing self-reported symptoms. Although pertussis signals are difficult to detect due to low incidence and symptom overlap with other infections, ONM collects free-text descriptions that may contain pertussis-specific i

Bronchopulmonary sequestration in an adult patient in his late 60s.

Bronchopulmonary sequestration (BPS) is a congenital anomaly of the lung characterised by non-functioning pulmonary tissue that receives its blood supply from an anomalous systemic artery, most commonly from the aorta. Although typically diagnosed in childhood or adolescence, BPS can present later in life and may mimic more common pulmonary conditions. We present the case of a male patient in his late 60s with a persistent non-productive cough, in whom imaging revealed a sequestered lung segment with a feeding vessel arising directly from the descending aorta. This case emphasises the importan

Foreign Body Aspiration Presenting as Recurrent Bronchiolitis: Case Report.

Foreign body aspiration (FBA) is a well-recognized pediatric emergency that is frequently misdiagnosed due to symptom overlap with common respiratory conditions such as bronchiolitis, asthma and pneumonia. This case report describes a toddler with recurrent wheezing and cough that persisted despite standard medical therapy. The child was evaluated at a pediatric urgent care center in which history, physical exam findings and imaging raised concern for airway obstruction. The child was transferred to a pediatric emergency department for suspected FBA prompting bronchoscopy evaluation, with conf

What Is the Impact of Maternal Pertussis Immunization in Pregnancy on the Quantity, Quality and Longevity of Infant Vaccine Responses?: A Review of the Current Evidence.

Immunizing pregnant women against pertussis has been a powerful tool adopted in many countries to effectively reduce morbidity and mortality from whooping cough in young infants when they are most vulnerable to complications and death. One of the main trade-offs, however, may be that maternally derived antibodies modify or modulate the infant's own antibody responses to routine childhood vaccines, although whether this is both a quantitative and a qualitative phenomenon, and its long-term clinical and epidemiological implications remain largely unclear. This review will summarize our current u

The ongoing challenge of prevention of pertussis in infants: what's new in 2024?

INTRODUCTION: Pertussis, a respiratory disease caused primarily by Bordetella pertussis, is undergoing a resurgence despite decades of high rates of vaccination. The prevention of pertussis in infants presents several challenges, including the waning immunity of the acellular pertussis (aP) vaccine, the limited protection afforded to newborns before they complete the vaccine series, and the existence of gaps in maternal vaccination. Furthermore, the unwillingness or refusal of a considerable number of individuals, including some healthcare workers, to receive vaccinations represents another si

Major pulmonary clinical manifestations of e-cigarette use by adolescents: a scoping review.

It is known that a mix of marketing, lack of regulation, broad accessibility, perceived minimal risk, and flavors appealing to young people may have played a role in the widespread adoption of vaping and, consequently, its harms. Because of that, this review aims to map the spectrum of clinical pulmonary manifestations associated with the use of electronic cigarettes in adolescents, which is the most vulnerable age group. By using PRISMA scoping review guidelines, seven electronic databases were consulted using MeSH terms. Three independent reviewers performed screening and full-text analysis,

Innovative adjuvant strategies for next-generation pertussis vaccines.

The acellular pertussis (aP) vaccine has increasingly replaced the whole-cell pertussis (wP) vaccine due to its superior safety profile. However, the aP vaccine is less effective at preventing infection and transmission of Bordetella pertussis, highlighting the need for more effective aP vaccines. Current aP vaccines do not elicit the robust cellular immunity necessary to eliminate intracellular bacteria and do not induce sufficient mucosal immunity to prevent bacterial colonization in the upper respiratory tract. Incorporating novel adjuvants represents a promising avenue for the future devel

Pertussis before, during and after Covid-19.

After the Covid-19 pandemic, pertussis has made a spectacular comeback in Europe and many other parts of the world, while during the pandemic it had essentially disappeared because of the social distancing requirements. However, even before the Covid-19 pandemic, the disease was on the rise in many countries, especially those that have replaced whole-cell pertussis vaccines by acellular pertussis vaccines. Several reasons may account for this upsurge, including strain adaptation to escape vaccine-induced immunity, rapid waning of immunity after vaccination and the failure of current vaccines t

Synthesis of oligosaccharides from terminal B. pertussis LPS pentasaccharide and definition of the minimal epitope recognized by anti-pertussis antibodies.

Pertussis vaccines have been very effective in controlling whooping-cough epidemics but are ineffective in controlling circulation in older children and adults, thus facilitating the onset of future outbreaks. Antibodies against the lipopolysaccharide could reduce the carriage of the bacteria, its circulation, and transmission. The oligosaccharide fragments from the lipopolysaccharide may become a potential complement to existing vaccines in the form of protein glycoconjugates. An important step in the development of this type of vaccine is defining the minimal oligosaccharide epitope recogniz

[Goods syndrome in chronic active SARS-CoV-2 infection: a case report].

A 54-year-old female patient presented with recurrent cough and sputum production over the past year and was hospitalized several times. CT examination revealed exudative lesions in both lungs, which were partially absorbed after treatment. However, they recurred shortly after discharge, and the patient had to be readmitted. In the past year, the patient had been hospitalized six times, and her throat swabs were positive for SARS-CoV-2 at four different points in time. After receiving anti-infective and antiviral treatment in other hospitals, the above symptoms were relieved. The patient teste

A whole-cell pertussis vaccine engineered to elicit reduced reactogenicity protects baboons against pertussis challenge.

Whole-cell pertussis (wP) vaccines introduced in the 1940s led to a dramatic reduction of pertussis incidence and are still widely used in low- and middle-income countries (LMICs) worldwide. The reactogenicity of wP vaccines resulted in reduced public acceptance, which drove the development and introduction of acellular pertussis (aP) vaccines in high-income countries in the 1990s. Increased incidence of pertussis disease has been observed in high-income countries following the introduction of aP vaccines despite near universal rates of pediatric vaccination. These increases are attributed to

Combined transcriptomic and ChIPseq analyses of the Bordetella pertussis RisA regulon.

The regulation of Bordetella pertussis virulence is mediated by the two-component system BvgA/S, which activates the transcription of virulence-activated genes (vags). In the avirulent phase, the vags are not expressed, but instead, virulence-repressed genes (vrgs) are expressed, under the control of another two-component system, RisA/K. Here, we combined transcriptomic and chromatin immunoprecipitation sequencing (ChIPseq) data to examine the RisA/K regulon. We performed RNAseq analyses of RisA-deficient and RisA-phosphoablative B. pertussis mutants cultivated in virulent and avirulent condit
